  • The study was to find out whether the more difficult each activities of daily living(ADLs) is, the more high subjective health status, or health insecurity, is among those in their 65 or older in Seoul and Gyeonggi Province. The subjects were asked up to 1 to 5 points for subjective health status and ADLs. And according to the performance level of each activities of daily living, the order trend was analyzed in four groups (very difficult, difficult, easy, and very easy) by Jonckheere-Terpstra. Order trends were also compared using a linear regression line. Depending on the degree of difficulty in "using toilet," "bathing" and "shopping," the insecurities in subjective health status showed a corresponding order differences. It has been shown that the slope of "using toilet" on a linear regression line is the largest. The more difficult it was to "use toilet," "bathing" and "shopping," the higher the score of health insecurity was.

The subjects were 140 elders with dementia residing in three nursing facilities in Chungnam, South Korea. Data were collected from May 30 to September 30 in 2016, using questionnaires. The collected data were tested by t-test, ANOVA, correlation, regression analysis. As a result of multiple regression analysis, significant influence variables on malnutritional status were Activities of Daily Living(${\beta}=0.379$, p=.001), feeding difficulty(${\beta}=0.264$, p=.001), cognitive function(${\beta}=-0.187$, p=.014) and these variables showed an explanatory power of 35.9% on malnutritional status. The results of this study are expected to be used as basic data for the development of malnutritional status improvement program for elders in facilities with dementia.

The data were analyzed using SPSS window ver. 19.0(IBM CO., Armonk, NY, USA) for t-test, one-way ANOVA, and multiple linear regression analysis. Results : Men and group of 59 years old tended to have higher stress perception than women and those who over 60 years old, but the results were not statistically significant. Stress perception was higher among the group with systemic dryness and oral dryness(each${\geq}1$) and the group reporting high difficulty in daily living due to oral problem(${\geq}2$)(p<0.05). Especially, the factor that was most correlated to stress perception was systemic dryness(${\beta}$=0.347) followed by problem of daily living for oral problem(${\beta}$=0.278, p<0.01). Conclusions : Systemic dryness and oral problem can increase the level of stress perception, mouth dryness and oral problems are a part of stress responses. The highest feeding difficulty item was "patient require close supervision while feeding". The variables influencing the feeding difficulty was ADL(${\beta}=.312$), cognitive function(${\beta}=-.172$) which explained about 20.0% of total variance. The results suggest the need of developing feeding difficulty coping strategy according to dietary behavior phase, In addition, the development and testing the educational program for caregivers to help feeding difficulty of the elders with dementia in long-term facility are recommended.

  • Purpose : The purpose of this pilot study was to examine the effects of task-oriented training program on balance, activities of daily living(ADL) performance, and self-efficacy in stroke patients. Method : Two subjects with stroke in experimental group participated in the task-oriented training program, while two subjects with stroke in control group received traditional rehabilitation therapy for 4 weeks, 30 minutes per session, four times per week. The task-oriented training program consisted of four tasks with 4 difficulty levels. In two groups, balance was examined with using the Berg Balance Scale(BBS), ADL performance was examined with using the Modified Barthel Index(MBI), and Self Efficacy was evaluated with using the Self-Efficacy scale(SES) before and after 4-week training. Result : After 4 weeks training, all scores of measurement variables increased in both the experimental group and the control group, but the average rates of change differed between the two groups. After the training program, the scores of BBS, MBI, and SES in experimental group increased to 11.4%, 9.9%, 15.4%, respectively than pre-training. Conclusion : According to the results of this study, task-oriented training program might be proposed as a intervention to improve balance ability, ADL performance, and self-efficacy in stroke patients.
