A Study on the Experts' Agreement on the Patterns Classification of the Prescription for the Use of Menopausal Disorder (갱년기장애 사용처방과 변증분류에 관한 전문가 합의 연구)

  • Objectives: This study attempted to provide prescriptions that can be used according to the classification of menopausal disorders in order to increase the clinical utilization of CaPSPI (Diagnostic System for Climacteric and Postmenopausal Syndrome Pattern Identification). Methods: Delphi method survey was conducted for expert consensus on the Pattern Identification that could be used for each prescription by selecting frequent prescriptions based on the literature researched on the prescription of Korean medicine used for women with menopausal disorders. 11 experts participated in the questionnaire for posterity and 7 experts for sasang constitution. The survey was divided into the main prescription that can be mainly used for the patterns and the minor prescription that can be used although the suitability is somewhat low. Expert consensus was obtained through the second survey, and the pattern classification of prescriptions that were not confirmed here was confirmed through a face-to-face meeting. Results: There were 56 prescriptions mainly used for treatment for menopause, 45 for posterity and 11 for Sasang constitution. There were 69 identifications of 45 posterity prescriptions and 17 classifications of 11 Sasang constitutional prescriptions including major and minor patterns. By pattern identifications, the number of major/minor prescriptions was as follows. 肝鬱 14/5, 腎陰虛 8/2, 腎陽虛 4/1, 肝腎陰虛 2/7, 腎陰陽兩虛 3/1, 心腎不交 6/3, 心脾兩虛 15/2, 心火 7/6. Conclusions: In the future, by accumulating and analyzing the data on the diagnosis and treatment using CaPSPI, the clinical status of Korean medicine for menopausal disorders can be reviewed, and it can be used as the basis for the clinical practice guidelines for menopausal disorders. In addition, it is thought that analyzing the results of the diagnosis of pattern identifications and the prescription of Sasang constitution can be helpful in research on the posterity and Sasang constitution.

Observational Clinical Study on Mibyeong Based on Korean Medicine Diagnosis, Questionnaire, and Radial Artery Tonometry (한의사의 진단, 설문지, 맥진을 이용한 미병 관리에 관한 관찰적 임상연구)

  • Objectives : This study was conducted to reveal the relationship between multiple factors of traditional Korean Medicine diagnosis and consider the further probabilities of treating people with physical and mental problems not defined as diseases, which is called 'Mibyeong' in traditional Korean Medicine. Methods : 40 healthy participants were included in the observational clinical trial. The participants were asked to complete health questionnaires (e.g. State-Trait Anxiety Inventory, Pittsburgh Sleep Quality Index, Stress Response Inventory) and they went through a traditional diagnosis process, including four stages of diagnosis (looking, listening/smelling, inquiring, and pulse taking), by a Korean Medicine doctor. Both the Korean Medicine doctor and an artery tonometry device performed the pulse diagnosis. Results : Although all participants were healthy people with no history of disease, more than half of participants had a problem related with severe level of fatigue (n=19), sleep disturbance (n=26) and stress (n=27) status according to the related questionnaires. Participants diagnosed with phlegm syndrome by the Korean Medicine doctor showed significantly greater score in phlegm pattern questionnaires than participants who were not. However, there was little agreement between the doctor's pulse diagnosis and radial artery tonometry results. Conclusions : We conducted a pulse diagnosis and measured health-related information along with the traditional Korean Medicine diagnose procedure, including four stages of diagnosis, and we found a linkage between diagnosis of phlegm and the phlegm pattern questionnaire score. The results suggest that a number of healthy participants, with no disease diagnosed, have Mibyoung symptoms which need further clinical management. Thus, we suggest that Mibyoung management programs based on qualified diagnosis tools and traditional Korean medicine diagnosis procedures be developed, and that future research using various diagnostic tools be carried out on a large population.

Impact of irregular marital cohabitation on quality of life and sexual dysfunction in infertile men from upper Egypt

  • Objective: Irregular patterns of marital cohabitation are a common problem in upper Egypt due to employment conditions. The objective of this study was to investigate the effect of irregular marital cohabitation on the quality of life and sexual function of infertile men. Methods: In total, 208 infertile men were included and divided into two groups. The first group included 134 infertile men with an irregular pattern of marital cohabitation and the second group included 74 infertile men with a regular pattern of marital cohabitation. All subjects were assessed through a clinical evaluation, conventional semen analysis, the fertility quality of life (FertiQoL) questionnaire, the International Index of Erectile Function (IIEF-5) score, and the premature ejaculation diagnostic tool (PMEDT). Results: The two groups were compared in terms of conventional semen parameters, FertiQoL, IIEF-5 score, and PMEDT. Infertile men with an irregular pattern of marital cohabitation had significantly lower subscale and total FertiQoL and IIEF-5 scores. Additionally, they had significantly higher PMEDT scores. Erectile dysfunction and premature ejaculation were more common in them than in infertile men with a regular pattern of marital cohabitation. Conclusion: Irregular patterns of marital cohabitation had an adverse effect on quality of life and sexual function in infertile men.

A Clinical Study on the Relationship between Pattern and Ryodoraku Test Identifications for Patients with Functional Dyspepsia (기능성소화불량 환자의 한의 변증 설문과 양도락 결과의 상관성 연구)

  • Objectives: This study aimed to analyze the correlation between Ryodoraku diagnosis and three pattern identification questionnaires, namely, Spleen Qi Deficiency, Stomach Qi Deficiency, and Food Retention in functional dyspepsia (FD) patients. Methods: Forty FD patients who met the Rome IV diagnostic criteria for FD participated in this study. The Ryodoraku test was conducted, and three pattern identification questionnaires were filled up by all patients. The average Ryodoraku score was called total average (TA), and the scores on the Ryodoraku points were measured. The degree of Spleen Qi Deficiency, Stomach Qi Deficiency, and Food Retention was assessed by Spleen Qi Deficiency questionnaire (SQDQ), Scale for Stomach Qi Deficiency pattern (SSQD), and Food Retention Questionnaire (FRQ). Results: The TA scores in the SQDQ patient group were lower than those in the SQDQ control group (p=0.091). The TA scores in the SSQD patient group were higher than those in the SSQD control group (p=0.651). The TA scores in the FRQ patient group were lower than those in the FRQ control group (p=0.851). The scores on the LH5, RH5, and RH6 points were significantly lower in the SQDQ patient group than in the SQDQ control group. However, no significant difference was found in the Ryodoraku scores among the other groups. Conclusions: The results suggest that the low TA and low Ryodoraku scores on the LH5, RH5, and RH6 points could be a quantitative indicator to diagnose Spleen Qi Deficiency in FD patients in a simpler manner. Larger studies on the Ryodoraku test in FD patients, the health control group, and other pattern identification groups are required.

Pattern analysis of patients with temporomandibular disorders resulting from unilateral mastication due to chronic periodontitis

  • Purpose: The purpose of the present study was to perform a pattern analysis in patients with temporomandibular disorder (TMD) resulting from unilateral mastication due to chronic periodontitis. Methods: Thirty participants with signs or symptoms of TMD who engaged in unilateral mastication due to periodontitis-related discomfort (test group) were selected. Another 30 subjects exhibiting signs or symptoms of TMD resulting from unilateral mastication not due to chronic periodontitis (control group) were also recruited. An interview-based questionnaire was administered, and an examination of the temporomandibular joint (TMJ) with determination of periodontal status was performed. Results: The duration of unilateral mastication was significantly longer in the control group than in the test group. There was a significant negative correlation between the duration of unilateral mastication and the Community Periodontal Index score. Using the Research Diagnostic Criteria for TMD (RDC/TMD) axis I algorithms, all the subjects were assigned to 3 main groups. The test group exhibited significantly a higher diagnostic distribution of group III (arthralgia, osteoarthritis, or osteoarthrosis), and in both the test and control groups, the number of diagnoses was larger for the non-chewing side. The control group showed a significantly higher diagnostic distribution of group I (myofacial pain), and in both the test and control groups, the number of diagnoses was larger for the chewing side. Conclusions: The results of the present study indicate that unilateral mastication due to chronic periodontitis could induce not only pain but also structural TMJ changes if adequate treatment is not administered and supported within a short time from the onset of the condition. Therefore, immediate treatment of chronic periodontitis is recommended to prevent not only the primary progress of periodontal disease, but also secondary TMJ-related problems. Furthermore, subjects who have suffered chronic long-term periodontitis without treatment should be urged to undergo a TMJ examination.

  • Objectives: This study aims to evaluate the severity and pattern of symptoms exhibited by teenage Korean temporomandibular disorder (TMD) patients. Materials and Methods: Among patients with an association of TMDs, teenage patients (11-19 years) who answered the questionnaire on the research diagnostic criteria for TMD (RDC/TMD) were recruited. Results: The ratio of patients who visited our clinic with a chief complaint of clicking sound (34.5%) or temporomandibular pain (36.6%) at the initial diagnosis (examination) was the highest. In the evaluation of the depression index, 75.8% of the subjects were normal, 12.9% were moderate, and 11.3% were severe. With regard to non-specific physical symptoms (including pain), 66.5% of the subjects were normal, 17.0% were moderate, and 16.5% were severe. Concerning non-specific physical symptoms (excluding pain), 70.6% of the subjects were normal, 14.4% were moderate, and 15.0% were severe. In terms of the graded chronic pain score, high disability (grade III, IV) was found in 9.3% of the subjects. Conclusion: Among teenage TMD patients, a portion have clinical symptoms and experience severe psychological pressure; hence requiring attention and treatment, as well as understanding the psychological pressure and appropriate treatments for dysfunction.

A Conceptual study on Shanghanlun Jueyinbing :Based on 2 cases treated by Shanghanlun provisions (『상한론(傷寒論)』 궐음병(厥隂病)에 대한 고찰: 변병진단체계(辨病診斷體系)에 입각해 치료한 증례 2례에 근거하여)

  • Objective : This study is to propose a new hypothesis about the interpretation of Jueyinbing in Shanghanlun by studying 2 Jueyinbing cases. Methods : We analyzed the original form of the chinese characters 'Jue(厥)', 'Jueyin(厥陰)' and 'Huoluan(?亂)' and tried to understand their contexts in Shanghanlun. At the same time, we analyzed 2 clinical cases diagnosed and treated as Jueyinbing according to newly hypothesized definition. Results : Both patients suffered from eye diseases which caused by concentrating on a sophisticated work by straining their hands, feet and chest during the night time. After the herbal intervention, their eye diseases are improved firstly. The National Eye Institute 25-item Visual Function Questionnaire (NEI-VFQ) score increased on both patients. Not only all the patients' chief complaints relieved but their other clinical problems such as limb pain, hand tremble also improved. Conclusions : These results highly insist that, at least in 15 characters provision of Shanghanlun, Jueyinbing had had the different usage from the conventional terminology, and it is assumed that Jueyinbing means the disease made by concentrating on a sophisticated work by straining hands, feet and chest during night time.

Comparison of Attention-Deficit/Hyperactivity Disorder Practice in Adults According to a Training Background in Child Psychiatry

  • Objectives: Awareness of attention-deficit/hyperactivity disorder (ADHD) in adults has significantly increased; however, clinical data specific to the Korean population are insufficient. Clinical experience of ADHD may differ based on whether psychiatrists have received pediatric psychiatry-specific training. In order to prepare a practice parameter for adult ADHD patients in Korea, we examined questionnaire data to observe how pediatric psychiatry training could affect clinical practice for adults with ADHD. Methods: A questionnaire about the diagnosis and treatment process was distributed to both general psychiatrists (GPsy) and child and adolescent psychiatrists (CAP) at the summer and winter workshop meetings of Korean Academy of Child and Adolescent Psychiatry. Results: In total, 142 psychiatrists participated in the survey (86 GPsy, 56 CAP). GPsy and CAP preferred pharmacotherapy (GPsy 82.40%, CAP 64.30%) as the primary treatment option and answered that the clinical psychiatric interview is the most necessary step in diagnostic assessment (GPsy 22.16%, CAP 19.00%). The GPsy responded with an optimal and average treatment duration that was shorter than that reported by CAP. Conclusion: Identification of the initial presenting symptom as the correct diagnosis and the optimal duration of pharmacotherapy differed between GPsy and CAP in practice, whereas concepts in diagnosis and treatment of ADHD in adults were similar for both groups. These results suggest the urgent need for the Continuing Medical Education program for psychiatrists treating adults with ADHD.

  • Objectives : This trial were conducted to evaluate the efficacy and adverse events of Bofutsusho-san(BTS) and Boiogiot-tang(BOT) by oriental obesity pattern identification on obese subjects, as compared to placebo. Methods : 166 subjects(body mass index ${\geq}25kg/m^2$) were recruited and randomized to receive BT(n=55), FH(n=55) or placebo(n=56) for 8 weeks. Anthropometric factors, serum lipid, glucose, blood pressure(BP), pulse rate, resting metabolic rate and oriental obesity pattern identification questionnaire were measured at baseline and 8 weeks. Adverse events and safety outcome variables were also checked during trials. Results : The frequency of top-scored oriental obesity pattern was ordered by indigestion(食積) > stagnation of the liver qi(肝鬱) > yang deficiency(陽虛) > spleen deficiency(脾虛) > phlegm(痰飮) > blood stasis(瘀血) in subjects. BTS group significantly decreased body weight, body mass index(BMI), waist circumference(WC), body fat mass, total cholesterol and HDL-cholesterol in stagnation of the liver qi(肝鬱) and WC in indigestion(食積). BOT group showed significant decrease of body weight, BMI, WC, and body fat mass in indigestion(食積) not in deficiency(虛症). Adverse events were reported most frequently in yang deficiency(陽虛) by BT group and stagnation of the liver qi(肝鬱) by BOT group. Conclusions : Bofu-tsusho-san was effective in treating obesity with stagnation of the liver qi(肝鬱). Obesity pattern identification could be a useful diagnostic tool predicting treatment effects and adverse events.

  • 1. Purpose of study There are many Constitution theories. But, In the Sasang Constitutional medicine, the theory and the practice are connected systematically. So it has been used in Korea popularly. Lee Je-Ma suggested that the external form, the mind, the symptom are the criteria of Sasang constitution diagnosis at the Discourse on the Four Principle and the Discourse on the Identifying Four Constitutions. But The clinical importance study of three diagnostic criteria - the external form, the mind, the symptom - has never been studied. Therefore, there are many obstacles of using the three diagnostic criteria totally. And I think that it needs a study which we apply the three diagnostic criteria to practice and approve the clinical importance of one another. I classify and investigate the previous methods of Sasang constitution diagnosis by the three diagnostic criteria in this study. 2. Method of study I investigate the previous methods of Sasang constitution diagnosis bibliographically by Dongyi Soose Bowon, Journal of Constitutional medicine and other books. 3. Result of study (1) Study of the external form The Study of Ki-sang(氣像) and Sa-ki(詞氣) by experience and intuition was done previously, and study of imaginay formulation of each constitution by measuring Chae-hyung(體形) and yong-mo(容貌) has being done recently. (2) Study of the mind For the objectification of study and diagnosis, self-reporting questionnaire has been used. The ideal choice to determine an indivisual constitution, as far as questionnaire go, is to take results from both the modified version Sasang Pattern Identification Questionnaire and QSCCII. (3) Study of the symptom For the study of the symptom - constitution symptom and constitution symptom of disease - the study of principles, the clinical study of the type of disease and symptom, the study which the pathologic view of Oriental medicine apply the view of Sasang Constitutional medicine have been done. We must try to objectfy Sasang Constitutional disease.
