Objective: To assess associations of Type II DM with hepatocellular carcinoma occurrence in Nepal. Materials and Methods: This case control study was carried out using data retrieved from the register maintained in the Department of Biochemistry of Nepalese Army Institute of Health Sciences between 1st January, 2012, and 31st August, 2012. The variables collected were age, gender, HbA1c. All biochemical parameters were analyzed in the Central Laboratory of our hospital by standard validated methods. One way ANOVA was used to examine the statistical significant difference between groups with the LSD post-hoc test for comparison of means of case groups. Odds ratios (OR) were calculated using simple logistic-regression analysis. Results: Etiological factors for HCC were HBV, HCV, alcohol and cryptogenic cirrhosis. The highest age group belonged to the etiological category of HCV with a mean of $71.9{\pm}3.6$ (CI 69.3, 74.5) years and the lowest age group to the etiological category of HBV with $61.7{\pm}5.3$(CI 57.9, 65.5) years. The main imperative basis of HCC in present study was HCV (39.5%) and second most significant cause of HCC was alcohol (26%). Glycated hemoglobin was found to be more in males with HCC (7.9%) as compared to females (7.3%). The percentage of Type II diabetes mellitus was greater in HCC patients when compared to controls. This difference was statistically significant with an odd ratio of 4.63 (p<0.001). Conclusion: Type II DM influences incidence, risk of recurrence, overall survival, and treatment-related complications in HCC patients.
The purpose of this study was to determine the dose effects of taurine supplementation and effects of taurine depletion in diabetic rats. Sprague-Dawley male rats were fed the purified diets and supplemented with 1, 2 or 3% taurine in drinking water for 7 weeks(E1, E2 and E3, respectively). To induce taurine depletion, rats were treated with 5% $\beta$-alanine in drinkeng water (EA). Diabetes was induced by streptozotocin injection(50mg/kg B.W) after 3 weeks. Taurine-depleted diabetic rats(EA)had significantly lower body weight compared to those of nondiabetic(CO), nontaurine-supplemented diabetic(E0) and taurine-supplemented diabetic rats(E1, E2 and E3). E0 had significantly higher food intake compared to that of CO and EA. E1, E2 and E3, however, had significantly lower food intake compared to that of E0, and E3 had significantly lower food intake compared to that of E1 and E2. E0 had significantly higher water intake compare to that of CO and EA. EI had significantly lower water intake compared to that of E0. Blood glucose concentration of E0 was significantly increased compared to that of CO and EA. E2 and E3 had significantly lower blood glucose concentration compared to E0 and E1. Also there was a dose effect in blood glucose concentration between E2 and E3. The total cholesterol and LDL-cholesterol concentrations of EA were significantly increased compared to those of other groups. Therefore, it may be suggested that taurine supplementation is necessary for diabetes in order to prevent diabetic complications such as cardiac vascular diseases.
Rhus verniciflua Stokes(RVS) has been widely used as a food and traditional herbal medicine in Korea. RVS has been reported that the extract from its wood and fruit has strong antioxidant activity and anticancer effect but there is little information on Lacca Sinica Exsiccata(LSE), the resin of RVS, as a medicinal use. The aim of this study was to evaluate the antidiabetic effect of ethanol-eluted extract of LSE on streptozotocin(STZ) - induced diabetic rats. Diabetes was induced in male Sprague-Dawley rats with STZ injection. Oral administration of LSE extract(50mg or 100mg/kg of body weight/day) was given to diabetic group. During 4 weeks of experiment, diabetic rats showed significant weight loss and decreasing feed efficiency ratios(FER) compared with normal rats, while the diabetic group orally fed with LSE extract showed a trend of decreasing weight loss and a significant increase of FER(p<0.05). In 4 weeks after induction of diabetes, diabetic rats showed an increase in weight of liver, kidney and heart, whereas the diabetic rats administered with LSE extract showed a reduction in the weight of heart. Blood glucose level was decreased in diabetic rats treated with LSE extract, but it was not statistically significant. Glutamic oxaloacetic transaminase, Glutamic pyruvate transaminase and total cholesterol levels were lower in the diabetic group treated with LSE extract than in untreated diabetic group, but not significant. These results present that LSE may partly have antidiabetic effect and may protect against the development of diabetic heart complications resulting from impaired glucose metabolism.
Diabetes mellitus(DM) is a metabolic syndrome caused by deficiency of insulin secretion and a consequence of insulin resistance. Poor glycemic control is a common finding in children with Type 1 DM(T1DM). Approximately 60% of the young patients with T1DM develop abnormalities in the eyes and 15-20% in the kidney. Diabetic nephropathy (DN) is a serious metabolic complication of T1DM that leads to renal failure. Some clinical studies report that the duration of prepubertal diabetes may contribute less to the development of microvascular complications than pubertal and postpubertal duration. There have been few cases of DN in prepubertal patients with T1DM in Korea. Thus we report a case of a 12-year-old female with T1DM who had poor glycemic control and was diagnosed as DN in a prepubertal period. It was proven by renal biopsy after microscopic hematuria and proteinuria were detected through the mass school urinary screening program.
Objectives: This study aims to investigate the effects of diabetes mellitus care mixture (DCM) on blood glucose and lipid metabolism in type 2 diabetic mellitus mice. DCM consisted of lagerstroemia speciose, allium hookeri, momordica charantia, amaranthus tricolor, and boesenbergia rotunda, which have been proven to have antidiabetic properties. Methods: In this study, we researched the effects of DCM in type 2 diabetic mellitus mice. C57BLKS/J mouse groups had no treatment, db/db mouse randomly assigned to 2 groups, and treated with distilled water and DCM (200 mg/kg/day). Blood glucose levels and body weight were checked every week. After 4 weeks of treatment, liver function indicators (AST, ALT, and LDH) and lipid metabolites (triglyceride, total cholesterol, LDL-cholesterol, HDL-cholesterol) were measured with a biochemistry analyzer. Diabetic factors (insulin, resistin, and leptin) were measured with ELISA. Results: DCM was decreased blood glucose, diabetic factors, liver function indicators, triglyceride, total cholesterol, and LDL-cholesterol significantly. Also, HDL-cholesterol was significantly increased in DCM group. The bodyweight of DCM group decreased but, no significant difference with the control group. DCM may have the potential to improved diabetes mellitus by regulating blood glucose levels and diabetic factors. Also protecting from diabetic complications by adjusting liver function indicators and lipid metabolites. Conclusions: These results suggest that DCM to be used as an oriental medicine for diabetes, the results of clinical trials are needed.

Diabetes mellitus is a common heterogeneous metabolic disorder, characterized by deposition of extracellular matrix, oxidative stress, and vascular dysfunction, thereby leading to gradual loss of function in multiple organs. However, little attention has been paid to gene expression changes in the lung under hyperglycemic conditions. In this study, we found that diabetes inuced histological changes in the lung of streptozotocin-induced diabetic mice. Global gene expression profiling revealed a set of genes that are up- and down-regulated in the lung of diabetic mice. Among these, expression of Amigo2, Adrb2, and Zbtb16 were confirmed at the transcript level to correlate significantly with hyperglycemia in the lung. We further evaluated the effect of human umbilical cord-derived perivascular stem cells (PVCs) on these gene expression in the lung of diabetic mice. Our results show that administration of PVC-conditioned medium significantly suppressed Amig2, Adrb2, and Zbtb16 upregulation in these mice, suggesting that these genes may be useful indicators of lung injury during hyperglycemia. Furthermore, PVCs offer a promising alternative cell therapy for treating diabetic complications via regulation of gene expression.
The purpose of this study was to determine the effects of taurine supplementation and taurine depletion on blood glucose and blood lipid concentrations in insulin-treated diabetic rats. Four groups of Sprague-Dawley male rats were fed the purified diet for 3 weeks ; nontaurine-supplemented diabetic rats(E0), nontaurine-supplemented diabetic rats with insulin treatment(E0+I), 1% taurine-supplemented diabetic rats with insulin treatment(E1+I) and taurine-depleted diabetic rats with insulin treatment(EA+I). Diabetes was induced by streptozotocin injection(50mg/kg B.W.). Isophane insulin was given subcutaneously into the abdominal wall of the diabetic rats(4 unit/rat/day). E1+I were supplemented with 1% taurine in drinking water. To induce taurine depletion, EA+I were treated with 5% $\beta$-alanine in drinking water. E1+I had significantly higher body weight compared to that of E0. The food intakes of E1+I and E0+I were significantly decreased compared to that of E0. There was no sigfniciant difference in food intake between E1+I and E0+I. The water intake of rats was significantly different among the groups ; E0>E0+I>E1+I>EA+I. The urine volume of E0 was significantly increased compared to those of insulin-treated goups. The blood glucose concentration of E0 was significantly increased compared to those of insulin-treated groups. In the oral glucose tolerance test(OGTT), E0+I and E1+I had significantly lower blood blucose concentrations compared to E0 after 30 min. Also EA+I had significantly lower bloodglucose concentrtion compared to E0 and E0+I. The plasma total cholesterol and LDL-cholesterol concentratons of EA+I were significantly incrased compared to those of other groups. Therefore, it may be suggested that tuarine supplementation is useful for insulin-dependent diabetes in order to prevent diabetic complications suchas cardiac vascular diseases.
This study was performed to investigate the effects of alcohol drinking frequency and foods consumed along with alcohol on anthropometry, serum lipid levels, and blood pressures in 73 male type 2 diabetic patients aged 30-59 years old. Dietary data for usual intake were obtained from the subjects by the 3-day food records. Separate data for foods consumed along with alcohol as accompaniment were collected and analyzed for energy and nutrient intakes. Both alcohol drinking frequency and/or the amount of energy consumed from accompaniment influenced clinical data as well as anthropometric measurements. The serum total- and HDL-cholesterol, triglyceride levels and systolic blood pressure were significantly higher in the group with a drinking frequency of ${\geq}$ 2/week than that of ${\leq}$ 1/week and also in the group whose daily energy intake from accompaniment was greater than the median (106.6 kcal/d) than that below the median. When the data were adjusted for age, amount of energy intake from alcohol and diet, the anothropometric measurements such as body weight, BMI, waist circumference were significantly higher in patients whose energy intake from accompaniment was greater than the median than that below the median. The results of our study suggest that both alcohol drinking frequency and the energy intake from foods consumed along with alcohol as accompanements are important contributing factors to clinical and anthropometric parameters whose associations with the cardiovascular complications are well established in patients with diabetes mellitus.

Diabetic nephropathy is one of the serious complications in patients with either type 1 or 2 diabetes mellitus but current treatments remain unsatisfactory. Results of clinical research studies demonstrate that Panax ginseng can help adjust blood pressure and reduce blood sugar and may be advantageous in the treatment of tuberculosis and kidney damage in people with diabetes. The heat-processing method to strengthen the efficacy of P. ginseng has been well-defined based on a long history of ethnopharmacological evidence. The protective effects of P. ginseng on pathological conditions and renal damage associated with diabetic nephropathy in the animal models were markedly improved by heat-processing. The concentrations of less-polar ginsenosides (20(S)-Rg3, 20(R)-Rg3, Rg5, and Rk1) and maltol in P. ginseng were significantly increased in a heat-processing temperature-dependent manner. Based on researches in animal models of diabetes, ginsenoside 20(S)-Rg3 and maltol were evaluated to have therapeutic potential against diabetic renal damage. These effects were achieved through the inhibition of inflammatory pathway activated by oxidative stress and advanced glycation endproducts. These findings indicate that ginsenoside 20(S)-Rg3 and maltol are important bioactive constituents of heat-processed ginseng in the control of pathological conditions associated with diabetic nephropathy.
BACKGROUND/OBJECTIVES: Unregulated inflammatory responses caused by hyperglycemia may induce diabetes complications. Hesperetin, a bioflavonoid, is a glycoside in citrus fruits and is known to have antioxidant and anticarcinogenic properties. However, the effect of inflammation on the diabetic environment has not been reported to date. In this study, we investigated the effect of hesperetin on proinflammatory cytokine secretion and its underlying mechanistic regulation in THP-1 macrophages with co-treatment LPS and hyperglycemic conditions. MATERIALS/METHODS: THP-1 cells differentiated by PMA (1 µM) were cultured for 48 h in the presence or absence of hesperetin under normoglycemic (5.5 mM/L glucose) or hyperglycemic (25 mM/L glucose) conditions and then treated with LPS (100 ng/mL) for 6 h before harvesting. Inflammation-related proteins and mRNA levels were evaluated by enzyme-linked immunosorbent assay, western blot, and quantitative polymerase chain reaction analyses. RESULTS: Hesperetin (0-100 µM, 48 h) treatment did not affect cell viability. The tumor necrosis factor-α and interleukin-6 levels increased in cells co-treated with LPS under hyperglycemic conditions compared to normoglycemic conditions, and these increases were decreased by hesperetin treatment. The TLR2/4 and MyD88 activity levels increased in cells co-treated with LPS under hyperglycemic conditions compared to normoglycemic conditions; however, hesperetin treatment inhibited the TLR2/4 and MyD88 activity increases. In addition, nuclear factor-κB (NF-κB) and Acetyl-NF-κB levels increased in response to treatment with LPS under hyperglycemic conditions compared to normoglycemic conditions, but those levels were decreased when treated with hesperetin. SIRT3 and SIRT6 expressions were increased by hesperetin treatment. CONCLUSIONS: Our results suggest that hesperetin may be a potential agent for suppressing inflammation in diabetes.
