In vitro $\alpha$-Glucosidase Inhibitory Potential and Antioxidant Activity of Selected Lamiaceae Species Inhabited in Korean Penninsula

  • In the current study, inhibitory activity of 8 selected Korean edible plants of Lamiaceae family against $\alpha$-glucosidases, prepared from rat small intestine acetone powder was evaluated. Total flavonoids and oxygen radical absorbance capacity (ORAC) were also investigated. Methyl alcohol extracts of Scutellaria indica (SI) had the highest $\alpha$-glucosidase inhibitory activity relevant for potentially managing hyperglycemia, followed by Clinopodium gracile (CG) and Thymus quinquecostatus (TQ). These 3 species also showed significant antioxidant activity in ORAC system. The $\alpha$-glucosidase inhibitory activity of the extracts was compared to selected phenolics. Among the standard phenolics tested quercetin which was major flavonoid in the extracts had the highest $\alpha$-glucosidase inhibitory activity. CG, TQ, and SI which had high quercetin content and ORAC values also exhibited significant sucrase inhibitory activity. Results suggested that selected 3 Korean Lamiaceae species have the potential development of effective dietary strategy for postprandial hyperglycemia and oxidative stress-linked diabetes complications.

Antioxidant and Anti-hyperglycemic Effects of a Sanghwang Mushroom(Phellinus linteusau) Water Extract (상황버섯 열수추출물의 항산화 활성과 식후 혈당 상승 억제 효과)

  • The inhibitory activities of a water extract of Sanghwang mushroom(Phellinus linteusau)(SWE) against ${\alpha}$-glucosidases were evaluated in this study. Inhibiting these enzymes involved in the absorption of disaccharides significantly decreases the postprandial increase in blood glucose level after a mixed carbohydrate diet. Oxygen radical absorbance capacity and 1,1-diphenyl-2-picryl hydrazyl scavenging activities of the SWE were evaluated to investigate the antioxidant activity of the SWE associated with complications of long-term diabetes. Furthermore, the postprandial blood glucose lowering effect of SWE was compared to a known type 2 diabetes drug($Acarbose^{(R)}$) in a Sprague-Dawley rat model. SWE significantly reduced the blood glucose increase after sucrose loading. These results suggest that SWE, which has high ${\alpha}$-glucosidase inhibitory activity and high antioxidant activities, has the potential to contribute to a useful dietary strategy for controlling postprandial hyperglycemia.

Effects of Uncooked Korean Food on Blood Glucose and Antioxidant Fnzyme Activities of Streptozotocin-induced Diabetic Rats (당뇨환자용 식사대용식이 Streptozotocin으로 유발된 당뇨쥐의 혈당 및 항산화 효소 활성에 미치는 영향)

  • Objectives : This study has been carried out to understand the effect of Uncooked Korean Food(F-DM) on blood glucose and antioxidant enzyme activities in streptozotocin-induced Diabetic Rats. Methods : SD rats were separated into four groups(each with 20 rats). Except normal two group, the other two groups were injected into intra-peritoneal with streptozotocin 60 mg/kg. Experimental group was eated Feed with 25% F-DM for 4 weeks. The change of plasma glucose level, body weight were observed. After 4 weeks, liver and kidney weight, antioxidant enzyme activities, survival rate were observed with histological changes on liver, kidney and pancreas. Results : In experimental group, body weight and survival rate increased, plasma glucose level were decreased significantly. Liver and kidney weight, XOD activity were decreased in experimental group compared to control group. GSH-px and CAT activities, insulin- immunoreactive granules in p-cells were increased significantly in experimental group compared to control group. Conclusions : This study shows that the F-DM might be effective for treatment of diabetes and its complications, as well as reduction of the oxidative stress.

Metabolically Obese Normal Weight (MONW) (정상체중 대사비만)

  • Metabolically obese but normal weight(MONW) syndrome is characterized, with potentially increased risks for development of the insulin resistance or metabolic syndrome despite their normal body mass index(BMI) < 25 kg/m2. Such characteristics could confer upon MONW individuals a type 2 diabetes mellitus and cardiovascular diseases(CVD) risk however, research on MONW is scarce. MONW individuals have metabolic disturbances typical of obese persons and are identified by having a high amount of visceral fat, a low BMI, a high fat mass, a low lean body mass, low insulin sensitivity, and high triglyceride concentrations. The purpose of this study is to review several markers as potential modulators in individuals displaying the "MONW". Body fat appears to be functionally comparable with a dynamic endocrine organ, producing and secreting various adipocy tokines, such as leptin, adiponectin, CRP, tumor necrosis factor(TNF-), interleukin(IL)-6, all of which play an important role in the onset of cardiovascular disease, and insulin resistance. Otherwise, physical activity and a lower inflammation state might be helped to reduce the number of persons at risk of diabetes, CVD complications, or premature mortality. We should provide a method to optimal treatments resolving the emerging public health problem to prevention of MONW by providing guideline for physical activity as an optimal treatment for the MONW Korean. Furthermore we expect to develop a new strategy to manage MONW Korean in this society in terms of reducing medical costs and enhancing public health care for uprising population with MONW.

Clinical Observation on Hearing Impairment in Diabetes Mellitus Patients (당뇨병 환자의 청각장애에 대한 관찰)

  • For a year from Jan. 1976 to Dec. 1976, the authors have performed the clinical observation in accordance with audiometric test and clinical findings of 43 cases (male 25: female 18) who were diagnosed of diabetes mellitus in diabetic clinic of our hospital. The results are as follows; 1) Eight of 43 cases reveal hearing impairment, and almostly abrupt drop or descending in hearing types. 2) The incidence of hearing impairment is higher in middle and older age group. 3) Hearing impairment has not related to duration of the disease but blood sugar level and total serum cholesterol level in some extent. 4) The incidence of hearing impairment is higher in cases accompanied by complications such as retinopathy, hypertension, neuropathy etc. 5) In etiology of hearing impairment, multiple factors seem to be concerned.

Clinical Study on one Patient with Vitreous Hemorrhage Caused by Diabetic Retinopathy (당뇨망막병증으로 유발된 유리체출혈(暴盲)환자 1례에 대한 임상적 고찰)

  • Objective: To carry out the oriental medical treatment on a patient with vitreous hemorrhage in the left eye caused by diabetic retinopathy and record the results of the treatment. Methods: 1. Diagnosis: Fundus photography, Colored paper, Dr. Hahn's standard test chart for 5M, Blood sugar measurement. 2. Treatment: Acupuncture, Electro-Acupuncture, Indirect moxibustion, Western medicines, Oryoungsan(Crude drug preparations) Results: Oriental treatment using Ohaeng-acupuncture, Electro-Acupuncture, Indirect moxibustion resulted in the Unaided visual acuity of 0.1 while it used to be the left eye visual acuity with only light sense I month ago. Looking from Fundus photography result, progress was achieved and diabetic retinopathy was found to be in progress in fluorescein fundus angiography to right eye also by revisiting the patient after treatment. Conclusions: 1. Vitrectomy has many advantages but there are instances where patients do not recover their visual acuity due to complications. Therefore it is necessary to prove the effect oriental medical treatment through more cases in future. 2 For diabetic retinopathy patients, diabetes must be treated together with visual acuity.

Isolation and Structural Determination of Aldose Reductase Inhibitor from Korean Fermented Soybean Paste

  • Aldose reductase catalyzes the conversion of glucose into sorbitol. Inhibiting this enzyme in diabetes mellitus can delay or prevent pathogenic process. Aldose reductase inhibitor was screened from Korean fermented soybean pastes (Doen-jang) and purified via sequential processes of ethanol extraction, HP-20 column chromatography, ethyl acetate extraction, silica gel column chromatography, and crystallization. Aldose reductase inhibitor was identified as genistein with molecular weight of 270 Da and molecular formula of $C_{15}H_{10}O_5$ based on UV spectrometry, $^1H$ and $^{13}C\;NMRs$, and mass spectrometry. Genistein inhibited aldose reductase of pig lens with $IC_{50}$ level of $20\;{\mu}M$. Because genistein was effective against aldose reductase of animal source, it may be a potential therapeutic agent for diabetic complications.

Life-Threatening Necrotizing Fasciitis of the Posterior Neck

  • Necrotizing fasciitis is an infection of the subcutaneous tissue that results in destruction of the fascia and is disproportionately common in patients with chronic liver disease or diabetes. Necrotizing fasciitis of the head and neck is rare, but has a high fatality rate. A 50-year-old man with a past medical history of diabetes reported a chief complaint of a wound in the posterior neck due to trauma. The wound had grown and was accompanied by pus and redness, and the patient had a fever. When the patient was referred to department of plastic & reconstructive surgery, the sternocleidomastoid muscle, semispinalis capitis muscle, splenius capitis muscle, and trapezius muscles were exposed, and the size of the defect was about 25×20 cm. Dead tissue resection was performed before negative-pressure wound therapy, followed by a split-thickness skin graft (STSG). After a 2-week course of aseptic dressing post-STSG, the patient recovered completely. No postoperative complications were observed for 1 year. Necrotizing fasciitis is a life-threatening, rapidly spreading infection, requiring early diagnosis and active surgical treatment. In addition, broad-spectrum antibiotics are required due to the variety of types of causative bacteria. Broad necrotizing fasciitis of the posterior neck is rare, but can quickly progress into a life-threatening stage.

The Effects of Regular Walking Exercise on Metabolic Syndrome, Cardiovascular Risk Factors, and Depressive Symptoms in the Elderly with Diabetic Mellitus (노인 당뇨병 환자의 규칙적 걷기운동 프로그램이 대사증후군, 심혈관 위험도 및 우울정도에 미치는 효과)

  • Purpose: This study aimed to estimate the effects of a regular walking exercise program on metabolic syndrome, cardiovascular risk factors, and depressive symptoms among the elderly with diabetic mellitus (DM) based on the Theory of Reasoned Action (TRA). Methods: This study has randomized and stratified experimental design with experimental and control groups. We developed a regular walking exercise program suitable for the elderly with DM based on the guidance of AAHPERD. The experimental group participated in the regular walking exercise program, which contains walking exercise 3 times a week and 50 minutes each time for 3 months and education on controlling diet and preventing complications once a week and 20 minutes each time for 4 weeks. Post-test was conducted after 3 months to estimate metabolic syndrome, cardiovascular risk factors, and depressive symptoms. Results: The regular walking exercise program was effective for decreasing the waist size, the level of fasting blood glucose (FBG) and triglyceride (TG), cardiovascular risk factors and the severity of depressive symptoms among the elderly with DM. Conclusion: The incidence of complications would be decreased by applying a regular walking exercise program.

Body Weight, Cardiovascular Risk Factors, and Self-Efficacy of Diabetic Control among Obese Type II Diabetic Patients (비만한 당뇨환자의 체중, 심혈관계 위험요소 및 자기효능감)

  • Purpose: The purpose of this study was to examine the effects of problem solving nursing counseling and walking exerciseon weight loss, cardiovascular risk factors, and self-efficacy of diabetic control among obese diabetic patients. The Polar heart rate monitor was used for walking exercise to utilize the Biofeedback mechanism. Method: Fifty nine diabetic patients were conveniently placed into experimental (n=35) and control groups (n=24). The experimental group participated inweekly nursing counseling for 12 weeks and was encouraged to do walking exercise using a Polar monitor. The control group remained in the same treatment as before. The data wascollected from November 2003 to August 2004 and analyzed using t-tests and ANCOVAs. Results: After 12 weeks, the participants in the experimental group reported significantly decreased body weight (p=.004) and total scores on the Parma scale (p=.00l). While the participants in the control group reported significantly increased levels of blood triglyceride (p=.046) and HDL (p=.018). Conclusion: Based on the findings, we concluded that problem focused nursing counseling with intensified walking exercise could reduce the risk of cardiovascular complications and body weight among obese diabetic patients. Future research to explore the long-term effects of nursing counseling on diabetic complications is warranted.
