The Relationships among Stress Perception, Locus of Control and Depressive Symptom of The Patients with Psychological Pruritis and Chronic Urticaria (심인성 소양증환자와 만성 담마진환자의 스트레스지각과 통제소재 및 우울증상과의 관계)

  • For investigating a disease specific psychological mechanism in various dermatological disorders, we tried to explore the difference in correlation among stress perception, locus of control as a coping strategy, depressive symptoms and pruritic symptoms in the patients with psychological pruritis and chronic urticaria. The subjects were composed of 32 patients with psychol-ogical pruritis and 67 patients with chronic urticaria(subject group), and 25 patients with organic pruritis and 59 patient with major depression(control group). Global assesment of recent stress scale(GARS), I-E locus of control scale, Beck Depression Inventory(BDI) and medical college of wisconsin center pain follow up questionnaire were used for assesement. The results were as follows: 1) There were a significant positive correlation between stress perception and depressive symptom in patients with psychological pruritis, chronic urticaria, and major depression but not hi organic pruritis. 2) In relationship between locus of control and depressive symptom, patients with psychological pruritis, organic pruritis, and major depression except chronic ruticaria showed a significant negative correlation. 3) For intensity and pattern of pruritis, there were positive relations with depressive symptom and stress perception only in patients with chronic urticaria. Above results indicate that stress perception and locus of contorl may play a significant role in the formation of psychological and dermatological symptoms in psychological pruitis and chronic urticaria. But the mechanisms of these processes are different in either disorder.

  • This study examined the importance of the skin care source on the consumer's general characteristics (lifestyle, residential district and income). The questionnaire survey results showed that women in their twenties choose dermatologic clinics as the best source for skin care. The three-way ANOVA using three variables (lifestyle, residential district and income) showed notable differences between the dermatologic clinic, newspaper?magazine, family?friends, and internet sources. Some suggestions for a marketing strategy were made. For example, newspaper?magazine targeting high income consumers is a good means for dermatologic clinics in Seoul and the Internet is better than other sources for other industries in Daejon.

  • Postinflammatory hyperpigmentation (PIH) is a reactive hypermelanosis of the skin that occurs as a result of variable inflammatory processes, such as trauma, and many inflammatory conditions. Although a range of modalities of managing PIH have been reported, the treatment of PIH is challenging. In this report, a patient with PIH was treated using picosecond-domain Nd:YAG lasers. After three sessions of 1,064-nm picosecond-domain Nd:YAG laser treatment, the patient showed almost complete improvement with no remarkable side effects or recurrence over the duration of six months. Overall, 1,064-nm picosecond-domain Nd:YAG laser can be used effectively and safely for treating pigmented lesions in the dermis, particularly PIH, in Asian patients.

  • Mitochondria-targeted vitamin E (MVE) is designed to accumulate within mitochondria and is applied to decrease mitochondrial oxidative damage. However, the protective effects of MVE in skin cells have not been identified. We investigated the protective effect of MVE against UVB in dermal fibroblasts and immortalized human keratinocyte cell line (HaCaT). In addition, we studied the wound-healing effect of MVE in animal models. We found that MVE increased the proliferation and survival of fibroblasts at low concentration (i.e., nM ranges). In addition, MVE increased collagen production and downregulated matrix metalloproteinase1. MVE also increased the proliferation and survival of HaCaT cells. UVB increased reactive oxygen species (ROS) production in fibroblasts and HaCaT cells, while MVE decreased ROS production at low concentration. In an animal experiment, MVE accelerated wound healing from laser-induced skin damage. These results collectively suggest that low dose MVE protects skin from UVB irradiation. Therefore, MVE can be developed as a cosmetic raw material.

  • The aesthetics of current dermatology clinics are the main space of dermatology, which is gradually becoming an area of specialty. Paths requiring spatial transition due to the expansion of treatment field in dermatology clinics are currently emerging, meaning that the duty to provide medical and aesthetic environment with high spatial connection must be accompanied. Contrary to past clinics where only functional aspects were emphasized, current clinics require a differentiated environment that considers both aspects of function and aesthetics, centered on the patient. The purpose of this research in this perspective is to study the efficient connection between dermatology and aesthetics, while also analyzing identity factors to indicate design factors differentiated from other functional spaces, to apply them as preliminary data for the planning of dermatology clinics. Based on the above, six dermatology clinics located in Seoul were designated for case studies through field studies. Overall, Aesthetics connected to dermatology clinics legally must have separate business registrations and have alternative entrance ways. Currently however, there were many cases where entrance, waiting, reception and receipt were not separated. there were efforts to partially display identities when analyzing design factors that formed the identity of dermatology clinics. However, there were insufficient cases where visual communication factors such as a clinic's spatial identity, logo, signing system, and applied products were integrated into a coherent theme. At this point when dermatology clinics are becoming brands, all fields must merge for integrated identity effects that go beyond the boundaries of contemporary H.I, to clearly display their identity with the clinics' professional image and consistent concept.

A Case of Non-small Cell Lung Cancer Patient Whose Dermatologic Adverse Reactions Associated with the Epidermal Growth Factor Receptor-inhibitors were Relieved with the Treatment of a Herbal medicine, Samultang-gagambang (비소세포폐암 환자의 표피성장인자 수용체 억제제 치료와 관련된 피부의 이상반응이 사물탕 가감방 투여로 호전된 1례)

  • Objectives : Epidermal growth factor receptor-inhibitors have demonstrated improved overall survival in patients with non-small cell lung cancer, but their use is associated with dermatologic adverse reactions that often require symptomatic treatment. Methods : A 44-year-old woman, who started the chemotherapy of Iressa$^{(R)}$ on August 2010, developed cutaneous symptoms such as papulopustular rash, dry skin, and pruritus on her face and scalp after taking Iressa$^{(R)}$ for four weeks. The patient visited our clinic with such symptoms on March 2011 and underwent herebal remedy targeted to alleviate the skin reactions. The severity of dermatologic symptoms was evaluated with the numeric rating scale and the Common Terminology Criteria for Adverse Events version 4.0. Results : Noticeable changes on the skin lesion were observed after the two months of treatment, without any dose modification of the Iressa$^{(R)}$. The cutaneous symptoms as papulopustular rash, dry skin and pruritus were improved and there was no adverse event induced by the treatment with herbal medicine. Conclusions : This case report suggests that the treatment with a herbal medicine, Samultang-gagambang be considered as a useful treatment to relieve EGFR-inhibitor induced dermatologic adverse reactions.

A Case Report of Contact Dermatitis treated by Mahwang-tang based on Shanghanlun Provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 마황탕(麻黃湯) 투여 후 호전된 접촉성피부염 1례)

  • Objective : The purpose of this paper is to report the improved contact dermatitis(CD) treated with the herbal medication based on Shanghanlun disease pattern identification diagnostic system. The level V steroid ointments were no use to the patient 3weeks before the treatment. Methods : In this case, the patient was diagnosed according to 'Disease Pattern Identification Diagnostic System based on Shanghanlun Provisions' and took Mahwang-tang for 36days since the final diagnosis was Taeyang-byung, number 46 provision. The photographs of patient's hands were taken to observe the process of the skin status and the discomfort from CD was measured with DLQI(Dermatologic Life Quality Index) and NRS(Numeric Rating Scale). Results : The DLQI score changed from 24 to 7 and NRS of itching changed form 8 to under 4. Conclusions : This case report shows that the herbal medication through the diagnostic system could be a remedy on CD, dealing with the skin status with no use of the level V steroid ointments. It is needed proving an alternative possibility of Korean Medicine, on other skin diseases, with later studies since there are many reports on the side-effects after some long-term steroid treatments.

Health Status of Migrant Workers in Korea (한국 이주노동자의 건강관련 특성)

  • Purpose: The purpose of the study is to investigate health problems of migrant workers in Korea. Methods: The data were collected from the records of the medical history of 2,233 migrant workers who visited the community health clinic in Ansan city, Gyeonggi province from Jan. 2005 to Jan. 2007. The data were analyzed by SPSS. Results: Men accounted for 56.4% of the total who participated in the study. Most of them were Chinese, Russian, and Bangladesh people. The 590 people out of all the migrant workers were employed in manufacturing industries and they comprised the largest percentage. The most commonly complained health problems were musculoskeletal (31.7%), digestive (20.7%), respiratory (12.5%), dermatologic (6.5%), cardiovascular (5.5%) and dental disorder (5.2%). The 1,853 (36.0%) workers were assumed to have hypertensive disorder and 1310 (11.1%) workers, some diseases due to high cholesterol level. Conclusions: It is necessary to establish a public health care system to improve health and welfare of migrant workers. Furthermore, the communities need to draw their attention to this matter. And it is also necessary to make accurate research on their health and medical service.

  • Kaposi's sarcoma (KS) is a cancer that causes patches of abnormal tissue to grow under the skin. It also occurs in the immunosuppressive population. KS is currently believed to be caused by infection with human herpes virus-8 (HHV-8) in non-human immunodeficiency virus patient. A 79-year-old female visited the outpatient clinic presenting with increasing number and size of palpable masses on both upper and lower extremities. She was first diagnosed as drug-erupted dermatitis and stopped her medications, but the symptoms got worse. We did partial biopsy, and KS with HHV-8 was diagnosed histopathologically. She planned to undergo further evaluations and proper treatments. This rare case suggests the need to consider a classic type of KS in the differential diagnosis of specific dermatologic symptoms such as macular, nodular, and darkish patches of upper or lower extremities in elderly patients. It is believed that this case helps to strengthen awareness of this rare disease.

Antimelanogenic effect of ginsenoside Rg3 through extracellular signal-regulated kinase-mediated inhibition of microphthalmia-associated transcription factor

  • Background: Panax ginseng has been used to prolong longevity and is believed to be useful for improving skin complexion. Ginsenosides are the most active components isolated from ginseng, and ginsenoside Rg3 (G-Rg3) in particular has been demonstrated to possess antioxidative, antitumorigenic, and anti-inflammatory properties. The aim of this study was to examine the ability of G-Rg3 to inhibit melanogenesis. Methods: The effects of G-Rg3 on melanin contents and the protein levels of tyrosinase, microphthalmia-associated transcription factor (MITF), and tyrosinase-related protein 1 (TRP1) were evaluated. Melanogenesis-regulating signaling molecules such as Akt and extracellular signal-regulated kinase (ERK) were also examined to explore G-Rg3-induced antimelanogenic mechanisms. Results: G-Rg3 was found to significantly inhibit the synthesis of melanin in normal human epidermal melanocytes and B16F10 cells in a dose-dependent manner. The activity of cellular tyrosinase and the expression of MITF, tyrosinase, and TRP1 were all reduced, whereas ERK was strongly activated. PD98059 (a specific inhibitor of ERK) attenuated the G-Rg3-induced inhibition of melanin synthesis and tyrosinase activity. Conclusion: Taken together, these results showed that G-Rg3 induces the activation of ERK, which accounts for its antimelanogenic effects. G-Rg3 may be a promising safe skin-whitening agent, adding to the long list of uses of P. ginseng for the enhancement of skin beauty.
