치성 낭종으로부터 유래된 원발성 골내 편평상피세포 암종: 증례보고 (Primary Intraosseous Squamous Cell Carcinoma Arising from Odontogenic Cyst: A Case Report)

  • Primary intraosseous squamous cell carcinoma (PIOSCC) is a rare form arising within the jaws. PIOSCC is not related to the oral mucosa, presumably developing from remnants of the odontogenic epithelium. Because odontogenic cyst epithelium often transforms malignantly into PIOSCC, it could be misdiagnosed as odontogenic cyst based on a relatively ill-defined radiolucent lesion. Therefore, definite diagnosis is established from histological examination of biopsy samples taken during cyst enucleation in many cases. The present study is reported with a case of patient complaining of discomfort on his mandible. He was diagnosed as a putative dentigerous cyst and underwent a cyst enucleation treatment. After definite diagnosis as PIOSCC was established based on histologic findings, partial mandible resection and mandible reconstruction were performed. Up to the present, 10 months follow up of the patient showed satisfactory healing without recurrence and abnormal findings; thereby, we are reporting this case with literature review.

Risk factors for postoperative infection of odontogenic cysts associated with mandibular third molar

  • Background: Odontogenic cysts associated with lower third molar are common. The prognosis for surgical treatment is relatively good. However, postoperative infection discourages the clinicians. Hence, we would like to investigate the factors associated with infection after surgical treatment of cysts associated with the mandibular third molar. Methods: We retrospectively reviewed the medical and radiographic records of 81 patients who were diagnosed with dentigerous cyst or odontogenic keratocyst and underwent cyst enucleation. The factors affecting postoperative infection were divided into host factor, treatment factor, and cystic lesion factor. To identify the factors associated with postoperative infection, we attempted to find out the variables with significant differences between the groups with and without infection. Results: A total of 81 patients (64 male and 17 female) were enrolled in this study. There was no statistical relationship about the postoperative infection between all variables (gender, smoking, diabetes mellitus, age, bone grafting, related tooth extraction, previous marsupialization or decompression, type of antibiotics, cortical perforation associated with cystic lesion, preoperative infection, preoperative cyst size). Conclusions: The results of this study suggest that it is not necessary to avoid bone grafts that are concerned about postoperative infection.

Management of rare ectopic teeth eruption: case series

  • Objectives: An ectopic tooth is a rare eruption of a tooth out of the normal dental apparatus and occurs commonly with the third molar. Thus, in this study, we reported a case series of ectopic teeth in rare jaw locations and highlight the associated pathology and our experience in the surgical management. Patients and Methods: All cases of ectopic tooth managed at the Department of Oral and Maxillofacial Surgery, University of Maiduguri Teaching Hospital from January 2011 to December 2020 were reviewed. The information retrieved includes biodata, location of the ectopic tooth, signs, symptoms, type of tooth and associated pathology, surgical approach and complications. Results: Ten cases of ectopic teeth were identified over the study period. This comprised 80.0% males with a mean age was 23.3 years. The antrum and lower border of the mandible accounted for 50.0% and 40.0% of the ectopic locations, respectively. Dentigerous cyst was the most associated pathology (70%) and usually presented with pain and swelling. Surgical intervention predominantly via the intraoral route was performed if indicated. Conclusion: Ectopic teeth are rare and not always associated with pathology. A high index of suspicion and radiological investigation are necessary for diagnosis. A more extensive multi-center study is however recommended to determine the prevalence of ectopic teeth other than the third molar.

함치성낭종에 대한 방사선학적 연구 (A RADIOLOGIC STUDY OF DENTIGEROUS CYSTS)

  • The purpose of the present study is to offer the fundamental materials for the diagnosis of dentigerous cyst by analysis of the clinical and radiographic findings of these lesions. The subject for this study consisted 118 patients admitted to the Department of Oral Radiology, Kyung Hee University and Seoul National University from 1974 to 1983. Conclusions from this study were as follows: 1. There was a higher incidence in males (70.34%) than in females (29.66%), and the incidence was highest in the second and third decade (26.27%) with the same incidence. (Average age: 25 year) 2. There was a higher incidence in the maxillary teeth (66.10%) than in the mandibular teeth (33.90%) and the maxillary supernumerary tooth (45.76%) was the most frequently involved one. The mandibular third molar (16.10%) was next in order of frequency of involvement followed by maxillary canine (7.63%), mandibular second premolar (5.08%). 3. There was a greater incidence of the central type than the lateral type in the maxilla, while a higher incidence of the lateral type in the mandible. 4. There was a greater incidence of the smooth type (84.26%) than that of the scalloped type (15.74%) and a higher incidence of minor type (70.91%) than the major type (29.09%) in the displacement of associated tooth. 5. Root apices of teeth encompassed or approached by the cysts showed variable resorption in 68.47% of cases.

Expression of cytokeratin 10, 16 and 17 as biomarkers differentiating odontogenic keratocysts from dentigerous cysts

  • Objectives: Odontogenic keratocysts (OKCs) have a tendency to recur and possess an aggressive nature. the aim of the present study was to evaluate cytokeratin (CK) expression patterns as a method for the differentiation between dentigerous cysts (DCs) and OKCs, as their histomorphologic appearance are often indistinguishable. Materials and Methods: Formalin-fixed, paraffin-embedded tissue sections of 43 OKCs and 38 DCs were immunohistochemically analyzed with i-solution in a quantitative manner in order to evaluate the immunoreactivity of CK 10, 16 and 17. Results: CK 10 expression was evident in 79.1% of OKCs but found in only 18.4% of DCs (P<0.05), and CK 10 expression was observed to occur more frequently in OKCs (mean 25.45%) than in DCs (2.19%) (P<0.05). The expression of CK 16 was evident in 79.1% of OKCs but found in only 7.9% of the DCs (P<0.05) and CK 16 expression was observed to occur more frequently in OKCs (mean 4.33%) than in the DCs (0.61%) (P<0.05). The expression of CK 17 was evident in 88.4% of OKCs but seen in only 15.7% of the DCs (P<0.05) and CK 17 expression was observed to occur more frequently in OKCs (mean 31.11%) than in the DCs (2.37%) (P<0.05). Conclusion: The immunohistochemical detection of CK 10, 16 and 17 can be utilized as a valuable biomarker for use in distinguishing between OKCs and DCs, which have clinically significant differential diagnoses.

상악 전치부에 발생한 함치성 낭종의 임상적 특징과 치료에 대한 후향적 연구 (Retrospective Study of Characteristics and Treatment of Dentigerous Cysts Involving the Maxillary Anterior Tooth)

  • 본 연구는 상악 전치부에 이환된 함치성 낭종의 다수의 증례 분석을 통해 임상적 특징 및 치료 결과에 대해 알아보고자 하였다. 상악 전치부에서 함치성 낭종으로 진단 받은 55명의 환아 중에서 절치부에서 33증례, 견치부에서 22증례를 분석하였다. 그 결과 절치부에서는 남자가 7.2배 더 높았고 종창을 주소로 내원한 경우가 많았으며, 견치부는 여자가 1.7배 더 높았고 정기검진에서 발견된 경우가 더 많았다. 이환된 낭의 영향으로 92.7%에서 치아 변위가 관찰되었고 치료는 41증례에서 조대술이, 14증례에서 적출술이 시행되었다. 낭종 처치 후 자발적 맹출은 견치부보다 절치부에서 더 높았다. 하지만 자발적 맹출 이후 절치부에서는 부정교합으로 인해 교정치료가 시행된 경우가 많았다. 본 연구를 통해서 상악 전치부에 발생한 함치성 낭종의 분포 및 특성을 알 수 있었고, 치료 결과를 제공함으로써 향후 치료계획을 세우는데 도움이 될 수 있으리라 생각된다.

치성낭에서 c-erbB-2 종양 단백의 발현에 관한 면역조직화학적 연구 (IMMUNOHISTOCHEMICAL STUDY ON THE EXPRESSION OF c-erbB-2 ONCOPROTEIN IN THE ODONTOGENIC CYSTS)

  • To investigate epithelial cell proliferation reactivity in the odontogenic cysts, the expression of c-erbB-2 oncoprotein by epithelial lining was studied in odontogenic keratocyst(OKC, n=10), dentigerous cyst(DC, n=12), radicular cyst(RC, n=12) and normal dental follicle(n=7). The c-erbB-2 immunoreactivity was studied using a streptavidine- biotin- peroxidase method with polyclonal rabbit antihuman antibody to c-erbB-2 oncoprotein which is known to react with formalin fixed, paraffin-embedded sections and the intensity of staining was determined by manually. In all of 10(100%) OKCs, showed positive expression for c-erbB-2 oncoprotein compared with 10/12(83.3%) in DCs, 11/12(91.7%) in RCs and 5/7(71.4%) in normal dental follicles. The expression within OKC was higher than that of DC, RC and dental follicle but statistically not significant(p>0.05) and but may reflects underlying genetic defect. These results demonstrate differences in c-erbB-2 expression between the epithelial linings of the three major odontogenic cyst types, indicating differences in proliferation activity and differentiation processes within these lesions. And, in particular, these results are able to explain the peculiar aggressive growth pattern of OKC.

낭성법랑모세포종, 함치성낭, 치성각화낭의 방사선소견과 Ki-67, PCNA, Cytokeratin 발현과의 연관성에 관한 연구 (Relation of the radiologic findings and labeling index of Ki-67, PCNA and cytokeratin in unicystic ameloblastoma, dentigerous cyst and odontogenic keratocyst)

  • Purpose: To compare the proliferation potential of the epithelial cells between unicystic ameloblastoma (UA), dentigerous cyst (DC), and odontogenic keratocyst (OKC) and to correlate this proliferation potential with the radiographic features of these three pathoses. Materials and Methods: Immunohistochemical expression of PCNA, Ki-67, and cytokeratin as a proliferation marker were assessed for 15 cases of UA, 15 cases of DC, and 15 cases of OKC. The degree of immunochemical expression of three proliferation markers were correlated with the radiographic features, especially cortical expansion (negative and positive) and shape of border (scalloped and round). Results: Using PCNA and Ki-67, OKC showed the highest proliferation potential and UA the lowest. Statistically significant differences were found between the OKC and the UA (p < 0.05). However, no statistically significant difference was present according to the radiographic features in all pathoses. Using cytokeratin, there was no significant differences of proliferation potential among three pathoses. Conclusions : OKC epithelium has the most intense proliferation potential, followed by the dentigeous cyst and then unicystic ameloblastoma. There is no significant relation between the radiographic features and the proliferation potential of epithelium of these three pathoses.

  • Multiple jaw cysts are one of the most constant features of the basal cell nevus syndrome. Basal cell nevus syndrome is inherited as an autosomal dominant trait with variable expressiveness. This syndrome comprises a number of abnormalities such as multiple nevoid basal cell carcinomas of the skin, skeletal abnormalities as bifid rib and fusion of vertebrae, central nervous system abnormalities as mental retardation, eye abnormalities with multiple jaw cysts. The odontogenic keratocysts in patients with this syndrome are often associated with the crowns of unerupted teeth and huge size; on radiographs they may mimic dentigerous cysts. The most important feature of the cyst is its extraordinary recurrence rate. Since recurrence may be long delayed in this lesion, follow-up of any case of odontogenic keratocyst with roentgenograms and clinical examination of basal cell carcinoma are essential for at least five years after surgery. We report the result of 7-year follow up after cyst enucleation associated with basal cell nevus syndrome with the literature of review.

하악골에 발생한 점액성 유상피암종의 치험1례 (MUCOEPIDERMOID CARCINOMA IN THE MANDIBLE : REVIEW OF A CASE)

  • Mucoepidermoid carcinoma is a common salivary gland tumor. It comprised 8% of all salivary gland tumor and originated mainly in parotid gland. Central mucoepidermoid carcinoma is rare. It comprised $2{\sim}3%$ of all mucoepidermoid carcinoma, but it occurs in the mandible two or three times more frequently than in the maxilla. Central Mucoepidermoid carcinoma are frequently associated with an odontogenic cyst, such as dentigerous cyst, in which mucous goblet cell would have neoplastic transformation. In May 2002, a 25 year-old male visits in our clinic, presented with a progressive facial swelling after surgical tooth extraction of left mandibular third molar at 1999 in the army. After incisional biopsy, the lesion was confirmed as mucoepidermoid carcinoma so we performed tumor resection and reconstruction surgery of mandible.
