최근 치과용 지르코니아 CAD/CAM 수복물의 임상적 고찰 (A clinical consideration of current dental zirconia CAD/CAM restorations)

  • Currently there is no dental ceramic material can be used in all dental situations need to be restored. However, in view of recent clinical reports, the most viable alternative is zirconia ceramic. Clinical success of dental zirconia restorations strongly depends on proper selection of materials, accurate laboratory procedure and final cementation, which can be achievable with the correct understanding of zirconia. As dental materials, zirconia ceramics have a very bright future, because they are being used increasingly in the anterior region as implant fixtures, as well as crown and bridge restorations and implant abutments. Many dental ceramics showing poor clinical performance have been gone from the dental market. However, in terms of outstanding mechanical properties and esthetic nature, new dental materials can replace zirconia ceramics will not be available in the foreseeable future.

장애인 치과 치료시 발생한 이물질 흡인 (Foreign Body Aspiration during Dental Treatment in the Disabled Patient)

  • There are a lot of dental emergency situation. Especially on disabled or pediatric patient with negative behavior, accidental aspiration or ingestion of foreign body (ex: dental instrument) is one of the common accidents in dental procedure. Dentists and dental hygienists must have knowledge about the precaution and be ready to deal with foreign body aspiration during dental treatment, especially on the disabled. This is a case of an accident during the dental treatment of 14-year-old female patient with cerebral palsy. During scaling, prolonged retained primary tooth which had resorbed roots was fell into left bronchi. So we tried to remove the crown by endotracheal approach. Most ingested foreign bodies pass through the gastrointestinal tract spontaneously, but some of them need endoscopic or surgical removal. Moreover aspiration into broncho-trachea can be more serious events and must be treated as emergency situation.

Considerations for Invasive Dental Treatment in Disabled Patients

  • Dental treatment for the disabled should be a customized that considers the characteristics and degree of cooperation of each disabled patient. There are additional considerations during implant treatment and tooth extraction in disabled patients. Since some brain lesion disorder or cardiac disease patients may be taking antiplatelet or anticoagulant medications, it is necessary to evaluate whether these medications should be discontinued before an invasive procedure. Precautions should be taken for patients with heart valve disease considering the risk of infective endocarditis, especially during invasive dental procedures. Moreover, disabled patients may have difficulty in following instructions and cautions. There are specific considerations for each stage of implant treatment in disabled patients. In the case of patients who are in the pre- or post-transplant state, it is necessary to assess their general condition and oral disease due to the risk of infection. Since disabled patients with various systemic diseases may visit the dental clinic, it is important to understand their characteristics and treatment process in order to flexibly adjust the dental treatment plan accordingly.

Nanomechanical properties and wear resistance of dental restorative materials

  • The effects of thermocycling procedure and material shade on the mechanical properties and wear resistance of resin-based dental restorative materials are investigated. The modulus of elasticity, hardness, plasticity index and wear resistance are determined for the conventional composite, the nanohybrid composite and the nanofilled dental composites. Disc-shape samples are prepared from each material to investigate the effects of thermocycling procedure on the mechanical properties and wear resistance of different types of dental restorative materials. In this respect, a group of samples is thermocycled and the other group is stored in ambient conditions. Then nano-indentation and nano-scratch tests are performed on the samples to measure their mechanical properties and wear resistance. Results show that the A1E shade of the dental nanocomposite possesses higher modulus of elasticity and hardness values compared to the two other shades. According to the experimental results, the mean values for the modulus of elasticity and hardness of the A1E shade of the nanocomposite are 13.71 GPa and 1.08 GPa, respectively. The modulus of elasticity and hardness of the conventional dental composite increase around 30 percent in the oral environment due to the moisture and temperature changes. The wear resistance of the dental composites is also significantly affected by moisture and temperature changes in the oral conditions. It is observed that thermocycling has no significant effect on the hardness, plasticity index and wear resistance of the nanohybrid composite and the nanocomposite dental materials.

금관 수복치료 후 발생한 좌측 기관지 내로의 금관 흡인에 관한 증례보고 (The Aspiration of Foreign Body in the Left Tracheobronchial Tree during Gold Crown Restoration -A Case Report-)

  • Foreign body aspiration (FBA) into the tracheobronchial tree could be life threatening requiringprompt intervention. Any objects placed in the oral cavity put patients at a risk of aspirating or swallowing the objects slipped or broken by physical injuries. Here, we report a case of 30 yr old patient with FBA during gold crown replacement was successfully treated with the use of the flexible bronchoscope. Case: A 30 yr old woman was admitted to Seoul National Dental Hospitalfor an amalgam restoration. She was scheduled to gold crown restoration for replacement of the damaged amalgam at #37 site. After performing crown lengthening procedure, the aspiration of gold crown occurred during the cementation of the crown. After aspiration, the patients complained of the subjective distress of respiration. Chest radiograph revealed that gold crown was enlodged to the left bronchus. Flexible fiberoptics was inserted to the bronchus to remove the aspirated crown. Fiberoptic assisted removal of the aspiratedcrown was successfully performed. After removal, there was no radiopaque material in the left bronchus on follow-up chest radiograph. Discussion: When aspiration of dental materials occurs, flexible fiberoptic can be used in the treatment of FBA. It is also very useful to take preventive management such as rubber dam, application of dental floss in dental procedure where there is high likelihood of FBA.

Comparison of anxiety and pain perceived with conventional and computerized local anesthesia delivery systems for different stages of anesthesia delivery in maxillary and mandibular nerve blocks

  • Background: Fear of local anesthesia (LA) is a significant impediment to dental care as many patients delay or avoid treatment to avert pain. Computer-controlled local anesthetic delivery system (CCLAD), with constant and controlled rate of flow, present a painless alternative. The present study aimed to compare anxiety and pain perceived with conventional and computerized systems, for different stages of anesthesia delivery when administering various nerve blocks. Methods: One hundred patients requiring bilateral LA participated in the study. One side was anesthetized using one system and the contralateral side was anesthetized using the other, in two separate appointments. Patients assigned anxiety scores on a 5-point scale and used the visual analogue scale (VAS) for pain determination at needle insertion, during delivery of anesthetic solution, immediately after injection, and at the end of the periodontal procedure. Each patient's preference for the delivery system of future injections was also recorded. Results: Patients reported significantly lower anxiety levels with CCLAD compared to the syringe. Significantly lower mean VAS scores for anesthesia deposition, pain immediately after, and at the end of the periodontal procedure were also noted. However, pain at needle insertion was comparable between the two systems, with no statistical significance. Overall, 64.4% patients preferred CCLAD for future anesthesia. Conclusion: Lower pain perceived with CCLAD and higher preference for the system suggest that CCLAD should replace conventional syringes to allow pain-free dental treatment.

혈압 상승으로 인한 의식하 진정 요법의 실패 -증례보고- (Failure of Intravenous Sedation due to Significant Hypertension -A Case Report-)

  • Sedation is often indicated for the relief of anxiety for outpatient oral surgery. In combination with local anesthesia, it is safe and effective method of treatment. However, it is not always effective in allowing the physician to complete the planned oral surgery procedure. On occasion, a procedure is left unfinished due to patient combativeness and discomfort and hypertension in spite of increase in sedative doses. Episodic increases in blood pressure were most commonly caused by light anesthesia or sedation and by the patient's experience of pain during treatment. Female patient was 42 years old. blood pressure is 150/90 mmHg. Extraction and implant surgery was done under IV sedation. During seadtion, her blood pressure was increased (200/100 mmHg). Surgery was stopped. She was done monitoring blood pressure. The blood pressure was decreased to 130/90 mmHg. Sedation was failed due to significant hypertension. Blood pressure is seldom increased during sedation but we should evaluate the patient's medical history and know guideline for hypertension crisis.

Original Article 2 - 의도적 재식술에 관한 임상적 고찰 (Clinical evaluation of Intentional replantation)

  • Although non-surgical endodontic procedures have high success rates, failures do occur, These can be managed by root canal re-treatment or surgical intervention. Intentional replantation is an accepted endodontic treatment procedure in which a tooth is extracted and treated outside the oral cavity and then inserted into its socket to correct an obvious radiographic or clinical endodontic failure. Intentional replantation is indicated when other endodontic treatments performed to maintain the tooth have failed, or when endodontic periradicular surgery is not feasible. Intentional replantation may be particularly useful in these cases because these difficult to access areas can be maximally treated while the tooth is out of the mouth without damaging the periodontal attachment in adjacent teeth. In conclusion, intentional replantation is a reliable and even predictable procedure, and should be considered more often as a treatment modality in our efforts to maintain the natural dentition.

의식하진정법을 적용한 성인 환자의 치과공포 및 불안의 변화 (Changes of dental anxiety and fear in adult patients applying conscious sedation)

  • Objectives: The purpose of this study was to determine the relationship between conscious sedation, a moderate sedation method used to reduce dental fear, and anxiety. Methods: The previously developed modified dental anxiety investigation scale (MDAS) and dental fear investigation scale (DFS) were investigated using a self-administered survey for adults aged 19 to 65 years old who visited a dental clinic located in Daegu City from April to November 2020. Those who met the ASA Recommendation Level 1 to 2 were the subjects of this study. The participants were evaluated using the sedation severity rating scale (OAA/S) during the implementation of conscious sedation. A lot of 106 valid questionnaires were analyzed using SPSS. Results: Dental fear and anxiety decreased significantly after dental care with conscious sedation. Dental fear after dental care accompanied by conscious sedation was statistically significant in females, those under the age of 40, those with a bachelor's degree or higher, and those who weighed 60 kg or less. Conclusions: Dental care using conscious sedation was found to have an effect on the reduction of dental fear after the dental procedure. Dental fear can occur in people of all ages, and, regardless of the individual's general and physical characteristics, conscious calming is considered an effective intervention for dental fear in all adults.

Miniscrews versus surgical archwires for intermaxillary fixation in adults after orthognathic surgery

  • Objective: We compared the skeletal and dental changes that resulted from the use of two methods of intermaxillary fixation (IMF)-miniscrews and surgical archwire-in 74 adult patients who had Class III malocclusion and were treated with the same orthognathic surgical procedure at a hospital in Korea. Methods: All the patients underwent Le Fort I osteotomy and bilateral sagittal split ramus osteotomy with rigid fixation. They were divided into two groups according to the type of IMF used-group 1 underwent surgical archwire fixation and group 2 underwent orthodontic miniscrew fixation. In a series of cephalograms for each patient, we compared vertical and horizontal tooth-position measurements: (a) immediately after surgery ($T_0$), (b) 3 months after surgery ($T_1$), and (c) 6 months after surgery ($T_2$). Cephalometric changes within each group were examined using one-way analysis of variance (ANOVA) while the independent samples t -test procedure was used to compare the two groups. Results: After surgery, the maxillary incisors tended to be proclined in both groups although there were no significant differences. Incisor overbite increased significantly in both groups from $T_0$ to $T_1$, and the miniscrew group (group 2) showed slightly greater overbite than the archwire group (group 1). Conclusions: This study suggest that the use of orthodontic miniscrews and orthodontic surgical archwire for IMF in adult patients results in similar skeletal and dental changes.
