구강건강신념이 구강병 예방 및 삶의 질에 미치는 영향 (The Effects of Oral Health Belief on Oral Disease Prevention and Quality of Life)

  • Objectives : This study analyzed the influencing relationship among oral health belief, prevention of oral diseases, and quality of life subject in adults over 20 years of age. Methods : This study enrolled 237 participants who visited Y hospital located in the Gyeongnam district as the final subjects for the analysis. From the collected data, multiple regression analysis was conducted to identify the relative influence of oral health belief on each parameter using SPSS 18.0. Results : Based on the regression analysis results, it was found that the factor in oral health belief with the strongest influence on the intention to prevent oral disease was importance. The behavior for prevention of oral disease was influenced by sensitivity, and the quality of life was affected by self-efficacy. Conclusions : These results, suggest that in order to promote oral health in adults, continuous support from oral health professionals is required to make patients recognize the importance and the sensitivity of oral health and reduce sensitivity.

A 2-year clinical evaluation of Sculpture crowns

  • Statement of problems. There are only a few studies available that deal with the clinical behavior of ceromer systems as potential substitutes for metal-ceramic crowns. Purpose. This prospective study was initiated to evaluate the clinical performance of 35 Sculpture crowns after 2 years in service. Material and methods. Thirty five Sculpture crowns were placed for 20 patients (7 men and 13 women). All patients were treated by the same dentist, and all restorations were fabricated by the same dental laboratory. Crown placement involved both the anterior and posterior regions of the dental arches. Patients were evaluated by two examiner at baseline, 12, and 24 months using the CDA quality assessment system in addition to periodontal criteria. Results. Of 34 crowns remaining in the study after 2 years, only one crown had experienced a marginal fracture. The crown was replaced as a result of recurrent caries. All remaining crowns were ranked as either excellent or acceptable for surface and color, anatomic form, and marginal integrity. Conclusion. The 2-year clinical observations and ranking with the CDA quality assessment criteria supported the conclusion that Sculpture crowns may be used in substitutes for metal-ceramic crowns.

치열 교정환자의 치과 의료서비스 만족도에 관한 연구 (Satisfaction with Dental Care in Orthodontic Patients)

  • 사회, 경제적으로 발전하면서 환자들이 치과를 내원하는 동기가 다양해졌다. 부정교합으로 인한 안모의 개선과 저작 기능을 개선하고자 치과를 찾는 인구가 점차 늘어나고 있으며, 교정치료에 대한 관심의 증가와 더불어 수요도 증가하고 있어서 교정환자들의 치과 의료서비스의 만족도를 측정하여 만족도에 영향을 미치는 요인들을 파악하여 의료 환경변화에 대한 치과경영대책 차원에서 치과 의료기관의 수익성과 의료의 질적 수준을 높이기 위한 방안을 제시하고자 2005년 3월 7일부터 3월 26일까지 20일간 편의 표본을 추출하여 설문조사를 실시하여 다음과 같은 결과를 얻었다. 1. 치과 의료서비스 만족도를 구성요인별로 비교한 결과 교정환자에서 치위생사요인 (4.21)에 대한 만족도가 높았고, 진료절차 및 대기시간요인(3.73)이 가장 낮았다. 2. 성별에 따른 교정환자와 비교정환자의 치과 의료서비스에 대한 만족도는 교정환자에서 남자가 여자보다 만족도가 높았다. 재이용의사에서는 교정환자의 남녀 모두가 비교정 환자보다 만족도가 높았다. 3. 연령에 따른 교정환자와 비교정환자의 치과 의료서비스에 대한 만족도는 재이용의사에서 교정환자의 모든 연령층이 만족도가 높았다. 4. 월수입에 따른 교정환자와 비교정환자의 치과 의료서비스에 대한 만족도는 교정환자가 치위생사요인과 재이용의사에서 고소득일수록 만족도가 높았다. 5. 학력에 따른 교정환자와 비교정환자의 치과 의료서비스 만족도를 살펴보면 치과의사 요인, 치위생사요인, 내부 환경요인, 외부환경요인에서 초등학교졸업환자의 만족도가 높은 것으로 조사되었다. 이러한 연구결과로 교정환자에서 비교정환자보다 치과 의료서비스 만족도가 높은 것을 알 수 있었다.

국내 장애인의 구강 관련에 대한 문헌고찰(2000년-2015년) (Literature review on oral health among people with disabilities in Korea (2000-2015))

  • Objectives: This study consisted of a literature review on oral health among people with disabilities, and to present the factors that should be considered in developing an oral health program for people with disabilities. Methods: The key words 'people with disabilities' and 'oral health' were searched in 4 Korean academic journals and 5 online search engines and a total of 635 papers were identified. Duplicate papers were removed, and the literature selection criteria were applied to the remaining papers. Finally, a total of 45 papers were used in the review. Results: First, people with disabilities were the most common research subjects, followed by dental students, dental hygiene students, dentists, dental hygienist and guardians and special education teachers. Second, the most frequently studied research topic was dental examination, followed by oral health behavior and behavior of using dental clinics. Third, research purposes included the status of oral health, the quality of oral health, dental treatment, the use of oral health service, and oral hygiene behavior and perception. Fourth, the most frequently discussed policy task was expansion of research subjects, followed by oral hygiene and policy development. Conclusions: To enhance the oral health of people with disabilities who have difficulty in keeping their teeth clean and healthy on their own, development of an oral hygiene training program is required not only for people with disabilities, but also for guardians and teachers. It is equally important to examine oral health behavior that could potentially affect the status of oral health and create a more accurate and systematic oral hygiene method. In addition, the government, together with various other research institutes, should conduct an oral health survey of a representative sample of people with disabilities to determine oral health status and facilitate improvements to oral hygiene programs.

임상치과위생사의 셀프리더십과 심리적 임파워먼트 및 직무만족도와의 관련성 (Relationship between Self-leadership, Psychological Empowerment and Job Satisfaction of Clinical Dental Hygienists)

  • 본 연구의 목적은 치과의료서비스에 직접적인 영향을 주는 주요 인력인 치과위생사의 셀프리더십, 심리적 임파워먼트와 직무만족도간의 관련성을 규명하여 조직 내 양질의 인적 향상 방안을 마련하고자 시행되었다. 자료수집은 2017년 5월 1일부터 6월 10일까지 대전·전라지역에 치과 병·의원에서 근무하는 임상치과위생사를 편의 표본추출 하였으며, 자기기입식 방법으로 작성하였다. 셀프리더십은 심리적 임파워먼트, 직무만족도와 정의 상관관계를 나타냈으며, 심리적 임파워먼트와 직무만족도도 정의 상관관계를 나타내었다. 심리적 임파워먼트, 치과시스템과 본인 적합성, 셀프리더십이 직무만족도에 영향을 미치는 요인으로 나타났다. 이에 임상치과위생사의 셀프리더십, 심리적 임파워먼트 및 직무만족도와 관련성이 있음이 파악되었고, 치과위생사들의 직무만족도를 향상시키기 위해 긍정적인 심리적 임파워먼트 활용과 적극적인 셀프리더십 융합 프로그램을 정립하여 올바른 치과 조직의 방향성 확보와 운영에 관심을 가질 필요가 있을 것으로 사료된다.

Factors Influencing the Use of Dental Services by Foreign Tourists in Thailand

  • This exploratory research investigated the factors that influence foreign tourists' decision to use dental services in Thailand. The questionnaire was used to collect data from foreign tourists who used dental services in a hospital or dental clinic in Thailand. Data from 233 respondents completed questionnaires were analyzed using frequency distribution, percentages, mean, and standard deviation. The findings revealed that respondents were most likely to be Asian (54.5%) followed by European (33.9%), females (64.8%) aged between 26-35 years (39.1%), business owners (45.9%), and came to Thailand with friends (47.6%). Regarding travel purposes and contact with dental services, most of them travel for pleasure (52.89%) and contact the clinic directly through the Internet(63.95%) The health/medical services used included health check-up (34.48%), dental services (29.22%), and day spa/massage (23.23%). The dental treatment that respondents had completed or planned to do included dental check-up (29.87%), tooth cleaning (26.68%), and tooth whitening (11.41%). Based on the influence of $_7P$ factors towards the respondent's decision to use the dental service, it was found that the average mean score range from 4.33-4.15 which were at the high to the very high level of influence to their decisions. The people factor had the highest average mean score (mean 4.33, SD= 0.604)followed by the price factor (mean 4.32, SD= 0.651), which both had the highest level of influence. The physical evidence factor had the lowest mean score (mean 4.15, SD+0.752) which was at the high level of importance. It should be noted that the factor concerning the modern technology and dental equipment (mean 4.33, SD=0.606) and factor on the quality of service (mean 4.43, SD= 0.598) had the highest mean score followed by the total cost of the trip including services being reasonable (mean 4.37, SD=0.582). Regarding the country factor, it was found that this factor also had highest influence level (mean 4.30, SD=0.67) and affordability had the highest mean score (mean 4.39, SD=0.614).

의료시장 개방에 대한 치과위생사의 인식과 대응방안 (Dental hygienists' perspective and coping measures towards medical market opening)

  • Objectives : This research seeks to present the data needed for the development of coping strategy, following medical market opening by identifying dental hygienists' perspective and coping measures towards the opening. Methods : One hundred eighty-eight dental hygienists were targeted to identify their level of perception towards medical market opening, attitude towards medical market opening, question of whether they agree or not with the opening and reasons, and coping measures and benefits of the medical market opening. t-test, chi-square test and cross-tabulation analysis were used for the analysis Results : First, team leaders are more aware of the medical market opening and hold greater sense of crisis towards opening compared to the rank and file. Second, the reasons cited for agreeing with the medical market opening included improvement of medical services' quality and diversification of services. As for the reasons for disagreeing, they cited the increase medical expenses paid by public. Third, limitation of the hospital management technique was cited the most when it comes to the scope of Korean hospitals' management crisis, followed by the limitations of the diagnosis procedure, limitations of the medical services, limitations of the medical techniques and increase in the number of large hospitals, in the order cited. Fourth, team leaders perceive greater need to seek coping measures from the aspect of realizing medical insurance fee from the policy development, service and system level aspects when it comes to the coping measures depending on their ranks. Conclusions : Therefore, Dental Hygienist has a comparatively low awareness of medical market opening, coping measures need to be explored to cope with the medical market opening by ensuring the dissemination of accurate knowledge through the education on the fees for dental hygienist and seminars in relation to the medical market opening.

환자기반 치과의료 환자안전에 대한 연구 (Development of patient-based patient safety questionnaire in dentistry)

  • The Primary Care Patient Measure of Safety (PC-PMOS) is a measure of safety that allows patients to evaluate contributing factors related to patient safety in primary care. This study aimed to examine which questionnaire items of the PC-PMOS could be used in Korean dental institutions. A survey of the Korean translation of the PC-PMOS, a self-reported questionnaire, was conducted in August 2022 by 400 adults who had used dental-care services within the last three years. Of the 77 items, 34 were selected using principal component analysis and two experts' evaluations based on face validity. Five domains were identified from factor analysis: patient centeredness, patient information update, complaint processing, communication, and information about the complaint process. The Cronbach's alpha of the data was 0.913, indicating high reliability. As a result of the generalized multiple regression analysis, regression coefficients were not statistically significant, except for household income. This indicated that there was no bias in the patient safety scores of dental institutions evaluated by patients within the range of independent variables used in this study. The five domains with 34 items identified in this study suggested the factors that contribute to the safety of patients who used dental care services in Korea. However, validation of this study result is still important to refine questionnaires suitable for dental institutions in Korea so as to further improve the quality of dental care.

취약계층의 재가 거주자를 대상으로 수행한 방문구강건강교육 경험에 관한 질적연구 (A qualitative study of home-visiting oral health care experience in vulnerable populations)

  • Objectives: The purpose of this study was to explore and analyze experiences with home-visiting oral health education in vulnerable populations and to provide foundational data necessary for the development of preventive strategies for oral health promotion among these patients. Methods: Using a phenomenological research, in-depth interviews were conducted between August 29 and October 31, 2023 with 20 vulnerable individuals participating in the Home Visit Oral Health Education Program under the Gwangju+ Gwangsan Integrated Care Service project spearheaded by the Gwangju Medical Welfare Cooperative. Results: After analyzing the contents of the participants' experiences, 130 meaningful words and 14 subcategories were identified and categorized into five major themes. Participants expressed discomfort when chewing or swallowing food and felt burdened by visits to the dentist. Although they experienced considerable loneliness because they spent a significant amount of time alone, they enjoyed communicating through visitation care and expressed gratitude for receiving oral health education. Conclusions: Oral health education through home visits cam increase awareness of oral health management among vulnerable populations and serve as an important means of improving their quality of life.

임상치과위생사의 감성지능이 소진에 영향을 미치는 요인 (Factors affecting clinical dental hygienist emotional intelligence on burnout)

  • 본 연구는 임상치과위생사의 감성지능이 소진에 영향을 미치는 요인을 파악하여 임상치과위생사의 감성지능이 소진에 영향을 미치는 요인을 파악하여 임상치과위생사의 인적자원의 효율적인 관리와 고객 서비스의 질을 향상시킬 수 있는 임상 치위생 조직 관리를 위한 기초자료를 제공하고자 연구하였다. 연구기간은 2017년 2월 6일부터 5월 31일까지 전라북도에 근무하는 임상치과위생사 394명을 대상으로 다음과 같은 결과를 얻었다. 임상치과위생사의 감성지능은 소진과 유의한 음의 상관관계(r=-.623, p<.001)를 나타났으며, 감성지능이 높을수록 소진은 유의하게 낮은 것으로 나타났다. 소진의 하위영역인 정서적 고갈(r=-.316, p<.001), 비인격화(r=-.474, p<.001), 성취감 저하(r=.639, p<.001)와 유의한 음의 상관관계를 나타났다. 임상치과위생사의 소진에 관련요인으로 자신의 감성이해, 타인의 감성이해, 감성활용이 영향을 미치는 것으로 나타났다. 이상의 연구결과로 감성지능이 소진에 연관성이 있음을 알 수 있었다. 임상치과위생사로 생산성을 높이기 위해서는 감성지능을 향상시킬 수 있는 중재프로그램 개발이 절실히 필요하다. 앞으로 임상치과위생사의 감성지능을 높여 소진을 감소시킬 수 있는 효율적인 전략 구축이 마련되어야 할 것으로 사료된다.
