Staphylococcus Species in the Dental and Medical Environment

  • Staphylococcus species are one of prevalent pathogens found in hospitals. Microbes that are a primary cause of nosocomial infection were isolated from a dental and medical environment it may assist the reader to explain what this is and how it differs from the 'dental health care providers and ward health care providers'. To investigate the distribution of staphylococcus species in this environment, we used vitek II to measure drug sensitivity, and further performed biochemical testing. The isolation rate of staphylococcus species from the dental and medical environment was 100% but from dental health care providers and ward health care providers were 44.4% and 33.3%, respectively. In the analyses, staphylococcus species showed resistance to diffusion of cefoxitin and oxacillin discs. These staphylococci may be sufficiently positive for the mecA gene. Our results suggest that staphylococci might be an important cause of nosocomial infection in the dental clinic.

치과의료기관의 선택요인에 관한 연구 (Study on Selection Factor in Choosing Dental Clinic)

  • This study examined and analyzed the demographical characteristics of the study target and their expectancy level on the factors in selecting dental medical institutes (basic elements of dental clinics, traffic convenience, physical environment of dental clinic and explicit qualification of dentist) with the questionnaire survey method with 222 questionnaires answered and returned (return rate 88.8%) out of 250 copies distributed to the 1st, 2nd and 3rdyear college students attending technical colleges located in Cheongwon-Gun, Chungbuk, Korea from March 26 to April 7, 2009. The results of this study are as follows: 1. The demographical characteristics of the study target group consisting of 225 college students include 92.3% of age 20s (205 students); 123 males (55.4%) and 99 females (44.6%); the number of those who had at least one experience to visit dental clinic was 199 (89.6%) and 23 (10.4%) of those with no-visiting experience. 2. The descriptive statistics for the selection factor showed that basic elements, traffic convenience, physical environment and explicit qualification of dentist were regarded important as in order. In particular, the factors considered in selecting dental clinic was ordered from most to least in their importance beginning with cleanness of dental clinic as the most important factor, dentist's trustworthiness, kindness of medical staffs, low medical fee, dentist's knowledge level, modernized medical equipments and convenience in using medical facilities as the least. On the contrary, brand name of dental clinic, age, college, sex and appearance of dentist were rated low in order in terms of their importance in selecting dental clinic. 3. As for the difference in the selection factors across demographical characteristics, while male students placed importance on traffic convenience and physical environment of dental clinic, female students placed more importance on dentist's qualification and basic elements of dental clinic. With respect to the difference in the selection factors across the visiting experiences, while those with visiting experience regard traffic convenience more importantly, those without visiting experience regarded physical environment of dental clinic, dentist's qualification and basic elements of dental clinic. 4. In terms of pearson correlation coefficient, the result showed that all aspects in selecting dental clinic were positively correlated. Particularly, the correlation between physical environment of dental clinic and dentist's explicit qualification were correlated highest. Significant positive correlations were found high in traffic convenience, physical environment of dental clinic, dentist's explicit qualification and basic elements of dental clinic in order.

  • Technological advances in contemporary medicine has discovered the causes of countless diseases and recorded a noticeable medical performance. As technology develops, the role of hospitals is expanding to include disease prevention of inpatients, on top of their fundamental role of treating diseases. Recently, hospitals are becoming more influential as they create environments to provide comfort and stability to patients. In this regard, contemporary hospitals are increasingly shifting their focus to create a patient-centric environment as well as develop into humanistic establishments. The same goes to dentists, as well. Since inpatients often have fear and frustration over treatment, hospitals should figure out the environmental factors that are more effective and relaxing for patients and design medical services to provide them. The patients' movement and spaces during their treatment were categorized by stages and collected for gazing information using eye tracking. It analyzed users' gaze information according to Heatmap analysis of distribution and frequency and was determined the presence or absence of stimuli on the components of space. This research is an advanced research to study and enhance treatment environment based on the analysis of patients' gazes. It attempted to create an opportunity to get closer to patient-centric environment by understanding the stimulants and obstacles and controlling the background settings.

일부지역 장애인구강진료센터 이용 만족도에 미치는 요인 (Factors affecting the satisfaction of persons with disabilities at community oral care centers)

  • Background: To identify the factors that affect the current status and satisfaction of people with disabilities at community oral care centers. Methods: A structured self-administered survey, including five questions on facility environment, five on usage procedure, four on medical skill, four on care cost, three on friendliness, and three on satisfaction, was administered to 218 residents of the G-disabled community care center. It comprised a Likert 5-point scale (strongly agree, 5 points; agree, 4 points; moderate, 3 points; disagree, 2 points; not at all, 1 point). The reliability of the measurement tool was 0.932 for Cronbach's α. Results: The evaluation of community oral care centers for the disabled showed that the environment was hygienic (4.42±0.73), reservation system was well maintained (4.18±0.95), and the dentist-in-charge was satisfied with the treatment (4.37±0.62). The participants agreed that the details were sufficiently explained (4.29±0.71). However, in terms of medical expenses, the score of "have fully heard the explanation of medical expenses and reductions" was 3.88±0.92. The factors affecting satisfaction were sex, final educational background in the facility environment, usage procedure, and medical skill. Conclusion: To increase the satisfaction of people with disabilities at community oral care centers, it is necessary to establish a facility environment and service according to the patients' need and increase the reduction or exemption benefits between different treatment cost categories. Oral health management policies for the disabled should be developed based on these factors, so that the oral care of vulnerable groups in blind spots can be maintained.

치과위생사의 의료서비스가 환자만족도 및 병원재이용 의사에 미치는 영향 (An effect of medical service of the dental hygienist upon patients' satisfaction and the intention of hospital revisit)

  • Objectives : This study aims at analyzing the quality of medical service of the dental hygienist upon patients' satisfaction and the intention of hospital revisit in order to figure out the role of the dental hygienist to improve patients' satisfaction. Methods : Questionnaire survey was carried out to patients in dental hospital and clinics in Seoul and Gyeonggi-do. Total 301 sheets except for insufficient ones were analyzed and the results are as follows. Results : As for the correlation analysis between variables affecting patients' satisfaction, every variables showed significantly positive relation, and the relation between medical treatment level of the dental hygienist and patients' satisfaction had the strongest positive (+) relation (p<0.001). Oral health education satisfaction by the dental hygienist and the medical treatment satisfaction by the dental hygienist had significant influence on patients' satisfaction, a dependent variable (p<0.001). Hospital environment (p<0.000), oral health education satisfaction by the dental hygienist (p<0.05), the satisfaction of medical treatment level by the dental hygienist (p<0.05) and patients' satisfaction (p<0.001) had significant influence on the intention of hospital revisit. Conclusions : In order to improve dental medical service quality and the intention of hospital revisit, the role of the dental hygienist is important. Thus, it seems necessary to prepare specific education plan on the medical service by the dental hygienist.

요인별 치과 의료서비스 질이 환자만족과 서비스 가치에 미치는 영향 (The effects of the quality of dental care service by factors on the patient satisfaction and service value)

  • The purpose of this study is to forecast patient satisfaction and service value through the staged regression analysis of the relation between each factor and its satisfaction and service value, following the measurement of the quality of dental care service that patients recognize. And a self-administered survey was used for this study. The subjects of this survey were 394 outpatients of dental clinics located in K city in the period from Dec. 5, 2004 to Feb. 19, 2005, the questionnaire was composed of a total of 31 questions to measure the quality of overall dental care service(22 questions), the patient satisfaction(6 questions) and the service value(3 questions). The 7 points Likert scale ranging from "very poor"(1-point) to "very high"(7-point) was used for these questions The results of study could be summarized as follows: 1. The most useful index to predict patient satisfaction turned out to be "internal environment", followed by "treatment process and waiting time", "dentist", "external environment", and "dental hygienist". 2. The service value had a significant effect on the "internal environment", "dentist", "treatment process and waiting time". 3. The most useful index to predict the service value turned out to be "treatment process and waiting time", followed by "internal environment" and "dentist". Today, the quality of dental care service is becoming a prime concern since it is directly connected to a matter of survival of medical service organizations. Dental clinics desperately need patient-oriented marketing strategies in order to actively cope with changing medical environment. They also need to thoroughly examine possible measures to maximize the service value by establishing a variety of service strategies which can promote service quality that patients recognize.

  • The aim of this article is to provide major announcements within the last decade or so about possible medical emergencies in dentistry. This would be helpful to improve the knowledge on first aid suitable for dental environment. Syncope was the most common medical emergencies in dentistry. Medical emergency situations can be divided into urgency and true emergency. Urgency situations can be solved well if proper treatment is taken. However, even in an urgency, if the correct treatment is not performed, it may soon turn into an emergency. With the joint efforts of the Korean Dental Anesthesiology Society and the Korean Cardiopulmonary Resuscitation Association, dental advanced life support (DALS) has been prepared since 2015. Through 17 training experiences until January 2020, scenarios and textbooks were developed. Dentists and dental care teams need to be prepared to strengthen their competence as professionals and to increase their team-level response capabilities..

김해 치과 의료기관의 치과진료 만족도 요인분석 (Analysis of Factors affecting the Patient's Service Satisfaction in Kimhae Dental Hospital)

  • 보건의료환경 변화에 따른 환자들의 의료수요에 부응하고 합리적인 경영대책을 마련하여 환자들에게 양질의 치과의료서비스를 제공하고 진료환경을 개선시킬 목적으로 계속구강건강관리 환자 149명을 대상으로 치과의료서비스를 제공 받은 후의 진료 만족도와 진료만족도에 영향을 미치는 요인을 파악하였다. SPSS Ver 13.0을 이용하여 일반적 특성에 따른 치과의료서비스 만족도, 치과의료서비스의 질에 대한 만족도는 평균분석을 하였고, 일반적 특성 및 치과의료서비스 만족도가 치과진료 만족도에 미치는 영향은 회귀분석을 하였다. 다중회귀분석에서 치과진료 만족도에 영향을 미치는 요인은 직원요인, 외부환경요인, 진료절차였다. 치과의료 서비스의 질을 향상시키고 환자의 만족도를 높이기 위해서는 직원들의 체계적인 관리 및 교육이 이루어져야 할 것이다.

치과위생사의 조직몰입도, 내부역량, 근무환경의 융합적 관련성 (Convergent Relationship between Organizational commitment, Empowerment, Work Environment of Dental Hygienist)

  • 본 연구는 치과위생사의 조직몰입도, 내부역량, 근무환경의 연관성을 알아보고자 한다. 연구대상자는 2019년 5월~10월 까지 대구, 경북 일부 지역의 치과위생사 245명에게 설문 조사를 실시하으며, SPSS 21.0 프로그램을 이용하여 기술통계, ANOVA, Pearson's 상관계수로 분석하였다. 연구결과 조직몰입도, 내부역량, 근무환경 관련성 결과에서는 양(+)의 상관관계를 보여 치과위생사의 조직몰입도가 높을수록 개인의 내부역량이 높고 근무환경이 좋은 것으로 나타났다. 본 연구는 의료기관에서 치과위생사의 조직몰입도 및 내부역량을 향상시킬 수 있는 방안을 제시하였으며, 치과병(의)원의 근무환경을 개선시킬 수 있는 프로그램 개발의 필요성을 제언한다.

치과위생사의 업무 실태 및 비중에 관한 연구 (A Study on the Current State and Weight of Dental Hygienists' Works)

  • With the change of medical environment, people are taking interest not just in treatment but in the prevention of oral diseases. As people's interest in oral diseases increases, the number of dental institutions is increasing continuously and this is heightening the necessity and importance of professional dental hygienists equipped with expert knowledge of oral health. Thus the present study purposed to survey and analyze the current state and weight of works carried out by dental hygienists, and to propose dental hygienists' works desirable in the medical environment changing its focus from treatment to prevention. The results of this study are as follows; 1. In the results of dividing dental hygienists' works into oral examination and preliminary examination, preventive works, assistance to dental treatment, oral health education, hospital administration and management, and surveying the performance rate of these works, the work of the highest performance rate in preliminary examination was collection of patients' systemic history (87.6%). It was also scaling (96.9%) in preventive dental treatment, preparation for treatment (96.5%) in assistance to dental treatment, education on the toothbrushing method (92.3%) in oral health education, and tool disinfection and management (72.2%) in hospital administration and management. 2. When the performance rate was surveyed according to age, institution, and work experience, the performance rate by work area was highest in dental hygienists aged between 27~28, those working at a dental clinic, and those with 4~6 years' work experience. 3. The weight of dental hygienists' works was high in order of assistance to dental treatment (59.1%), observation of oral health condition and preliminary examination (12.4%), oral health education (12.4%), hospital administration and management (10.4%), and preventive dental treatment (5.8%), but in the results of surveying perception on the importance of works, the importance was high in order of oral health education (34.7%), preventive dental treatment (29.0%), observation of oral health condition and preliminary examination (19.3%), assistance to dental treatment (11.2%), and hospital administration and management (5.8%), showing that the weight of works carried out current was different from perception on the weight of works regarded as important. 4. To the question on parts to be developed in response to the change of medical environment, the most frequent answers were the establishment of their roles as preventive dental practitioners (75.7%), and the establishment of their roles as oral health educators (74.9%). This was consistent with the works that dental hygienists gave the highest weight to, and suggested that dental hygienists regarded preventive dental treatment as most important. Development of abilities to give counsel to patients was 55.2%, acquisition of theoretical knowledge of dental treatment 42.1%, improvement in quality as a hospital manager 28.2%, and acquisition of skills to assist dental treatment 23.2%. These show that, even with regard to the development of future oriented dental hygienists' capacities, assistance to dental treatment was perceived less important as it was with regard to the importance of work.
