Statement of problem : Accurate impression is essential to success of implant prostheses. But there have been few studies about the accuracy of fixture-level impression technique in internal connection implant system. Purpose: This study evaluated the accuracy of splinted fixture-level impression technique using clinical methods and the effect of internal hex on fit of superstructure in internal connection implant system (Astra Tech). Material and method : Two metal master frameworks made from two abutments (Cast-to Abutment ST) each for parallel and divergent conditions and a corresponding. passively fitting, dental stone master cast with four future replicas (Fixture Replica ST) were fabricated. Ten dental stone casts were made with vinyl polysiloxane impressions from the master cast by acrylic resin splinted fixture-level impression technique. To evaluate the accuracy of impression technique, the fit of master frameworks for test models was evaluated using screw resistance test (SRT) and one-screw test. The results of SRT were recorded as SRT values from grade 1 to grade 5 by 1/4 turn. And to evaluate the effect of hex on fit of superstructure, the same tests were performed after removing hexes of master frameworks. Results: 1. There was only one case (2.5%) showing SRT value of test model below ade 2 in total before and after removing hexes of master frameworks. And, by removing hexes. SRT values decreased in only one test model (5%) and did not change in 17 test models (85%). 2. SRT values of the 1$^{st}$ screws were grade 2 in 80% of cases before, and grade 1 in 80% of cases after removing hexes. And, by removing hexes, SRT values decreased in 72.5% of cases. 3. SRT values of the 2$^{nd}$ screws were grade 3 in 85% of cases before, and grade 3 in 95% of cases after removing hexes. And, by removing hexes, SRT values did not change in 85% of cases. 4. There were only 2 cases regarded as acceptable fit by one-screw test, and SRT values of 2$^{nd}$ screws of both cases were grade 2. Conclusion. Within the limitations of this study, future-level impression of internal connection implant system is considered to obtain inaccurate working cast, even using acrylic resin splinted impression technique. And, it is considered to be unable improve the fit to remove the hexes of implant restoration.
Disinfection of dental impressions are necessary due to contamination with patient’s saliva and blood, which is a potential for cross-infection. The purpose of this study was to evaluate the effects of disinfection of four hydrophilic rubber impression materials with three disinfecting solutions, on the dimensional stability and surface hardness of improved stone casts. Three hydrophilic vinyl polysiloxane impression materials(Express, Reprosil, Exafine) and one polyether impression material(Impregum-F) were mixed according to the manufacturer’s directions and impressions were made on a ADA specification No. 19 stainless-steeldie. On removal of the impressions, each impression was immersed in one of the disinfectants(Banicide, Potadine, Clorox) for 10 minutes. After disinfection, type IV improved stone. casts were poured. On this cast, the linear dimension and surface hardness were measuredusing a Measurescope(Nikon, Japan) and a Barcol hardness tester(Barber, Colman Co U. S. A). The results were as follows : 1. The improved stone casts from disinfected Reprosil and Impregum-F impression material did not show dimensional changes(P>0.01). Those from disinfected Express and Exafine impression material showed dimensional changes(P<0.01). The amount of shrinkage was not clinically significant. 2. The improved stone casts from disinfected Express impression material did not exhibit changes in surface hardness(P>0.01), but those from disinfected Reprosil, Exafine, Imp regnum-F impression material showed changes in surface hardness(P<0.01). 3. The dimensinal stability and surface hardness of the improved stone casts were satisfactory using Banicide on Express, all disinfectants used in this study on Reprosil, Potadin and Clorox on Exafine, Banicide and Clorox on Impregum-F. According to these results, immersion disinfection of hydrophilic rubber impression mate rials did not adversely affect the resultant casts. Nevertheless compatibility tests of impression materials and disinfectants should be done when disinfecting impressions.
This study was performed to measure the retentive force of dental impression tray according to retention form. The 9 resin beams($30{\times}60{\times}2.5mm$) were made of visible light-curing tray resin according to the surface texture, the size of hole and the number of rim. The resin block was fabricated in width 50mm, length 30mm, heght 40mm to maintain an even hydrocolloid impression material. The retentive force between the resin beam and hydrocolloid impression material was measured by Universal Testing Machine(Zwick Z020, Zwick Co., Germany). The results obtained in this study were as follows : 1. The retentive force of the resin beam with bilateral 4 rims, 2mm holes(9group) was highest(9.18kg), and the polishing resin beam(2group) was worst(4.85kg). 2. There was no significant difference between the polished the resin beam(2group) and the contrast resin beam(1group). 3. The retentive force of the rimmed resin beam was higher than the perforated resin beam. 4. The retentive force of the 2mm perforated resin beam(4group) was higher than the 3mm perforated resin beam(3group). 5. As increasing the number of rim increased the retentive force.
Purpose: To evaluate the three-dimensional trueness of upper and lower denture bases produced using a digital light processing (DLP) printer and immersed in a constant-temperature water bath. Methods: An edentulous model was prepared and fitted with denture bases and occlusal rims manufactured using base plate wax. After scanning the model, denture bases, and occlusal rims, complete denture designs were created. Using the designs and a DLP printer, 10 upper and 10 lower complete dentures were manufactured. Each denture was scanned before (impression surface of upper denture base before constant temperature water bath [UBC] and impression surface of lower denture base before constant temperature water bath [LBC] groups) and after (impression surface of upper denture base after constant temperature water bath [UAC] and impression surface of lower denture base after constant temperature water bath [LAC] groups) immersion in the constant-temperature water bath. Scanned files were analyzed by comparing reference and scanned data, with statistical analysis conducted using the Kruskal-Wallis test (α=0.05). Results: Statistical analysis revealed no significant differences between the UBC and LBC groups, nor between the UAC and LAC groups (p>0.05). However, significant differences were observed between the UBC and UAC groups and between the LBC and LAC groups, i.e., before and after the constant-temperature water bath for both maxillary and mandibular denture bases (p<0.05). Conclusion: Denture bases not immersed in the constant-temperature water bath (UBC and LBC groups) exhibited error values within 100 ㎛, whereas those immersed in the water bath (UAC and LAC groups) showed error values exceeding 100 ㎛.
The purpose of this study was twofold. One was to evaluate the wettability of 4 elastomeric impression materials on tooth and different kinds of filling materials. The other was to identify the effect of topical surfactants sprayed on the surface of each impression. The elastomeric impression mate rials used in this experimental study were one polyether(Impregum F) and three polyvinyl siloxanes(Provil novo, Zerosil, Imprint). The abutments were prepared for 3/4 crown or onlay on the extracted human first molar. The filling materials used in this study for the duplication of prepared abutment were composite resin, amalgam, and casting metal. Impression was taken by manufacturer's recommendations and the number of voids on the impression surface was counted. The topical surfactants were Spannex $II^{(R)}$ and $Cohere^{(R)}$. The wettability was evaluated by comparing the number of voids between non-treated group and treated groups. The results were as follows : 1. $Zerosil^{(R)}$ showed the least number of voids on the impression surface. The number of voids increased in order of Provil $novo^{(R)}$, Impregum $F^{(R)}$, and $Imprint^{(R)}$. 2. Impregum $F^{(R)}$ and $Zerosil^{(R)}$ showed the least number of voids on the surface of dental stone master cast. The number is inclosed in order of Provil $novo^{(R)}$ and Imprint 3. When abutment material is tooth, the number of voids on the surface of master cast was smallest compared with that of other abutment materials. The number of voids increased in order of casting metal, amalgam and composite resin 4. The number of voids on the surface of the dental stone master cast was smallest when Spannex $II^{(R)}$ was used, followed by $Cohere^{(R)}$ treated group and non-treated group. These results suggest that the difference in wettability caused by the types of rubber base impression materials as well as abutment materials can affect the number of voids. And the use of topical surfactant for rubber base impression materials can improve the wettability of the materials and enhance the accurate master cast which has fewer surface voids.
PURPOSE. The aim of this study was to evaluate the repeatability of the digitizing of silicon rubber impressions of abutment teeth by using a white light scanner and compare differences in repeatability between different abutment teeth types. MATERIALS AND METHODS. Silicon rubber impressions of a canine, premolar, and molar tooth were each digitized 8 times using a white light scanner, and 3D surface models were created using the point clouds. The size of any discrepancy between each model and the corresponding reference tooth were measured, and the distribution of these values was analyzed by an inspection software (PowerInspect 2012, Delcamplc., Birmingham, UK). Absolute values of discrepancies were analyzed by the Kruskal-Wallis test and multiple comparisons (${\alpha}$=.05). RESULTS. The discrepancy between the impressions for the canine, premolar, and molar teeth were $6.3{\mu}m$ (95% confidence interval [CI], 5.4-7.2), $6.4{\mu}m$ (95% CI, 5.3-7.6), and $8.9{\mu}m$ (95% CI, 8.2-9.5), respectively. The discrepancy of the molar tooth impression was significantly higher than that of other tooth types. The largest variation (as mean [SD]) in discrepancies was seen in the premolar tooth impression scans: $26.7{\mu}m$ (95% CI, 19.7-33.8); followed by canine and molar teeth impressions, $16.3{\mu}m$ (95% CI, 15.3- 17.3), and $14.0{\mu}m$ (95% CI, 12.3-15.7), respectively. CONCLUSION. The repeatability of the digitizing abutment teeth's silicon rubber impressions by using a white light scanner was improved compared to that with a laser scanner, showing only a low mean discrepancy between $6.3{\mu}m$ and $8.9{\mu}m$, which was in an clinically acceptable range. Premolar impression with a long and narrow shape showed a significantly larger discrepancy than canine and molar impressions. Further work is needed to increase the digitizing performance of the white light scanner for deep and slender impressions.
오늘날 알지네이트 인상재는 다른 어떠한 인상재보다 널리 사용되고 있다. 이는 알지네이트 인상재가 ①사용하기 쉽고, ②환자에게 거부감이 적고, ③특수한 장비의 사용이 필요하지 않으며, ④정확히 사용하면 상당히 정밀한 인상이 가능하기 때문이다. 알지네이트 인상재의 정확한 사용법을 정리한다.
Purpose: The purpose of this study was to evaluate the internal fitness of the resin coping that was fabricated by the traditional and Digital manufacturing methods through 3-dimensional analysis. Methods: maxillary right second molar was chosen implant master model. Custom-built impression trays were manufactured. After screwing the pick-up impression coping onto the master cast, impressions were made with silicone impression. The Working model was then made with type IV stone. The coping was fabricated: SLAC group (n=8), APPC group (n=8), LAPC group (n=8) Resin coping data was measured by using a three-dimensional evaluation program. Internal fitness was calculated by RMS (Root Mean Square).It measures mean and Standard Deviation (SD). Results: Three groups are measured $47.11{\pm}(3.08){\mu}m$ total RMS of SLAC group, $48.35({\pm}1.55{\mu}m)$ for total RMS of LAPC group, $43.45{\pm}2.09{\mu}m$ for total RMS of APPC group. Measured value is gradually increased. Followed by autopolymerized pattern resin; Stereolithography resin, Light-activated pattern resin But there were no differences stastically(P>0.321). Conclusion: Evaluation of internal fitness on Resin copings was fabricated by three-ways methods showed that no differences statistically significant and clinically acceptable results.
목적: 본 연구에서는 전통적 인상채득과 디지털 인상채득을 모두 경험한 환자들의 만족도를 조사하여 다양한 항목에 대하여 비교 분석하고자 하였다. 재료 및 방법: 구강 내 디지털 인상채득의 방법을 사용하는 5개의 치과병원에서, 보철 치료를 위해 기존의 인상재를 이용한 방법과 디지털 인상채득을 모두 경험한 만 20세 이상의 성인 환자 170명을 대상으로, 2015년 10월부터 2016년 4월까지 설문조사를 실시하였다. 총128부의 설문지를 분석 자료로 이용하였으며, 빈도분석, 다중응답 빈도분석, 기술통계, 비모수 검정, 교차 분석을 시행하였고, 유의수준은 0.05로 설정하였다. 결과: 디지털 인상채득의 이유는 '임플란트치료'(43.8%), '크라운치료'(30.5%), '인레이치료'(15.6%) 순서로 나타났다. 전통적 인상채득의 평균 만족도보다 디지털 인상채득의 평균 만족도가 유의하게 더 높았다 (P<.05). 재선택하고 싶은 인상채득 방법으로는 전통적 인상채득(11.7%) 보다 디지털 인상채득(60.2%)이 더 높게 나타났다. 디지털 인상채득을 재선택한 이유로는 '구토반사가 없어서'(35.1%), '3D디지털 스캔이라 신뢰가 가서'(33.8%), '시간이짧아서'(33.8%) 순으로 나타났다. 결론: 환자 만족도 및 다시 선택하고 싶은 인상채득의 방법으로 전통적 인상채득 보다 디지털 인상채득의 선택이 더 높게 나타났다.
목적: 하악 부분 무치악 모형에서 pick-up type 인상용 코핑을 사용한 개방형 인상 채득법과 transfer type 인상용 코핑을 사용한 폐쇄형 인상 채득법, 개방형 인상 채득법에서 pick-up type 인상용 코핑 고정 재료에 따른 인상 채득 정확성을 비교 하고자 하였다. 연구 재료 및 방법: 하악 부분 무치악 모형의 좌측 구치부에 직경 4.5 mm, 길이 11.5 mm의 임플란트 고정체 두 개 식립 후 3D 프린터를 이용하여 개방형 개인 트레이 30개 폐쇄형 개인 트레이 10개를 제작하였다. 제작한 개인 트레이를 이용하여 PN군: 개방형 인상 채득법(non-splint), PG군: 개방형 인상 채득법(splint GC pattern resin), PH군: 개방형 인상 채득법(splint Herifix), TN군: 폐쇄형 인상 채득법으로 나누어 각 군당 10회씩 총 40회의 인상 채득을 진행하였다. 인상채득 후 석고 모형을 제작하여 모델 스캐너로 스캔 후 역설계 프로그램으로 스캔오차를 측정하여 95% 유의수준으로 평가 하였다. 결과: 폐쇄형 인상 채득법 TN군이 개방형 인상 채득법 PN군 보다 더 낮은 오차값을 보였다. 개방형 인상 채득법(splint)에 사용된 인상용 코핑 고정 재료 간의 비교 결과에서는 PH군에서 가장 낮은 오차값을 보였으며 PN군, PG군 순으로 정확한 인상 채득 정확성을 보여주었다. 결론: 임플란트 인상 채득 방법이나 인상용 코핑의 연결 고정 유무에 따른 각 군 간의 정확성에 유의한 차이는 없다.
