Purpose: Optical coherence tomography (OCT) has been investigated as a novel diagnostic imaging tool. The utilisation of this equipment has been evaluated through several studies in the field of dentistry. The aim of this preliminary study was to determine through basic experiments the effectiveness of OCT in implant dentistry. Materials and Methods: To assess detection ability, we captured OCT images of implants in each of the following situations: (1) implants covered with mucosae of various thicknesses that were harvested from the mandibles of pigs; (2) implants installed in the mandibles of pigs; and (3) implants with abutments and crowns fixed with temporary cement. The OCT images were captured before cementation, after cementation, and after removing the excess submucosal cement. Results: If the thickness of the mucosa covering the implant body was less than 1 mm, the images of the implants were clearly detected by OCT. In the implants were installed in pigs' mandibles, it was difficult to capture clear images of the implant and alveolar bone in most of the samples. Remnants of excess cement around the implants were visible in most samples that had a mucosa thickness of less than 3 mm. Conclusion: Currently, OCT imaging of implants is limited. Cement remnants at the submucosal area can be detected in some cases, which can be helpful in preventing peri-implant diseases. Still, though there are some restrictions to its application, OCT could have potential as an effective diagnostic instrument in the field of implant dentistry as well.
PURPOSE. To determine whether the fracture strengths and failure types differed between metal and zirconia frameworks veneered with pressable or layering ceramics. MATERIALS AND METHODS. A phantom molar tooth was prepared and duplicated in 40 cobalt-chromium abutments. Twenty metal (IPS d.SIGN 15, Ivoclar, Vivadent, Schaan, Liechtenstein) and 20 zirconia (IPS e.max ZirCAD, Ivoclar) frameworks were fabricated on the abutments. Each framework group was randomly divided into 2 subgroups according to the veneering material: pressable and layering ceramics (n=10). Forty molar crowns were fabricated, cemented onto the corresponding abutments and then thermocycled ($5-55^{\circ}C$, 10,000 cycles). A load was applied in a universal testing machine until a fracture occurred on the crowns. In addition, failure types were examined using a stereomicroscope. Fracture load data were analyzed using one-way ANOVA and Tukey HSD post-hoc tests at a significance level of 0.05. RESULTS. The highest strength value was seen in metal-pressable (MP) group, whereas zirconia-pressable (ZP) group exhibited the lowest one. Moreover, group MP showed significantly higher fracture loads than group ZP (P=.015) and zirconia-layering (ZL) (P=.038) group. No significant difference in fracture strength was detected between groups MP and ML, and groups ZP and ZL (P>.05). Predominant fracture types were cohesive for metal groups and adhesive for zirconia groups. CONCLUSION. Fracture strength of a restoration with a metal or a zirconia framework was independent of the veneering techniques. However, the pressing technique over metal frameworks resisted significantly higher fracture loads than zirconia frameworks.
PURPOSE. This study examined the effects of the abutment types and dynamic loading on the stability of implant prostheses with three types of implant abutments prepared using different fabrication methods by measuring removal torque both before and after dynamic loading. MATERIALS AND METHODS. Three groups of abutments were produced using different types of fabrication methods; stock abutment, gold cast abutment, and CAD/CAM custom abutment. A customized jig was fabricated to apply the load at $30^{\circ}$ to the long axis. The implant fixtures were fixed to the jig, and connected to the abutments with a 30 Ncm tightening torque. A sine curved dynamic load was applied for $10^5$ cycles between 25 and 250 N at 14 Hz. Removal torque before loading and after loading were evaluated. The SPSS was used for statistical analysis of the results. A Kruskal-Wallis test was performed to compare screw loosening between the abutment systems. A Wilcoxon signed-rank test was performed to compare screw loosening between before and after loading in each group (${\alpha}$=0.05). RESULTS. Removal torque value before loading and after loading was the highest in stock abutment, which was then followed by gold cast abutment and CAD/CAM custom abutment, but there were no significant differences. CONCLUSION. The abutment types did not have a significant influence on short term screw loosening. On the other hand, after $10^5$ cycles dynamic loading, CAD/CAM custom abutment affected the initial screw loosening, but stock abutment and gold cast abutment did not.
The author installed $3.75mm\times10mm$$Br{\aa}nrmark$ osseointegrated implants and $3.5mm\times8mm$ I.T.I. hollow screw implants(Benefit) as a free standing abutment to replace lower left and right molars of 44-year-old female patient. Form the foregoing procedure, the author obtained the following results. 1. Free-standing $Br{\aa}nrmark$ and I.T.I. Hollow screw implants(Benefit) could be used to replace molars effectively without reduction of adjacent natural teeth, 2. Rotation of occlusal gold screw of $Br{\aa}nrmark$ implants was observed under occlusal force. 3. Adjustment of height of I.T.I. abutments was necessary in the mandibular second molar because of the limited occlusal clearance. 4. Form the oral examination, patient comments, panoramic and periapical radiographs, there were no pain, gingival imflammation and mobility, but slight reduction of alveolar bone level was shown around both implant systems. 5. Patient comments were comfortable on both systems. but the patient complained about economic problem of $Br{\aa}nrmark$ system and long treatment treatment time of both systems.
PURPOSE. The purpose of the study is to evaluate the repeatability and reproducibility of the abutment angle using a blue light scanner. MATERIALS AND METHODS. 0°, 6°, and 10° wax cast abutment dies were fabricated. Each of the silicone impression was produced using the replicable silicone. Each study die was constructed from the prepared replicable stone used for scans. 3-dimensional data was obtained after scanning the prepared study dies for the repeatability by using the blue light scanner. The prepared 3-dimensional data could have the best fit alignment using 3-dimensional software. For reproducibility, each abutment was used as the first reference study die, and then it was scanned five times per each. 3-dimensional software was used to perform the best fit alignment. The data obtained were analyzed using a nonparametric Kruskal-Wallis H test (α=.05), post hoc Mann-Whitney U test, and Bonferroni correction (α=.017). RESULTS. The repeatability of 0°, 6°, and 10° abutments was 3.9, 4.4 and 4.7 ㎛, respectively. Among them, the 0° abutment had the best value while the 10° abutment showed the worst value. There was a statistically significant difference (P<.05). The reproducibility of 0°, 6°, and 10° abutments was 6.1, 5.5, and 5.3 ㎛, respectively. While the 10° abutment showed the best value, the 0° abutment showed the worst value. However, there was no statistically significant difference (P>.05). CONCLUSION. In repeatability, the 0° abutment showed a positive result. In reproducibility, the 10° abutment achieved a positive result.
PURPOSE. Cement-retained implant prostheses can lack proper retrievability during repair, and residual cement can cause peri-implantitis. The purpose of this in vitro study was to evaluate the influence of abutment height and convergence angle on the retrievability of cement-retained implant prostheses with lingual slots, known as retrievable cement-type slots (RCS). MATERIALS AND METHODS. We fabricated six types of titanium abutments (10 of each type) with two different heights (4 mm and 6 mm), three different convergence angles ($8^{\circ}$, $10^{\circ}$, and $12^{\circ}$), a sloped shoulder margin (0.6 mm depth), a rectangular shape ($6mm{\times}6.5mm$) with rounded edges, and a rectangular ledge ($2mm{\times}1mm$) for the RCS. One monolithic zirconia crown was fabricated for each abutment using a dental computer-aided design/computer-aided manufacturing system. The abutments and crowns were permanently cemented together with dual-curing resin cement, followed by 24 hours in demineralized water at room temperature. Using a custom-made device with a slot driver and torque gauge, we recorded the torque ($N{\cdot}cm$) required to remove the crowns. Statistical analysis was conducted using multiple regression analysis and Mann-Whitney U tests (${\alpha}=.05$). RESULTS. Removal torques significantly decreased as convergence angles increased. Multiple regression analysis showed no significant interaction between the abutment height and the convergence angle (Durbin-Watson ratio: 2.186). CONCLUSION. Within the limitations of this in vitro study, we suggest that the retrievability of cement-retained implant prostheses with RCS can be maintained by adjusting the abutment height and convergence angle, even when they are permanently cemented together.
Purpose: To investigate the effects of simultaneous soft and hard tissue augmentation and the addition of polydeoxyribonucleotide (PDRN) on regenerative outcomes. Materials and Methods: In five mongrel dogs, chronic ridge defects were established in both mandibles. Six implants were placed in the mandible, producing buccal dehiscence defects. The implants were randomly allocated to one of the following groups: 1) control: no treatment; 2) GBR: guided bone regeneration (GBR) only; 3) GBR/PDRN: GBR+PDRN application to bone substitute particles; 4) GBR/CTG: GBR+connective tissue grafting (CTG); 5) GBR/VCMX: GBR+soft tissue augmentation using volume stable collagen matrix (VCMX); and 6) group GBR/VCMX/PDRN: GBR+VCMX soaked with PDRN. The healing abutments were connected to the implants to provide additional room for tissue regeneration. Submerged healing was achieved. The animals were euthanized after four months. Histological and histomorphometric analyses were then performed. Results: Healing abutments were gradually exposed during the healing period. Histologically, minimal new bone formation was observed in the dehiscence defects. No specific differences were found between the groups regarding collagen fiber orientation and density in the augmented area. No traces of CTG or VCMX were detected. Histomorphometrically, the mean tissue thickness was greater in the control group than in the other groups above the implant shoulder (IS). Below the IS level, the CTG and PDRN groups exhibited more favorable tissue thickness than the other groups. Conclusion: Failure of submerged healing after tissue augmentation deteriorated the tissue contour. PDRN appears to have a positive effect on soft tissues.
Objective : This research compared stabilities between two types of dental implant ($SLA^{TM}$, Institut Straumann AG, Waldenburg, Switzerland and $SSII^{TM}$, Osstem co, Busan, Korea) using Osstell Mentor (Integration Diagnostics AB, Goteborg, Sweden) considering surgery methods, surgery area, diameter of implant, systemic disease, and smoking for obtaining prognosis information when installing fixture of dental implant. Materials & Methods : 206 implants of 131 patients taken by resonance frequency analysis (RFA) were determined as a final sample. Dental implants were installed as protocol of supplier by a excellent dentist who had 10 years experience about dental implants. Before connecting abutments (3 months after installation of fixture), RFA were measured twice for buccal and lingual direction to obtain average value. Results : Dental implants at mandible showed significantly higher stabilities significantly than at maxilla (p<0.001). Diameter 4.8 implants had also higher stabilities than diameter 4.1 in case of $SLA^{TM}$ implants (p<0.001). $SLA^{TM}$ implants showed more excellent stabilities than $SSII^{TM}$ implants, especially at posterior area of mandible (p=0.045) and premolar area of maxilla (p=0.032). Conclusions : This research revealed higher stabilities of $SLA^{TM}$ implants than $SSII^{TM}$ implant, especially at posterior area of mandible (p=0.045) and premolar area of maxilla (p=0.032).
목적: 이 연구의 목적은 심미적인 임플란트 지대주의 종류와 두께에 따른 파절 강도를 측정하여 구강 내 저작압에 견디는 최소한의 두께를 평가하기 위함이다. 재료 및 방법: 대조군으로 0.5 mm 두께의 티타늄 임플란트 지대주를(Ti-0.5), 실험군으로 지르코니아 임플란트와 레진 나노 세라믹 지대주를 사용하여 각각 0.5 mm, 0.8 mm, 1.0 mm 두께로 각 그룹에 10개씩 총 70개의 시편을 제작하였다(그룹Zir-0.5, Zir-0.8, Zir-1.0, RNC-0.5, RNC-0.8, RNC-1.0). 모든 시편은 파절 실험 이전에 열순환을 시행하여 구강 내에서의 사용을 재현한 후, universal testing machine을 이용하여 각 시편의 파절 강도를 측정하여 평균값을 측정하였다. 그룹들의 평균 파절 값을 측정하였으며 이원분산분석을 이용하여 통계학적으로 분석하였다. 결과: Zir-1.0군이 가장 높은 파절 강도 $2,476.3{\pm}342.0N$를 보였으며 뒤를 이어 Zir-0.8 ($1,518{\pm}347.9N$), Ti-0.5 ($1,041.8{\pm}237.2N$), Zir-0.5 ($631.4{\pm}149.0N$), 의 순이었다. RNC 그룹의 경우에 Ti와 Zir 그룹에 비교하여 유의하게 낮은 파절 강도값을 나타내었으며(RNC-1.0 $427.5{\pm}72.1$, RNC-0.8 $297.9{\pm}41.2$), 모든 실험군에서 지대주 두께가 감소할수록 파절 강도 값도 유의하게 감소했다(P < .01). RNC-0.5 ($127.4{\pm}35.3N$) 그룹은 다른 모든 군에 비해 유의하게 낮은 값을 보였다(P < .05). 결론: 이번 실험에서 사용된 모든 두께의 지르코니아 지대주는 전치부와 구치부의 교합압을 견딜 수 있는 정도의 파절 강도를 보여주었다. 레진 나노 세라믹 지대주의 경우 0.8 mm 두께 이상에서 전치부의 교합압을 견딜 수 있는 가능성을 보여주었다.
The purpose of this study was to apply the etched metal ceramometal retainer using a composite resin and acid-etch procedure with minimal tooth reduction of abutments for the replacement of one missing anterior or posterior tooth. Author obtained the following conclusions.
1. conservation of tooth structure and minimal chair time and patient expense were the primary advantages of etched metal retainer.
2. This fixed partial denture permitted good esthetic results.
3. This retainer was successfully applied for the replacement of one missing anterior or posterior tooth.
4. Proper retainer etching ws an important procedure.
5. Etched castings could be applied to periodontal splinting and post orthodontic fixation.
