  • Purpose: The purpose of this study is a finite element analysis of supporting bone according to custom abutment angle. Methods: Implant fixture was selected with a diameter of 4 mm and the length of 13 mm. The fixture and abutment was designed by a combination of the abutment screw clamping force to produce a custom abutment model of $0^{\circ}$, $15^{\circ}$, $25^{\circ}$ and $35^{\circ}$. The loading condition of 176 N was applied to the lingual surface of the crown, near to the incisor edge, and horizontal load. An oblique load of $90^{\circ}$ was applied long axis of the implant fixture analyze the stress of supporting bone. Results: The result of mechanical analysis was observed that the supporting bone stress analysis of the horizontal load, the von Mises stress values (MPa) are given in the order of TH00 (432.6) > TH25 (418.0) > TH15 (417.4) > TH35 (415.8), the oblique load, the von Mises stress values are given in the order of TO00 (459.3) > TO15 (399.6) > TO25 (374.8) > TO35 (343.4) Conclusion: The $35^{\circ}$ abutment over the current clinical tolerance limits will be available for clinical application.

  • Purpose: This study was performed fracture strength test by conducted change of abutment and coping shape for suggesting monolithic all ceramic crown which has thin thickness and superior strength of the occlusal surface. Methods: The specimens on the four kinds abutment was made according to thickness of occlusal surface and angle of axis surface. And All ceramic coping specimens of 6 different kinds was made by the CAD/CAM Method. Compression strength test using the UTM and the verification of compression-stress situation using the 3D finite element method were conducted under optimum conditions. Results: 516C specimen was showed the strongest compression-fracture strength, followed by 516FR, 516F45, specimens. Did not show significant differences between 516FR and 516F45. 516C of the universal testing machine the specimen's surface that are within the vertical load is small, finite element method of a uniformly distributed load, so the value received suggests otherwise. Conclusion: In conclusion, abutments of monolithic ziconia ceramic when having a same thickness of the occlusal, as the angle of occlusal edge is small, the stress is well dispersed and it can endure well in the fracture.

IMPACT OF IMMEDIATE AND NON-IMMEDIATE PROVISIONALIZATION ON THE SOFT TISSUE ESTHETICS OF FINAL RESTORATIONS ON IMMEDIATELY PLACED IMPLANTS

  • STATEMENT OF PROBLEM: Interpoximal papilla could be re-established without immeidate support with a provisional resotration following an immdiate implant placement. PURPOSE: Successful esthetic outcomes were reported utilizing immediate provisionalization following immediate implant placements. The aim of this study was to evaluate the soft tissue esthetics around immediately placed single tooth implant restorations with or without immediate provisional restorations. METHODS: A total of ten patients, who had a hopeless maxillary anterior tooth, were enrolled in this study. Screw retained provisional restorations were delivered to the randomly chosen 5 patients (immediate provisionalization group) on the day of immediate implant placement and maintained for about 5 months. For the remaining five patients (non-immediate provisionalization group), healing abutments were delivered on the day of surgery, replaced with screw retained provisional restorations approximately 3 months afterwards, and the provisional restorations were maintained for about 3 months. Digital photographs were taken at the delivery of final restorations in order to assess following variables; mesial papilla, distal papilla, soft tissue level, soft tissue contour and facial soft tissue prominence. The variables were compared to those of the contralateral natural tooth and scored by prosthodontists, periodontists, orthodontists and dental students. RESULTS: The immediate provisionalization group marked significantly higher scores on the following variables; soft tissue level and facial soft tissue prominence. In evaluating each variable, there were no notable differences in opinion between four dentist groups. CONCLUSION: Immediate provisionalization can be a treatment option to achieve superior soft tissue esthetics around immediately placed single implant restorations rather than non-immediate provisionalization approaches.

건강보험 급여화 관련 치과임플란트보철물 제작실태 연구 (A Study on The Dental field of Present Health Insurance for custom-made Prosthetic implant by Dental technicians)

  • Purpose: The Purpose of this study are to describe the Dental field of present health insurance for custom-made prosthetic implant by dental technicians' work. Results: A total of 300 dental technicians working at dental laboratories in Korea were randomly selected and surveyed, 206(68.7%) of them were used for the statistical analysis. Conclusion: Average daily working time was 10 hours 66%. The average cumulative credit of the clinic for dental prosthesis fabrication rates was Less than 10 million won(21.8%), 10~80 million won(11.7%), more than one hundred million won(1.5%). Remake dental prosthesis was one more than the monthly average of 98.5%. Causes of remake dental prosthesis was dentist impression 83% but did not pay 62.5%. Dental technicians Implant production period was 7 days(48.5%), 10 days(35%) was commissioned by dentists production time is 5 days(46.1%), 7 days(36.5%). President of dental laboratories 3.86 points and dental technicians 3.06 points knew differently about starting of implant health insurance coverage(p<.001). They alike were in favor of insurance coverage for the implant. Dental technicians were lower by 2.36 points for work do you know whether your health insurance application of dental prostheses. Dental technicians are 2.16 points on whether confidence in the pores payment of insurance coverage dental prosthesis, dental laboratory president was lower by 1.85 points. They are very low with 1.97 points on whether confidence in the rate payment of health insurance coverage dental prosthesis(p<.01). The implant prosthesis abutment selected, the abutments designed, design of the implant upper prosthetic, the upper prosthetic fitting dental technicians participate of dental laboratory president showed higher score (p <.05). Conclusion: Hours of dental technicians were making this short period of remake dental prosthesis-related dental prosthesis. Dental clinic and a detailed representation of the dental prosthodontic fabrication request is required for communication between the laboratory in order to reduce the remake of a dental prosthesis, dental insurance coverage written dental prosthodontic fabrication request should be legislated. Implant classification standard medical practice 1-3 Step conduct a thorough costing a total of no. 73 of the correct classification standard medical practice in addition to eight times defined by the act of dental technicians should be defined.

Three-dimensional finite element analysis of platform switched implant

  • PURPOSE. The purpose of this study was to analyze the influence of the platform switching concept on an implant system and peri-implant bone using three-dimensional finite element analysis. MATERIALS AND METHODS. Two three-dimensional finite element models for wide platform and platform switching were created. In the wide platform model, a wide platform abutment was connected to a wide platform implant. In the platform switching model, the wide platform abutment of the wide platform model was replaced by a regular platform abutment. A contact condition was set between the implant components. A vertical load of 300 N was applied to the crown. The maximum von Mises stress values and displacements of the two models were compared to analyze the biomechanical behavior of the models. RESULTS. In the two models, the stress was mainly concentrated at the bottom of the abutment and the top surface of the implant in both models. However, the von Mises stress values were much higher in the platform switching model in most of the components, except for the bone. The highest von Mises values and stress distribution pattern of the bone were similar in the two models. The components of the platform switching model showed greater displacement than those of the wide platform model. CONCLUSION. Due to the stress concentration generated in the implant and the prosthodontic components of the platform switched implant, the mechanical complications might occur when platform switching concept is used.

The reliability of Anycheck device related to healing abutment diameter

  • PURPOSE. The purpose of this in vitro study was to examine the reliability of the Anycheck device and the effect of the healing abutment diameter on the Anycheck values (implant stability test, IST). MATERIALS AND METHODS. Thirty implants were placed into three artificial bone blocks with 10 Ncm, 15 Ncm, and 35 Ncm insertion torque value (ITV), respectively (n = 10). (1) The implant stability was measured with three different kinds of devices (Periotest M, Osstell ISQ Mentor, and Anycheck). (2) Five different diameters (4.0, 4.5, 4.8, 5.5, and 6.0 mm) of healing abutments of the same height were connected to the implants and the implant stability was measured four times in different directions with Anycheck. The measured mean values were statistically analyzed. RESULTS. The correlation coefficient between the mean implant stability quotient (ISQ) and IST value was 0.981 (P<.01) and the correlation coefficient between the meant periotest value (PTV) and IST value was -0.931 (P<.01). There were no statistically significant differences among the IST values with different healing abutment diameters. CONCLUSION. There was a strong correlation between the Periotest M and Anycheck values and between the ISQ and IST. The diameter of the healing abutment had no effect on the Anycheck values.

Effect of span length on the fit of zirconia framework fabricated using CAD/CAM system

  • PURPOSE. The purpose of this study was to evaluate the effect of the span length on the fit of zirconia framework fabricated using CAD/CAM system. MATERIALS AND METHODS. Abutments for single, 4-unit and 6-unit fixed partial prostheses were fabricated. Ten zirconia frameworks were fabricated for each group. The marginal and internal gap were presented by means of replica technique and measured by measuring microscope ($AXIO^{(R)}$, Carl Zeiss, Rochester, NY) and software (I-$solution^{(R)}$, IMT i-solution Inc., Vancouver, BC, Canada). The results were statistically analyzed by multivariate analysis test and Dunnett T3 test for post hoc test (${\alpha}$=.05). RESULTS. There were statistically significant differences at 2, 4, 7, 8 points (mesio-distal section) and b, d, e, f, g (labio-lingual section). In some marginal reference points of 6-unit group (P<.05), the marginal gap were larger than 120 ${\mu}m$. CONCLUSION. Span length of zirconia core may have an influence on marginal and internal fit. Within the limitation of this study, the increase of span length of zirconia framework of 6 or more-unit fixed partial denture may decrease the marginal and internal fit.

Evaluation of marginal and internal gaps in single and three-unit metal frameworks made by micro-stereolithography

  • PURPOSE. The purpose of this study is to compare single and three-unit metal frameworks that are produced by micro-stereolithography. MATERIALS AND METHODS. Silicone impressions of a selected molar and a premolar were used to make master abutments that were scanned into a stereolithography file. The file was processed with computer aided design software to create single and three-unit designs from which resin frameworks were created using micro-stereolithography. These resin frameworks were subjected to investment, burnout, and casting to fabricate single and three-unit metal ones that were measured under a digital microscope by using the silicone replica technique. The measurements were verified by means of the Mann-Whitney U test (${\alpha}=.05$). RESULTS. The marginal gap was $101.9{\pm}53.4{\mu}m$ for SM group and $104.3{\pm}62.9{\mu}m$ for TUM group. The measurement of non-pontics in a single metal framework was $93.6{\pm}43.9{\mu}m$, and that of non-pontics in a three-unit metal framework was $64.9{\pm}46.5{\mu}m$. The dimension of pontics in a single metal framework was $110.2{\pm}61.4{\mu}m$, and that of pontics in a three-unit metal framework was $143.7{\pm}51.8{\mu}m$. CONCLUSION. The marginal gap was smaller for the single metal framework than for the three-unit one, which requires further improvement before it can be used for clinical purposes.

Peri-implant bone length changes and survival rates of implants penetrating the sinus membrane at the posterior maxilla in patients with limited vertical bone height

  • Purpose: The aim of this study was to measure the peri-implant bone length surrounding implants that penetrate the sinus membrane at the posterior maxilla and to evaluate the survival rate of these implants. Methods: Treatment records and orthopantomographs of 39 patients were reviewed and analyzed. The patients had partial edentulism at the posterior maxilla and limited vertical bone height below the maxillary sinus. Implants were inserted into the posterior maxilla, penetrating the sinus membrane. Four months after implant insertion, provisional resin restorations were temporarily cemented to the abutments and used for one month. Then, a final impression was taken at the abutment level, and final cement-retained restorations were delivered with mutually protected occlusion. The complications from the implant surgery were examined, the number of failed implants was counted, and the survival rate was calculated. The periimplant bone lengths were measured using radiographs. The changes in initial and final peri-implant bone lengths were statistically analyzed. Results: Nasal bleeding occurred after implant surgery in three patients. No other complications were found. There were no failures of the investigated implants, resulting in a survival rate of 100%. Significantly more bone gain around the implants (estimated difference=-0.6 mm, P=0.025) occurred when the initial residual bone height was less than 5 mm compared to the >5 mm groups. No significant change in peri-implant bone length was detected when the initial residual bone height was 5 mm or larger. Conclusions: This study suggests that implants penetrating the sinus membrane at the posterior maxilla in patients with limited vertical bone height may be safe and functional.

적합도에 따른 ITI 임플란트 지지 고정성 국소의치의 삼차원 유한요소 분석 (Three Dimensional Finite Element Analysis on ITI Implant Supported Fixed Partial Dentures with Various Fitting Accuracy)

  • The purpose of this study was to investigate the effects of prostheses misfit, cantilever on the stress distribution in the implant components and surrounding bone using three dimensional finite element analysis. Two standard 3-dimensional finite element models were constructed: (1) 3 ITI implant supported, 3-unit fixed partial denture and (2) 3 ITI implant supported, 3-unit fixed partial denture with a distal cantilever. variations of the standard finite element models were made by placing a $100{\mu}m$ or $200{\mu}m$ gap between the fixture, the abutment and the crown on the second premolar and first molar. Total 14 models were constructed. In each model, 244 N of vertical load and 244 N of $30^{\circ}$ oblique load were placed on the distal marginal ridge of the distal molar. von Mises stresses were recorded and compared in the crowns, abutments, crestal compact bones, and trabecular bones. The results were obtained as follows: 1. In the ITI implant system, cement-retained prostheses showed comparatively low stress distributions on all the implant components and fixtures regardless of the misfit sizes under vertical loading. The stresses were increased twice under oblique loading especially in the prostheses with cantilever, but neither showed the effects of misfit size. 2. Under the oblique loading and posterior cantilever, the stresses were highly increased in the crestal bones around ITI implants, but effects of misfit were not shown. Although higher stresses were shown on the apical portion of trabecular bones, the effects by misfit were little and the stresses were increased by the posterior cantilever. 3. When the cement loss happened in the ITI implant supported FPD with misfit, the stresses were increased in the implant componets and supporting structures.
