Clinical Experience of Surgical Treatment for Penetrating Pulmonary Gunshot Wound of a Civilian in Korea: A Case Report

  • Gunshot-induced chest trauma is exceedingly rare among civilians in South Korea due to strong firearm control policies. In contrast to military reports emphasizing the use of emergent open thoracotomy to increase chances of survival, most penetrating non-cardiac injuries in civilian settings are managed conservatively, such as through chest tube insertion, as they typically result from lower-energy bullets. However, early surgical intervention for penetrating gunshot wounds can help reduce delayed fatalities caused by septic complications from pneumonia or empyema. The advent of minimally invasive thoracic surgery has provided cost-effective and relatively non-invasive treatment options, aided in the prevention of potential complications from undrained hematomas, and facilitated functional recovery and reintegration into society. We successfully treated a patient with a penetrating gunshot wound to the chest using video-assisted thoracoscopic surgery.

Enhancing VANET Security: Efficient Communication and Wormhole Attack Detection using VDTN Protocol and TD3 Algorithm

  • Due to the rapid evolution of vehicular ad hoc networks (VANETs), effective communication and security are now essential components in providing secure and reliable vehicle-to-vehicle (V2V) and vehicle-to-infrastructure (V2I) communication. However, due to their dynamic nature and potential threats, VANETs need to have strong security mechanisms. This paper presents a novel approach to improve VANET security by combining the Vehicular Delay-Tolerant Network (VDTN) protocol with the Deep Reinforcement Learning (DRL) technique known as the Twin Delayed Deep Deterministic Policy Gradient (TD3) algorithm. A store-carry-forward method is used by the VDTN protocol to resolve the problems caused by inconsistent connectivity and disturbances in VANETs. The TD3 algorithm is employed for capturing and detecting Worm Hole Attack (WHA) behaviors in VANETs, thereby enhancing security measures. By combining these components, it is possible to create trustworthy and effective communication channels as well as successfully detect and stop rushing attacks inside the VANET. Extensive evaluations and simulations demonstrate the effectiveness of the proposed approach, enhancing both security and communication efficiency.

Numerical study on battery thermal management system using phase change material with oscillating heat pipe (상변화물질과 맥동형 히트 파이프를 이용한 배터리 열 관리 시스템에 대한 수치해석적 연구)

  • To effectively control heat generation resulting from advancements in fast discharging technology for electric vehicle batteries, hybrid Battery Thermal Management Systems (BTMS) are gaining attention. In this study, a BTMS combining Phase Change Material (PCM) with Oscillating Heat Pipe (OHP) was designed. During the phase change process of the PCM, the maximum battery temperature increased slowly. Additionally, due to the excellent heat transfer capability of the OHP, the PCM/OHP BTMS delayed the time when the maximum battery temperature exceeded 50 ℃ by 810 s compared to the PCM/copper fin BTMS, resulting in the maximum battery temperature that was 41.29 ℃ lower at 3600 s. Furthermore, in the section where the latent heat of the PCM had the greatest impact, the slope of the battery temperature difference was 0.0017 lower than that of the PCM/copper fin BTMS. Therefore, the PCM/OHP BTMS demonstrates its potential as a viable hybrid BTMS.

Corn-Based Forage Cropping Systems in Rice Black-Streaked Dwarf Virus Prevalent Area (흑조위축병이 심한 남부지방에서 옥수수를 중심으로 한 사료작물 작부체계)

  • Forage productivity of cropping systems of rye - silage corn, silage corn - oats, silage corn - rape was studied in the south-eastern part of Korea where rice black-streaked dwarf virus(RBSDV) infection of corn are severe. Rye(cv. Paldanghomil) was planted on Oct. 20 of 1986 and harvested 10 times from April 5 to May 5 at the 5-day intervals in 1987, corn (cv. Suweon 19 and Jinjuok) was planted 5 times from April 5 to May 15 at the 10-day intervals in 1987, and oats(cv. Megwiri) and rape (cv. Velox) were planted 4 times from Sept: 4 to 25 at the 7-day intervals and harvested 4 times from Nov. 10 to Dec. 10 at the 10-day intervals in 1987. Considering yield, nutrition value, and in vitro dry matter digestibility (IVDMD), forage productivity of the cropping systems was compared. As harvesting time of rye delayed, plant height, dry matter(DM) yield, percent DM, crude fiber, and digestible DM yield increased, but crude protein, crude fat, and IVDMD decreased. However, nitrogen free extract was not different among the harvesting dates. As planting date of corn delayed, RBSDV infection rate increased. but DM yield of silage decreased. However, silage yield of Jinjuok was higher, but RBSDV infection rate was lower compared with Suweon 19 at all planting dates. DM yield of oats and rape decreased as planting date delayed. However, at Sept. 4 and 11 plantings yield of oats on Nov. 10 was much lower than that of rape, but the differences in yield between two crops decreased with delayed harvesting, and yield was similar on Dec. 10. A cropping system harvesting rye around April 20 and followed by planting corn in late April was best among the rye-corn systems considering yield and nutrition value of both crops. However, among the corn-oats or corn-rape cropping systems early April planting of corn and followed by early Sept. planting of oats or rape showed best results with similar yield potential of the best rye-corn cropping system.

P3 in the Auditory Event-Related Evoked Potential of Schizophrenia(I) -P3 in the Schizophrenics- (정신분열증의 사건관련유발전위에 대한 연구(I) -정신분열증 환자의 사건관련유발전위-)

  • Objectives: To examine a defect in inhibitory gating of auditory evoked response in schizophrenics, to compare P3 latency and amplitude in negative and positive schizophrenics, and to assess the association of P3 with family history of the psychiatric disorders, electroconvulsive therapy, and clinical features. Methods: 54 schizophrenics(male 31, female 23) and 75 controls(male 33, female 42) were tested with event-related potential paradigm designed to elicit P3 response and Frankfurter Beschwerde Fragebogen. Results: In schizophrenics, the latency of P3 was significantly more delayed and the amplitude of P3 was significantly more reduced than in the controls. Significant differences in P3 latency and amplitude between negative and positive schizophrenics were not found. And significant difference in the P3 latency and amplitude between schizophrenics with family histories of psychiatric disorder and those without family histories of psychiatric disorder was not found also. The P3 latency and amplitude was not significantly related with electroconvulsive therapy and other clinical features such as age, duration of illness, onset of inllness, number of admission, and doses of antipsychotics etc. Conclusion: These results suggested that schizophrenics had a dysfunction in the process of selective attention and that P3 was not significantly related with family history of the psychiatric disorders, positive and negative symptoms, electro1convulsive therapy, and clinical features in schizophrenics.

Climatic Yield Potential Changes Under Climate Change over Korean Peninsula Using 1-km High Resolution SSP-RCP Scenarios (고해상도(1km) SSP-RCP시나리오 기반 한반도의 벼 기후생산력지수 변화 전망)

  • The changes in rice climatic yield potential (CYP) across the Korean Peninsula are evaluated based on the new climate change scenario produced by the National Institute of Agricultural Sciences with 18 ensemble members at 1 km resolution under a Shared Socioeconomic Pathway (SSP) and Representative Concentration Pathways (RCP) emission scenarios. To overcome the data availability, we utilize solar radiation f or CYP instead of sunshine duration which is relatively uncommon in the climate prediction f ield. The result show that maximum CYP(CYPmax) decreased, and the optimal heading date is progressively delayed under warmer temperature conditions compared to the current climate. This trend is particularly pronounced in the SSP5-85 scenario, indicating faster warming, except for the northeastern mountainous regions of North Korea. This shows the benef its of lower emission scenarios and pursuing more efforts to limit greenhouse gas emissions. On the other hand, the CYPmax shows a wide range of feasible futures, which shows inherent uncertainties in f uture climate projections and the risks when analyzing a single model or a small number of model results, highlighting the importance of the ensemble approach. The f indings of this study on changes in rice productivity and uncertainties in temperature and solar radiation during the 21st century, based on climate change scenarios, hold value as f undamental information for climate change adaptation efforts.

Characterization of Voltage-Gated Potassium Currents in Dorsal Root Ganglion Neurons of Neonatal Rats (신생흰쥐 척수후근신경절 세포에서 전압의존성 $K^+$ 전류의 동정)

  • Dorsal root ganglion (DRG) is composed of neuronal cell bodies of primary afferents with diverse functions. Various types of ion channels present on DRG neurons may reflect those functions. In the present study, voltage-gated potassium currents in DRG neurons of neonatal rats were characterized by whole-cell voltage clamp method. Two types of delayed rectifier and three types of transient potassium currents were identified according to their electrophysiological properties. The delayed rectifier currents were named $I_{Ke}$ (early inactivating) and $I_{K1}$ (late inactivating). Steady state inactivation of $I_{Ke}$ began from -100 mV lasting until -20 mV. $I_{K1}$ could be distinguished from $I_{Ke}$ by its inactivation voltage range, from -70 mV to +10 mV. Three transient currents were named $I_{Af}$ (fast inactivation), $I_{Ai}$ (intermediate inactivation kinetics), and $I_{As}$ (slow inactivation). $I_{Af}$ showed fast inactivation with time constant of $10.6{\pm}2.0$ msec, $I_{Ai}$ of $36.9{\pm}13.9$ msec, and $I_{As}$ of $60.6{\pm}2.9$ msec at +30 mV, respectively. They also had distinct steady state inactivation range of each. Each cell expressed diverse combination of potassium currents. The cells most frequently observed were those which expressed both $I_{K1}$ and $I_{Af}$, and they had large diameters. The cells expressing $I_{Ke}$ and expressing $I_{Ke}$, $I_{Ai}$, and $I_{As}$ usually had small diameters. Judging from cell diameter, capsaicin sensitivity or action potential duration, candidates for nociceptor were the cells expressing $I_{Ke}$, expressing $I_{Ke}$ and $I_{Ai}$, and expressing $I_{Ke}$ and $I_{As}$. The types and distribution of potassium currents in neonatal rat DRG were similar to those of adult rat DRG (Gold et al, 1996b).

Comparison of P300 between Schizophrenia and Bipolar Disorder (정신분열병과 양극성 장애의 P300 비교)

  • Objective : Asymmetries in evoked potential P300 topography of schizophrenia and bipolar disorder are still controversial. The purpose of this study was to examine the difference in P300 topography between schizophrenia and bipolar disorder. Method : P300 was recorded from 16 schizophrenic, 15 bipolar manic, and 16 control subjects. All were right-handed. Subjects silently counted target stimuli(2.0kHz) among trains of standard stimuli(1.0kHz). Averages were constructed from brain responses to target stimuli. Results : 1) Schizophrenics displayed significantly smaller peak amplitude of P300 over Cz, Pz, T3 than controls. 2) Schizophrenics displayed significantly smaller peak amplitude of P300 over T3 than bipolar manics. 3) Schizophrenics displayed significantly smaller peak amplitude of P300 over T3 than their T4. 4) Schizophrenics displayed significantly delayed latency of P300 over T3 than bipolar manics. 5) Schizophrenics displayed significantly delayed latency of P300 over T3 than their T4. Conclusions : Left-sided P300 abnormality, especially left superior temporal gyrus, in schizophrenics relative to bipolar manics and controls suggests that psychophysiological cause of schizophrenia and bipolar disorder is different and P300 asymmetry is specific to the psychophysiological cause of schizophrenia.

A Comparative Study on the Growth & Developmental Status of Premature and Full Term Infants During the First 3Years (미숙아와 정상아의 영유아기 성장발달상태 비교연구)

  • The problems of growth & development due to maladjustment are gradually increasing while need for the treatment of children's diseases is decreasing. The level of developmental deficiency or delay correlates with neonatal birth weight and also with gestational age, i.e. degrees of prematurity. There-fore, developmental defects and potential risk factors' are more Common in premature infants than in full term infants. The purpose of this study is to define the difference in the growth at developmental status between premature and full term infants, and to define the relation between the developmental status and the physical growth during the first 3 years' Data were collected from January 10, 1985 to April 6, 1985 at 3 hospitals including St. Mary's Hospital, and through home visiting. The subjects of this study consisted of 79 Premature infants (G.A. <37wks. & B.W. <2.5kg) and 94 full term infants (G.A.≥37 wks. & B.W.≥2.5kg). The study method used was a questionnaire, anthropometric assessment and DDST for normative data of growth & development. The collected data were analyzed using descriptive statistics, chi-square test and t-test. The results of the study were as follows: Hypothesis: 1 : That the prematures will differ from the full term infants in the physical growth status during the first 3 years was partially supported (p<0.02) : The prematures reached up the full term infants in the physical growth status in the first 6 months. And, the first hypothesis was supported (P<0.01) : There are more cases which is below‘the Korean children's physical. growth standards’in prematures than in full term infants. Hypothesis 2 : That the prematures will differ from the full term infants in the developmental status during the first 3 years was supported (P< 0.001);‘Normal’developmental status due to DDST was less in prematures than in full term infants. And, the second hypothesis was Partially supported (P<0.02) : The developmental status of the pre-matures was different from that of the full term infants within the first 3 months by analysis of passed items in DDST, Hypothesis 3 : That the prematures' developmental status will relate to their physical growth during the first 3 years was supported (P<0.001) : If the prematures' developmental status is in delayed status, then, their physical growth status is also in delayed status. This study shows that the prematures differed significantly from the full term infants in the growth at developmental status during their infancy. This means that the nurse can foster the growth & development of the prematures by supportive care during their infancy. Further longitudinal study is needed to verify these findings for the environmental factors.

  • Recent advances in techniques and strategies use to detect cancer in its early stages and to treat it effectively has the survival rate of cancer patients and the number of long-term cancer survivors continually increasing. Unfortunately, many cancer survivors are at risk for various late and long-term effects of cancer treatments including the radiotherapy. Long-term cancer survivors can be also seen for a hospice and palliative care because of cancer recurrence and they are at risk of delayed reactions to radiotherapy. So, the understanding and knowledge of radiation reactions is required for the proper medical diagnosis, management, and coordination of the potential reactions that may occur in these care setting. In effort to increase the survival rate in cancer patients and to decrease the adverse effects of cancer treatment, many clinical studies have been and continue to be conducted. The efforts of these studies have thus resulted in the advancement of cancer treatments. Regrettably, the overall interest in how to manage adverse effects of cancer treatment such as radiotherapy appears seemingly low in clinical practice and its advanced studies as a whole are delayed and deficient. It is imperative that the medical community show an enthusiastic interest in the aftercare of cancer patients and cancer survivors in order to create a complementary integrative approach that will eliminate radiotherapy related pain/discomfort or illness in hospice and palliative care settings.
