Permanent Pacemaker Lead Induced Severe Tricuspid Regurgitation in Patient Undergoing Multiple Valve Surgery

  • Severe and permanent tricuspid regurgitation induced by pacemaker leads is rarely reported in the literature. The mechanism of pacemaker-induced tricuspid regurgitation has been identified, but its management has not been well established. Furthermore, debate still exists regarding the proper surgical approach. We present the case of a patient with severe tricuspid regurgitation induced by a pacemaker lead, accompanied by triple valve disease. The patient underwent double valve replacement and tricuspid valve repair without removal of the pre-existing pacemaker lead. The operation was successful and the surgical procedure is discussed in detail.

  • In this paper, I review the historical background of "the principle of the permanence of equivalent forms" and sum- marize properties of "the principle of the permanence of equivalent forms" as a kind of heuristic. 1 think that "the principle of the permanence of equivalent forms" can be used effectively for student's discovery of the algebraic structure. There are three ways of using "the principle of the permanence of equivalent forms" in extending number system - an extension on the base of set theory(SMSC), the formal or axiomatic extrapolation and the inductive-extrapolatory method. All those three methods are mixed up and being used potentially at various levels in current Korean text books. "The principle of the permanence of equivalent forms" is used most effectively in the subject of the exponent. 1 try to present a situation that makes the students find more general definition and cultivate their desirable attitudes for the mathematics in the process of extending the exponent through summarizing the debate between Goel & Robillard(1997) and Tirosh S, Even(1997).

  • Radioactive iodine therapy using I-131 for hyperthyroidism has been used for more than 50 years, and generally considered safe and devoid of major side effects. Appropriate patient selection criteria and clinical judgement concerning patient preparation should be employed for its optimal use. It has not been possible to resolve the trade-off between efficient definite cure of hyperthyroidism and the high incidence of post-therapy hypothyroidism. The dose of the I-131 needed to maintain euthyroid state remains an area of uncertainity and debate. Early side effects are uncommon and readily managable. Other than the need for long-term monitoring and, in most cases, lifelong thyroid hormone treatment for late adverse consequences of this treatment remains only conjectural. We have reviewed general principles and recent advances in radioiodine treatment for Graves' hyperthyroidism, specially regarding to several controversies.

  • This paper will discuss three different ways of framing relationships between identity and built forms mainly through the theoretical frame works of David Harvey, Christine M. Boyer, Jane M. Jacobs, Doreen Massey, Paul Rabinow, and Michel Foucault. From these scholars, this paper will argue the relationships between identity and built forms are categorized as such: "Becoming", "Politics of Difference", and "Construction of Self". Besides these three approaches of framing identity and built forms, relevant ideas will be drawn from the work of other scholars in so far as their theoretical positions relate and support these three key frameworks. To approach the critical points of each debate, these three categories are further analyzed by juxtaposing the epistemological positions between them. Through the comparisons, this paper illustrates the interrelationships and interdependence of these three categories whose discursive power gains rapid popularity in Western scholarships. By incorporating the three ways to view the relationship between built form and the identity of social groups, drawn is a suggestion for a broader imagining of new spatial identity.

  • Despite low mortality and excellent long-term results after repair of Tetralogy of Fallot, certain associated anomaly such as single pulmonary artery continues to be told to have a high operative mortality and morbidity, and there is still some debate on appropriate surgical intervention. During the 4 year period from 1981 to 1984, surgical repair was performed on 5 patients with tetralogy of Fallot and congenital [4 cases] or acquired [1 case] absence of left pulmonary artery. Previous left pneumonectomy had been performed in the patient with acquired absence of a pulmonary artery. Transannular patch or RVOT patch alone with or without pulmonic valvotomy was used with some modifications to reduce pulmonic insufficiency in individual patient. In contrast with previous reports, all patients survived operation and have exhibited marked symptomatic improvement without pulmonic valve insertion nor valved conduit.

  • There has been a long debate whether the Sun revolves the Earth or the Earth revolves the Sun. Also, students are very interested in the solar systems, which means the entire system of planets, satellites, minor planets, comets, and meteoroids that orbit the sun. However, students are not sure about them. New models which enhance teaming about them are strongly needed. This study is intended to develop the new models on the solar systems and to investigate how the preservice elementary teachers' concepts are affected by them. Subjects are 20 preservice elementary teachers, One instrument including 11 items is used. Data are collected before using the new models and after using them through the tests. As a result, learning through the new models has a positive effects on the preservice elementary teachers' concepts on the solar system.

Microsurgical Excision of Symptomatic Sacral Perineurial Cyst with Sacral Recapping Laminectomy : A Case Report in Technical Aspects

  • Perineurial cysts (Tarlov cysts) are lesions of the nerve root that are often observed in the sacral area. There is debate about whether symptomatic perineurial cysts should be treated surgically. We presented three patients with symptomatic perineurial cyst who were treated surgically, and introduced sacral recapping laminectomy. Patients complained of low back pain and hypesthesia on lower extremities. We performed operations with sacral recapping technique for all three. The outcome measure was baseline visual analogue score and post operative follow up magnetic resonance images. All patients were completely relieved of symptoms after operation. Although not sufficient to address controversies, this small case series introduces successful use of a particular surgical technique to treat sacral perineural cyst, with resolution of most symptoms and no sequelae.

  • In the framework of linear representation which allows for no internal structure within features, there is no way to represent nonlinear phonological phenomena such as complex segments. This paper shows how we carl solve some problems of the linear feature theory in relation to the hierarchical feature theory. The purpose of this paper is to explain lateral assimilation under hierarchical feature representation. Although arguments for the position of classes of distinctive features have been made the position of (lateral) remains the issue of debate. Sagey(1988) argues that the feature [lateral] is structurally dependent on the root node. In contrast Rice & Avery (1991) put the feature (lated) under the spontaneous voicing. I have discussed previous studies of feature hierarchy and I propose a revised model of feature representation. Within this model I have shown how well feature geometry describes lateralization as feature spreading.

  • It is reported that Korea is a county of water shortage. The government make plans to construct dam to cope with this problem. But, it become difficult to construct dam, because of public opinion of residents and objection of environmentalist. In the sense, the government set up new policy of demand-management which means water saving. It is known that the efficiency of agricultural water is so poor that the first target to save water is focussed on agricultural water. In the study, the function of agricultural water was evaluated and it is found that it is not a waste, but a good ability on eco-system by return flow.

A Review on the Taguchi Method and Its Alternatives for Dynamic Robust Design (다구치의 동적 강건설계와 그 대안에 관한 고찰)

  • Taguchi's robust design is a method for quality improvement by making a system insensitive to uncontrollable variations incurred by noise factors and it has received much attention in a wide range of engineering fields. Robust design can be broadly classified into static and dynamic ones. This paper is concerned with dynamic robust design. Taguchi suggested to use a signal-to-noise ratio as a robustness measure, but there has been much debate and criticism on its blind use. In order to cope with this drawback, many alternatives have been proposed. They are divided into performance measure modeling (PMM) and response function modeling (RFM) approaches. In this paper, both PMM and RFM approaches for dynamic robust design are reviewed. An example for illustration is provided as well.
