  • Purpose: This study tried to identify changes in family burden after the introduction of the long-term care insurance and to examine the factors influencing subjective and objective caring burden and depression of family caregivers of elders receiving home-based long-term care. Methods: Data were collected from 203 family caregivers of elders from August 1 to 31, 2015 using questionnaires. They were analyzed in descriptive statistics, t test, ANOVA test, and multiple regression analysis. Results: The mean score of depression was 7.24, which suggested mild depression level. The subjective family burden was 2.71 and the objective burden 3.04. The factors affecting depression included subjective burden (t=5.08, p<.001), objective burden (t=2.80, p=.006), time of elderly care per day (t=-3.61, p< .001), caregiving duration (t=3.33, p=.001), age (t=3.13, p=.002), family relationship (t=2.48, p=.014), and economic status (t=1.99, p=.047). Conclusion: The family burden was most important influencing factor on caregiver's depression. Therefore, services and supports to alleviate caregivers' burden in the home-based care should be added to long-term care.

  • The purpose of this study is to investigate the relationship between the OKgram and abuse awareness, and between the the characteristics of the major field and abuse awareness of pre-service child care teachers who are taking the (child care) course as part of their university curriculum. Positiveness isclassified into 2 subfactors,self-positiveness and other positiveness, while abuses are classified into 4 sub factors, viz. body, sex, emotion and neglect, in order to investigate and analyze the effectsof each sub factor. Asurvey was carried out for students of the department of Child Care and the department of Social Welfare of the universities in Gyeonggi-do, Gangwon-do and Seoul, during the 5 day period from 8th May to 12th May, 2017. After conducting a frequency analysis, correlation and multiple regression analyses were performed, from which we madethe following conclusions. First, it was found that positiveness influences abuse awareness, and that when other positiveness is higher than self-positiveness, abuse awareness is increased. We also concluded that we should provide more personality programs that enhance others positiveness. Second, the seriousness of abuse level showed an average value of 4.594, which is highly recognized, while the level (required) to recognize the index wasin the order of sexual(4.789), physical(4.587), neglect(4.510), emotional(4.498), with the index of emotion beingthe lowest. This indicatesthe necessity to strengthen the social standard on emotion abuse awareness. Third, in the characteristics of the major field, the respondents answered that their actual work experience of being a pre-service child care teacher was helpful to raise their abuse awareness, and thatchild care practice wasthe most important. Therefore,we concluded that it is necessary to arrange practical courses which increase the chance of obtaining actual work experience and provide child care practice during child care teacher training.

  • Purpose: The purpose of this study was to measure home health resource utilization using a Case-Mix Adjustor Model developed in the U.S. Method: The subjects of this study were 484 patients who had received home health care more than 4 visits during a 60-day episode at 31 home health care institutions. Data on the 484 patients had to be merged onto a. 60-day payment segment. Based on the results, the researcher classified home health resource groups (HHRG). Result: The subjects were classified into 34 HHRGs in Korea. Home health resource utilization according to clinical severity was in order of Minimum (C0) < 'Low (Cl) < 'Moderate (C2) < 'High (C3), according to dependency in daily activities was in order of Minimum (F0) < 'High (F3) < 'Medium (F2) < 'Low (Fl) < 'Maximum (F4). Resource utilization by HHRGs was the highest 564,735 won in group C0F0S2 (clinical severity minimum, dependency in daily activity minimum, service utilization moderate), and the lowest 97,000 won in group C2F3S1, so the former was 5.82 times higher than the latter. Conclusion: Resource utilization in home health care has become an issue of concern due to rising costs for home health care. The results suggest the need for more analytical attention on the utilization and expenditures for home care using a Case-Mix Adjustor Model.

Only 8.9% of the centers employed a dietitian, whereas 92.5% of the centers employed cook or assistant cook, thus food service was not managed by professionals in most centers. All of the centers had been self-operated and meals were prepared in a conventional manner. In approximately 89.2% of the centers servings snacks twice a day. Menu planner of the centers which have no dietitian was the child care information center (47.5%) or the director (34.7%). In most centers, the directors was also purchasing manager and 36.0% of the center purchased food every day. These results indicated that food service management guidelines need to be established by the child care center type with the government control and financial support. • Evaluation Project on Appropriate Prescribing (EPAP) which is analysing prescribing pattern and providing physicians feedback has begun in 2001. EPAP indicators are related to antibiotics for acute respiratory tract infections, overuse of injection, polypharmacy (no. of drugs prescribed together, no. of prescriptions with 6 or more drugs), prescribing of specific medication group (drugs for acid related disorders, NSAIDs, corticosteroids) and medication expenditure per prescription day. The aim of this study was to suggest a development plan for EPAP indicators suitable for domestic situation. A consensus group consisting of seventeen health care professionals evaluated significance of each therapeutic class for EPAP indicators expansion considering information such as magnitude of issue, prescribing indicators of foreign countries, reimbursement criteria by each therapeutic class. Based on the data and group survey, 5 classes were selected as candidates for prescribing indicators and we presented 24 indicators regarding 5 classes. The results suggested that we need to augment evaluation indicators of additional area.
