  • The Korean Society of Heart Failure (KSHF) Guidelines provide evidence-based recommendations based on Korean and international data to guide adequate diagnosis and management of heart failure (HF). Since introduction of 2017 edition of the guidelines, management of advanced HF has considerably improved, especially with advances in mechanical circulatory support and devices. The current guidelines addressed these improvements. In addition, we have included recently updated evidence-based recommendations regarding acute HF in these guidelines. In summary, Part IV of the KSHF Guidelines covers the appropriate diagnosis and optimized management of advanced and acute HF.

  • The Korean Society of Heart Failure (KSHF) guidelines aim to provide physicians with evidence-based recommendations for the management of patients with heart failure (HF). After the first introduction of the KSHF guidelines in 2016, newer therapies for HF with reduced ejection fraction, HF with mildly reduced ejection fraction, and HF with preserved ejection fraction have since emerged. The current version has been updated based on international guidelines and research data on Korean patients with HF. Herein, we present Part II of these guidelines, which comprises treatment strategies to improve the outcomes of patients with HF.

Changes in Cardiac Structure and Function After Kidney Transplantation: A New Perspective Based on Strain Imaging

  • BACKGROUND: We aimed to investigate left ventricular (LV) global longitudinal strain (GLS) in end-stage renal disease patients and its change after kidney transplantation (KT). METHODS: We retrospectively reviewed patients who underwent KT between 2007 and 2018 at two tertiary centers. We analyzed 488 patients (median age, 53 years; 58% male) who had obtained echocardiography both before and within 3 years after KT. Conventional echocardiography and LV GLS assessed by two-dimensional speckle-tracking echocardiography were comprehensively analyzed. Patients were classified into three groups according to the absolute value of pre-KT LV GLS (|LV GLS|). We compared longitudinal changes of cardiac structure and function according to pre-KT |LV GLS|. RESULTS: Correlation between pre-KT LV EF and |LV GLS| were statistically significant, but the constant was not high (r = 0.292, p < 0.001). |LV GLS| was widely distributed at corresponding LV EF, especially when the LV EF was > 50%. Patients with severely impaired pre-KT |LV GLS| had significantly larger LV dimension, LV mass index, left atrial volume index, and E/e' and lower LV EF, compared to mildly and moderately reduced pre-KT |LV GLS|. After KT, the LV EF, LV mass index, and |LV GLS| were significantly improved in three groups. Patients with severely impaired pre-KT |LV GLS| showed the most prominent improvement of LV EF and |LV GLS| after KT, compared to other groups. CONCLUSIONS: Improvements in LV structure and function after KT were observed in patients throughout the full spectrum of pre-KT |LV GLS|.

Unexpected Restart Failure of Durable Left Ventricular Assist Devices: A Report of Two Cases

  • The HeartWare Ventricular Assist Device (HVAD) was widely used for mechanical circulatory support in patients with end-stage heart failure. However, there have been reports of a critical issue with HVAD pumps failing to restart, or experiencing delays in restarting, after being stopped. This case report describes 2 instances of HVAD failure-to-restart during heart transplantation surgery and routine outpatient care. Despite multiple attempts to restart the pump using various controllers and extensions, the HVAD failed to restart, triggering a hazard alarm for pump stoppage. In one case, the patient survived after receiving a heart transplantation, while in the other, the patient died immediately following the controller exchange. These cases highlight the rare but life-threatening complication of HVAD failure-to-restart, underscoring the importance of awareness among clinicians, patients, and caregivers, and adherence to the manufacturer's guidelines and recommendations for HVAD management.

Relationship between temporal variability of TPW and climate variables (가강수량의 변화패턴과 기후인자와의 상관성 분석)

  • Water vapor is main absorption factor of outgoing longwave radiation. So, it is essential to monitoring the changes in the amount of water vapor and to understanding the causes of such changes. In this study, we monitor temporal variability of Total Precipitable Water (TPW) which observed by satellite. Among climate variables, precipitation play an important part to analyze temporal variability of water vapor because it is produced by water vapor. And El $Ni{\tilde{n}}o$ is one of climate variables which appear regularly in comparison with the others. Through them, we analyze relationship between temporal variability of TPW and climate variable. In this study, we analyzed long-term change of TPW from Moderate-Resolution Imaging Spectroadiometer (MODIS) data and change of precipitation in middle area of Korea peninsula quantitatively. After these analysis, we compared relation of TPW and precipitation with El $Ni{\tilde{n}}o$. The aim of study is to research El $Ni{\tilde{n}}o$ has an impact on TPW and precipitation change in middle area of Korea peninsula. First of all, we calculated TPW and precipitation from time series analysis quantitatively, and anomaly analysis is performed to analyze their correlation. As a result, TPW and precipitation has correlation mostly but the part had inverse correlation was found. This was compared with El $Ni{\tilde{n}}o$ of anomaly results. As a result, TPW and precipitation had inverse correlation after El $Ni{\tilde{n}}o$ occurred. It was found that El $Ni{\tilde{n}}o$ have a decisive effect on change of TPW and precipitation.

  • Water vapor leading various scale of atmospheric circulation and accounting for about 60% of the naturally occurring warming effect is important climate variables. Using the Total Precipitable Water (TPW) from Moderate Resolution Imaging Spectroradiometer (MODIS) operating on both Terra and Aqua, we study long-term Variation of TPW and define relationship among TPW and climatic parameters such as temperature and precipitation to quantitatively demonstrate the impact on climate change over East Asia focusing on the Korea peninsula. In this study, we used linear regression analysis to detect the correlation of TPW and temperature/precipitation and harmonic analysis to analyze changeable aspects of periodic characteristics. A result of analysis using linear regression analysis between TPW and climate elements, TPW shows a high determination coefficient ($R^2$) with temperature and precipitation (determination coefficient between TPW and temperature: 0.94, determination coefficient between TPW anomaly and temperature anomaly: 0.8, determination coefficient between TPW and precipitation: 0.73, determination coefficient between TPW anomaly and precipitation anomaly: 0.69). A result of harmonic analysis of TPW and precipitation of two-year to five-year cycle, amplitude contribution ratio of 3.5-year cycle are much higher and two phases are similar in 3.5-year cycle.

Analysis of net radiative changes and correlation with albedo over Antarctica (남극에서의 위성기반 순복사 장기변화와 알베도 사이의 상관성 분석)

  • Antarctica isimportant area in order to understand climate change. In addition, this area is complex region where indicate warming and cooling trend according to previous studies. Therefore, it is necessary to understand the long-term variability of Antarctic energy budget. Net radiation, one of energy budget factor, is affected by albedo, and albedo cause negative radiative forcing. It is necessary to analyze a relationship between albedo and net radiation in order to analyze relationship between two factors in Antarctic climate changes and ice-albedo feedback. In thisstudy, we calculated net radiation using satellite data and performed an analysis of long-term variability of net radiation over Antarctica. In addition we analyzed correlation between albedo. As a results, net radiation indicates a negative value in land and positive value in ocean during study periods. As an annual changes, oceanic trend indicates an opposed to albedo. Time series pattern of net radiation is symmetrical with albedo. Correlation between the two factors indicate a negative correlation of -0.73 in the land and -0.32 in the ocean.

Landsat 8-based High Resolution Surface Broadband Albedo Retrieval (Landsat 8 위성 기반 고해상도 지표면 광대역 알베도 산출)

  • Albedo is one of the climate variables that modulate absorption of solar energy, and its retrieval is important process for climate change study. High spatial resolution and long-term consistent periods are important considerations in order to efficiently use the retrieved albedo data. This study retrieved surface broadband albedo based on Landsat 8 as high resolution which is consistent with Landsat 7. First of all, we analyzed consistency of Landsat 7 channel and Landsat 8 channel. As a result, correlation coefficient(R) on all channels is average 0.96. Based on this analysis, we used multiple linear regression model using Landsat 7 albedo, which is being used in many studies, and Landsat 8 reflectance channel data. The regression coefficients of each channel calculated by regression analysis were used to derive a formula for converting the Landsat 8 reflectance channel data to broadband albedo. After Landsat 8 albedo calculated using the derived formula is compared with Landsat 7 albedo data, we confirmed consistency of two satellite using Root Mean Square Error (RMSE), R-square ($R^2$) and bias. As a result, $R^2$ is 0.89 and RMSE is 0.003 between Landsat 7 albedo and Landsat 8 albedo.

Components Analysis of Korean Pollens and Pollen Extracts (국내산 화분 및 화분 추출물의 성분 분석)

  • The chemical components of Korean pollens and pollen extracts were analysed to provide fundamental data for pollen processing and products development. The contents of crude protein, crude lipid, crude ash, crude fiber and carbohydrate of pollens were $14.0{\sim}22.3%,\;3.0{\sim}8.8%,\;2.2{\sim}3.3%,\;0{\sim}37.5%,\;and\;31.9{\sim}64.4%$ respectively, showing the specially high crude fiber content (37.5%) in Pine pollen. Almost all the carbohydrate, lipid, protein and ash components except fiber were extracted by water and n-hexane. Free sugars identified in water extracts $(10\;^{\circ}Bx)$ of pollens were fructose and glucose as major component and maltose as minor component. The proportions of three free sugars to total soluble solid content of Mixed, Acorn, and Pine water extracts were 73.6%, 85.4% and 47.7% respectively. Potassium and phosphorus content in pollens and pollen water extracts were high, but Ca, Na, Mg, Fe and Zn were not major mineral components. The essential amino acids such as leucine, phenylalanine, methionine, lysine, valine, isoleucine and threonine were contained richly in pollens and those were almost completely extracted by water. Vitamin $B_1$ and C were not detected, but only small amount of vitamin $B_2$ was detected in the pollens and pollen extracts tested. The essential fatty acids such as linoleic acid and linolenic acid were contained richly in pollens. The proportion of unsaturated fatty acids to total fatty acid content in Mixed, Acorn and Darae pollen except Pine was higher than 50%.

Quality Consistence Analysis of Satellite-based Sea Ice Concentration Products (위성기반 해빙 농도 산출물들의 품질 일관성 분석)

  • We compared sea ice concentration(SIC) and sea ice extent(SIE) using EUMETSAT Ocean and Sea Ice Satellite Application Facilities(OSI SAF) and NASA Team(NT) sea ice algorithm in the Arctic during 1980-2010 to investigate the difference between sea ice data applied different algorithms. SIC and SIE of the two data showed different consistency by season and by sea area. Seasonally, SIC of OSI SAF was 0.85 % overall, 0.48 % in spring, 0.97 % in summer, 1.38 % in autumn and 0.66 % in winter higher than NT SIC. By sea area, OSI SAF SIC was 2.7 %, SIE was $198,000km^2$ higher than NT in Arctic Ocean, but in Lincoln Sea, OSI SAF SIC was 2.3 %, SIE was $20,000km^2$ lower than NT.
