  • Background: The Ministry of Public Health (MOPH) in Lebanon provides cancer drugs free of charge for uninsured patients who account for more than half the total case-load. Other categories of cancer care are subsidized under more stringent eligibility criteria. MOPH's large database offers an excellent opportunity to analyze the cost of cancer treatment in Lebanon. Materials and Methods: Using utilization and spending data accumulated at MOPH during 2008-2013, the cost to the public budget of cancer drugs was assessed per case and per drug type. Results: The average annual cost of cancer drugs was 6,475$ per patient. Total cancer drug costs were highest for breast cancer, followed by chronic myeloid leukemia (CML), colorectal cancer, lung cancer, and Non-Hodgkin's lymphoma (NHL), which together represented 74% of total MOPH cancer drug expenditure. The annual average cancer drug cost per case was highest for CML ($31,037), followed by NHL ($11,566). Trastuzumab represented 26% and Imatinib 15% of total MOPH cancer drug expenditure over six years. Conclusions: Sustained increase in cancer drug cost threatens the sustainability of MOPH coverage, so crucial for socially vulnerable citizens. To enhance the bargaining position with pharmaceutical firms for drug cost containment in a small market like Lebanon, drug price comparisons with neighboring countries which have already obtained lower prices may succeed in lowering drug costs.

  • Although educational facilities are important considerations from a social equity standpoint, the educational environment generally found in rural areas has been continuously deteriorated due to numerous social problems following birth rate declines and school abolition numbers. The purpose of this study is to evaluate the commuting accessibility changed by the abolition of elementary schools. The village-level accessibility is assessed following physical accessibility criteria based on legally defined commuting distances. Moreover, this study shows the implications of commuting accessibility by school district zones through comparing the minimum commuting distances with real commuting distance. The target area is the Gangwon state, the coverage area in which for schools is the widest in South Korea (44.1 ㎢/school). We evaluated the commuting distances to the nearest elementary school for 992 rural villages and 74 urban villages. Of the 1,066 villages in urban and rural areas evaluated, most of the villages (about 77%) are exposed to commutes to elementary schools were the marginal distance is over 1,000 meters. The annual rate of increase of commuting distance for 1,066 villages in Gangwon state has steadily increased by 2.8%p since 2000, average commuting distance from 2.8 km to 3.6 km. By the designation of the school districts, elementary school students have to go to a faraway school located within administrative district, even though there is a school nearby. It is expected that the results of this study will be made use of as basic data for the establishment of policies such as a reasonable basis for closing schools measurements considering the locally unique environment.

  • Purpose: The aim of this study was to evaluate the clinical efficiency of the subepithelial connective tissue graft (SCTG) with and without plasma rich in growth factor (PRGF) in the treatment of gingival recessions. Methods: Twenty bilateral buccal gingival Miller's Class I and II recessions were selected. Ten of the recessions were treated with SCTG and PRGF (test group). The rest ten of the recessions were treated with SCTG (control group). The clinical parameters including recession depth (RD), percentage of root coverage (RC), mucogingival junction (MGJ) position, clinical attachment level (CAL), and probing depth (PD) were measured at the baseline, and 1 and 3 months later. The data were analyzed using the Wilcoxon signed rank and Mann-Whitney U tests. Results: After 3 months, both groups showed a significant improvement in all of the mentioned criteria except PD. Although the amount of improvement was better in the SCTG+PRGF group than the SCTG only group, this difference was not statistically significant. The mean RC was $70.85{\pm}12.57$ in the test group and $75.83{\pm}24.68$ in the control group. Conclusions: Both SCTG+PRGF and SCTG only result in favorable clinical outcomes, but the added benefit of PRGF is not evident.

  • The focal mechanism of the Hongsung Earthquake (1978. Oct. 7, M$_L$=5.0, Latitude 36.62N, Longitude 1 26.67E) was evaulated using the polarity distribution of the P-Waveforms. Through the non-linear computer process, the compatibility of polarity distributions of the 9 P-Waveforms observed at teleseismic distances from the Hongsung Earthquake epicenter was investigated to those of the focal mechanism determined from the varying strike, dip and rake angles. The resultant values for the strike and dip angle of the principal fault plane, which apparently matches very well the sunface lineament of the Hongsung region, are determined to be about 247 degree and 78 degree with uncertainties, respectively. However, the rake angle of the focal mechanism has wide range of 40 degree to 160 degree, which is mainly due to the poor coverage of the azimuthal angle of the observed seismic stations. Due to the consistency of principal stress axes, the resultant focal mechanism could support the current stress regime of that region, which may be caused by subduction of the Pacific Plate under the Eurasia Plate along the Japan Trench. It also provides information of seismic source characteristics of the part of the Korean Peninsula for aseismic design criteria such as Site Specific Response Spectrum and Strong Ground Motion Time History for the nuclear power plants and related nuclear waste disposal facility sites.

  • In the Seoul Metropolitan City, the PM(pariculate matter) application used by the citizens provides the PM concentration of the nearest monitoring stations located on the PM monitoring stations. Currently, the selecting method of the PM monitoring network considered by the Ministry of Environment is based on considering the monitoring station distribution and population density only. In this study, we analyzed the distance between PM monitoring station and the administrative center point in addition to the above considerations. The number of test sites was verified and the range of coverage of each monitoring stations was indicated by using the Voronoi algorithm and hexagon grid. The spatial position problem of the PM monitoring station was suggested by spatial data analysis. The variables of spatial data analysis are single-family houses, apartments, $1^{st}$ class neighborhood, $2^{nd}$ class neighborhood, garbage disposal plant, hazardous material disposal facility, factory, and the density map. The analysis result of the selection criterion considering the additional variables for new PM monitoring stations was presented, in addition to the selection criteria provided by the Ministry of Environment.

  • Objectives: Access to maternal and neonatal care services (MNCS) is an important goal of health policy in developing countries. In this study, we proposed a 3-level hierarchical location-allocation model to maximize the coverage of MNCS providers in Iran. Methods: First, the necessary criteria for designing an MNCS network were explored. Birth data, including gestational age and birth weight, were collected from the data bank of the Iranian Maternal and Neonatal Network national registry based on 3 service levels (I, II, and III). Vehicular travel times between the points of demand and MNCS providers were considered. Alternative MNCS were mapped in some cities to reduce access difficulties. Results: It was found that 130, 121, and 86 MNCS providers were needed to respond to level I, II, and III demands, respectively, in 373 cities. Service level III was not available in 39 cities within the determined travel time, which led to an increased average travel time of 173 minutes to the nearest MNCS provider. Conclusions: This study revealed inequalities in the distribution of MNCS providers. Management of the distribution of MNCS providers can be used to enhance spatial access to health services and reduce the risk of neonatal mortality and morbidity. This method may provide a sustainable healthcare solution at the policy and decision-making level for regional, or even universal, healthcare networks.

  • Cranioplasties are common procedures in plastic surgery. The use of tissue expansion (TE) in staged cranioplasties is less common. We present two cases of cranioplasties with TE and systematically review literature describing the use of TE in staged cranioplasties and postoperative outcomes. A systematic review was performed by querying multiple databases. Eligible articles include published case series, retrospective reviews, and systematic reviews that described use of TE for staged bony cranioplasty. Data regarding study size, patient demographics, preoperative characteristics, staged procedure characteristics, and postoperative outcomes were collected. Of 755 identified publications, 26 met inclusion criteria. 85 patients underwent a staged cranioplasty with TE. Average defect size was 122 cm2, and 30.9% of patients received a previous reconstruction. Average expansion period was 14.2 weeks. The most common soft tissue closures were performed with skin expansion only (75.3%), free/pedicled flap (20.1%), and skin graft (4.7%). The mean postoperative follow-up time was 23.9 months. Overall infection and local complication rates were 3.53 and 9.41%, respectively. The most common complications were cerebrospinal fluid leak (7.1%), hematoma (7.1%), implant exposure (3.5%), and infection (3.5%). Factors associated with higher complication rates include the following: use of alloplastic calvarial implants and defects of congenital etiology (p = 0.023 and 0.035, respectively). This is the first comprehensive review to describe current practices and outcomes in staged cranioplasty with TE. Adequate soft tissue coverage contributes to successful cranioplasties and TE can play a safe and effective role in selected cases.

  • This paper presents an optimal path planning strategy for aerial searching and surveying of a user-designated area using multiple Unmanned Aerial Vehicles (UAVs). The method is designed to deal with a single unseparated polygonal area, regardless of polygonal convexity. By defining the search area into a set of grids, the algorithm enables UAVs to completely search without leaving unsearched space. The presented strategy consists of two main algorithmic steps: cellular decomposition and path planning stages. The cellular decomposition method divides the area to designate a conflict-free subsearch-space to an individual UAV, while accounting the assigned flight velocity, take-off and landing positions. Then, the path planning strategy forms paths based on every point located in end of each grid row. The first waypoint is chosen as the closest point from the vehicle-starting position, and it recursively updates the nearest endpoint set to generate the shortest path. The path planning policy produces four path candidates by alternating the starting point (left or right edge), and the travel direction (vertical or horizontal). The optimal-selection policy is enforced to maximize the search efficiency, which is time dependent; the policy imposes the total path-length and turning number criteria per candidate. The results demonstrate that the proposed cellular decomposition method improves the search-time efficiency. In addition, the candidate selection enhances the algorithmic efficacy toward further mission time-duration reduction. The method shows robustness against both convex and non-convex shaped search area.
