The Study for Apical Pulse Measurement Technique Through Hospitalized Children (입원한 영유아의 심첨 맥박 측정 방법에 관한 연구)

  • The purpose of this study was to determine the most accurate technique measuring the apical pulse rate, using three counting duration 15, 30 and 60 seconds, and two methods start ‘0’ and start ‘1’. The instrument used in the study was the EKG monitor, stethoscope and stopwatch. Data was analyzed by utilizing SPSSWIN program. General characteristics of the subjects were analyzed by frequency, percentile, mean, SD. The subject of this research is made up of 46 children and 20 nurses. The children were infants, & under the age of 5. They were hospitalised in PICU & NICU in 2 tertiary hospitals in seoul from Jan. 1. 1998 to Sep. 10. 1998. The measurement of starting 1 & measurement of starting ‘0’ used the T-test to find out the measurement error. Apical pulse duration of 15, 30, 60 seconds were used to find out measurement error, the measurement error depend on experience of Nurse were analyzed by using ANOVA. The result of this study are as follows. 1. When comparing the starting poin of apical pulse 0&1, starting with 1 the measurement error is less, but not statiscally significant. 2. When counting the apical pulse by 15, 30,60 sec. ; 60 seconds counting duration was more accurate, but not statistically significant. 3. The mean of measure error ; Group under 100/min, is 10.33 ; from 100 re 119/min, is 8.30 ; from 120 to 139/min, is 4.76 ; from 140 to 159/min, is 6.09 ; above 160, is 17.83. The differences of these groups are statistically significant. When 60sec were counted, under 140/min the mean of measurement error is 3.4. Also when 30 seconds were counted from 140/min to 159/min the measurement error is 7.14, above 160/min the measurement error is 16.4. That measurement mean is the smallest than the other durations. During the 15 sec. count the measurement error was the largest of them all. 4. By the experience of the nurses, the apical pulse count measurement error was discovered. Under a year experience this measurement error was the largest(11.09), 1 year to under 3 years, the error is the smallest(4.86). 3 year to under 6 years the error is 8.33, 5 years above the error is 6.11 but this is not statistical significant. Under a year experience when counting 15, 30, 60 seconds the error is the largest. The group of the nurses from a year to under 3 years, the measurement error is the smallest of all the groups. The result of the study is to determine the technique measuring the apical pulse rate, Hargest (1974), starting point ‘0’ is not proved. When the pulse rate increases the 30 sec measurement rate is accurate. Under 140/min the 60 sec measurement rate is the most accurate. Depending on the nurses experiences, there is a variable difference to the apical pulse rate measurement. Especially new nurses training courses should enforce the children’s pulse rate count and the basic vital signs.

Impact of Seasonal Conditions on Quality and Pathogens Content of Milk in Friesian Cows

  • Heat stress negatively affects milk quality altering its nutritive value and cheese making properties. This study aimed at assessing the impact of seasonal microclimatic conditions on milk quality of Friesian cows. The study was carried out in a dairy farm from June 2013 to May 2014 at Beni-Suef province, Egypt. Inside the barn daily ambient temperature and relative humidity were recorded and used to calculate the daily maximum temperature-humidity index (mxTHI), which was used as indicator of the degree of heat stress. The study was carried out in three periods according to the temperature-humidity index (THI) recorded: from June 2013 to September 2013 (mxTHI>78), from October 2013 to November 2013 (mxTHI 72-78) and from December 2013 to April 2014 (mxTHI<72). Eighty Friesian lactating dairy cows were monitored in each period. The three groups of cows were balanced for days in milk and parity. Milk quality data referred to somatic cell count, total coliform count (TCC), faecal coliform count (FCC), Escherichia coli count, percentage of E. coli, and Staphylococcus aureus, percentage of fat, protein, lactose, total solid and solid non-fat. Increasing THI was associated with a significant decrease in all milk main components. An increase of TCC, FCC, and E. coli count from mxTHI<72 to mxTHI>78 was observed. In addition, the isolation rate of both S. aureus and E. coli increased when the mxTHI increased. The results of this study show the seriousness of the negative effects of hot conditions on milk composition and mammary gland pathogens. These facts warrant the importance of adopting mitigation strategies to alleviate negative consequences of heat stress in dairy cows and for limiting related economic losses.

Serum levels of interleukin-6 and interleukin-8 as diagnostic markers of acute pyelonephritis in children

  • Purpose: Early diagnosis and treatment of acute pyelonephritis in children is of special importance in order to prevent serious complications. This study was conducted to determine the diagnostic value of serum interleukin (IL)-6 and IL-8 in children with acute pyelonephritis. Methods: Eighty-seven patients between 1 month to 12 years old with urinary tract infection (UTI) were divided into 2 groups based on the result of 99m-technetium dimercapto-succinic acid renal scan: acute pyelonephritis (n=37) and lower UTI (n=50) groups. White blood cell (WBC) count, neutrophil (Neutl) count, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP) concentration, platelet count, and serum IL-6 and IL-8 concentrations of both groups were measured and compared. Results: There was a significant difference between two groups regarding WBC count, Neutl count, ESR, and CRP concentration (P<0.05). In addition, the difference between the two groups regarding serum IL-6 and IL-8 concentrations was not significant (IL-6, 60 and 35.4 pg/mL and IL-8, 404 and 617 pg/mL, respectively). The sensitivity and specificity of serum IL-6 and IL-8 for diagnosis of acute pyelonephritis were 73%, 42% and 78%, 32%, respectively. Sensitivity, specificity, negative and positive predictive values of serum IL-6 and IL-8 were less than those of acute phase serum reactants such as CRP. Conclusion: This study showed that there was no significant difference between acute pyelonephritis and lower UTI groups regarding serum IL-6 and IL-8 levels. Therefore, despite confirming results of previous studies, it seems that IL-6 and IL-8 are not suitable markers for differentiating between acute pyelonephritis and lower UTI.

Minimum LQI based On-demand Routing Protocol for Sensor Networks (Minimum LQI 기반의 On-demand 센서 네트워크 라우팅 프로토콜)

  • A number of on-demand routing protocols for sensor networks have been proposed yet. However, the majority of proposed on-demand routing protocols for sensor networks are not suitable for a relatively poor wireless environment and sensor applications requiring reliable data transmission due to using a hop-count metric for their protocols. In this paper, we proposed a minimum LQI(Link Quality Indicator) based on-demand sensor network routing protocol that is suitable for a relatively poor wireless environment and implemented the proposed routing protocol on a TinyOS. We also compared the implemented protocol with typical hop count based routing protocol by carrying out performance experiments on a multi-hop testbed. The results from these experiments showed that the successful transmission rate of the proposed routing protocol is higher than that of typical hop count based routing protocol over a poor wireless link.

Pulsed Photostimulated Luminescence (PPSL) of Irradiated Importation Sesame and Perilla Seeds (방사선 조사된 수입 참깨, 들깨의 광여기 발광)

  • A study was carried out to establish a detection method for imported sesame and perilla seeds using pulsed photostimulated luminescence (PPSL) whether they have been irradiated or not. Samples were packed in polyethylene bags and irradiated at 1, 5, and 10 kGy with a dose rate of 10 kGy/h. The whole sample of sesame and perilla and the minerals separated from seeds were introduced in the sample chamber and measured PPSL photon counts for 60 and 120 s. The PPSL photon counts of samples increased with increasing irradiation dose and showed a higher correlation coefficients in separated minerals than in sesame and perilla seeds themselves. These results suggest that imported sesame and perilla seeds be possibly detected by both their whole sample and separated minerals by PPSL measurements.

  • Improving TCP performance has long been the focus of many research efforts in 802.11 wireless networks study. Hop count and Round Trip Time (RTT) are the critical sources which serious affect the TCP performance on end to end connection. In this paper, we analytical derived the affection and based on the analysis we propose TCP should Change its Expected Value (TCP-CEV) when hop count and RTT change by setting a reasonable CWND change rate to improve the performance. The proposed scheme is applicable to a wide range of transport protocols using the basic TCP mechanism, and the protocol behavior is analytically tractable. We show that our simple strategy improves TCP performance at least over 12% in a chain topology, 4.9% in a grid topology and improve the TCP convergence.

The Retrospective Study on Modified Fasting Therapy in Inpatients (절식요법을 시행한 입원환자의 후향적 연구)

  • Objectives : The purpose of this study was to investigate the effects of modified fasting therapy on body composition, blood cell count and blood chemistry in inpatients. Methods : The modified fasting therapy program consists of three phases, a period of reducing food intakes, fasting and refeeding. Body weight, body composition, and the degree of obesity were estimated in before fasting therapy and after refeeding. Blood cell count and blood chemistry were estimated in before fasting therapy, before refeeding and after refeeding. Results : After modified fasting therapy, body weight, body fat mass, percent body fat, body mass index(BMI), body matabolic rate(BMR) and muscle mass was decreased significantly. WBC count and platelet count decreased significantly, RBC count and hemoglobin increased significantly and hematocrit was not changed significantly but within normal limit. AST and ALT increased significantly, especially patients group over BMI 25 within normal liver function. This study was not founded correlation between decreased muscle mass and increased liver function test. ALP, BUN, Total-cholesterol, and triglyceride decreased significantly. Creatinine increased significantly, but within normal limit. Sodium was not changed significantly, potassium decreased significantly but within normal limit. Conclusions : These results suggest that modified fasting therapy will be beneficial if several measures complement.

Laboratory Markers Indicating Gastrointestinal Involvement of Henoch-Schönlein Purpura in Children

  • Purpose: To determine clinically useful biochemical markers reflecting disease activity and/or gastrointestinal (GI) tract involvement in Henoch-$Sch{\ddot{o}}nlein$ purpura (HSP). Methods: A total of 185 children with HSP and 130 controls were included. Laboratory data indicating inflammation, standard coagulation, and activated coagulation were analyzed for the HSP patients, including measurements of the hemoglobin level, white blood cell (WBC) count, absolute neutrophil count (ANC), platelet count, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP) level, prothrombin time, activated partial thromboplastin time, and fibrinogen, D-dimer, and fibrin degradation product (FDP) levels. The clinical scores of the skin, joints, abdomen, and kidneys were assessed during the acute and convalescence phases of HSP. Results: The WBC count, ANC, ESR, and CRP, fibrinogen, D-dimer, and FDP levels were significantly higher in the acute phase compared with the convalescent phase of HSP (p<0.05). The total clinical scores were more strongly correlated with the D-dimer (r=0.371, p<0.001) and FDP (r=0.369, p<0.001) levels than with inflammatory markers, such as the WBC count (r=0.241, p=0.001), ANC (r=0.261, p<0.001), and CRP (r=0.260, p<0.001) levels. The patients with GI symptoms had significantly higher ANC (median [interquartile range], 7,138.0 [4,446.4-9,470.0] vs. 5,534.1 [3,263.0-8,153.5], p<0.05) and CRP (0.49 [0.15-1.38] vs. 0.23 [0.01-0.67], p<0.05), D-dimer (2.63 [1.20-4.09] vs. 1.75 [0.62-3.39]), and FDP (7.10 [0.01-13.65] vs. 0.10 [0.01-7.90], p<0.05) levels than those without GI symptoms. Conclusion: D-dimer and FDPs are more strongly associated with disease activity and more consistently reflect GI involvement than inflammatory markers during the acute phase of HSP.

Enhancing Raw Bovine Milk Quality using Ultraviolet-C (UV-C) Irradiation: A Microbial and Lipid Peroxidation Study

  • This study investigated the efficacy of ultraviolet-C (UV-C) irradiation in enhancing the quality of raw bovine milk by targeting microbial populations and lipid peroxidation, both of which are key factors in milk spoilage. We categorized the raw milk samples into three groups based on initial bacterial load: low (<3 Log 10 CFU/mL), medium (3-4 Log 10 CFU/mL), and high (>4 Log 10 CFU/mL). Using a 144 W thin-film UV-C reactor, we treated the milk with a flow rate of 3 L/min. We measured the bacterial count including standard plate count, coliform count, coagulase-negative staphylococci count, and lactic acid bacteria count and lipid peroxidation (via thiobarbituric acid reactive substances assay) pre- and post-treatment. Our results show that UV-C treatment significantly reduced bacterial counts, with the most notable reductions observed in high and medium initial load samples (>4 and 3-4 Log 10 CFU/mL, respectively). The treatment was particularly effective against coliforms, showing higher reduction efficiency compared to coagulase-negative staphylococci and lactic acid bacteria. Notably, lipid peroxidation in UV-C treated milk was significantly lower than in pasteurized or untreated milk, even after 72 hours. These findings demonstrate the potential of UV-C irradiation as a pre-treatment method for raw milk, offering substantial reduction in microbial content and prevention of lipid peroxidation, thereby enhancing milk quality.

Bacteriological Survey for Food/Food Contacting Surfaces in Large Grocery Stores in Korea

  • A bacteriological survey for 20 large grocery stores (M 1 to M20) in Korea was investigated for one year. The average detection rate of Esherichia coli was $22\%$ (166/763) for 7 kinds of ready-to-eat food through the year, where each grocery store and each type of food showed different detection rates. Eleven grocery stores showed lower detection rates, while 9 grocery stores showed a higher than average rate. Especially, M3 showed a rate that was twice as high as the average and one which was 7 times higher than M14, which had the lowest rate of $6\%$ E. coli detection. The detection rate for each type of food was: $38\%$ (41/109) for Kimbop, $31\%$ (34/109) for vegetable salad, $19\%$ (21/109) for bean-curd, $18\%$ (20/109) for the cooked materials used in making Kimbop, $17\%$ (19/109) for Hoe (sliced raw fish) and Sushi (Japanese vinegared rice delicacies), and $11\%$ (12/109) for cooked pork hock. During the summer, the E. coli detection rate averaged $43\%$ (71/166), which was twice as high as other seasons. Most (89/100) of the food contacting surfaces contained more than the critical limit $(1.3\;log_{10}\;CFU/10cm^2)$ of aerobic viable cell counts (AVC). The $log_{10}$ AVC and $log_{10}$ coliform count (CC) of 218 meat samples (beef, pork, and chicken) ranged between 4.6-7.1 CFU/g and 1.9-6.4 CFU/g, except for 41 meat samples $(19\%)$ which were found to contain no coliform. There was a definite correlation between the $log_{10}$ AVC and $log_{10}$ CC, and the values of $log_{10}$ CC made a more accurate straight than the $log_{10}$ AVC, which are variable. From these results, it is suggested that a detection rating of less than 2.1 CFU/g of $log_{10}$ CC (correspond to 5.0 CFU/g of $log_{10}$ AVC) is the critical point of freshness, and a rating of more than 6.3 CFU/g of $log_{10}$ CC (correspond to 7.0 CFU/g of $log_{10}$ AVC) can be considered an initial spoilage point.
