The Analysis of Corneal Patterns in Korean 20s by Corneal Topography and Corneal Radii by Astigmatic Degree (각막지형도를 이용한 20대의 각막형상 및 난시도에 따른 각막곡률반경의 분석)

  • Purpose: In order to provide the fundamental information for the design development of RGP lenses and its clinical prescription, the corneal types of Korean twenties were analyzed according to corneal shapes, astigmatic degree, and the certain distance from the corneal apex. Methods: Corneal types of total 252 eyes in 20s were firstly classified, then their corneal radii from the certain distance from the corneal apex were measured by corneal topography and further analyzed based on the astigmatic degree. Results: Korean 20s' corneal types were classified as 14.3%(36 eyes) of round, 31.3%(79 eyes) of oval, 28.6%(72 eyes) of symmetric bow tie, 17.5%(44 eyes) of asymmetric bow tie, 8.3%(21 eyes) of irregular shapes. The round and oval typed corneas had mild astigmatic degree whereas the higher astigmatic degree in symmetric and asymmetric bow tie typed corneas were shown. The relative corneal radii of round and oval typed corneas at each distance from corneal apex were shown to consistently increase regardless of astigmatic degrees when they measured at certain distances from the corneal apex. However, the relative corneal radii of symmetric and asymmetric bow tie typed corneas within 1.0-1.5mm from the corneal apex were decreased, which showed steeper slope than it within 1.0 mm and somewhat different based on astigmatic degrees. Bigger change of corneal radii outer 3.5 mm from the corneal apex in symmetric bow tie typed corneas with astigmatism of 1.50-2.00 D and 2.25-2.75 D appeared. Conclusions: The consideration of radial change from the central cornea to peripheral cornea is necessary for manufacturing RGP lens and its prescription since they showed different change in corneal radii by corneal patterns and astigmatic degrees.

Analysis on the corneal thickness and anterior chamber depth of the Keratoconus using Pentacam

  • Purpose: To investigate the corneal thickness and anterior chamber depth (ACD) of eyes with Keratoconus in the Korean population with the Pentacam. Methods: The subjects consisted of 84 eyes from Keratoconic adults aged 7-59 years during 2010. The thinnest area, apex zone, and pupil centre of the corneal thickness were measured using the Pentacam pachymetry. ACD value was also measured with Pentacam. Results: There was a statistically significant relationship between thickness of the cornea at the apex area and the pupil centre (p=0.0001). However, there was no statistical difference (p>0.5) in the mean thickness of thinnest area, apex zone, pupil centre of cornea between right eye and left eye. Also, correlation between ACD and corneal thickness in all subjects had no statistical differences (p>0.05) in all subjects. Conclusion: These results suggested that the regional thickness of cornea and ACD with Pentacam can provide correct and useful diagnostic information of the morphology of Keratoconus for the RGP contact lens and diagnosis of abnormal corneal refraction surgery.

Analysis of the Central Corneal Thickness and Anterior Chamber Depth of Eyes in Korean Young Population using the Pentacam (Pentacam을 이용한 한국 청소년 집단에서 각막의 중심두께와 전방깊이 분석)

  • Purpose. This study was to investigate the analysis of the central corneal thickness and anterior chamber depth of eyes in Korean Young Population using the Pentacam topography. Methods. The subjects consisted of 126 eyes from normal cornea of Korean aged 7-12($8.857{\pm}1.501$) years during 2009. The thinnest location, pachy apex, and pupil central region of the cornea thickness was measured using the Pentacam pachymetry. The anterior chamber depth of normal corneas was measured with Pentacam. Results. The thinnest location of corneal thickness was $547.413{\pm}37.893$um. The pachy apex of cornea thickness was $552.103{\pm}36.016$um. Also, the pupil center of corneal thickness was measured $552.143{\pm}36.418$um. On the other hand, the mean thickness in the central cornea was a statically significant relationship(t-test, p=0.0002) between pachy apex and thinnest zone. Also the mean thickness of the central cornea was a statically significant correlation(pearson p=0.0001) among the pupil center, pachy apex and thinnest zone. The anterior chamber depth was $3.137{\pm}0.308$mm. The mean depth in the anterior chamber of cornea was a statically relationship(t-test, p>0.05) between OD and OS. Conclusions. The results of this clinical study suggested that the analysis of the central corneal thickness and anterior chamber depth using Pentacam in Korean young population can provide the useful morphological information in diagnosis of cornea for the contact lens fitting and corneal refraction surgery.

  • Purpose: To represent the shape of toric corea in the elliptical function for the determination of curvature distribution and lacrimal thickness between cornea and contact lens when the lens is fitted. Methods: Topography measurements of corneal curvature and curvature equation derived from the assumed elliptical function were evaluated using the Excel program which included the necessary equation derived. Results: Mathematical expressions for the cornea whose ribbon shaped-topography image, in which the center does not coincide with the corneal apex, can be determined. Conclusions: For the application where the higher accuracy on the cornea is not required, such as higher order aberration, the cornea cal be expressed in the simple elliptical function.

The Change in Corneal Eccentricity on the Correction of Refractive Error using Reverse Geometry Lens (역기하렌즈(Reverse Geometry Lens)의 굴절교정시 각막 편심률(Eccentricity)의 변화)

  • Purpose: In this study we investigated the correlation between the effect of myopia correction and the change of corneal eccentricity using reverse geometry lens. Methods: The 23 students (46 eyes) continuously wearing reverse geometry lens during 3 months were divided into Group I and Group II by different parameter fitting methods of wearing Reverse Geometry Lens. We measured a corneal eccentricity for Group I and Group II at $10^{\circ}$, $20^{\circ}$, and $30^{\circ}$ positions from corneal apex before wearing reverse geometry lens, 1 week, 1 month, and 3 months after wearing reverse geometry lens. We also measured an uncorrected visual acuity, a spherical equivalent, and a corneal radius and analyzed the correlation between them and the change of corneal eccentricity using statistical significance test. Results: There were the statistical significances of a change of corneal eccentricity (p=0.03, t=-2.29) for Group I and Group II at 10 position from corneal apex in a week after wearing reverse geometry lens, but were not those (p>0.05) in 1 month, and 3 months after wearing reverse geometry lens. There were the statistical significances of correlation between the change of corneal eccentricity and a corrected visual acuity, and a corneal radius, respectively. Particularly, the high correlation between the change of corneal eccentricity and a corrected visual acuity (r=-0.36, p=0.00, t=6.5), and a spherical equivalent (r=-0.72, p=0.00, t=-70.5) for Group II in a week after wearing reverse geometry lens showed. Conclusions: We knew from these results that the high correlation between the effect of myopia correction and the change of corneal eccentricity in a week after wearing reverse geometry lens represented.

Interspecies comparative morphological evaluation of the corneal epithelial stem cell niche: a pilot observational study

  • Background: The corneal and limbal morphology relevant to corneal epithelial maintenance in ten different species was examined using histological methods. Objectives: The presence of a Bowman's layer, limbal epithelial cell, and superficial stromal morphology was examined in the following species to evaluate the differences in corneal thickness and epithelium: Java sparrows, frogs, macaws, spoonbills, red pandas, penguins, horses, Dobermans, orangutans, and humans. Methods: Corneal sections (4 ㎛) were obtained from ten ocular globes from three different animal classes: Aves, Amphibia, and Mammalia. All sections were stained with hematoxylin and eosin and periodic acid-Schiff reaction. After microscopy, all stained slides were photographed and analyzed. Results: Significant morphological differences in the corneal and limbal epithelia and their underlying stroma between species were observed. The number of corneal epithelial cell layers and the overall corneal epithelial thickness varied significantly among the species. The presence of a Bowman's layer was only observed in primates (orangutans and humans). Presumed supranuclear melanin caps were noted in four species (orangutans, macaws, red pandas, and horses) in the limbal basal epithelial layer (putative site of corneal epithelial stem cells). The melanin granules covered the apex of the cell nucleus. Conclusions: Supranuclear melanin capping has been described as a process within the epidermis to reduce the concentration of ultraviolet-induced DNA photoproducts. Similarly, there may be a relationship between limbal stem cell melanin capping as a protective mechanism against ultra-violet radiation.

A Relationship between Corneal Eccentricity and Stable Centration of RGP Lens on Cornea (각막 이심률과 RGP 렌즈의 중심안정위치와의 상관관계)

  • Purpose: The present study was performed to investigate a relationship between the stable centrations of spherical RGP lens and aspherical RGP lens on cornea and corneal eccentricity. Methods: Two RGP lenses with different designs were fitted in alignment, steep or flat on total 84 eyes having corneal eccentricity of 0.28~0.78. The stable centration of lenses on cornea was analyzed by taking photographs with a high-speed digital camera. Results: The stable centrations of spherical and aspherical RGP lenses in horizontal direction were decentrated to temporal side. More centration to median side was shown when corneal eccentricity was larger. The difference between the stable centrations of spherical and aspheric RGP lenses according to corneal eccentricity was bigger when the fitting state was flatter. The difference in the stable centrations of aspherical RGP lens was smaller than that of spherical RGP lens regardless of fitting status. The stable centrations of spherical and aspherical RGP lenses in vertical direction were located below corneal apex regardless of fitting status however, there was no significant difference analyzed by the variation of corneal eccentricity. However, there were many cases that RGP lenses were in upper eyelid with increasing corneal eccentricity. Conclusions: The consideration of corneal eccentricity is required for RGP lens fitting and manufacturing aspherical RGP lens since the stable centration of spherical RGP lens as well as aspherical RPG lens' centration was changed depending on corneal eccentricity.

A MECHANICAL INVESTIGATION OF CORNEAL REFRACTIVE SURGERIES AND PROPOSITION OF NEW TECHNIQUES (각막굴절수술의 역학적 고찰 및 새로운 기법의 시도)

  • This study investigated the effects of mechanical factors involved in several corneal refractive surgeries on the surgical outcomes. Then we proposed possible new techniques from the mechanical point of a view utilizing finite element method. The models studied are: circumferential keratetomy, combination of excimer laser photorefractive keratectomy and circumferential keratotomy for myopia treatment, arcuate keratotomy for astigmatism treatment. The cornea was assumed to be nonlinear elastic and almost incompressible material as the most soft tissue in the human body. In the circumferential keratotomy the effect of the incision location was investigated. The angle and location of the incision were varied to predict the surgical outcomes in the arcuate keratotomy. The finite element analysis results showed that the location of incision was a critical factor affecting the surgical outcomes in the circumferential keratotomy. In the combination of the excimer laser photorefractive keratectomy and circumferential keratotomy, it was predicted that the circumferential can increase or decrease the refractive power depending on the incision location or it can be used to adjust the overcorrection of undercorrection. In the arcuate keratotomy for astigmatism, the most diopter changes were predicted when the location and the angle of the incision were 3.0mm from the apex and $90^{\circ}$, respectively. In the arcuate keratotomy, the effects of an incision were studied within the incision area as well as outside the incision area. Also, the arcuate keratotomy with two incisions located on the opposite area of the cornea was also studied. As a conclusion, the finite element method is a useful technique in the area of corneal refractive surgeries to develop new techinques.

A Mechanical Model of Excimer Laser Surgery (엑시머 레이저 수술의 역학적 모델)

  • A finite element-based computer simulation of excimer laser susery was conducted to study some factors on the surgery. In particular, the radius of curvature at the apex of the cornea was examined under various surgical conditions. Corneal tissue was assumed to be a nearly incompressible, linear elastic, homogeneous, isotropic material under very small deformation. The geometry of the human cornea was taken from the experimental data[1]. The simulation utilized ANSYS(Swanson Analysis System Inc.Rev.4.4A). In this study, the major factors which affect the outcomes of the excimer laser surgery were investigated. First, two patterns of surgery with various surgery thickness(40-70micrometers) were examind. The pattern#1 describes the meridian from the apex to the edge of the surgery area to be straight. And the corresponding meridian of the pattern 2 can be expressed as a quardratic function. The results show that the pattern #2 is more realistic and effective. Then, the effects of other factors were investigated based on the pattern #2. Other factors are:various diameters of the surgical area (3-8 milimeters), Young's modules(3.5-4.5MPa), and depth of surgery at the apex(40-70micrometers). Compared with the computer simulation of the radial keratotomy surgery[2], the excimer laser surgery was proven to be more effective in treating myopia patients. In conculusion, the results of the simulation are qualitative agreement with clinical experience[3] indicating the potential of the finite element model of the surgery as a guideline to the surgeon before actual surgery.

The Investigation and Development of Astigmatism Correction Treatments by Finite Element Method and Animal Experiments (유한요소법과 동물실험을 통한 난시교정술의 고찰 및 개발)

  • The purpose of this study is to investigate the effects of various factors in keratotomy for astigmatism correction on surgical outcomes by finite element method as well as animal experiments. Three kinds of surgical techniques were mechanically investigated : arcuate, straight, and inverse arcuate keratotomy. Among the three techniques the arcuate keratotomy is the most popular one while the other two techniques are being investigated in this area. The arcuate keratotomy was found to be more controllable and effective in reducing the refractive power than the others. In arcuate keratotomy it was found most effective when the incision was located in the middle position between the apex and the edge of the cornea from the results of experiment as well as finite element study. Regarding to the range of the corneal incision in arcuate keratotomy, the incision angle of 90$^{\circ}$ was found th be most effective in reducing refractive power than other angles even it was incised up to 150$^{\circ}$. Therefore, it was concluded that 90$^{\circ}$ of incision angle results in the largest decrease in refractive power in arcuate keratotomy. However, other important findings were that the effect of the surgery decreased with time so the visco-effect of the cornea and auto-healing process. Therefore, these factors should be considered in future studies.
