폴리페놀 및 항산화 관점에서의 쌍화탕 및 단미혼합쌍화탕의 동등성 (Equivalence of Traditional and Individual Preparation of Ssanghwa-tang in terms of Polyphenol Contents and Radical Scavenging Activity)

  • Traditional medicines usually use combinational formula that is prepared by mixing different varieties of medicinal herbs and boiling them in water to yield a decoction. In recent years a modified method has been proposed and practiced wherein the individual herbs are boiled with water separately and later these extracts are mixed together for use. This practice has lead to a constant controversy concerning the chemical and biological equivalence between the preparations arriving out of these two methodologies. In this research, we attempted to study the effects of traditional and individual preparation of Ssanghwa-tang (ST) to evaluate their equivalence in terms of its antioxidant activity and polyphenol content. The study results indicate that the polyphenol contents as well as the extraction yields of these two preparations were very similar to each other.

  • 2006년 3 월부터 보건복지부에서는 비급여 항목으로 환자의 부담이 컸던 환자식을 급여화하기로 함에 따라 입원환자에 제공하는 병원에서 제공하는 환자식 가격과 관련한 논란이 심화되고 있다. 본 연구는 과학적 방법으로 병원 모집단을 층화 추출한 후 71 개라는 많은 병원 자료를 이용해 급식 유형별 원가와 적정가격을 산정한 우리나라 최초의 연구이다. 활동원가계산과 상대가치 개념을 사용해 급식 유형별 원가를 산출 한 후, 급식 유형별 원가에 영향을 미치는 변수를 알아보았다. 환자식 가격에 영향을 미치는 가장 의미있는 변수는 급식종사인력과 급식 수이므로 보험수가에서는 이들 변수를 고려하여야 한다.

자궁경부무력증 진단을 받고 내원한 환자 임상 1례 (A Case Report of a Cervical Incompetence Patient)

  • Objectives: This study is to report the effect of oriental medicine on a cervical incompetence patient. Methods: The effectiveness of surgical cerclage in the cervical incompetence have had prolonged controversy. Herbal medicine was given to a cervical incompetence patient whom was suggested surgical cerclage. The patient was a 40-year-old female and treated with Dojeukgangki-tang gagambang. Results: After oriental herbal treatment, the clinical symptom of a cervical incompetence was improved and maintained, the patient was delivered in 38 weeks through cesarian section. Conclusions: Oriental herbal medicine and treatment have possibility to be a useful alternative to the surgical cerclage on cervical incompetence on the basis of more persistent study.

  • Cloned animals are a result of asexual reproduction of animals using somatic cell nuclear transfer. Ever since the first report of a cloned sheep 'Dolly' produced by SCNT, increasing numbers of livestock, such as bovine and swine clones, have been generated worldwide. Foods derived from cloned animals have not been produced yet. However, the food safety of cloned animals has provoked controversy. The EU Food Safety Authority and U.S. Food and Drug Administration announced that milk and meat from cloned and non-cloned animals have no difference regarding food safety. However, food derived from cloned animals is considered unsuitable for eating vaguely. Moreover, there were scant information about cloned animals in Korea. Therefore, we surveyed the number of cloned animals worldwide including Korea and summarized the reports for cloned animals and discussed predictable problems.

  • 단일 다발 십자인대 재건술의 임상적 결과를 보다 향상하고자 제안된 이중 다발 전방십자인대 재건술은 해부학적, 생역학적, 생물학적, 운동역학적으로 많은 이론적 장점이 있고 이에 따른 임상적 측면에서의 결과 향상이 기대되지만 이를 입증할 만한 충분한 관찰 기간을 가진 임상적 연구들이 매우 부족한 실정이다. 따라서 저자는 이중 다발 전방십자인대 재건술에 관련되어 최근까지 발표된 문헌을 통해 임상적 결과를 분석하고 이에 제기된 문제점을 살펴보고자 한다.

  • Optimal treatment of the torn anterior cruciate ligament (ACL) remains controversial. The complexity of surgically reproducing the natural biomechanical and anatomical function of the ACL has led to a diversity of reconstructive procedures. Controversy continues to exist regarding the best reconstructive procedure for the ACL deficient knee, but currently, there is no ideal method. Because of the increased frequency of ACL injury and the functional impairment resulting from that, the role of mechanoreceptors in the ACL recently has attracted considerable attention. Proper reconstruction of the ruptured ACL does not always have good results. Success after operation may depend not only on the mechanical stability but also on the quality of recovery of proprioception. It is well known that most ACL are ruptured in proximal half and most mechanoreceptors have been reported to be located in the subsynovial layer and near the tibial insertion of the ACL. Expected roles of tibial remnant is to enhance the revascularization and cellular proliferation of the graft, to preserve proprioceptive function, and to be able to acquire anatomical placement of the graft without roof impingement. The remnant of the ruptured ACL has been removed to clearly visualize the ACL footprint or decrease the risk of impingement and Cyclops lesion in most current techniques for ACL reconstruction. Therefore it seems reasonable to assume that preserving the tibial remnant as much as possible as a source of reinnervation, if technically possible without causing impingement, would be of potential benefit to the patient. In addition, it will facilitate the vascular ingrowth and ligamentization of the grafted ACL.

마취유도시 Desflurane의 효과 (The Effects of Desflurane on Anesthesia Induction)

  • Background: There is controversy regarding the relative perioperative benefits of desflurane when used for induction of anesthesia. Inhalation induction with desflurane alone causes adverse airway events, such as coughing, bronchospasm, laryngospasm, and copious secretion of varying severity. The aim of this study was to determine whether desflurane minimize cardiovascular activation during induction. Methods: Sixty ASA I and II patients were randomized to receive 1 MAC or 1.5 MAC of desflurane during manual vernilation or not. Patients received propofol (2 mg/kg) to induce loss of consciousness (LOC). Rocuronium (0.8 mg/kg) was given at LOC and the trachea was intubated after 90 seconds of manual breathing support with or without inhaled anesthetics. Vital signs and adverse airway events were recorded until 10 minutes post-intubation. Results: A significant increase in blood pressure and heart rate were seen in no desflurane group. The stable vital signs were seen in desflurane groups. The adverse airway events were increased in 1.5 MAC group but 1 MAC group. Conclusions: Desflurane was able to be stable blood pressure and heart rate at 1 MAC but adverse airway events were increased at 1.5 MAC of desflurane.

Theoretical Studies of the Low-Lying Electronic States of Diazirine and 3,$3^{\prime} $-Dimenthyldiazirine

  • The low-lying electronic states of diazirine and 3,3'-dimethyldiazirine have been studied by high level ab initio quantum chemical methods. The equilibrium geometries of the ground state and the first excited singlet and triplet states have been optimized using the Hartree-Fock (HF) and complete active space SCF (CASSCF) methods, as well as using the Møller-Plesset second order perturbation (MP2) theory and the single configuration interaction (CIS) theory. It was found that the first excited singlet state is of 1 B1 symmetry resulting from the n- π* transition, while the first excited triplet state is of 3 B2 symmetry resulting from the π- π* transition. The harmonic vibrational frequencies have been calculated at the optimized geometry of each electronic state, and the scaled frequencies have been compared with the experimental frequencies available. The adiabatic and vertical transition energies from the ground electronic state to the low-lying electronic states have been estimated by means of multireference methods based on the CASSCF wavefunctions, i.e., the multiconfigurational quasidegenerate second order perturbation (MCQDPT2) theory and the CASSCF second-order configuration interaction (CASSCF-SOCI) theory. The vertical transition energies have also been calculated by the CIS method for comparison. The computed transition energies, particularly by MCQDPT2, agree well with the experimental observations, and the electronic structures of the molecules have been discussed, particularly in light of the controversy over the existence of the so-called second electronic state.

반도체 산업 노동자 암 발생 위험 논란과 과제 (Challenges and issues of cancer risk on workers in the semiconductor industry)

  • Objectives: The objectives of this study are to summary controversy over health risks among semiconductor workers, to review major cancer risk results conducted in semiconductor operation and to evaluate occupational health activities in Korea for controlling hazardous agents generated in semiconductor operations Methods: Major occupational health issues that has been social controversies among semiconductor workers since 2007 were reviewed through an extensive literature, report and article review. Results: Since a female semiconductor worker aged 22 died from leukemia in 2007, job-association of a number of former semiconductor workers with various types of cancer and rare diseases have been denied by the Korea Workers' Compensation and Welfare Service (KWCWS), but some of them were later awarded compensation as an occupational disease by the administrative court. Two epidemiologic cancer risk studies conducted in Korea found increased risks in leukemia and non-Hodgkin's lymphoma among semiconductor workers. Various legal occupational health activities taken in semiconductor industry were found to fail to assess a complex characteristics of semiconductor operations, such as drastic changes in chemical use, processes, and technology, multiple exposure. National compensation regulation also showed the limitation to evaluate job-association of semiconductor workers who had worked in semiconductor operation. Conclusions: National legal measures should be taken to improve several occupational health activities and duties for protecting workers. In addition, the KWCWS program should be revised so that all workers who meet minimal job or environment associations can be compensated.

  • Theories of capital have historically been amongst the most fertile sources of economic controversy. Many aspects of the modern debate, if not the techniques employed in its exposition, would have been as familiar to Marx, Ricardo, Bohm-Bawerk or Wicksell as to any present-day Cambridge economist. That conotroversies should arise in the course of theorizing cm the concept of capital and the function of profit is not surprising: that these debates have been so vigorous and virluent cannot be divorced from the general ideological and specific implications associated with the theories. In the context of a textbook on the theories of economic growth, the central question associated with capital that must he faced is whether the existence in the real world of heterogeneous capital goods inevitably invalidate; the conclusions of simple theories of "parables" which incorporate no more than a conception of a single, malleable, capital good. All the Cambridge writers have, to a greater or lesser extent, been concerned to deny that any unit can be found in which heterogeneous capital goods can be aggregated so as to simultaneously satisfy the pair of neoclassical requirements described above. Some of them have been more prepared than others to countenance such a possibility or, for practical purposes, to use some concept of aggregate capital in their models, but they are all deeply suspicious of attempts to use aggregate production function, incorporating aggregate "capital", so as to explain the flow of output, relative factor prices and the distribution of income.tribution of income.
