Epigenetics: general characteristics and implications for oral health

  • Genetic information such as DNA sequences has been limited to fully explain mechanisms of gene regulation and disease process. Epigenetic mechanisms, which include DNA methylation, histone modification and non-coding RNAs, can regulate gene expression and affect progression of disease. Although studies focused on epigenetics are being actively investigated in the field of medicine and biology, epigenetics in dental research is at the early stages. However, studies on epigenetics in dentistry deserve attention because epigenetic mechanisms play important roles in gene expression during tooth development and may affect oral diseases. In addition, understanding of epigenetic alteration is important for developing new therapeutic methods. This review article aims to outline the general features of epigenetic mechanisms and describe its future implications in the field of dentistry.

Effects of eye dominance on shade matching and color perception among the dentist population

  • Objectives: The purpose of this study was to evaluate the influence of eye dominance on color perception, and shade matching. Materials and Methods: A total of 104 participants were selected for the study. There were 3 groups: Group I: 3rd and 4th year dental students and interns (n = 40); Group II: postgraduates (n = 34); Group III: senior residents and faculty members (≥ 6 years of clinical experience) (n = 30). All participants were evaluated for congenital color blindness with Ishihara plates, their dominant eye with Mile's test, and their color perception with the Farnsworth-Munsell 100 hue test. The shade guide test was used for shade matching with a second corresponding set of Vitapan classical shade guides. Results: The results of Mile's test revealed that 60.6% were right-eye dominant and 39.4% were left-eye dominant. There was a statistically significant difference among all participants between the dominant eye and the non-dominant eye in shade matching. Conclusions: The dominant eye has a positive effect on shade matching and the ability to match shades becomes better with an increase in clinical experience.

Clinical Analysis of the Conservative Treatment for Diabetic Foot Osteomyelitis (당뇨병성 족부 골수염 치료에서 보존적 치료에 대한 임상적 고찰)

  • Purpose: The question of surgical versus non-surgical treatment for diabetic foot osteomyelitis remains subject to debate. The aims of this study were to analyse the outcome of conservative treatment (antibiotic treatment and conservative surgery) for diabetic foot osteomyelitis and identify the predictive factors of remission in conservative treatment of diabetic foot osteomyelitis. Materials and Methods: Seventy-seven patients with diabetic foot osteomyelitis who initially received conservative treatment from January 2004 to July 2013 were identified, and their medical records were reviewed. Diabetic foot osteomyelitis was defined by imaging studies or histological evidence. Remission was defined as the absence of any sign of infection at the initial or contiguous site assessed at least 12 months after the end of treatment. The demographic, clinical, and therapeutic factors were analysed. Results: The mean age of the patients was $62.7{\pm}12.2$ years, and 47 patients (61.0%) were male. The median diabetes duration was $15.7{\pm}11.2$ years and mean HbA1c was $8.7%{\pm}2.4%$. Forty-eight patients (62.3%) healed with conservative treatment (antibiotic treatment and conservative surgery). Twenty-five patients (32.5%) underwent amputation. In the multivariate analysis, concomitant peripheral artery disease and inadequate antibiotic therapy were associated with failure of conservative treatment. Conclusion: Antibiotics alone, or with conservative surgery, were successful in treatment of diabetic foot osteomyelitis in 62.3% of the patients. Concomitant peripheral artery disease and inadequate antimicrobial therapy were risk factors for remission in conservatively treated diabetic foot osteomyelitis.

Evaluation of reparative dentin formation of ProRoot MTA, Biodentine and BioAggregate using micro-CT and immunohistochemistry

  • Objectives: The purpose of this study was to assess the ability of two new calcium silicate-based pulp-capping materials (Biodentine and BioAggregate) to induce healing in a rat pulp injury model and to compare them with mineral trioxide aggregate (MTA). Materials and Methods: Eighteen rats were anesthetized, cavities were prepared and the pulp was capped with either of ProRoot MTA, Biodentine, or BioAggregate. The specimens were scanned using a high-resolution micro-computed tomography (micro-CT) system and were prepared and evaluated histologically and immunohistochemically using dentin sialoprotein (DSP). Results: On micro-CT analysis, the ProRoot MTA and Biodentine groups showed significantly thicker hard tissue formation (p < 0.05). On H&E staining, ProRoot MTA showed complete dentin bridge formation with normal pulpal histology. In the Biodentine and BioAggregate groups, a thick, homogeneous hard tissue barrier was observed. The ProRoot MTA specimens showed strong immunopositive reaction for DSP. Conclusions: Our results suggest that calcium silicate-based pulp-capping materials induce favorable effects on reparative processes during vital pulp therapy and that both Biodentine and BioAggregate could be considered as alternatives to ProRoot MTA.

Experimental validation of ASME strain-based seismic assessment methods using piping elbow test data

  • To quantify the conservatism of existing ASME strain-based evaluation methods for seismic loading, this paper presents very low cycle fatigue test data of elbows under various cyclic loading conditions and comparison of evaluation results with experimental failure cycles. For strain-based evaluation methods, the method presented in ASME BPVC CC N-900 and Sec. VIII are used. Predicted failure cycles are compared with experimental failure cycle to quantify the conservatism of evaluation methods. All methods give very conservative failure cycles. The CC N-900 method is the most conservative and prediction results are only ~0.5% of experimental data. For Sec. VIII method, the use of the option using code tensile properties gives ~3% of experimental data, and the use of the material-specific reduction of area can reduce conservatism but still gives ~15% of experimental data.

  • Objectives: The object of this study is to report a clinical effect regarding the conservative treatment of the patients who were diagnosed as the herniated intervertebral lumbar disc(HIVD). Methods: The patients were treated by the oriental medical conservative treatment (Acupuncture, Moxibustion, oriental medicine, and Chu-na therapy). Result of conclusions: 1) Oriental medicine conservative treatment proved its effectiveness during the treatment of HIVD, patients. Especially, Chu-na therapy was superior on pain relive. 2) Treatment of HIVD, took less than 3 months in most cases except protruded or extruded disc. 3) In case of seniors who cannot have a surgical operation because of organ and muscle wickness, the oriental treatment was effective in terms of strengthening muscle and organs.

  • Treatment of patellofemoral joint injuries consist of conservative and surgical methods. Conservative treatemnt is effective in many cases. The goal of rehabilitation of patellofemoral joint injuries is to reestabilish the functional integrity of the affected lower extremity. Each patient presents with diffenent aspects of positive findings. Therefore, individualized treatment for each patient must be applied.

  • Although an ameloblastoma is a benign tumor histologically, it may act malignantly. It has locally destructive and recurrent tendencies. Many different strategies have been attempted in order to cure an ameloblastoma including curettage, enucleation, marsupialization, and resection with a safty margin. Curettage, enucleation, and marrsupialization can be classified into a conservative treatment and resection with a safty margin can be classified into a radical treatment. Radical treatment has better results than the conservative treatment. Thus, more radically conservative treatment methods are needed in order to improve the treatment results. The cryosurgery can be applied an ameloblastoam. In particular, with regards to the solid and intramural type, the application of the cryosurgery has its advantages over the conservative treatment. After resection of the diseased area we don't need to discard the diseased segment. Instead, by placing the segment in liguid nitrogen, the diseased segment can use the autogenous tray for packing several bone materials.

  • The purpose of this article was to provide basic knowledge and treatment principles of patient with lymphedema, which was usually not treated at all, or the treatment given didn't work efficiently. Lmphedema is defined as an abnormal accumulation of protein-rich fluid, edema, and chronic inflammation within an extremity. Lmphedema may be classified as either primary results from defects with aplasia, hypoplasia, and hyperplasia in the lymphatic system at birth or secondary is caused by known precipitating factors such as cancer, infection, inflammation, radiation, surgery, or trauma etc. There are essentially several conservative treatment methods which has been utilized successfully to treat lymphedema in Samsung Medical Center. We used following procedures: CPT (Complex Physical therapy) or CDP (Complex Decongesitive Physical therapy) such as skin care, MLD (Manual Lymph Drainage), compression with short-stretch bandage, exercise, elevation, elastic stocking, and pneumatic compression. Our experiences shows that conservative treatments can significantly reduce lymphedema and prevent different complications.

  • This study is designed to evaluate the effect of conservative oriental medical treatment with Kyungkuen Chuna for Herniated intervertebral lumber disc(HIVD) patient. The improvement of the patient was evaluated by Range of Motion(ROM), Straight Leg Rasing Test(SLRT), Visual Analogue Scale(VAS), Oswestry Low-back Pain Disability Index(ODI). After conservative oriental medical treatment with Kyungkuen Chuna, ROM, SLRT, VAS and ODI were significantly improved. It is suggested that Conservative oriental medical treatment with Kyungkuen Chuna could be effective methods to treat the HIVD patient.
