A study of Moral Reasoning by the Defining Issues Test among medical students (도덕판단력 진단검사(Defining Issues Test)에 의한 의과대학생의 도덕적 사고)

  • The purpose of this study was to investigate moral reasoning among 77 senior medical students. Data were collected through selfreported questionnaires in June, 1995. The short form of the DIT (Rest's Defining Issues Test) was adopted to measure the stage of moral development, which was classified with the stage 2(instrumental relativist orientation), the stage 3(interpersonal concordance), the stage 4(law and order), the stage 5A(societal consensus), and the stage 5B(intuitional humanism), stage 6(universal ethical practice). In particular, the level of principled thinking(P) was measured by summing those scores of the stages 5A, 5B, and 6. The possible range of P is O to 95. The data were analyzed by t-test, ANOVA. The results were as follows. 1. The mean score of P(%) was 44.67 (SD=12.82). And the mean score of the stage 5A was higher than the scores of other stages. The mean score of P was not significantly different by general characteristics of the students. 2. The mean score of the stage 5B revealed significant difference by religion (3.17, P=.019) ; The score was highest in buddhist (8.0), which was followed by protestant (6.1), catholic (5.6) and no religion (4.7). 3. The mean score of the stage 4 revealed significant difference by educational background of mother (3.24, P=.017) ; the Score was highest in graduate school (25.0), which was followed by high school (14.1), under-graduate school (13.9), elementary school (12.4), middle school (8.3).

Electroencephalography for the diagnosis of brain death

  • Electroencephalography (EEG) is frequently used to assist the diagnosis of brain death. However, to date there have been no guidelines in terms of EEG criteria for determining brain death in Korea, despite EEG being mandatory. The purpose of this review is to provide an update on the evidence and controversies with regarding to the utilization of EEG for determining brain death and to serve as a cornerstone for the development of future guidelines. To determine brain death, electrocerebral inactivity (ECI) should be demonstrated on EEG at a sensitivity of $2{\mu}V/mm$ using double-distance electrodes spaced 10 centimeters or more apart from each other for at least 30 minutes, with intense somatosensory or audiovisual stimuli. ECI should be also verified by checking the integrity of the system. Additional monitoring is needed if extracerebral potentials cannot be eliminated. Interpreting EEG at high sensitivities, which is required for the diagnosis of brain death, can pose a diagnostic challenge. Furthermore, EEG is affected by physiologic variables and drugs. However, no consensus exists as to the minimal requirements for blood pressure, oxygen saturation, and body temperature during the EEG recording itself, the minimal time for observation after the brain injury or rewarming from hypothermia, and how to determine brain death when the findings of ECI is equivocal. Therefore, there is a strong need to establish detailed guidelines for performing EEG to determine brain death.

The Development of the Korean Medical Insurance Economic Index(MIEI) (의료보험 수가수준의 조정을 위한 의료보험경제지수 (Medical Insurance Economic Index: MIEI)의 개발)

  • The current method of rate adjustment for inflation is based on the evaluation of the financial performance of hospitals. The method has the disadvantage such as too complicated, expensive process as well as low reliability. This study, therefore, develops the 'Korean Medical Insurance Economic Index(MIEI)' as a new model for the rate adjustment with the use of the macro economic indices. In addition, we calculate the 1992∼1998 rate adjustment with the MIEI, and examines the validity of the MIEI by comparing with the conventional method. Medical costs are classified into nine categories : physician salaries, nurse·pharmacist·medical technician salaries, assistants & others salaries, material cost(by imports), material cost(by domestics), depreciation & rent paid(by imports), depreciation & rent paid(by domestics), power utilities, other administrative costs. Then the category weight which is the ratio of category in the total cost is calculated. Macro economic indices are selected for each cost category in order to reflect the concept of the each cost category and inflation during the year of 1992∼1998. Finally MIEI which integrate all category according to the category weight and selected macro indices is calculated. The mean of hospital MIEI which weighting by amount paid by insurers was cacluated. The result from the application of empirical data to the MIEI model is very similar to that of the current method. Furthermore, this method is very simple and also easy to get social consensus. This MIEI model can be replaced the current method based on the analysis of the financial performance for the adjustment of medical fees.

Evaluation of the Quality of the Case Reports in Journal of Korean Medicine from Year 2015 to 2020 : Using CARE Guidelines (CARE 지침에 따른 대한한의학회지의 증례 보고에 대한 질 평가 연구: 2015년~2020년 증례 보고를 중심으로)

  • Objectives: The purpose of this study is to assess the quality of the Journal of Korean medicine case reports. Methods: Case reports published in the Journal of Korean Medicine from January 2015 to March 2020 were selected by using Oriental Medicine Advanced Searching Integrated System (OASIS) and the journal search system of the Korean Medical Association (https://www.jkom.org). The quality of the case reports was assessed using the Consensus-based Clinical Case Reporting Guideline Development (CARE) guidelines. Results: 33 case reports were selected for the assessment. Based on the CARE guidelines, 61.54% of the case reports included necessary information, but the quality level was uneven. More than 60% of the reports were missing data regarding 'Discussion of the strengths and limitations in your approach to this case', 'Intervention adherence and tolerability', 'Timeline', 'Medical, family, and psychosocial history including relevant genetic information', 'Patient perspective or experience', 'Adverse and unanticipated events', 'Administration of intervention', and 'De-identified demographic information and other patient specific information'. In most reports of over 90%, data regarding 'Diagnostic challenges', 'Intervention adherence and tolerability', and 'Key word' were not included. Conclusions: Efforts are needed to improve the quality of case reports in the Journal of Korean Medicine, and it is necessary to develop appropriate guidelines for case reporting for the Journal of Korean Medicine. In addition, all articles submitted to the Journal of Korean Medicine are to be complied with submission instructions and CARE guidelines.

A study on the Classification Schemes of Internet Resources for Industry (산업 분야 인터넷 자원의 분류체계에 관한 연구)

  • The industry information grows faster than any other information resources in the Internet age. Unfortunately, however, there is no consensus on the standard of the classification among the information providers of the industry fields. This may a problematic issue not only in building a continuous and systematic development of the industry information, but also in the use of the information among the users. This study aims to propose a well-structured and/or an efficient classification scheme for the industry information to help the users with easy to retrieve the Internet resources. To do this, we analyzed the subject classification scheme of the domestic industry information on the web sites, which is largely adopted the \"Korean Standard for the Industry Classification\". In addition, we suggested the principle of the subject classification and their hierarchial structure derived from the analysis of the knowledge and document classification scheme. As a result, it was suggested an optimized industry classification scheme based on the analysis of the validity test of classification item measured by the quantitative analysis of the industry information, which it currently accessible through the Internet. Role of Cerebrospinal Fluid Biomarkers in Clinical Trials for Alzheimer's Disease Modifying Therapies

  • Until now, a disease-modifying therapy (DMT) that has an ability to slow or arrest Alzheimer's disease (AD) progression has not been developed, and all clinical trials involving AD patients enrolled by clinical assessment alone also have not been successful. Given the growing consensus that the DMT is likely to require treatment initiation well before full-blown dementia emerges, the early detection of AD will provide opportunities to successfully identify new drugs that slow the course of AD pathology. Recent advances in early detection of AD and prediction of progression of the disease using various biomarkers, including cerebrospinal fluid (CSF) $A{\beta}_{1-42}$, total tau and p-tau181 levels, and imagining biomarkers, are now being actively integrated into the designs of AD clinical trials. In terms of therapeutic mechanisms, monitoring these markers may be helpful for go/no-go decision making as well as surrogate markers for disease severity or progression. Furthermore, CSF biomarkers can be used as a tool to enrich patients for clinical trials with prospect of increasing statistical power and reducing costs in drug development. However, the standardization of technical aspects of analysis of these biomarkers is an essential prerequisite to the clinical uses. To accomplish this, global efforts are underway to standardize CSF biomarker measurements and a quality control program supported by the Alzheimer's Association. The current review summarizes therapeutic targets of developing drugs in AD pathophysiology, and provides the most recent advances in the clinical utility of CSF biomarkers and the integration of CSF biomarkers in current clinical trials.

Development of Standard Contents and Methods of the Prevent Education of Sexual Harassment in High School Students Preparing for Employment Using Delphi Method (취업준비 고등학생을 위한 성희롱 예방 표준교육내용 및 방법 개발 : 델파이 기법을 활용)

  • This study aimed to develop the standardized contents and methods for preventing sexual harassment for hight school students preparing for employment after graduation. Delphi methodology was used with 30 experts. Three-round Delphi surveys were analyzed using Kendall's coefficient of concordance W analysis, appropriateness for reliability of consensus opinion. Data were analyzed respectively by descriptive statistics using SPSS Win version 20.0. Results of this study showed that standardized contents were classified with 3 domains and 83 contents, then 46 contents were selected from Kendall's W analysis and weight rank. Finally, 37 contents have taken as standards contents for prevention education. If high school students are educated with systematic standardized education before employment, it will be prevented sexual harassment in the workplace after graduation.

A Comparative Study on BMD of Lumbar Spine and Proximal Femur in Post-Menopausal Women Using Dual Energy X-ray Absorptiometry (이중에너지 X선 흡수계측법을 이용한 폐경기 여성의 요추 및 근위 대퇴부의 골밀도 비교 연구)

  • Osteoporosis, which causes mainly fracture of the spine, proximal femur and distal radius by minimal trauma, is a major public health problem and its prevalence is steadily increasing in Korea according to the development of public health care. There are reliable methods for diagnosis based on bone densitometry. Early detection and intervention are important for reducing the incidence of fractures. A consensus definition of osteoporosis, based on bone density measurement, has been developed by the World Health Organization(WHO). In this study, bone mineral density(RMD) was measured by dual energy x-ray absorptiometry(DEXA) at the proximal femur and lumbar spine in 132 post-menopausal women. The purpose of this study is to find influential factors on the BMD of the proximal femur and the lumbar spine and to analyze correlation between BMD and the problematic factors. We obtained the following results : 1. Mean BMD score, T-score and Z-score of the proximal femur were $0.81(g/cm^2)$, -2.45(S.D.) and -2.09(S.D.) respectively and in the lumbar spine were $0.83(g/cm^2)$, -2.02(S.D.), -2.43(S.D.) respectively. 2. In correlation analysis between BMD and many factors, correlation coefficients were -0.467, 0.212, -0.321 and 0.241 in age, height, duration after menopause respectively. BMI and the residuals were comparatively small. 3. Correlation coefficients to age matched BMD, in height and body weight were 0.222 and 0.241, in age and duration after menopause were -0.268, -0.282. 4. The fracture threshold of proximal femur BMD to the 90th percentile was $0.845(g/cm^2)$. 5. At the result of multiple regression analysis, age, body weight, $BMI(kg/m^2)$ and duration after menopause described as significant variables.

Significance and Development of the Integrated Environment Management System Implementation (통합환경관리제도 시행의 의의와 발전방향)

  • The significance of an integrated environmental management system implementation can be summarized into three sections. The first is that the paradigm of a permit system will be changed from the existing media-specific permits to cross-media permits. In the near future, the cross-media license will be granted based on the diagnostic results of the plant. The second is that it will be established through a social concensus, i.e., consultations with stakeholders and direct participation in the system design process of stakeholders. The third is that it will have flexibility in its operating system. In the near future, it will be possible to management an environment through its industrial workplace itself and determine if emission standards are being complied with or not through statistical methods. In addition there is also an urgent need for complementary parts in order to enhance the driving force of the integrated environmental management system which is based on transparency, reliability and flexibility. In other words, it is necessary to continuously promote in order to diffuse consensus and increase an understanding of implementation through an integrated environmental management system. It is also necessary to establish a new organization and educate authorized representatives so that they have the tools to smoothly perform the system.

  • The Radio-frequency electromagnetic(RFEM) spectrum is defined as waves that range in frequency from>0 to $3{\times}1012Hz$. Although there are several thousands of reports that present data or opinion of the biological response to RFEM radiation, no consensus has emerged regarding thresholds and mechanisms of injury. This review presents a overview of the subject on mechanisms of interaction of RFEM fields with tissue, chromosomal and mutagenic effect. carcinogenic effects. The scope of the review is expanded to include systemic effects such as those on reproduction, growth, and development, hematological effects. Some biological end points, those with associated with behavior and cataractogenesis is discussed.
