Single-Stage Reconstruction with Titanium Mesh for Compound Comminuted Depressed Skull Fracture

  • Objective : Traditionally, staged surgery has been preferred in the treatment of compound comminuted depressed fracture (FCCD) after traumatic brain injury (TBI) and involves the removal of primarily damaged bone and subsequent cranioplasty. The main reason for delayed cranioplasty was to reduce the risk of infection-related complications. Here, the author performed immediate reconstruction using a titanium mesh in consecutive patients with FCCD after TBI, reported the surgical results, and reviewed previous studies. Methods : Nineteen consecutive patients who underwent single-stage reconstruction with titanium mesh for FCCD of the skull from April 2014 to June 2018 were retrospectively analyzed. The demographic and radiological characteristics of the patients with FCCD were investigated. The characteristics associated with surgery and outcome were also evaluated. Results : The frequency of TBI in men (94.7%) was significantly higher than that in women. Most FCCDs (73.7%) occurred during work, the rest were caused by traffic accidents. The mean interval between TBI and surgery was 7.0±3.9 hours. The median Glasgow coma scale score was 15 (range, 8-15) at admission and 15 (range, 10-15) at discharge. FCCD was frequently located in the frontal (57.9%) and parietal (31.6%) bones than in other regions. Of the patients with FCCDs in the frontal bone, 62.5% had paranasal sinus injury. There were five patients with fractures of orbital bone, and they were easily reconstructed using titanium mesh. These patients were cosmetically satisfied. Postoperatively, antibiotics were used for an average of 12.6 days. The mean hospital stay was 17.6±7.5 days (range, 8-33). There was no postoperative seizure or complications, such as infection. Conclusion : Immediate bony fragments replacement and reconstruction with reconstruction titanium mesh for FCCD did not increase infectious sequelae, even though FCCD involved sinus. This suggests that immediate single-stage reconstruction with titanium mesh for FCCD is a suitable surgical option with potential benefits in terms of cost-effectiveness, safety, and cosmetic and psychological outcomes.

급성 아세트아닐라이드계 제초제 중독의 임상 분석 (Clinical Features of Acute Acetanilide Herbicide Poisoning)

  • Purpose: Acetanilide has been in widespread use as an amide herbicide compound. However, available data regarding acute human poisoning is scarce. The aim of this study was to analyze the clinical characteristics of acetanilide poisoning in order to identify the risk factors associated with severity. Methods: We conducted a retrospective observational study encompassing the period January 2005 to December 2010, including adult ED patients suffering from acetanilide intoxication. Toxicological history, symptoms observed, clinical signs of toxicity, and laboratory test results were collected for each patient. The patients were classified into two groups for analysis, according their poisoning severity score (PSS). Resulting clinical data and prognostic variables were compared between mild-to-moderate poisoning (PSS 1/2 grades), and severe poisonings and fatalities (PSS 3/4 grades). Results: There were a total of 37 patients, including 26 alachlor, 6 s-metolachlor, 4 mefenacet, and 1 butachlor cases. The majority of patients (81.1%) were assigned PSS 1/2 grades. Changes in mental status and observation of adverse neurologic symptoms were more common in the PSS 3/4 group. The median ingested volume of amide herbicide compound was 250 ml (IQR 200-300 ml) in the PSS 3/4 group, and 80 ml (IQR 50-138 ml) in the PSS 1/2 group. Also, the median GCS observed in the PSS 3/4 group was 13 (IQR 10-14), which was markedly low as compared to a median GCS of 15 in the PSS 1/2 group. Overall mortality rate was 5.4%, and profound cardiogenic shock was observed prior to death in all fatalities. Conclusion: When compared to previous reports, acute acetanilide poisoning resulted in relatively moderate severity. The presence of neurologic manifestations, hypotension, lower GCS score, and larger ingested volumes was associated with more serious effects and mortalities.

Thimerosal의 발생독성에 관한 연구 (Study on the Developmental Toxicity of Thimerosal)

  • The purpose of our study was to evaluate the toxicity of the thimerosal in embryos and neonates. Thimerosal (also known as mercurothiolate) is a mercury-containing compound used in trace amounts to prevent bacteria and other organisms from contaminating vaccines, especially in opened multi-dose vials. The toxicity of mercury is well known and those most at risk occurrs in unborn babies and newborn babies. Test methods included in vitro whole embryo culture (WEC) system and in vivo test of neonatal toxicity in Wistar rats. Ethylmercury and methylmercury were used as positive controls for the evaluating of toxic effects of mercury. In WEC assay, treated concentrations of thimerosal, ethylmercury and methylmercury were up to 0.01, 0.025, 0.05, 0.1, 0.25, 0.5, 1, 2.5 and 5 $\mu\textrm{g}$/$\textrm{m}{\ell}$, respectively. All compounds didn't show any morphological abnormalities, but showed retardation of growth and development in dose dependent manner (> 0.5 $\mu\textrm{g}$/$\textrm{m}{\ell}$). These data indicated that thimerosal showed developmental toxicity in vitro. In vivo neonatal toxicity, Wistar rats were administered subcutaneously with thimerosal, ethyl mercury, or methylmercury (5, 25, 50, 250, and 500 $\mu\textrm{g}$/kg) during from postnatal day (PND) 4 to 25. Significant effects of these compounds on relative organ weights and organ morphology were not observed in this experiment. However, accumulation of mercury was detected in the kidney and testis when treated with thimerosal, ethylmercury, or methylmercury. These results suggest that thimerosal may be a harmful compound to embryo and neonate, but used concentration of thimerosal in these experiments is much higher than that of clinical application. Further investigation is needed on the safety of vaccine components, i.e. a thimerosal using in vitro and in vivo tests in the future.

무균조제 항암제 취급의 안전관리 (Assessment of Occupational Exposure to Antineoplastic Agents in a Healthcare Setting)

  • Most antineoplastic agents are nonselective in their mechanism of action, affecting noncancerous as well as cancerous cells, and resulting in acute effects such as irritation of mucous membranes and chronic effects such as genotoxicity, teratogenicity, and carcinogenicity. Healthcare workers occupationally exposed to antineoplastic agents are at risk. The present study aimed to develop and apply methods to monitor occupational exposure to antineoplastic agents, using cyclophosphamide (CP) as the model compound. To monitor environmental and biological exposure, potentially contaminated surfaces were wiped and 24 hour urine samples were collected from workers. Liquid chromatography combined with tandem mass spectrometry was performed, with a limit of detection of 0.05 ng/ml. Measurable amounts of CP were detected on 92% of the sampled surfaces, with a geometric mean of 175.22 $ng/m^2$. Despite the environmental contamination of the model compound, CP was below the detection limit in all urine samples. If workplace contamination cannot be completely avoided, it is importance to reduce exposure to the lowest possible levels. To this aim, efforts to minimize occupational exposure along with biological and environmental monitoring are required. The standardized sampling techniques, and specific and sensitive analytical methods reported in this study may be helpful in assessing occupational exposure and devising strategies to reduce exposure.

분산메모리시스템에서의 핫콜드 데이터 분류를 이용한 복합 백업 기법 (Compound Backup Technique using Hot-Cold Data Classification in the Distributed Memory System)

  • IT 기술의 발전으로 인해 데이터 처리 시스템은 많은 양의 데이터를 처리 및 가공해야 한다. 하지만, 기존에 사용되던 온디스크(On-Disk) 시스템으로는 급증하는 데이터를 빠르게 처리하는 데 한계점을 가졌다. 이로 인해 많은 분야에서 하드디스크에 데이터를 저장하는 것이 아닌 속도가 빠른 메모리에 데이터를 저장 및 관리하는 인메모리(In-Memory) 시스템이 도입되고 있다. 하지만, 메모리에 데이터를 관리하는 것은 메모리의 특성 중 하나인 휘발성으로 인해 데이터 손실이라는 위험을 갖기 때문에 항상 결함 허용 기법이 뒤따라야 한다. 결함 허용 기법은 인메모리 시스템의 처리 속도를 낮추는 성능 저하 원인이 된다. 따라서 본 논문에서는 인메모리 시스템의 데이터 사용 특성을 고려하여 핫콜드 데이터로 분류하고, 데이터 영속성 보장을 위한 복합 백업 기법을 제안한다. 제안하는 기법은 영속성을 높이고, 성능 저하 원인을 보완한다.

연소시험에 의한 붕소 화합물 처리 목재의 화재위험성 (Fire Risk of Wood Treated With Boron Compounds by Combustion Test)

  • 붕산과 5붕산암모늄으로 처리한 편백목재 시험편의 연소특성에 관한 실험을 ISO 5660-1 표준에 따른 콘칼로리미터를 이용하여 수행하였다. 그 결과 붕소화합물로 처리한 시험편의 화재성능지수(FPI)는 공시편보다 1.2~2.1배 증가하였고, 화재성장지수(FGI)는 공시편보다 1.6~8.4% 증가하였다. 또한 총연기발생률(TSR)은 공시편보다 9.0~28.3.% 낮았다. 이것은 붕소화합물로 처리한 시험편이 난연효과로 탄화층을 생성하기 때문으로 이해된다. 공시편은 418 s에서 가장 높은 CO농도인 0.01112%를 보였으나 붕소화합물로 처리한 시험편은 공시편보다 13.2~37.5% 감소하였다. 따라서 붕소화합물로 처리한 목재는 화재위험성과 위해성이 낮아질 것으로 예상된다.

붕소/실리콘 졸 화합물로 도포된 편백 목재의 연기유해성 평가 (Smoke Hazard Assessment of Cypress Wood Coated with Boron/Silicon Sol Compounds)

  • 본 연구는 건축 및 내구재용 목재인 편백 목재에 붕소/실리콘 졸 화합물을 처리한 후 화재위험성을 연기 유해성에 대하여 연기성능지수(SPI), 연기성장지수(SGI)와 연기강도(SI)를 중심으로 조사하였다. 화합물은 Tetraethoxyorthosilicate를 Boronic acid와 Boric acid 유도체와 반응시켜 합성하였다. 연기 특성은 편백목재에 대하여 Cone calorimeter (ISO 5660-1) 장비를 이용하여 조사하였다. 화재강도는 50 kW/㎡의 외부 열 유속(External heat flux)으로 고정시켰다. 연소 반응 후 측정된 연기성능지수는 편백목재와 비교하여 13.4~126.7% 증가하였다. 연기성능지수에 의한 화재위험성은 편백목재, PBA/Si, IBBA/Si, BA/Si 순서로 감소하였다. 연기성장지수는 편백목재와 비교하여 12.0~57.5% 감소하였다. 연기성장지수에 의한 화재위험성은 편백목재, PBA/Si, IBBA/Si, BA/Si 순서로 낮아졌다. 연기강도에 의한 화재위험성은 3.2~57.8% 감소하였으며 편백목재, PBA/Si, IBBA/Si, BA/Si 순서로 낮아졌다. COpeak 농도는 85~93 ppm였으며 공시편과 비교하여 37~43% 감소하였다. 화재위험성을 연기유해성에 대해 종합적으로 평가하면 편백목재, PBA/Si, IBBA/Si, BA/Si순서로 낮아졌다.

바이오디젤 혼합물의 함량변화에 따른 열적 특성에 대한 실험적인 연구 (Experimental Study on the Thermal Characteristics According to the Content Change of Biodiesel Mixture)

  • 연구목적: 신규로 사용되는 바이오연료를 기존 연료와 혼합하여 사용하는 경우 발생하는 위험성과 물성 변화를 열분석 방법(DSC, TGA)을 사용하여 화학 화재의 원인물질의 위험성을 확인하고, 평가할 방법과 그에 따른 물질의 위험성 관련 데이터를 확보함으로써 화재 원인 감식과 감정에 활용하기 위함이다. 연구방법: 본 실험에 사용된 연구 방법으로는 시차주사열량계(DSC : Differential Scanning Calorimeter)에 의하여 피크의 위치, 모양, 개수, 피크의 면적으로부터 열량 변화의 정량적인 정보를 통하여 열유속 차이(Difference in heat flux)를 측정하였고, 열중량분석(TGA : Analyzer)을 시행함으로써 특정한 온도에서 분해열 등에 의해 발생한 무게 변화를 연속적으로 측정하였다. 연구결과: 먼저 열 유속의 그래프에서 물질의 끓는점과 물질이 가지고 있는 고유 특성치 또는 분해에 필요한 에너지를 확인할 수 있다. 둘째 바이오디젤의 함량이 증가할수록 많은 피크를 확인 할 수 있었다. 셋째 비점이 낮은 물질들이 함유하고 있다는 것을 분석 결과로 확인할 수 있었다. 결론: 현재 새로운 에너지원으로 사용되고 있는 바이오디젤의 위험성을 다양한 물리·화학적 분석기법(DSC+TGA)을 통하여 사용함으로써 물질의 물적 위험성을 평가할 수 있음을 보여주었다. 아울러 본 연구의 시험방법별 차이의 비교와 실험에 대한 노하우를 축적하고 활용한다면 향후 위험물의 물성 연구와 물질 위험성 평가 연구에 있어 도움이 되리라 기대한다.

상업용 공기정화기 사용 차량 내 휘발성 유기물질 수준 (Volatile Organic Compound Levels inside Vehicles using Commercial Air Cleaning Devices)

  • 차량 내부의 높은 휘발성 유기물질 농도와 이들 휘발성 유기물질의 독성 때문에 휘발성 유기물 질에 대한 차량 내부에서의 노출이 주요한 관심을 받고 있다. 본 연구에서는 115회의 도시 지역 (대구 광역시) 출퇴근과 9회의 정차 실험을 통해 상업용 공기정화기의 6 종류 휘발성 유기물질에 대한 제거 효율이 평가되었다. 네 종류의 운전 조건에서 수행된 정차 및 출퇴근 연구 결과, 본 연구에서 이용된 두 개의 시중 시판 차량용 공기정화기는 휘발성 유기물질에 대해 제거 효율이 없는 것으로 나타났다. 벤젠을 제외한 다섯 종류의 휘발성 유기물질의 농도는 최근 차량보다 오래된 차량 내부에서 높게 나타났다 (p<0.05). 본 연구에서 측정된 벤젠과 톨루엔의 평균 농도는 로스앤젤레스에서 측정된 값과 유사하게 나타났고, 그 이외의 미국내 비교지역의 측정 값보다는 높게 나타났다. 휘발성 유기물질에 대한 노출 및 해당 발암 위해성은 차량 내부에서 실외 공기의 약 8배 정도 높게, 그리고 실내 공기의 약 절반 정도로 산정 되었다.

사방댐 입지조건에 관한 연구 - 충청지역과 경북지역을 중심으로 - (A study on Location Condition for Erosion Control Dam - Focus on Chungcheong region and Kyeongsangbuk-do -)

  • This study is to analyze location conditions for erosion control dams to be constructed in Chungcheongnam-do, Daejeon Metropolitan City, Chungcheongbuk-do and Gyeongsangbuk-do in order to establish proper conditions for erosion control dams in the future. 199 sites where erosion control dams are expected to be built in 2010 were chosen and investigated in terms of 12 factors including basin area, basin slope, and landslide risk. The results showed that erosion control dams for Chungcheongnam-do and Daejeon Metropolitan City are mostly impermeable gravity dams mainly composed of concrete. In contrast, Chungcheongbuk-do and Gyeongsangbuk-do are increasing the number of permeable or compound erosion control dams. Basin analysis at planned erosion control dam sites showed that at least 44.5% of the total area has high landslide risk. Gyeongsangbuk-do had the largest basin area for erosion control dam sites at 157.3ha, followed by Chungcheongbuk-do at 64.4ha and Chungcheongnam-do at 54.8ha. Analysis of sand deposits in the Chungcheongnam-do erosion control dam built in 2010 confirmed an average deposit of 971.8m3. The sand deposit capacity and amount of sediment control for erosion control dams have a very low correlation with basin area or flow path slope, and this needs to be addressed in future sand deposit capacity designs.
