Current Status and Management of Congregate Meal Service Program for the Elderly at Community Centers (사회복지관의 노인 경로식당 급식 서비스 현황)

  • To investigate current status of the elderly meal service program, community centers nationwide were surveyed for congregate meal services. The survey was conducted during the month of December 2002 by mail using self-developed questionnaire on administration, staffs, funding, and meal service management. Only 95 out of 356 community centers (27%) answered and returned the questionnaires, and congregate meal menus were also collected from 20 centers for 492 days. Results were analyzed using SAS package program. In more than half of the elderly congregate meal service programs, following criteria were used in multiple form to allow participation; 65 years old or older, low income, or living alone. All centers served lunch, most of them free of charge, and 88.4% of them provided service 5 days a week. About 79% of the total cost occurred on the congregate meal service was supported by the government. Most of the menus served at congregate meal service were Korean style dish with rice. On the average, each meal provided 3 side dishes including kimchi, in addition to soup or stew. Only 19% of the centers have employed dietitian working for meal service programs; meal menus were prepared by social welfare workers or other non-dietetic personnels in more than 80% of the centers. Food purchasing and food hygiene control was also practiced mainly by social welfare workers or cooks. This study suggests that dietetic professionals are needed to better manage meal service programs for the elderly in both aspects, food hygiene and nutrition. Development of more nutritious menus and determination of appropriate serving sizes for the elderly meal service programs are necessary to meet the RDAs and dietary action guide for the elderly.

A survey of the satisfaction of a dental hygiene student's in the program of public health center for quality improvement of community oral health practice (지역사회구강보건실습의 질 향상을 위한 일개 치위생학과 학생들의 보건소실습 만족도 조사)

  • Little evidence has been provided on the standardization of community dental hygiene as one of the subjects of dental hygiene course. The goals of this study were to: examine the satisfaction of a dental hygiene student's in the practice program according to public health centers and explore the satisfaction factors of a dental hygiene student's in the program of public health center. Data on 105 were obtained from the undergraduate students in the university in Gyeonggi-do. As a result of satisfaction of the practice at 11 public health centers, the overall mean score was 5.3 out of 7 score. The means of satisfaction score were different cross public health centers with the 3.7 lowest and 6.8 highest score. Among the factors affecting satisfaction of the practice at public health center, 'comfortability of practice' was identified by 55.2%, followed by 'accessibility (46.7%)' and 'academic achievement (31.4%)'. Different proportions of respondents were obtained between 3rd year and 4th year students. In conclusion, this study highlighted the importance of the program of public health center for quality improvement of community oral health practice. A survey of the satisfaction of a dental hygiene student of community oral health practice is to provide evidence data as a starting point for this purpose.

The Possibility of Regional Health Insurance Data in Blueprinting the Local Community Health Plan (지역보건의료계획 수립에 있어 지역의료보험자료의 활용가능성)

  • The health center has to play an important role in promoting community health and satisfying a variety of community health needs and demands in the decentralized Korea. The nearly enacted Community Health Act compels every health center to make its own health plans which intend to deal with local health problems and plan its future health care. This obligation is obviously a big burden to most health centers. They do not have experiences in and abilities of making local health care plans. In order to establish a systematic community health plan, health centers have to concentrate their efforts on enhancing the ability of making health care plan through gathering and analysing the local health informations. However, it is very difficult in reality. This is simply because it will take long time to accomplish these activities. It seems natural that various professionals and researchers participate in carrying out the process of making community health plan in the initial stage. No standardized methodology and analysing framework exist even in the health professional society. Nonetheless, it is common to introduce survey research methodologies in analysing consumer's health care utilization and cost, and in identifying factors influencing health behaviors. Many researchers and professionals have applied social survey methodologies in obtaining information on providers and health policy makers as well. The authors have found that few studies have ever utilized local health data stored at the self-employed medical insurance society as the data source of planning activities. The purpose of this study is to illustrate the usefulness of the data stored at the Sung-Dong Gu Self-employed Medical Insurance Society in establishing the community health plan. The major contents of this study are as follows ; 1. frequency of utilization by age, area, sex, type of medical care institutions, and some major diseases 2. Medical treatment by type of medical care institutions, by classification of 21 diseases, by frequency of three-character categories 3. Medical treatment of major neoplasm and some chronic diseases by age, sex, and area. The conclusion of this study is that it is of great potentiality to find out the local health problems and to use them in blueprinting the community health plan through comparing the frequency of medical utilization analyzed by a variety of variables with NHI health data or the health data from survey research.

  • In recent years, the recipients of the services of the health centers in Korea have been shifted from individual sick persons to families as a unit. As a result, the home visiting care records which are all filled out manually, will be increased. Since there is virtually no increase in the number of community health nurses, the CHNs are required to work more efficiently. One of the ways to make the CHNs' work more efficient is to reduce recording time by using a computer. However, a computer system that can manage the families as a unit has not yet been developed. In response to this need, we developed a database system that can be utilized in home visiting care service. The family assessment data is collected. diagnosed. and evaluated according to the family diagnosis classification. The system for family diagnosis consists of seven areas. Those areas are family structure. maintenance of the family system, interaction and interchange. support. coping and adaptation, health management. and housing environment. The areas of the family diagnosis consists of 99 items in all. We expect the following from this system. First. the CHNs will be able to identify family problems more easily. Second. the community's health level can be confirmed by the statistics the system produces. Thirdly, the CHNs' nursing services will be cost effective via reduced recording time. Finally, the family problems of the sick individuals which have been neglected under the health system oriented on individual persons can be effectively managed.

The Effects of Sophrologic Prenatal Education Program in Community Health Centers (일개 보건소의 소프롤로지 식 분만교육의 효과)

  • Purpose: The purpose of this study was to identify the effects of the Sophrologic Prenatal Education Program. Method: The subjects of this study were 16 antepartum women who had been registered at community health centers at S-gu in Pusan. Data were collected from women who visited for antenatal care during the period from the 7th of May to 28th of June in 2003. Participants were evaluated before and on completion of the 8 weeks' educational program using 3 instruments: Breast-feeding knowlege, Maternal-fetal attachment and Antenatal stress. The Wilcoxon Signed-Ranks Test was conducted for data analysis using SPSS/WIN 10.0. Results: Statistically significant difference was found between the pretest and posttest in the mean score of the Breast-feeding knowledge. The mean scores of the experiment group and the control group were $31.44{\pm}5.51\;and\;37.06{\pm}4.27$ respectively (p=.001). No statistically significant difference was found between the pretest and posttest in the mean score of the Maternal-fetal attachment (p=.315, p=.578). Conclusions: Community health care providers must consider the emotional needs of Korean people and make efforts to adapt the Sophrologie Prenatal Education Program for a larger number of antepartum women in community.

A Study on the Actual Conditions and Needs for Nutrition Education for Pregnant Women in Health Centers (보건소에서의 임신부 영양교육 프로그램 운영 실태와 요구도 조사)

  • This study was conducted to investigate actual conditions and needs of nutrition education in order to develop a nutrition education program for pregnant women in health centers. The questionnaires were mailed to 245 health centers and 146 questionnaires were returned. Most health centers(76%) had nutrition education program for pregnant women. About 63% of supervisors were the nurses and 43% of educators were dieticians. The teaching method which was used most frequently was lecturing(34%). Teaching material which was used most frequently was material brought by invited speakers(31%). The subjects of education were the relationship between nutrition for pregnant women and the baby's health(19%), dietary guide and directions for pregnancy(19%), nutrient supplement for pregnant woman(17%), weight gain during pregnancy(16%), abnormal symptoms of pregnancy and health(15%), pregnancy complications and health(13.0%), and others. These subjects were the same ones which educators thought were needed in education. Important success factors in education were giving accurate information and guide and practice, while failure factors were lack of proper space, lack of practice, and others. Lack of a standardized nutrition education program was the biggest barrier to running a program. The subjects which were taught and the needs in nutrition education were significantly different according to respondents' age, educational level, job position, and residence of health center. Therefore, a standardized program, proper space for practice, and professional educators are needed to promote the effectiveness of nutrition education.

A Content Analysis of the Experience of Participation in Community Leisure Programs among Older Adults (노인복지관 여가 프로그램 참여노인의 경험에 대한 질적내용분석)

  • Purpose: This study aimed to explore the experiences of participation in community leisure programs among older adults. Methods: Data were collected using face-to-face interviews with nine older adults participated in community leisure programs and analyzed using a qualitative content analysis. Results: Seven themes emerged from the data: 1) A deep regret not learned, 2) unboring life, 3) living together, 4) having a happy life, 5) gaining recognition from family members, 6) cherishing their own lives, and 7) living for others. Conclusion: Findings indicate that there is a need for health professionals to understand meanings of participation in community leisure programs before planning community senior programs. More studies are needed to develop community leisure programs for older adults' health promotion in community health centers.

  • Purpose: The purpose of this study is to identify the operational status and current management of Sub-Health Centers in Limpio, Paraguay. Second, understanding the use of Sub-Health Centers and Perceptions of Sub-Health Centers in Limpio, Paraguay. Third, Providing policy implications for strengthening the Health Delivery System in Paraguay. Methods: The survey of the current status of sub-health centers in Limpio was conducted with observation and interview. Utilization of Sub-Health centers was analyzed in the 2018 Paraguay Community Health Survey. A face to face interview was conducted to complete a questionnaire and 831 samples were collected for the study. Results: In order to perform the normal function of the sub-health center, it will be necessary to allocate manpower that meets the standard. A common problem with sub-health centers in Limpio is that they have an environment vulnerable to rain. Currently, there are no health promotion and communicable disease management programs in sub-health center. Satisfaction of users about treatment, equipment, medicines and cleanleness of rooms. Implications: First, it is necessary to allocate human resources and organize spaces according to the standard. Second, there was a problem caused by moisture, and continuous maintenance and repair are required. Third, water and sewage related facilities must be safely improved to prevent contamination of groundwater. Forth, it is necessary to implement a program that fits the role of the sub-health center. Fifth, it is necessary to form a health delivery system considering the accessibility of residents. Finally, it is necessary to discuss the location of sub-health center considering travel time of Limpio residents.

Current Status of Community Health Nursing Practicum in Bachelor Program in Korea (4년제 간호대학(과) 지역사회간호학 실습교육의 현황분석)

  • Purpose: This study was to investigate the current situation of community health nursing practicum in bachelor programs. Method: Data were collected using a questionnaire consisting of questions on education goals, teaching items, teaching methods, and evaluation methods and items. Forty five nursing departments/colleges or 84.9% of four-year nursing schools in Korea responded to the survey. Result: Nursing process application and understanding about the role and function of community health nurses were major goals of practice education. Community health centers were the most significant practice fields because all nursing schools mentioned them as places for practice. All nursing schools used a specific evaluation tool to measure students outcomes and utilized guidebooks to help students. Conclusion: The results of this study suggest that the goals of practice education should be standardized to improve the quality of education. Besides, evaluation tools that can be used commonly at all nursing schools should be developed to measure the effectiveness of practice education of community health nursing.

In order to activate primary health care for the poverty in urban area the following measures should be taken : 1. Health center must be expended or establish subhealth center. 2. Health center must monitor neighbour's workplace's health management for their working population. 3. Health centers must do active home visiting nursing care for the urban-poor. 4. Health center must carry out flexible problem-centered practice according to the area. 5. For the urban-poor's health care must have organization of the health center & practice according to community's characteristics. 6. Public health care must be closely connected with welfare. 7. For the health care of the urban-poor must demand active community participation. 8. Health center is closely connected with Community hospital. 9. Active management of public health resource system is demanded.
