Status of Early Childhood and Maternal Nutrition in South Korea and North Korea (남북한 영유아 및 가임기 여성의 영양상태 비교)

  • The purpose of this study was to compare the nutritional status of children aged 5 or under and women aged 20 to 34 years between the Republic of Korea (South Korea) and the Democratic Peoples' Republic of Korea (DPRK: North Korea). For the source of nutritional status of North Koreans, the DPRK 2004 Nutrition Assessment-Report of Survey Results was used. As the comparable data of South Koreans, the anthropometric data for children and women were obtained from the reports of the Korean Pediatric Society and the Korean Agency for Technology and Standards, respectively. The blood hemoglobin data of South Korean women were obtained from the data file of the 2001 National Health and Nutrition Survey and analyzed. In regard to the North Korea, the Prevalence of underweight (weight for age Z-score < -2.0) in children under 12 months was about $10\sim15%$, and thereafter progressively increased until 30 to 35 months reaching 30%. In South Korea, the prevalence of underweight was less than 3% in most age groups both in boys and girls. In North Korea, the prevalence of stunting (height for age Z-score < -2.0) reached 20% in children under 12 months and increased with age over the level of 50% in children aged 54 to 59 months. In South Korea, the prevalence of stunting was less than 3% in children under 12 months and was less than 10% throughout the age groups. Maternal protein-energy malnutrition and anemia were assessed for the women aged 20 to 34 years using mid-upper arm circumference (< 22.5 cm) and blood hemoglobin level (< 12g/DL), respectively. The prevalence of protein-energy malnutrition was 39.6%, 30.7%, 31.7% in North Korea and 12.5%, 5.0%, 1.5% in South Korea fir the women in $20\sim24,\;25\sim29,\;30\sim34$ years, respectively. The prevalence of anemia in the North Korean women was about $34\sim36%$ while that in the South Korean women was $15\sim18%$. In conclusion, the disparity of nutritional status in early childhood and maternity between South Korea and North Korea is so huge that active and well-planned nutrition support policy and programs for women and children in North Korea is imperative to prepare for the future unified nation.

Relation among Hair Mineral Contents, Nutrient Intakes and Clinical Symptoms of Male Middle School Students in Urban and Local Area (도시와 지방에 거주하는 남자 중학생의 두발 중 무기질 함량, 영양섭취상태 및 임상증상과의 관련성)

  • Minerals play important roles in biological processes. Hair mineral analysis has the advantages of conveniences in sampling and observing the profiles of multiple minerals simultaneously. The purpose of this study was to compare hair mineral contents between urban and local middle school students, and to find out the relation with nutrient intakes and clinical symptoms. Ten nutritional minerals and 7 environmental mineral contents were analyzed in the hair samples of 44 and 37 male students who resided in urban and local area, respectively. And their nutrient intake and clinical symptom score were estimated from questionnaire. The average age, height, and weight were 15.1 years, 168.4 cm, 56.8 kg in urban subjects and 15.4 years, 169.1 cm, 61.9 kg in local subjects, respectively. The residence types were apartment (63.6%) and small-sized apartment (22.7%) in urban subjects and apartment (51.4%) and house (37.8%) in local subjects. The primary water sources were purified water (38.3%), running water (18.2%) in urban subjects and purified water (32.4%), underground water (27.0%) in local subjects, respectively. Daily energy and nutrient intakes were not significantly different between two groups. Among the surveyed 17 clinical symptoms, the scores of constipation, cold, anxieties and total score of clinical symptoms in urban subjects were significantly love. than those in local subjects. Hair contents of Na, Mg, Ca, Cu, V Al, As were significantly higher, while U was lower in urban subjects compared to those in rural subjects. Hair contents of Ni and U were significantly correlated with food intake, and Cr, V, Al, Sb, As, U showed a correlation with energy intake. Hair Mg level showed significantly negative correlation with total clinical symptom score. In conclusion, hair mineral profile between urban and local students was significantly different. And some of these minerals were significantly correlated with food and nutrient intakes. Especially hair magnesium was significantly correlated with total clinical symptom score. Therefore, hair mineral levels would be needed for more systematic study elucidating potentiality as a useful clinical tool.

Assessment of Intake of Nutrient and Dietary Fiber among Children at the 6th Grade of Elementary School in Gimhae (김해지역 초등학교 6학년생들의 영양소와 식이섬유 섭취상태평가)

  • The aim of this study was to assess dietary fiber intake and the relationship between dietary fiber intake and nutrient intakes of elementary school students. Food consumption of 200 elementary school students (100 boys, 100 girls) in the 6th grade in Gimhae was assessed by 2-day estimated food records. Nutrient intakes were calculated using CAN-pro and total dietary fiber (TDF) intake using the TDF tables of the common Korean foods. The mean height and weight of the boys were 147.7 cm and 42.1 kg. The mean height and weight of the girls were 149.1 cm and 41.1 kg. The mean body mass index (BMI) of the boys (19.3 $kg/m^2$) was significantly higher than that of the girls (18.4 $kg/m^2$, p < 0.05). Average daily intake of energy of the boys and girls was 1733 and 1778 kcal, respectively. Intakes of protein, P, and niacin of the boys and girls exceeded $110\%$ of current RDA. Intakes of Fe, vitamin $B_1$ and vitamin $B_2$ of the boys were similar to RDA. Intake of vitamin $B_1$ exceeded $110\%$ of current RDA and intakes of Fe and vitamin A were similar to RDA among the girls. Ca intake of the boys and girls was $74.2\%$ and $60.1\%$ of RDA, respectively. Vitamin C intake of the boys and girls were $63.6\%$ and $71.7\%$ of RDA, respectively. Intakes of fat, Ca, P, and vitamin $B_2$ of the boys were significantly higher than the girls. Intakes of Fe and vitamin A of the boys were significantly lower than the girls. Average daily intakes of TDF of the boys and girls were 11.2 g and 11.1 g, respectively. TDF intake of the children at the 6th grade of elementary school was about $70\%$ of 'age (yr) + 5 g', a recommended level for American children. Major food groups to supply dietary fiber were vegetables, grains, and seasonings among the boys and girls. Major sources of dietary fiber were Kimchi, rice, and red pepper powder which supplied $17.85\%$. $15.71\%$, and $5.18\%$ of TDF among the boys and $18.02\%$, $16.22\%$, and $6.58\%$ among the girls. Daily fiber intake showed significant positive correlations with intakes of grains, vegetables, fruits, legumes, and seasonings among the boys and girls. Daily fiber intake showed significant positive correlations with intakes of energy, carbohydrates, protein, lipids, Ca, P, vitamin A, vitamin $B_1$ vitamin $B_2$, niacin, and vitamin C among both the boys and girls. From the results of this study we could conclude that it is recommended to increase dietary fiber intake in elementary school students at upper level.

The Status of Eating Habits and Nutrient Intakes of Preschool Children in Kyungjoo (경주지역 유치원 아동들의 식습관과 영양소 섭취상태)

  • The purpose of this study is to estimate the nutrient intakes and food habits among preschool children in Kyungjoo city. The subjects were 210 preschool children, aged 4 - 6 years. Measurements of the weight, height, chest circumference, and head circumference of the children were conducted. And general home environment and factors related to eating habits for preschool children were collected using a questionnaire that included information about family income, parents' education and occupations. The average weight-length index (WLI) for the subjects was $103.9\%$. Using the WLI, $20.0\%$ of the preschool children were underweight, $48.6\%$ were normal, $19.0\%$ were overweight, and $12.3\%$ were obese. On the R$\ddot{o}$hrer index, $13.8\%$ of the preschool children were underweight, $38.5\%$ were normal, and $47.7\%$ were over weight or obese. The average daily intake ($\%$ of RDA) of energy and each nutrient was 1323.5 kcal ($81.3\%$), Ca 484.3 mg ($80.7\%$), Fe 7.05 mg ($88.1\%$), vitamin A 420.0 RE ($105\%$), vitamin $B_{2}$ 0.76 mg ($95.0\%$), vitamin $B_{2}$ 0.87 mg ($87.0\%$), and vitamin C 53.1 mg ($106.2\%$) , respectively. In particular, older subjects had lower intake in RDA $%$ of calcium and iron. The energy intake ratio from snacks was much higher than the recommended level of the preschool children. With regard to frequency of regularity of breakfast, $1.9\%$ of preschool children skipped every morning and $7.6\%$ of the children skipped more than 5 per week. With regard to the intake frequency of vegetables, fruits, complex carbohydrates, and milk, $13.3\%$, $19.9\%$, $22.8\%$, and $41.8\%$ of the children ate more than 5 times per week. The eating habit score was positively correlated (r = 0.18, P < 0.05) with household income. This study suggests that nutrition education to increase fruit and vegetable consumption for preschool children should be emphasized. Also a nutrition education program is needed to enhance consuming calcium and iron intake for adequate growth.

Dietary Factors and Serum and Urinary Electrolytes Affecting Blood Pressure in Adolescents (사춘기 혈압에 영향을 미치는 식이 인자 및 혈액과 뇨중 무기 이온 농도)

  • Correlations of adolescents' blood pressure with dietary factors and blood or urinary electrolytes were investigated in this study. Through the screening for blood pressures of 960 middle school students aged 13-16 years, 30 students with the mean value of SBP and DBP above 96th percentile(high blood pressure group) and the other 30 with the mean blood pressure between 48th and 52th percentiles(normal blood pressure group) were selected as the subjects. SBP/DBP of the high and talc normal blood pressure groups were $141.8{\pm}9.0$ / $83.6{\pm}5.1$ mmHg and $116.4{\pm}3.5$ / $69.8{\pm}3.7 mmHg$, respectively. The average values of age, weight, height, BMI, degree of relative crude physical activity, and family income were not different between two groups. Among nutrients for which intakes were determined by 24 hour-recall and expressed as percent RDA, intakes of total and animal calcium were lower in the high blood pressure group compared to the normal, and were negatively correlated with both SBP and DBP even when the effects of related general and other dietary factors were excluded. Whereas energy, total, animal and vegetable protein, total and animal lipid, and riboflavin were higher in intake in the high blood pressure group than in fille normal, and they were in positive correlations with SBP and/or DBP. Vitamin A and ascorbic acid intakes were also negatively correlated with DBP, though ascorbic acid intake was not different between two groups. High blood pressure group preferred higher concentration of thin rice gruel than normal group and talc preferred salt concentrations was ill positive correlation with SBP. Serum and urinary levels of Ca, Mg, Ca/Mg ratio, Na, K and Na/K ratio did not show any differences between two groups. However serum Ca/Mg retio was in negative and se겨m Mg was ill positive correlations with SBP, and urinary Na excretion was negatively correlated with SBP. Calcium intake showed negative correlation with serum Ha and Na/K ratio. Our data indicated that dietary intake of Ca, energy, protein, lipid, vitamin A and C as well as salt preference were associated with blood pressure in adolescents. It might be assumed that Ca lowered blood pressure by increasing serum Ca/Mg ratio and decreasing serum Ca/K ratio though urinary excretion of Na.

Microbial Diversity in Three-Stage Methane Production Process Using Food Waste (음식물 쓰레기를 이용한 3단계 메탄생산 공정의 미생물 다양성)

  • Anaerobic digestion is an alternative method to digest food wastes and to produce methane that can be used as a renewable energy source. We investigated bacterial and archaeal community structures in a three-stage methane production process using food wastes with concomitant wastewater treatment. The three-stage methane process is composed of semianaerobic hydrolysis/acidogenic, anaerobic acidogenic, and strictly anaerobic methane production steps in which food wastes are converted methane and carbon dioxide. The microbial diversity was determined by the nucleotide sequences of 16S rRNA gene library and quantitative real-time PCR. The major eubacterial population of the three-stage methane process was belonging to VFA-oxidizing bacteria. The archaeal community consisted mainly of two species of hydrogenotrophic methanogen (Methanoculleus). Family Picrophilaceae (Order Thermoplasmatales) was also observed as a minor population. The predominance of hydrogenotrophic methanogen suggests that the main degradation pathway of this process is different from the classical methane production systems that have the pathway based on acetogenesis. The domination of hydrogenotrophic methanogen (Methanoculleus) may be caused by mesophilic digestion, neutral pH, high concentration of ammonia, short HRT, and interaction with VFA-oxidizing bacteria (Tepidanaerobacter etc.).

A Comparison between Postmenopausal Osteoporotic Women and Normal Women of Their Nutrient Intakes and the Evaluation of Diet Quality (폐경 후 골다공증 여성과 정상 여성의 영양소 섭취상태와 식사의 질 평가에 관한 비교 연구)

  • The purpose of this study was to compare the nutrient intake and diet quality of postmenopausal osteoporotic women to those of control subjects, and to investigate the relationship among diet quality and bone mineral density in postmenopausal women. In this study, we classified the subjects into the postmenopausal osteoporotic women (n =38) and control (n = 43) according to their lumbar spine bone mineral density and age. Dietary intakes, anthropometric measurements and dietary quality indices were measured and evaluated. The average age of osteoporotic and control group were 60.4 yrs and 58.3 yrs, respectively and there was no significant difference. Body weight, body mass index of osteoporotic group were significantly lower than those of control group. The average energy intake of osteoporotic and control group were 1243.3 kcal and 1475.8 kcal, respectively and there was a significant difference. The osteoporotic group consumed significantly lower quantities of protein, plant protein, vitamin A, vitamin $B_1,\;vitamin\;B_2$, folate, vitamin C, calcium, animal calcium, plant calcium, iron and zinc compared to the control group. The osteoporotic group consumed significantly lower of food, vegetables, mushrooms, fruits intakes compared to the control group. In the diet quality, protein, vitamin A, vitamin $B_1,\;vitamin\;B_2$, folate, vitamin C, calcium, iron, Zinc nutrient adequacy ratio (NAR) of osteoporotic group were significantly lower than that of control group. Mean adequacy ratio (MAR) of osteoporotic and control group were 0.63 and 0.78, respectively and there was significant difference. To evaluate nutrient density, Index of nutritional quality (INQ) was calculated by dividing nutrient content per 1,000 kcal of diet with RDA per 1,000 kcal. The average dietary variety score (DVS) of osteoporotic and control group were 22.4 and 33.2, respectively and there was significant difference. DVSs of pulses (p<0.01), seeds (p<0.01) and vegetables (p<0.05) in osteoporotic group were significantly lower than those of the control. In conclusion, postmenopausal osteoporotic women had lower protein, vitamin A, folate, vitamin C, calcium, iron zinc intake quality and vegetables, mushrooms, fruits DVSs than those of the control. Therefore, to promote skeletal health enough energy and food should be consumed, and the maintenance of vitamin and mineral balance by increasing of vitamin A, folate, vitamin C, calcium, iron of intakes are very important.

Reproducibility and Validity of a Self-Administered Semiquantitative Food Frequency Questionnaire (자기기록식 반정량 식이섭취 빈도조사의 신뢰도 및 타당도 연구)

  • This study evaluated the reproducibility and validity of the self-administered semiquantitative food frequency questionnaire used in a large prospective cohort study(Korean Cancer Research Survey) in middle-aged men. The questionnaire was administered twice at an interval of approximately two years(December, 1992-January, 1995), and four or five 24-hour recalls for each subject were collected at intervals of approximately three months. The results were as follows; 1) Although the distributions of the data estimated by the questionnaire were somewhat wider, the mean nutrient intakes of group estimated by our questionnaires and the multiple 24-hour recalls were roughly comparable. 2) The reproducibility determined by correlation of absolute(unadjusted nutrient intake) and calorie adjusted nutrient intakes from two semiquantitative food frequency questionnaires were more than 0.5, and the weighted kappa values were more than 0.4. 3) The Pearson correlation coefficients between unadjusted nutrient intakes values were average 0.40 on the average(Ca, 0.13-Carbohydrate, 0.58) at the first questionnaire vs. 24-hour recalls, and 0.28 at the second questionnaire vs. 24-hour recalls. The spearman rank order correlation coefficients were similar. When energy intake was adjusted, there was a slight reduction : 0.28 at the second questionnaire, 0.25 average on the second. In order to correct the measurement error of 24-hour recall data, the deattenuated correlation coefficient was calculated. It averaged 0.53 on the first questionnaire, 0.37 on the second questionnaire for unadjusted nutrient intake. for calorie-adjusted nutrient intake, it averaged 0.44 on the first questionnaire, 0.37 on the second questionnarie. 4) There was lower agreement(k<0.4) between the questionnaries and the 24-hour recalls. And the subjects classified in the same quartile by 24-hour recalls and first questionnaire were average 37$\%$(energy-adjusted values) and 40$\%$(unadjusted values) on the average. More than k10$\%$(average) of subjects were in the extreme quartile of the questionnarie and 24-hour recall method. But 8.2$\%$(average) of subjects classified in the lowest quartile of unadjusted nutrient intake level by the 24-hour recalls were in the highest quartile by the first questionnaire. These data indicate that our self-administered semiquantitative food frequency questionnarie is reproducible. Correlation coefficients comparing nutrient intakes measured by two different dietary assessment methods were less than 0.5. The validity of our questionnarie is not high enough.

Effect of 12-week Low Calorie Diet and Behavior Modification on the Anthropomeric Indices and Biochemical Nutritional Status of Obese Woman (12주 동안의 저열량식사와 행동수정요법이 비만여성의 체격지수와 생화학적 영양상태에 미치는 영향)

  • This study was conducted to investigate the effect of a 3 week low calorie diet (LCD) and a 9 week of behavior modification (BM) program on the weight loss, mineral and vitamin status in 22 obese women. The subject were healthy, obese (PIBW> $120\%$) women aged 20 - 50 Yr and not taking any medications known to influence body composition, mineral or vitamin metabolism During the LCD program, subjects were provided commercial liquid formulas with 125 kcal per pack and were instructed to have a formula for replacement of one meal and at least one regular meal per day within the range of daily 800 - 1200 kcal intake. During the BM program the subjects weekly attended the group nutrition counseling session to encourage themselves to modify their eating behavior and spontaneously restrict their energy intakes. The BM program focused on stimulus control, control of portion sizes and modification of binge eating and other adverse habits. The initial mean energy intake of subjects was 2016.9 $\pm$ 129.8 kcal ($100.8\%$ of RDA) and dropped to 1276.5 $\pm$ 435.7 kcal at the end of a 3 week of LCD program and elevated to 1762 $\pm$ 329.3 kcal at the end of a 9 week of BM program. Carbohydrate, protein and fat intakes were significantly decreased at the end of the LCD but carbohydrate was the only macro nutrient that showed significant decrease (p < 0.05) at the end of the BM program compared to baseline. Calcium and iron intakes decreased significantly (p < 0.01, respectively) with no significant changes in other micronutrients at the end of the LCD. The mean weight of the subjects decreased from 73.8 $\pm$ 8.0 kg to 69.2 $\pm$ 7.7 kg with LCD and ended up with 67.7 $\pm$ 7.1 kg after 9 weeks of BM. The 3 weeks of LCD reduced most of the anthropometric indices such as BMI, PIBW, fat weight, wast-to-hip ratio and subscapular and suprailiac skinfold thickness. The 9 weeks of behavior modification showed slight change or maintenance of each anthropometric measurements. Weight loss and decreased WHR with the diet program induced significantly decreased systolic blood pressure. SGOT, SGPT and serum insulin levels with improved serum lipid profiles. Biochemical parameters related to iron status such as hemoglobin, hematocrit were significantly decreased (p < 0.01) at the end of the LCD. But their mean values were within normal range. The mean serum 25 (OH) vitamin $D_3$ level significantly increased after whole diet program. Serum folate level significantly decreased after 12 weeks of diet program. In conclusion 3 weeks of LCD brought 4.6 kg reduction in body weight without risk of iron, zinc or vitamin D deficiency and 9 weeks of the BM was effective to maintain nutritional status with slightly more weight reduction (1.5 kg). However calcium intake and serum folate should be monitored during the LCD and BM because of increased risk of deficiencies.

Effects of Body Composition and Nutrients Intake on the Calcaneal Broadband Ultrasound Attenuation in College Students (대학생의 신체계측치와 영양섭취실태가 종골초음파상태에 미치는 영향)

  • The purpose of this study was to analyze the relationship of nutritional status measured by the body composition and dietary nutrients intakes with calcaneal broadband ultrasound attenuation in college students. Total of 886 (462 male and 424 female) students who received health examination in May 2007 participated in this research. Participants bone status was measured by a quantitative ultrasound method and t-score was calculated via WHO guideline. For body composition measurements, body fat, height and weight were measured and BMI was calculated. Dietary data were collected by a 24-hour recall method. Based on Asia-Pacific standard of WHO, BMI was divided into 3 groups; UW (BMI < 18.5), NW + OW (18.5 $\leqq$ BMI < 25) and Obese group (25 $\leqq$ BMI). Among male students, 2.4% belong to the UW group, 45.0% to the NW+OW group and 52.6% belong to the obese group, bwhile 10.4% of female students belong to the UW group, 71.9% to the NW + OW group and 17.7% of female students belong to the Obese group. Differences among male and female students were statistically significant (p < 0.001). Students with higher BMI showed significantly higher bone health status. Male students did not show any significant differences in nutrients intakes by BMI groups while female students showed the higher intakes of energy, protein, pyridoxin, phosphorus, iron and zinc among NW + OW group than other groups (p < 0.05). The qualitative and quantitative evaluation of diet by BMI groups did not show any significant differences in both male and female students. The result of the multiple regression analyses showed that the body fat and bone status was negatively related while energy intake was positively related with the bone status. These results revealed that bone health status was positively affected by BMI but not by body fat. In conclusion, among those who are at their twenties, the period when the bone density becomes maximized, body fat may negatively affect bone health unlike during other life cycle stages.
