Purpose : Primary writing tremor(PWT) can be classified as either type A or type B depending on whether tremor appeared during writing or whilst writing and also on adopting the hand postures normally used for writing. Through the clinical experience author has had an impression that PWT type B may not be purely dependant on specific writing postures. The objective of this study was to clarify whether PWT type B have writing posture-specificity or not. Results : The data indicated that type B PWT is not writing posture-specific. Various pronation and supination postures could evoke tremor as well as writing postures. Furthermore most of other pronation- and supination-related tasks could evoke tremors as well as action of writing. Conclusions : The present data suggest that PWT should be limited only on the pure form of task-spesific PWT type A.
Background: The prevalence of esophageal cancer (EC) with second primary cancers (SPC) is increasing worldwide. This study was aimed to understand the clinical features of EC patients with SPC in the Taiwanese population. Materials and Methods: Clinical and laboratory data for 180 EC patients with or without SPC were collected between January 2009 and December 2013. Information on treatment approaches, location of SPCs and ABO blood type were also collected and stratified. Results: The most common SPC in EC patients was hypopharyngeal cancer, followed by laryngeal cancer and hepatocellular carcinoma in our study. Malignancies of colon, prostate and lung were also found. There was a significant higher portion of blood type A in the EC patients with SPC compared with those without (42.4% vs 19.5%, P=0.006). Conclusions: The frequency and SPC site distribution and blood type A should be considered in clinical evaluation of EC patients with a high risk of developing SPC in the Taiwanese population.
■ Objectives The purpose of this study is to report on a case that showed improvement in type 2 diabetic patients by using herbal medicine, Yeoldahansotang-gami. ■ Methods Yeoldahansotang-gami was given to patients with type 2 diabetes for 71days. To evaluate the effect, blood glucose was measured 4 times a day. As measured blood sugar, the frequency of hyperglycemia, changes in fasting blood sugar, changes in postprandial blood sugar, and changes in glucose variability were analyzed. The patient's insulin injection dose change was observed, and HbA1c and glycated albumin were measured. Follow-up was performed for 7 months to observe whether the treatment effect was maintained. ■ Results During treatment, the patient's blood sugar control, glucose variability, and HbA1c were improved, and insulin injection dose was gradually reduced and stopped. HbA1c and glycated albumin levels maintained improvement without insulin injection during the follow-up period. ■ Conclusion This study showed the effect of yeoldahansotang-gami on type 2 diabetes patient.
Purpose: The purpose of this study was to examine clinical competence and organizational socialization according to communication style of preceptors as perceived by new nurses. Methods: From May 1 to July 31, 2014, data were collected from 180 new graduated nurses who had been working for 3~12 months in hospitals in Busan and one in Gyungnam. Data were analyzed using descriptive statistics, t-test, ANOVA, and Pearson correlation coefficients. Results: Preceptors's communication style perceived by the new nurses included supportive type, reflective type, emotive type, and directive type in order of precedence. The average score for clinical competence was $2.82{\pm}0.31$, and the average score for organizational socialization was $3.05{\pm}0.40$. There were significant differences in nurses' clinical competence(F=9.087, p<.001) and organizational socialization (F=30.129, p<.001) according to preceptor's communication style. Conclusion: Results indicate that supportive and emotive communication styles of preceptors were important factors in the success of the preceptees' clinical competence and organizational socialization.
Purpose: Mucopolysaccharidosis type II (MPS II or Hunter syndrome) is a rare lysosomal storage disorder caused by iduronate-2-sulfatase (IDS) deficiency. MPS II causes a wide phenotypic spectrum of symptoms ranging from mild to severe. IDS activity, which is measured in leukocyte pellets or fibroblasts, was reported to be related to clinical phenotype by Sukegawa-Hayasaka et al. Measurement of residual plasma IDS activity using a fluorometric assay is simpler than conventional measurements using skin fibroblasts or peripheral blood mononuclear cells. This is the first study to describe the relationship between plasma IDS activity and clinical phenotype of MPS II. Methods: We hypothesized that residual plasma IDS activity is related to clinical phenotype. We classified 43 Hunter syndrome patients as having attenuated or severe disease types based on clinical characteristics, especially intellectual and cognitive status. There were 27 patients with the severe type and 16 with the attenuated type. Plasma IDS activity was measured by a fluorometric enzyme assay using 4-methylumbelliferyl- ${\alpha}$-iduronate 2-sulphate. Results: Plasma IDS activity in patients with the severe type was significantly lower than that in patients with the attenuated type ($p$=0.006). The optimal cut-off value of plasma IDS activity for distinguishing the severe type from the attenuated type was 0.63 $nmol{\cdot}4hr^{-1}{\cdot}mL^{-1}$. This value had 88.2% sensitivity, 65.4% specificity, and an area under receiver-operator characteristics (ROC) curve of 0.768 (ROC curve analysis; $p$=0.003). Conclusion: These results show that the mild phenotype may be related to residual lysosomal enzyme activity.
Objectives : This study was undertaken in order to establish clinical application of Taegeuk acupuncture treatment by Sasang(4-type) Constitutional medicine. Methods : 1. Dr. Lee Byung-haeng suggested nine applications of Taegeuk acupuncture treatment of Soyang(lesser Yang) type men. 2. The author summarized the application of Taegeuk acupuncture treatment gathered by clinical experience. 3. The author researched the effect of Taegeuk acupuncture on patients after treatment. Results & Conclusions : 1. Taegeuk acupuncture treatment is effective against psychogenic disease. 2. Taegeuk acupuncture treatment is effective against autonomic nervous system dysfunction(For example, blood circulation disorder, etc.) and has an effect on recovery from sub-health. 3. The author conclude that Taegeuk acupuncture treatment for incurable diseases or diseases of unknown etiology needs a further clinical study in the future.
본 연구는 간호대학생의 에니어그램 성격유형과 성격유형별 임상실습 스트레스 정도를 파악하여 추후 학생상담 방향 설정 및 관리프로그램 개발을 위해 시도되었다. 이를 위해 임상실습을 하고 있는 일 지역 간호학과 3, 4학년을 대상으로 한국에니어그램영성연구소에서 개발한 에니어그램 성격유형 검사지와 임상실습스트레스 측정도구를 사용하여 측정하였고, SPSS 25.0프로그램을 사용하여 분석하였다. 본 연구결과 에니어그램 성격유형은 9개 유형 중 2유형이 47.66%, 힘 중심영역은 감정형 영역이 64.84%로 가장 많았다. 에니어그램 성격유형에 따른 임상실습스트레스정도는 스트레스 하부 영역 중 실습교육환경은 6유형, 바람직하지 못한 역할모델은 4유형, 실습업무부담과 대인관계갈등은 5유형, 환자와의 갈등은 4유형에서 높았다. 에니어그램 힘중심 영역에 따른 임상실습 스트레스정도는 사고형이 실습교육환경과 바람직하지 못한 역할 모델, 실습업무부담, 대인관계 영역의 스트레스가 높았고, 환자와의 갈등영역은 감정형이 높았다. 본 연구결과 간호대학생들의 에니어그램 성격유형은 다양하고, 성격유형별 힘 중심영역별 임상실습스트레스의 차이가 있었다. 따라서 향후 간호대학생의 성격유형 분석을 통한 학생 상담과 지도가 필요하다.
Background: The consensus is that a bony Bankart lesion shorter than 25% of the length of glenoid does not affect the clinical result; hence, such lesions were often neglected. However, small bony Bankart lesions are associated with various types of capsulolabral lesions. Methods: A total of 82 patients who had undergone arthroscopic capsulolabral lesion repair surgery for anterior shoulder dislocation were reviewed. The prevalence rates of early and late type of capsulolabral lesions were compared between a group of patients with and a group without small bony Bankart lesions. In addition, the types of accompanying capsulolabral lesion were analyzed according to the type of bony Bankart lesion. Finally, the clinical outcomes were evaluated (active range of motion, American Shoulder and Elbow Surgeons score and Rowe's score). Results: Among the 13 patients who had small bony Bankart lesions, the prevalence rate of early and late type of capsulolabral lesions was 38.5% and 61.5%, respectively. Among the 69 patients without bony Bankart lesion, the prevalence rates of early and late type of capsulolabral lesions were 74% and 26%, respectively. Significantly worse clinical outcome was observed for the group of patients with both small bony Bankart lesions and late type of capsulolabral lesion. Conclusions: More severe type of small bony Bankart lesion appears to be associated with late type of capsulolabral lesion. The significantly worse clinical outcome for patients with both small bony Bankart lesion and late type of capsulolabral lesion indicates that small bony Bankart lesions cannot always be neglected.
Purpose This study aims to categorize the types of health, analyze the effects among health types based on network analysis find the most important type of health, and explain whether the results between health types vary depending on demographic characteristics. Design/methodology/approach This study investigated individual physical, clinical, mental, and social health(social capital and social support) levels through a survey of 100 people. Network analysis was applied to the survey data to confirm the degree centrality of nodes. Furthemore, we investigated the differences in core nodes according to gender and age groups. Findings According to the analysis result, social support was the most important health type in the entire group. Furthermore, the importance of health type was different depending on the characteristics of the groups. In the case of men, clinical health was the most important health type, and social support was analyzed to be the most important for women. In the case of young people, clinical health was the most important health type, and mental health was the most important health type in the middle-aged.
Over 60 cases were enlisted, but only 31 cases among 24 patients were eligible with a minimum follow-up of 1 year and complete medical documents with imaging data. There were 18 boys and 6 girls, and 7 patients had bilateral lesions. The age of the patients ranged from 2 to 20 years(mean:10.5 years). At their first visit, most lesions had a highly characteristic location and radiographic appearance of radiolucent lesion(s) ranging from 1 to 3cm, except for one case of 5.5 cm in the posteromedial comer of distal femoral metaphysis. The margins were generally well-defined, although some were ill-defined. After reviewing our cases from the viewpoint of clinical course and radiographic patterns, we divided these lesions into two types. Type I is the osteolytic lesion excavated into the posteromedial aspect of the distal femur without cortical defect; and type II is the buldged out lesion of the femur with cortical irregularity into the surrounding soft tissues. Both types have distinctive clinical courses. Type I lesions were easy to make a definite diagnosis with plain radiographs alone, but in type II, it was sometimes very difficult to differentiate it from malignant tumors or chronic localized osteomyelitis. For this lesion, Gd-enhanced MRI was the most effective method for differential diagnosis. In this study, biopsy was not necessary to confirm the diagnosis. Clinical symptoms of type I were very minor or even absent. Many of them were accidentally found after minor trauma around the knee joint. Clinical symptoms disappeared far earlier than radiographical lesions. No treatment such as restriction of activity or drugs was necessary. For type II, the clinical symptoms were more accentuated and lasted longer, and it was necessary to restrict the activity for a certain period in many cases. However, all were self-limited.
