The Relationship of Clinical Symptoms with Social Cognition in Children Diagnosed with Attention Deficit Hyperactivity Disorder, Specific Learning Disorder or Autism Spectrum Disorder

  • Objective One of the areas of social cognition is Theory of Mind (ToM) is defined as the capacity to interpret, infer and explain mental states underlying the behavior of others. When social cognition studies on neurodevelopmental disorders are examined, it can be seen that this skill has not been studied sufficiently in children with Specific Learning Disorder (SLD). Methods In this study, social cognition skills in children diagnosed with attention deficit hyperactivity disorder (ADHD), SLD or Autism Spectrum Disorder (ASD) evaluated before puberty and compared with controls. To evaluate the ToM skills, the first and second-order false belief tasks, the Hinting Task, the Faux Pas Test and the Reading the Mind in the Eyes Task were used. Results We found that children with neurodevelopmental disorders as ADHD, ASD, and SLD had ToM deficits independent of intelligence and language development. There was a significant correlation between social cognition deficits and problems experienced in many areas such as social communication and interaction, attention, behavior, and learning. Conclusion Social cognition is an important area of impairment in SLD and there is a strong relationship between clinical symptoms and impaired functionality.

Electroencephalographic Abnormalities in Children and Adolescents with Autism Spectrum Disorder (자폐스펙트럼장애 아동청소년에서 뇌파 이상)

  • Objectives : The aim of this study was to estimate the prevalences of electroencephalographic (EEG) abnormalities and epilepsy in children and adolescents with autism spectrum disorder (ASD). In addition, we intended to identify demographic and clinical correlates of epilepsy in ASD. Methods : A total of 140 children and adolescents (age $7.3{\pm}4.8yrs$, 106 boys) with ASD underwent EEG from January 2010 to December 2013 at Asan Medical Center. Medical records were reviewed for demographic information, clinical characteristics, psychiatric diagnoses and comorbidities, EEG findings and neurological diagnoses. Results : The prevalences of EEG abnormalities and epilepsy in children and adolescents with ASD was 62.1% and 38.6%, respectively. In subjects with seizure-like movements, EEG abnormalities and epilepsy were more frequent than those without seizure-like movements (EEG abnormalities : 92.5% vs. 43.7%, p<.001 ; epilepsy : 90.6% vs. 5.7%, p<.001). ASD subjects who had epilepsy were older (p=.001), had lower full scale intelligence quotient (p<.001) and took more antipsychotics (p=.006) than those who did not. Conclusion : The prevalences of EEG abnormalities and epilepsy in our sample were similar to those from Western countries. Our results suggested a possible association of older age, lower intelligence quotient, and antipsychotics use with epilepsy in ASD. Conduct of further prospective study in a larger sample is needed.

Discriminant Validity of the Child Behavior Checklist for Ages 1.5-5 in Diagnosis of Autism Spectrum Disorder (자폐스펙트럼장애 진단에서 Child Behavior Checklist 1.5-5 유아 행동평가척도 부모용의 변별력)

  • Objectives: The purpose of this study was to verify the validity and clinical cutoff score of the Child Behavior Checklist for ages 1.5-5 (CBCL 1.5-5) for diagnosis of autism spectrum disorder (ASD). Methods: 44 ASD infants and 100 normal infants participated. T-test, discriminant analysis, receiver operating characteristic (ROC) curve analysis, and odds ratio analysis were performed on the data. Results: Discriminant validity was confirmed by mean differences and discriminant analysis on the subscales of Withdrawn, Attention problems, Internalizing problems, Externalizing problems, Total problems, and all Diagnostic and Statistical Manual of Mental Disorders (DSM)-oriented scales between the two groups. ROC curve analysis showed that Withdrawn, Attention problems, Internalizing problems, Externalizing problems, Total problems, DSM pervasive developmental problems, DSM attention deficit/hyperactivity problems, and DSM oppositional defiant problems significantly predicted ASD infants compared to normal infants. In addition, the clinical cutoff score criteria adopted in the Korean CBCL 1.5-5 for subscales of Withdrawn, Attention problems, Internalizing problems, Externalizing problems, Total problems, DSM pervasive developmental problems, DSM attention deficit/hyperactivity problems, and DSM oppositional defiant problems were shown to be valid. Conclusion: The subscales of Withdrawn, Attention problems, Internalizing problems, Externalizing problems, Total problems, DSM pervasive developmental problems, DSM attention deficit/hyperactivity problems, and DSM oppositional defiant problems significantly discriminated for the diagnosis of ASD.

Extended-Spectrum $\beta$-Lactamase (ESBL) Typing of Klebsiella pneumoniae Isolated from Clinical Specimen in Pusan (부산시내 종합병원의 임상 검체에서 분리된 Extended -Spectrum $\beta$-Lactamase 생성 Klebsiella pneumoniae의 형별 분류)

  • This study was performed to investigate the biological characteristics of twenty isolates of extended spectnlm $\beta$-lactamase (ESBL) producing Klebsiellapnezm~onia collected kom the various clinical specimens of three hospitals in Pusan. Isoelectric focusing (IEF) and PCR were used to determine the types of $\beta$-lactamase gene in this study. Twenty isolates of ESBL producing Klebsiellnp~ieun~or~iae could be divided by PCR, such as TEM type (I1 strains), SHV type (8 strains); non TEM non SHV type (1 strain). In the isoelechic focusing test, the PI of TEM type was 5.2-6.0 and that of SHV type was 6.9-7.4. According to the pI value and PCR bands, twenty strains of ESBL Klebsiellapneumoniae were divided into 5 types: TEM type @I 5.2-6.0; 1080 bp on PCR band), TEM + SHV type (pI 5.2-6.0; andpI 7.0-7.4; 1080 bp and 599 bp on PCR band), SHV type (pl7.0-7.4; 599 bp on PCR band), non TEM non SHV type, and otber type (PCR result was SHV type but pI was not detected).

Mutation spectrum of NF1 gene in Korean unrelated patients with neurofibromatosis 1: Six novel pathogenic variants

  • Purpose: Neurofibromatosis 1 (NF1) is one of the most common autosomal dominant diseases caused by heterozygous mutation in the NF1 gene. Mutation detection is complex owing to the large size of the NF1 gene, the presence of a high number of partial pseudogenes, and the great variety of mutations. We aimed to study the mutation spectrum of NF1 gene in Korean patients with NF1. Materials and Methods: We have analyzed total 69 unrelated patients who were clinically diagnosed with NF1. PCR and sequencing of the NF1 gene was performed in all unrelated index patients. Additionally, multiplex ligation-dependent probe amplification (MLPA) test of the NF1 and SPRED1 gene analysis (sequencing and MLPA test) were performed in patients with negative results from NF1 gene sequencing analysis. Results: Fifty-five different variants were identified in 60 individuals, including six novel variants. The mutations included 36 single base substitutions (15 missense and 21 nonsense), eight splicing mutations, 13 small insertion or deletions, and three gross deletions. Most pathogenic variants were unique. The mutations were evenly distributed across exon one through 58 of NF1, and no mutational hot spots were found. When fulfilling the National Institutes of Health criterion for the clinical diagnosis of NF1, the detection rate was 84.1%. Cafe-au-lait macules were observed in all patients with NF1 mutations. There is no clear relationship between specific mutations and clinical features. Conclusion: This study revealed a wide spectrum and genetic basis of patients with NF1 in Korea. Our results aim to contribute genetic management and counseling.

Clinical Characteristics of Methylphenidate Use in Korean Children and Adolescents with Autism Spectrum Disorder : A Retrospective Study (소아청소년 자폐 스펙트럼장애에서 메칠페니데이트 사용의 임상적 특징에 대한 후향적 분석)

  • Objectives : The purpose of this study was to investigate clinical characteristics of children and adolescents with autism spectrum disorders (ASDs) using methylphenidate (MPH). Methods : Retrospective review of the charts of 79 children and adolescents with ASDs, who visited the Department of Child and Adolescent Psychiatry of Seoul National Hospital, from July 2010 to July 2011, was conducted. Changes in illness severity and improvement were measured using the Clinical Global Impression-Severity of illness (CGI-S) and Clinical Global Impression-Improvement (CGI-I) Scales. Results : We found that MPH was prescribed in 23 (29.1%) children and adolescents. Of the 23 patients on MPH, 4 patients (17.4%) were on MPH monotherapy and 18 patients (78.3%) were using risperidone concomitantly. MPH was prescribed primarily for symptoms of hyperactivity and impulsivity in ASDs patients. The mean dosage of MPH was $26.2{\pm}11.1$mg/day and mean duration of treatment was $31.9{\pm}28.7$ months. Mean CGI-S score improved significantly from baseline to endpoint (from $5.4{\pm}0.6$ to $4.1{\pm}0.9$ ; p<.01). MPH was reported to be effective in 17 patients (17/23, 73.9%), and 10 patients (10/23, 43.5%) reported side effects. Side effects included decreased appetite (4/23, 17.4%), tic (2/23, 8.6%), sleep disturbances (2/23, 8.6%), headache (1/23, 4.3%) and irritability (1/23, 4.3%). Conclusion : The results of this study demonstrate that MPH may be used effectively and safely in children and adolescents with ASDs with hyperactivity and impulsivity. Future controlled trials are needed to confirm these findings.

A Retrospective Study of Autism Spectrum Disorder in Children Treated by Korean Medicine (한의학적 치료를 받은 자폐 스펙트럼 장애 환아 63례에 대한 후향적 차트리뷰)

  • Objectives This study aimed to analyze the clinical characteristics of 63 patients with autism spectrum disorder (ASD) who received Korean medicine treatment. Methods This study identified and retrospectively reviewed the first visit records and clinical characteristics of all of the patients who visited the Kyung Hee University Korean Medicine Hospital and Kyung Hee University Korean Medicine Hospital at Gangdong in South Korea with ASD as their chief complaints. The herbal medicines that were prescribed according to the patient's symptoms were analyzed by separating them into categories. Results The data of 83 children with ASD were collected. Among them, the primary medical record charts of 63 patients were reviewed. Additional analysis was performed for 56 patients who were treated with herbal medicine. Through a retrospective chart review, the patients' clinical characteristics and the tendency of the prescribed herbal medicine was identified. Conclusions This study was conducted to provide preliminary data that proposes Korean medicine as a new alternative medical treatment. Our study is meaningful as it used the demographic and clinical records of 63 patients with ASD in Korea who visited an Korean medicine hospital in the last 11 years to identify tendencies and analyzed the prescriptions of two or more Korean medicine pediatricians.

The prevalence of Extended Spectrum β-Lactamase type produced by Clinical Isolates of Escherichia coli from ChungCheong Area (충청 지역에서 분리된 대장균이 생성하는 Extended Spectrum β-Lactamase 유형의 검출 빈도)

  • The study aims primarily to evaluate the resistance of antibiotics and the prevalence of these enzymes among Escherichia coli the most frequent isolate of Enterobacteriaceae producing Extended-Spectrum ${\beta}$-Lactamase(ESBLs), to differentiate the types of enzymes in these isolates. Total 74(26.2%) Strains of producing ESBLs among the 282 E. coli isolates were isolated from hospitals of Chungcheong area (Daejeon, Chungnam, and Chungbuk) during a 6 month-period from February to July, 2013. 282 E. coli isolates including ESBL shown resistance rates of aztreonam 30.8%, Cefotaxim 30.9%, and Ceftazidime 32.2%, 74 isolates producing ESBLs in E. coli were resistant rates to Aztreonam 58.1%, Cefotaxim 100%, and Ceftazidime 63.5% of ${\beta}$-lactam antibiotics. CTX-M-2 (48 isolates) was the most prevalent type of ESBLs identified. Followed the order of frequency by PER-1 (28 isolates), VEB-1 (26 isolates) and CTX-M-8 (20 isolates), of the 74 isolates, 2 isolates only showed GES-1 in Chungnam province. Accurate identification type of ESBLs would aid in hospital infection control. This would give aid to the physician to prescribe more appropriate antibiotics.

Febrile Urinary Tract Infections Caused by Community-Acquired Extended-Spectrum β-Lactamase-Producing and-Nonproducing Bacteria: A Comparative Study (Extended-Spectrum β-Lactamase 생성 균주와 비생성 균주에 의한 지역사회 획득 요로 감염 비교)

  • Purpose: The purpose of this study was to investigate the clinical characteristics and outcome of febrile urinary tract infections (UTIs) caused by community-acquired extended-spectrum ${\beta}$-lactamase (CA-ESBL)-producing and -nonproducing bacteria. Methods: We analyzed febrile UTIs in children hospitalized at Gachon University Gil Medical Center from January 2011 to December 2013 through retrospective data collection from their medical records. Results: Among pathogens causing 374 episodes of UTIs, the proportion of ESBL-producing bacteria was 13.1% (49/374). The proportion of ESBL-producing Escherichia coli and Klebsiella spp. was 13.6% (48/354) and 5.0% (1/20), respectively. There was no significant difference between the CA-ESBL and CA non-ESBL groups in duration of fever ($4.2{\pm}2.7$ vs. $3.7{\pm}2.1$ days, P=0.10) and bacterial eradication rate with empirical antibiotics (100% vs. 100%). The risk of cortical defects on renal scan significantly depended on existence of vesicoureteral reflux rather than ESBL production of pathogen. Conclusions: There was no significant difference between the CA-ESBL and CA non-ESBL groups in renal cortical defects and clinical outcome. Careful choice of antibiotics is important for treatment of community-acquired UTI in children.

  • This study assess their quality of radiation on analysis of the spectrum of resolution suggesting IEC 61267 in radiation quality that RQA3, RQA5, RQA7, RQA9 and combination of clinical condition using several quality of radiation. In experiments edge method first, the spatial resolution assessment used image of the additional filter and SID is obtained the IEC 62220-1, spatial resolution and sharpness of the obtained image was evaluated in the MTF value 10%(0.1), MTF value 50%(0.5) using a Matlab program. Second, MCNPX simulation used spatial resolution analysis was radiation quality particle fluence and spectrum analysis in energy. As a result, make use of additional filter, image quality evaluation of SID that RQA3 radiation quality combination qualification is higher spatial resolution and sharpness make unused of additional filter and SID 100cm. RQA7 radiation quality combination qualification is higher that spatial resolution make unused of additional filter and SID 150cm. RQA9 radiation quality combination qualification is higher that spatial resolution and sharpness make used of additional filter and SID 180cm. spectrum analysis of radiation quality by reducing consequent errors occurring in the experiment that error due to the reproducibility of the X-ray tube, occur in an error of correction the detector suggest ideal conditions from spectrum analysis through MCNPX simulation. In conclusion, by suggesting spatial resolution and sharpness of result for various radiation quality, It provide basic data that radiation quality condition and quantitative assessment method for laboratory in clinical using detector evaluation.
