The influence of the Clinical Learning Environment and Learning Transition on Satisfaction with a Gerontological Nursing Clinical Practicum in Nursing Students

  • This study used a descriptive survey design to examine the impact of the clinical learning environments and learning transition nursing students experienced the gerontological nursing clinical on the satisfaction with clinical practice. A convenient sample of 211 4th year nursing students who had the gerontological nursing clinical practicum from one College of Nursing at Private University in South Korea was recruited and completed the surveys from October to December 2019. This study showed that the satisfaction with a gerontological nursing clinical practicum was significantly correlated with clinical learning environments and learning transition. The results of this study highlights the need to create a safe and positive clinical learning environment for quality gerontological nursing clinical practicum, so hospitals and nursing schools need to make efforts to promote clinical sites as an educational learning environment in collaborative relationships.

한의 임상 정보의 효율적 통합을 위한 한의임상 데이터베이스 및 E-CRF 입력 시스템 구축 (Implementation of database and E-CRF for efficient integration of Korean clinical data)

  • 최근 의학기술의 발전과 더불어 서양의학 뿐만 아니라 한의학 분야에서도 임상 데이터에 대한 통합 및 표준화에 관한 연구가 활발히 진행 중에 있다. 유사한 임상시험 뿐만 아니라 전혀 다른 임상시험의 데이터도 하나의 표준에 맞춰 통합 구축된다면 통합된 의료데이터는 암묵적 한의의료지식 도출연구에 활용될 수 있다. 따라서 본 논문에서는 한의임상 정보를 효율적으로 저장하기 위하여 국제표준으로 널리 사용되는 CDISC 표준안을 기반으로 한의임상 데이터베이스를 구축하였고, 임상현장에서 편리한 데이터 입력을 위해 E-CRF를 구축하였다. 아울러, 실제 4개의 임상연구에 대한 데이터 저장과정을 거쳐 한의임상 데이터 통합에 대한 예를 보였다. 우리의 연구 결과는 통합된 데이터로부터 암묵적 의료지식도출을 위한 기반을 마련하였고, 데이터 통합을 통한 효율적 관리뿐만 아니라 반복적이거나 불필요한 임상시험 방지, 정제 된 데이터의 재배포를 통하여 연구의 편리성과 협업을 촉진할 수 있다.

간호대학생의 회복탄력성, 임상실습 수행능력, 임상실습 스트레스 간의 융합관련성 (Convergence among Resilience, Clinical Performance ability, Stress in Clinical Practice of Nursing Students)

  • 본 연구는 간호대학생의 회복탄력성과 임상실습 수행능력, 임상실습 스트레스 간의 융합관련성을 파악하기 위한 서술적 조사연구이다. 연구 대상자는 6개월 이상 임상실습을 경험한 간호대학생 433명이었다. 연구 결과는 다음과 같다. 대상자의 회복탄력성은 평균 3.58(${\pm}.39$)점으로 나타났으며, 임상실습 수행능력은 평균 3.65(${\pm}.50$)점, 임상실습 스트레스는 3.40(${\pm}.62$)점으로 나타났다. 또한 대상자의 회복탄력성과 임상실습 수행능력 및 임상실습 스트레스에 유의한 영향을 미치는 요인은 대학생활 만족도, 간호학 전공만족도 및 임상실습 만족도로 나타났다. 본 연구에서 회복탄력성, 임상실습 수행능력, 임상실습 스트레스 간의 상관분석을 실시한 결과, 회복탄력성과 임상실습 수행능력은 정적 상관관계(r=.510)를, 회복탄력성과 임상실습 스트레스 사이에는 부적 상관관계(r=-.153),를 보였다. 본 연구 결과에 기초하여 간호대학생의 회복탄력성을 증진시키고, 임상실습 수행능력을 향상시켜 임상실습 스트레스에 긍정적으로 대처할 수 있는 프로그램 개발 및 적용이 필요하다.

적극적 스트레스 대처매개 중재가 임상실습 스트레스와 만족도에 미치는 영향 (Effects of Aggressive Stress Response Mediation on Clinical Practice Stress and Satisfaction)

  • Purpose: This study aimed to determine the mediating effects of aggressive stress response on the association between clinical practice stress and clinical practice satisfaction in ophthalmic optics majors and provide basic data that could help form opticians' professionalism and make clinical practice more effective. Methods: This study was conducted in 236 sophomores and juniors in Daegu and North Gyeongsang Province who had finished clinical practice. Hierarchical regression analysis and mediating effect testing were performed to determine the associations among clinical practice stress, clinical practice satisfaction, and aggressive stress response and determine the mediating effects of aggressive stress response on the association between clinical practice stress and clinical practice satisfaction. Results: Clinical practice stress was significantly negatively correlated with clinical practice satisfaction and aggressive stress response (p<0.01), and aggressive stress response was significantly positively correlated with clinical practice satisfaction (p<0.01). The association between clinical practice stress and clinical practice satisfaction was more likely to be explained by the addition of clinical practice stress than by being aggressive stress response controlled, which statistically significantly met the criteria for mediating effects (p<0.001). Conclusions: It is necessary to pay more attention to program development and teaching methods within the department of ophthalmic optics so that ophthalmic optics majors can utilize aggressive stress response to overcome clinical practice stress effectively and improve clinical practice satisfaction.

간호교육에서의 비판적 사고, 임상적 추론, 임상적 판단 개념의 고찰 (A Review for Concept Clarification of Critical Thinking, Clinical Reasoning, and Clinical Judgment in Nursing Education)

  • Purpose: This article is a comprehensive review for concept clarification of critical thinking, clinical reasoning, and clinical judgment, which still lack a consensus and are of mixed use. Methods: Norris's method of concept clarification was used to review concepts that have no clear definition or conceptualization yet. Results: This review summarized literature from various disciplines, classified each concept based on similarities and differences, and provided hypothetic conceptual schema. Conclusion: Clinical reasoning and clinical judgment are clinical situation specific concepts, while critical thinking is a concept applied in general situations. Critical thinking is a broader concept and serves as a foundation for clinical reasoning and clinical judgment. Clinical reasoning precedes clinical judgment. Clinical judgement implies the end point or conclusion of clinical reasoning. Each of critical thinking, clinical reasoning, and clinical judgment is a cognitive and affective process not a psychomotor process. The concept of clinical competency involves action taken after the cognitive processes of clinical reasoning and clinical judgment.

한방임상시험센터의 수요 예측에 대한 조사연구 (Survey on the Demand for Clinical Trial Centers for Oriental Medicine)

  • Objectives: This study was carried out to estimate the demand for clinical trials and contribute to the consolidation of function and role in the clinical trial centers of oriental medicine. Methods: A survey of the demand was made of 379 faculties of oriental medicine in 11 oriental medical colleges throughout the country using two?part investigations by e-mail questionnaire over 1 month. The final data from 56 faculties were analyzed. Results: Clinical trials of oriental medicine have been continuously increasing. Furthermore, the demand and necessity for clinical trials of oriental medicine have also been continuously enhanced. Although there were arguments for and against the establishment of a clinical trial center for oriental medicine, the demand and necessity for the establishment was the overall consensus. Concerning objections, a high intention of use was observed. Conclusions: These results imply that the demand for clinical trials will increase remarkably through the establishment of a clinical trial center for oriental medicine. Further study is needed about the demand for such a center.

암 관련 인지 장애의 한의학적 치료에 대한 문헌 고찰 (A review of traditional Korean medical treatment for cancer-related cognitive impairment)

  • Objectives: This article aims to review clinical studies related to traditional Korean medicine (TKM) for cancer-related cognitive impairment (CRCI) up to date. Methods: EMBASE, Pubmed and eight Korean databases were searched for clinical studies about TKM for CRCI up to July 2016. We selected articles about subjects with cancer, using TKM interventions including acupuncture, moxibustion or herbal medicine, containing assessment about cognitive function. Results: Two randomized controlled trials (RCTs) and one randomized controlled feasibility study were included. Selected studies were containing assessments about cognitive function but not as primary outcomes. The three studies included one acupuncture study and two herbal-medicine studies. Only one herbal-medicine study showed improvement in cognitive function. Conclusions: Few clinical studies mainly focusing CRCI treated with TKM have been performed and have shown contradictory results until now. Future studies should be performed considering findings from pre-clinical and clinical studies altogether, and they should be controlled to minimize risk of bias thorough out overall courses of designing, conducting and reporting them.

SBAR 프로그램이 암 병동 간호사의 의사소통명확성, 임상수행능력과 자기효능감에 미치는 효과 (Effects of SBAR Program on Communication Clarity, Clinical Competence and Self-efficacy for Nurses in Cancer Hospitals)

  • Purpose: The purpose of this study was to develop Situation, Background, Assessment, Recommendations(SBAR) program and to test the effects of the program on communication clarity, clinical competence, and self-efficacy for clinical nurses in cancer hospitals. Methods: This study applied a nonequivalent control group non-synchronized design. There were 28 participants in the intervention group and 27 in the control group. SBAR program consisted of 4 parts and applied for 6 weeks in the intervention group. Data were analyzed with ${\chi}^2$ test, Fisher's exact test, t-test and ANCOVA using the SPSS program. Results: Communication clarity, clinical competence and self efficacy were significantly increased in the intervention group compared to those in the control group. Conclusion: These results suggest that SBAR program may improve communication clarity, clinical competence and self-efficacy for clinical nurses in cancer hospitals. Therefore, it is required to actively take advantage of this program to improve communication clarity among medical staffs, clinical competence, and self-efficacy in clinical nursing practices.

근거이론적 접근방법을 적용한 간호학생들의 임상실습 경험 (The Study on the Experience of Clinical Nursing Practice among Nursing Students using Grounded Theory Approach.)

  • This study is to understand and to explain how nursing students experience in the clinical nursing practice using grounded theory approach. Ten nursing students were participated in the study. Seven of them were junior students at the junior college of nursing, and the rest were the senior students at the university. The data were collected by in-depth individual interviews by investigators during May and June in 1995. The results of the study were as follows ; Twenty-eight concepts and 9 categories were emerged by the constant comparative analysis. The 9 categories include 'the need of role model', 'non-educational practical setting', 'knowledge deficit', 'emotional changes', 'the attitudes of clinical practice', 'fatigue', 'skepticism', 'pride', and 'the product of clinical practice'. The core category which encompasses all 9 categories was emerged as 'the process of formulating the nursing view'. Five hypotheses were derived from the analysis. 1) The desirable role model would enhance to pride of nursing in students. 2) Non-educational practical setting would increase skepticism of nursing in students. 3) Knowledge deficit would negatively affect on experience of clinical practice. 4) The pride of clinical practice would formulate a positive nursing view. 5) The skepticism of clinical practice would formulate a negative nursing view. The results of this study are to use as basic data for students attending clinical experience.

경기지역 일부 치위생과 학생의 대인관계능력, 사회적 지지와 임상실습만족도와의 관계 (Relations between Interpersonal Ability, Social Support, and Clinical Practice Satisfaction of Some Dental Hygiene Students in Gyeonggi province)

  • Objectives: This study aimed to provide the high-quality clinical practice environment education to students by researching the social support, major satisfaction, and interpersonal ability of dental hygiene students, analyzing factors having influence on the clinical practice satisfaction, and then increasing the satisfaction with clinical practice. Methods: 312 students who have completed clinical practice among dental hygiene major of three colleges in Gyeonggi province were collected by convenience sampling from April 18th to April 27th 2016. Using SPSS 20.0, descriptive analysis, t-test, ANOVA, correlation analysis, and regression analysis were conducted. Results: The clinical practice satisfaction was correlated with interpersonal ability(r=0.383, p<.001), social support(r=0.239, p<0.01), subjective health condition(r=0.226, p<0.01), and major satisfaction(r=0.287, p<0.01). When interpersonal ability and social support were high, subjective health condition was good, and major satisfaction was high, the clinical practice satisfaction was also high. Conclusions: Based on the results, it would be necessary to develop operational reinforcing social support and interpersonal ability of dental hygiene students, and also to make efforts to increase the clinical practice satisfaction of dental hygiene students by establishing measures to increase their major satisfaction.
