Clinical Competency for Directing of Registerd Nurses정 National Examination. (간호사 국가시험 방향설정을 위한 임상수행 능력- 기본간호학, 성인간호학, 정신간호학, 여성건강간호학, 지역사회간호학, 아동간호학, 간호행정을 중심으로 -)

  • For producing large numbers of professional nurses who could manage 21th century's human health, it is necessary to review the direction of registered nurses' national examination which evaluates the nursing education and is granted a licence. For adapting to social expectation of the nurse, we have to nurture the nurses' problem solving capability in clinical setting. Seven divisions of Korean Academy of Nursing suggested clinical competency according to their categories. This paper was presented in the work-shop for setting up direction of registered nurses' national examination. We expect that this paper would be more refine and confirm through reviewing subdivisions' learning objectives and discussing clinical minimum level of competence contents with clinical leaders.

Factors Influencing Metacognition, Communication Skills, and Confidence in the Performance of Core Basic Nursing Skills on Clinical Competency in Nursing Students (간호대학생의 메타인지, 의사소통능력 및 핵심기본간호술 수행자신감이 임상수행능력에 미치는 영향)

  • Purpose: The aim of this study was to identify the effect of metacognition, communication skills, and confidence in the performance of core basic nursing skills on clinical competency in nursing students. Methods: A total of 158 students were recruited from two nursing colleges in B metropolitan city. Data were collected by means of self-reported questionnaires from December 3-31, 2018 and analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients and multiple linear regression with SPSS 21.0. Results: Metacognition, communication skills, and confidence in the performance of core basic nursing skills were higher than normal and the relationship between the variables metacognition (r=.69, p=<.001), communication skills (r=.69, p=<.001), and confidence in the performance of core basic nursing skills (r=.59, p=<.001) were highly positively correlated with clinical performance. Factors influencing these three variables accounted for 64.4% of clinical competency. Conclusion: Further research on the development of the curriculum and intervention program, and the verification of the effect are needed.

The Effect of Emotional Intelligence and Self-Efficacy on Clinical Competence of the Nursing Students (간호 대학생의 감성 지능과 자기 효능감이 임상수행능력에 미치는 영향)

  • The purpose of this study aimed to find the effect of emotional intelligence and self-efficacy on clinical competence of the Korean nursing students. A total of 199 nursing students participated in this study. Data collection was conducted through the use of questionnaires constructed to include Wong & Law Emotional Intelligence Scale (WLEIS), General Self-efficacy Scale, and the Clinical Competence Scale. As a result, emotional intelligence, self-efficacy, and clinical competency of the Korean nursing students were found to be at moderate levels(Emotional intelligence: M=4.7, SD=0.81; Self-efficacy: M=3.2, SD=0.34; Clinical competency: M=3.4, SD=0.56). Also, the nursing students with higher grades and satisfaction on clinical practice were found to have significantly higher emotional intelligence, self-efficacy, and clinical competence. Moreover, there were significant positive correlations between emotional intelligence and clinical competency(r=.566, p<.001), self-efficacy and clinical competency(r=.440, p<.001). The factors affecting clinical competency were emotional intelligence, self-efficacy, and gender. They amounted to 49.3% in clinical competency. These results indicate a need to develop effective teaching methods and learning strategies to promote clinical competency of the nursing students.

  • Purpose: The purpose of this study was to evaluate the effects of standardized patient simulation on nursing students' communication skills, problem-solving skills, critical thinking dispositions, and clinical competency. Methods: A one-group pretest-posttest design was used. Data were collected from a convenient sample of 47 junior nursing students at W University. Scenarios to train SP and checklists to evaluate the students''competence were developed by our research team. The collected data were analyzed with descriptive analysis t-test, Pearson's Correlation Coefficient and Cronbach's ${\alpha}$ using SPSS WIN 21.0 Program. Results: The simulation practicum using a standardized patient was conducted based on four steps, namely, selections of scenario modules, standard patient training, implementation, and evaluation. The differences between the pre and post-test scores of problem-solving skills(t=-2.94, p=.005) and clinical competency(t=-2.84, p=.007) were statistically significant. But communication skill(t=-.32, p=.747) and critical thinking(t=-.68, p=.498) was not different in group. Conclusion: Standardized patient simulation in nursing education may be useful the improvement of problem-solving skills and clinical competency.

Convergence Analysis of the Factors Influencing Clinical Competency among Nursing Students Participated in Simulation-based Practice (시뮬레이션 실습을 경험한 간호대학생의 임상수행능력에 영향을 미치는 융합적 요인)

  • Objectives: The purpose of the study was to identify the factors influencing the nursing students' Clinical Competency. Methods: A sample of convenience was 185 nursing students, and a questionnaire was used to measure their self-leadership, critical thinking disposition, self-directed learning ability, problem solving ability and clinical competency. Results: A significant positive correlation was found among clinical competency, self-leadership, critical thinking disposition, self-directed learning ability and problem solving ability. Grade of which the participant was in, interpersonal relationship, critical thinking disposition, problem solving ability, and self-directed learning ability were significant predictive variables of which accounted for 53% of the variance in clinical competency. Conclusions: The results from this study can be used to develop the programs for improving clinical competency.

  • As medical practices and procedures become more specialized and information technology develops in clinical settings, health organizations need medical personnel with special skills, knowledge and competency. But the lack of practical experience in clinical settings may impair competency in basic nursing skills among nursing students. This study used a cross-sectional survey to analyze factors related to nursing competency among nursing students in order to establish effective teaching methods to increase the clinical competency of nursing students. The data were collected from a questionnaire distributed in several universities among 106 nursing students who expressed a willingness to participate in the study during December 2011 in order to measure self-directedness, professional self-concepts, communication ability, learning satisfaction. The data were analyzed using the SPSS window program. There were differences regarding competency in basic nursing skills according to interpersonal relationships, grades, existence of an open lab. The level of learning satisfaction, communication skills and self-directedness were deemed influencing factors regarding competency in basic nursing skills. These 3 elements account for 49.9% of competency with regard to basic nursing skills. According to existing research, blended learning methods which consist of problem based learning, cyber education or case centered education should be considered as effective teaching methods for developing clinical nursing skills.

Influence of Career Plateau on the Job Satisfaction and Nursing Competency of General Hospital Nurses (종합병원 간호사의 경력정체가 직무만족과 간호역량에 미치는 영향)

  • Purpose: This study was conducted to examine the degree of career plateau in general hospital nurses, to examine the affects of career plateau on nurses' job satisfaction and nursing competency. Methods: The sample consisted of 234 general hospital nurses. Data were analyzed using t-test, ANOVA, Pearson's Correlation and Hierarchical Multiple Regression. Results: Nurses' career plateau was associated with their job satisfaction, nursing competency. A statistically significant difference in job satisfaction was career plateau, charge nurse or higher in position and resilience predicted 41% of variance in job satisfaction of general hospital nurses. A statistically significant difference in nursing competency was total clinical experience, career plateau, preceptorship experience, charge nurse or higher in position and resilience predicted 43% of variance in nursing competency of general hospital nurses. Conclusion: These results of this study as presented above show that general hospital nurses' career plateau is associated with their job satisfaction and nursing competency. Career plateau is the most important variable in nursing competency. Based on the findings, general hospital nurses' career plateau management is empirically verified as a useful and effective method to increase their job satisfaction and nursing competency.

Survey of use of Evaluation tools for Student구s Clinical Competency (간호학 실습교육 평가도구에 관한 현황조사)

  • The purpose of this study was to survey the use of a evaluation tools of clinical competency for nursing students. The sample consisted of the departments of nursing in 14 universities and 20 Junior colleges of nursing. Data analysis was done by frequency, percentages and factor analysis. The results of the study were as follows : 1. A common measurement tools for evaluation in the clinical area was used by 74.4% of universities and Junior colleges of nursing. Only 0-4.5% of Junior colleges of nursing and 1.5-7.4% of universities used a evaluation tools developed according to their major. 2. Theoretically, 3% of those sampled applied the nursing process as an instrumental means of nursing practice. Bloom's theory was applied by 35.8% of the schools. Most of them used their own measurement tools for evaluating their students. 3. One half of them used quantitative scales, the other half used others. 4. Professional attitudes wire included in their contents bvy 93.9% of universties and 94.1% of Junior colleges of nursing. The major areas of evaluation were knowledge, skills, attitudes and interpersonal relationships in that order. Results ; From this study can be concluded that regardless of the number of academic years of nursing and professional area, common standard evaluation tools for nursing competency were found to be needed. Theoretically, an evaluation scheme which applies the nursing process should be required. Knowledge, skill, attitude and interpersonal relationship would be essential elements to be evaluated. Maximizing the clinical competency and minimizing the conflict elements for nursing students is important. Nursing, education, students and environmental aspects must be consider in the goal of clinical education. A diagram, a checklist and a anecdote note in addition to the quantitative scale are necessary for efficient evaluation.

Effects of a Simulation-based High-risk neonatal Care Education on Learning Satisfaction, Class Participation, Learning Motivation and Clinical Competency in Nursing Students (고위험 신생아간호 시뮬레이션 교육이 간호학생의 수업만족도, 수업참여도, 학습동기 및 임상수행능력에 미치는 효과)

  • The aim of this study was to verify the effects of simulation-based high-risk neonatal care education on learning satisfaction, class participation, learning motivation and clinical competency of Nursing Students. The participants were 118 nursing students in year 3 in D college. The questionaire survey on learning satisfaction, class participation, learning motivation and clinical competency were conducted before and after education to the nursing students. The data was analysed by the SPSS WIN 17.0 program. The results showed that the scores of learning satisfaction, class participation, learning motivation and clinical competency were significantly higher than before education. Also, clinical competency have significant relationships with learning satisfaction, class participation and learning motivation.

Effects of Core Competency and Teaching Style on Preceptor Self-efficacy Among Preceptors (프리셉터의 핵심역량과 지도유형에 따른 프리셉터 자기효능감)

  • Purpose: This study was aimed to find out the effect of core competency and teaching style on preceptor self-efficacy among preceptors. Methods: One hundred twelve nurses working at four university hospitals with previous preceptor experience participated in the survey. The data were analyzed by t-test, ANOVA, Pearson correlation coefficients and multiple regression. Results: The preceptors used 'judgment-initiative' teaching style most frequently, and reported the highest scores in the role model dimension of core competency. There were significant positive relations between age (r=.266, p=.005), clinical experience (r=.274, p=.004), preceptorship experience (r=.204, p=.032), core competency (r=.593, p<.001) and preceptor self- efficacy. But preceptor self-efficacy was not significantly different depending on the teaching style (F=0.72, p=.54). The most predictive factors of the preceptor self-efficacy were core competency and teaching style (judgment)(F=31.849, p<.001). The explained variance for preceptor self-efficacy was 35.9% in the model. Conclusion: The preceptor self-efficacy is essential for the preceptors' successful teaching experience and the clinical competency improvement of the entry level nurses. Management for an effective preceptor training program needs to focus on the improvement of core competency of preceptors, which will lead to strengthen their self-efficacy.
