신약개발을 위한 임상연구간호사의 역할수행과 관련 요인 (Role Performance and Related Factors of Clinical Research Nurses in New Drug Development)

  • Purpose: The purpose of this study was to explore the role performance and related factors of clinical research nurses (CRN) in the process of clinical trials of new drug development. Methods: Study participants were CRN whose affiliation was in non-capital areas and who had been working longer than three months at the time of the data collection. The data collection was conducted with a semi-structured questionnaire development by Hwang & Go, 2011 comprising 16 items of participants' characteristics and 60 items of role performance via in-person and online. Additionally, opinions which would be necessary to establish the CRN role were questioned for the future. A total of 141 questionnaires of 151 questionnaires responses were analyzed using descriptive statistics, t-test, and ANOVA for data analysis as appropriate. Results: The level of overall role performance was above average (5.61±0.90) with the highest in subject care (6.04 ± 0.96) and management and the lowest in self-development (4.39 ± 1.52). The only factor affecting overall role performance was employment status and the only sub-category affected by factors of employment status, and some general characteristics was self-development. Conclusion: From study results, it can be concluded that the CRN seem to perform their proper role. Nonetheless, self-development should be considered as critical aspect for better CRN competency, which is an important aspect toward improving the CRN role performance. Additionally, efforts to improve the level of role performance must be established through stable employment and concrete departmental placement as suggested in this study.

종합병원 입원 환자의 간호대학생 실습허용에 대한 견해 및 인식 (Opinions and Perceptions on Allowing Nursing Students' Practice among Inpatients at a University Hospital)

  • Purpose: The aim of this study was to explore the patients' perspectives on nursing students' clinical practices in the wards, and to investigate their willingness for allowing students to practice on them. Methods: This was a descriptive study. 116 inpatients were recruited from the S University Hospital. A 60-item questionnaire was applied to collect the data. The participants were 19 years and older with sound judgement, and were not in special or intensive care units. Data analysis was done in SPSS/WIN 22.0 using descriptive statistics, Fishers exact test, and the ANOVA test. the participant answered to questionnaire from April 29th 2016 to May 10th. Results: 40 participants (34.5%) stated they would allow students' practice, while 72 (61.2%) said they would allow only under staff supervision. 5 participants (4.3%) stated they would not allow whatsoever. The 3 most allowed were emotional support, oral care, and vital signs measurement while the 3 least allowed were gastric feeding, intravenous catheterization, and urinary catheterization. Conclusion: Patients were more inclined to allow students to practice on them when a member of the medical team was present. A fair number of participants said they would be more inclined to allow students' practice if they felt the student was competent; hence, reinforcing simulation sessions is vital in enhancing students' competency and ultimately practice allowance.

간호교육에서의 전문직 간 교육에 대한 현황과 발전방향 (Current Status and Future Direction of Interprofessional Education in Nursing Education)

  • This study examined the perception and readiness of nursing educators regarding interprofessional education (IPE), and discussed the validity and application of IPE in nursing. From December 2016 to January 2017, 239 nursing professors and nurses completed a structured questionnaire consisting of general characteristics, the Interdisciplinary Education Perception Scale, the Readiness for Interprofessional Learning Survey (RIPLS), and an IPE action plan. The collected data were analyzed by descriptive statistics and t-test using the IBM SPSS ver. 23.0 program (IBM Corp., Armonk, NY, USA). The analysis revealed that 91.6% of the participants had not experienced IPE, and only 11.7% knew about IPE. However, approximately 80.0% answered that IPE is necessary. The results of this study showed that the score of the perceived need for cooperation was higher in nurses than it was in professors, while the score on competency and autonomy was higher in professors than it was in nurses. With reference to the scores on the RIPLS, those of professors were high on the sub-scales of teamwork and collaboration, professional identity, and roles and responsibility. The results revealed that participants considered the upper-grade undergraduate years as the ideal time for imparting IPE, and it was deemed suitable to include communication, simulation, and clinical practice in IPE. Doctors, pharmacists, and physiotherapists were thought to require cooperation for IPE the most. Despite the presence of several barriers to IPE, the participants thought that IPE can achieve learning outcomes such as interprofessional communication and cooperation, conflict resolution, and teamwork. It is necessary to cooperate with professionals in the complex clinical environment as professional areas are specialized and subdivided. Therefore, it is necessary to examine the application of IPE in undergraduate education and in on-the-job training.

중환자실 환자가 경험한 인간중심 간호에 관한 질적 내용분석 (A Qualitative Content Analysis of the Person-Centered Care Experienced by Critical Care Patients)

  • Purpose : To explore the person-centered care (PCC) experienced by critical patients. Method : This qualitative study deductively examined the attributes of the PCC model proposed by Jakimowicz and Perry. The participants were 16 patients who were recently discharged from intensive care units at a university hospital. Data were collected through in-depth interviews and were analyzed using the deductive method of content analysis. Results : A total of 4 categories, 16 subcategories, and 33 codes were generated from 171 meaningful statements. The final 4 categories were "compassionate presence," "professional interaction," "outstanding competency," and "patient identity." These were consistent with the main attributes of Jakimowicz and Perry's model. However, most of the codes belonged to the "compassionate presence" and "patient identity" categories. Among the attributes of the model, "continuity of nursing," "therapeutic relationship," "expert knowledge," "clinical knowledge," "evidence-based intervention," and "patient's rights" were not derived as codes. Conclusion : These findings deepen the understanding of the PCC model from the patient's point of view. The main attributes of PCC identified in the current study can be applied to the development of practical guidelines for intensive care nursing. In addition, we recommend the development of a PCC measurement tool for critical care patients.

시뮬레이션 기반 정맥주사 교육이 간호학생의 임상수행능력과 수행자신감 및 대상자 만족도에 미치는 효과 (Effects of Simulation-Based Training on the Clinical Competence and Confidence of Nursing Students in Intravenous Injection Performance and the Satisfaction of Clients)

  • 본 연구의 목적은 시뮬레이션 기반 정맥주사 교육의 효과를 시뮬레이션 매체별로 비교 분석하고 대상자 만족도를 평가함으로써 시뮬레이션 기반 교육이 간호학생의 임상실무수행능력과 수행 자신감을 향상시킴과 동시에 대상자 만족도를 높이는데 도움이 되는 교육 방법임을 검증하는 것이다. 본 연구에서는 간호학과 2학년생 90명을 무작위로 동영상군에 31명, 모형군에 28명, 컴퓨터시스템군에 31명을 배정하였다. 동영상군은 기본간호학회에서 개발한 교육용비디오를, 모형군은 정맥주사모형을, 컴퓨터시스템군은 컴퓨터로 작동되는 정맥주사용 교육프로그램을 이용하여 1회 교육과 5회 실습을 실시한 후 대상자들끼리 짝을 이루어 손등 부위에 정맥주사를 시행하였다. 간호학생의 임상수행능력을 정맥주사 성공률로 분석한 결과, 동영상군 16.1%, 모형군 64.3%, 컴퓨터시스템군 38.7%가 성공한 것으로 나타났으며 세 군간에 유의한 차이가 있었다. 수행 자신감은 정맥 주사 전, 후 모두 세 군간에 유의한 차이가 없었으나 정맥주사 성공여부에 따른 차이를 비교한 결과, 정맥 주사 전, 후에 모두 유의한 차이가 있었다. 대상자 만족도는 세 군간에는 유의한 차이가 없었으나 정맥주사 성공여부에 따라서는 유의한 차이가 있었다. 이상의 연구결과에서 정맥주사 기술은 동영상과 컴퓨터시스템보다 모형을 이용한 시뮬레이션 기반 교육이 간호학생의 임상실무 능력을 향상시키는데 기여한 것으로 나타났으며 정맥주사에 성공한 학생이 성공하지 못한 학생보다 수행 자신감이 더 높은 것으로 나타났다. 그러나 이는 한 가지 간호 기술과 관련된 결과로 추후 다양한 간호기술에 적합한 시뮬레이션 교육매체를 평가하는 연구가 필요하다.

응급실 간호사의 자아탄력성, 재난에 대한 경험 및 재난간호 핵심수행능력과의 관계 (A Study on Ego-resilience, Disaster Experience and Core Competencies among Emergency Room Nurses)

  • Purpose: The study was aimed to describe the relationships between ego-resilience, disaster experience, and core competencies among emergency room (ER) nurses. Methods: Data were collected from ER nurses in Incheon and Kyunggi province from May to June 2014. Data were analyzed with descriptive study, t-test, ANOVA, and Pearson's correlation coefficient using SPSS/WIN 18.0 version. Results: Male respondents showed significantly higher ego-resilience than female respondents (t=-2.04, p=.043), and education (F=4.96, p=.002) and position (F=7.70, p=.001) were statistically significant. The differences in disaster experience was found in gender (t=-2.29, p=.023), age (F=4.25, p=.006), marital status (t=-2.02, p=.045), education (F=6.34, p<.001), religion (F=3.69, p=.015), and position (F=3.55, p=.031). Regarding core competencies for disaster nursing, age (F=7.80, p<.001), marital status (t=-3.00, p=.001), education (F=16.53, p<.001), career (F=5.40, p<.001), position (F=12.52, p<.001) were significantly different. Positive associations were found between core competencies for disaster nursing and ego-resilience (r=.66, p<.001), disaster experience and core competencies for disaster nursing (r=.52, p<.001), and disaster experience and ego-resilience (r=.33, p<.001). Conclusion: Core competencies were higher among ER nurses when they have higher ego-resilience, and more disaster experience. This finding suggests the need for improving ego-resilience among ER nurses. In addition, disaster education needs to be considered as a part of core nursing curriculum in college and clinical settings.

임상실습을 경험한 간호대학생의 공감피로, 공감만족, 영성과 삶의 질의 관계 (The Relationships among Compassion Fatigue, Compassion Satisfaction, Spirituality and Quality of Life in Undergraduate Nursing Students with Clinical Practice)

  • 본 연구는 임상실습을 경험한 간호대학생의 공감피로, 공감만족, 영성과 삶의 질의 관계를 파악하기 위한 서술적 조사연구이다. 연구 대상은 C도에 소재한 3개 대학에서 임상실습을 수행한 4학년 간호대학생 172명이며, 자료는 2020년 11월 7일에서 12월 11일까지 구조화된 설문지를 이용하여 수집되었다. 연구결과, 공감피로의 하위요인 중 소진은 삶의 질과 유의한 부적 상관관계를, 공감만족은 삶의 질과 유의한 정적 상관관계를 나타내었다. 영성은 소진과는 부적 상관관계를, 공감만족과는 정적 상관관계를 나타내었다. 따라서 간호대학생의 공감피로와 공감만족을 조절하여 삶의 질을 향상시키기 위해 영성증진전략의 개발과 적용을 제언한다.

간호학 만족도에 따른 비판적 사고성향과 임상수행능력의 변화 (Differences between Critical thinking Disposition and Clinical practice according to Satisfaction for Nursinga Practice)

  • 본 연구의 목적은 간호대학생에게 수술환자 간호에 대한 교육을 적용하여 간호학 만족도에 따른 비판적 사고성향과 임상수행능력에서 어떤 차이가 있는지 확인하여, 추후 수술환자 간호에 대한 효과적인 교육법 개발의 기틀을 마련하기 위함이다. 연구방법으로 대상자는 Y대학의 간호학과 3학년 155명이었으며, 대상자들은 3학년 첫 수업시간에 교육과정에 대한 전반적인 설명과 함께 실험참여에 대한 동의서와 사전 자가 설문지를 작성하였다. 이 후, 1학기와 2학기 마지막 수업에서도 자가 설문지를 작성하였다. 자료 분석은 SPSS/WIN 23.0 Program을 이용하여 실수, 백분율, 평균, 표준 편차, 𝑥2-test, t-test, repeated measure ANOVA로 분석하였다. 연구 결과, 만족도가 높은 간호학생들과 중간이하로 낮은 학생들의 일반적 특성 중 연령을 제외하고는 두 군 간에 동질성이 검증되었다. 비판적 사고성향은 만족도가 중간 이하인 간호학생들의 비판적 사고성향에 비하여 유의하게 높은 것으로 나타났으며(F=4.57 p=.012). 시간적 변화에 따라서는 유의한 차이가 없는 것으로 나타났다(F=0.20, p=.821). 이에 반해, 임상수행능력은 간호학에 대한 만족도와(F=1.08, p=.343) 시간에 따라서 모두 유의한 차이가 없는 것으로 나타났다(F=1.58, p=.210). 즉, 간호학생들의 만족도를 향상시키는 것이 간호사로서의 핵심역량 중 하나인 비판적 사고성향을 향상시킬 수 있는 방안이 될 수 있음을 유추할 수 있었다. 그러므로 추후 비판적 사고성향을 높여 간호대학생의 문제해결능력을 향상시키기 위해서는 간호학 만족도를 높일 수 있는 노력이 필요할 것이다.

주제범위 문헌고찰에 기반한 교육전담간호사의 역할과 성과에 대한 탐색 (Exploring the Roles and Outcomes of Nurse Educators in Hospitals: A Scoping Review)

  • This study was conducted to examine trends in research about nurse educators in Korean hospitals. The methodological framework consisted of previous work by Arksey and O'Malley. A literature search was performed of electronic databases (RISS, KISS, CINAHL, and PubMed) from January 2000 to June 2022. Each study was collated and analyzed, and the studies' data were abstracted into the following categories: publication year, study design, study participants, and education program details. Eight studies were reviewed. Most of the studies were published after 2020 (87.5%). The most frequently used design was quasi-experimental (50.0%). There were three studies whose subjects were nursing educators and five studies were conducted on programs in which nurse educators participated. In Korean medical institutions, clinical nurse educators have various roles, including the development and operation of programs and serving as facilitators and evaluators. In most studies, knowledge and skills were measured as outcome variables of educational programs led by nurse educators. The roles of nurse educators were diverse in Korean medical institutions. Therefore, a program to enhance the competency of nurse educators is needed. Judging from the effects of nurse educators in Korean medical institutions, it is necessary to expand the nurse educator system, and additional research on nurse educators should be conducted.

임상실습을 경험하는 간호학생의 환자안전에 관한 융·복합연구 (A Convergence Study on the Patient Safety of Nursing Students who have experienced Clinical Practice)

  • 본 연구는 임상실습을 경험한 간호학생들의 환자안전관리 실천에 영향을 미치는 요인을 분석하기 위해 수행하였다. 연구대상자는 국내 5개 도시의 5개 대학에 재학 중인 간호학생 232명이었다. 자료수집은 2019년 6월 17일부터 7월 30일까지 시행되었고, 구조화된 자가보고식 설문지 작성을 통해 수집하였다. 자료분석은 SPSS 22.0 windows program을 이용하여, T-test, ANOVA, Pearson's correlation coefficient, multiple regression을 이용하였다. 상관분석 결과, 환자안전관리 실천은 환자안전관리 중요성 인식(r= .42, p<.001), 지식(r= .29, p<.001), 태도(r= .35, p<.001)와 유의한 양의 상관관계가 있었다. 다중회귀분석 결과, 환자안전관리 중요성 인식은 환자안전관리 실천에 유의한 영향(β=.19, p<.001)을 미치는 것으로 확인되었으며, 모형의 설명력은 36.2%였다. 이에 본 연구에서는 간호학생들의 환자안전관리 역량을 개선시킬 수 있는 대학단위의 교육이 꾸준하게 이루어질 필요가 있음을 강조한다.
