운동신경원성 질환과 유사하게 발현된 샌드호프병 (Adult Sandhoff Disease Presenting as Motor Neuron Disease Phenotype)

  • We report a 23-year-old woman with adult Sandhoff disease, who presented with motor neuron disease phenotype. The patient had experienced progressive motor weakness in four extremities since 1 year prior to admission. Electrophysiological study revealed wide-spread denervation potentials, and the assay of total hexosaminidase involving A and B activities showed decreased levels of these activities, which was consistent with Sandoff disease. This is the first Korean case of adult Sanhoff disease presented as a motor neuron disease phenotype.

청간소요산으로 호전된 설통 환자 3례 (Three Case Reports of Glossodynia Patients Treated with Cheonggansoyo-san)

  • Objectives: This clinical study reports the effects of Cheonggansoyo-san (CSS) on three patients with glossodynia. Methods: Three patients with glossodynia were treated with CSS (i.e., an herbal medicine) and acupuncture. Numeric rating scales (NRSs) for glossodynia and dry mouth and hwa-byung questionnaire scores were evaluated, and the results from heart rate variability (HRV) evaluations were analyzed. Results: After treatment with CSS, three patients' glossodynia symptoms, such as burning sensations, tingling, and numbness of the tongue, were improved. NRS ratings for glossodynia and dry mouth and the hwa-byung questionnaire scores also improved, and HRV parameters were closer to the normal range. Conclusion: Cheonggansoyo-san could be used to treat glossodynia.

약인성 급성 이상운동증 환자의 한방 치험 1례 (A Case of Drug-Induced Acute Dyskinesia Treated with Korean Medicine)

  • Objectives: The objective of this study was to report a clinical case that suggests potential beneficial effects of Korean medicine therapy for the treatment of levosulpiride-induced acute dyskinesia. Methods: A patient having drug-induced acute dyskinesia was given a series of Korean medicine therapy treatments, including administration of the herbal medicine Chengsimyeonja-tang-gamibang, acupuncture, electroacupuncture, pharmacopuncture, and moxibustion over the 17 days of the hospitalization period. During the therapy, the progression of the disease was measured by the Abnormal Involuntary Movement Scale (AIMS). Results: During the therapy, the remission of orofacial dyskinesia and improvement of AIMS scores were observed. Conclusion: Korean medicine therapy has potential benefits for the treatment of drug-induced acute dyskinesia.

  • Objective: This study examined the effectiveness of Korean medical treatment for an 81-year-old female patient with pontine infarction complaining of discomfort in both legs. Methods: The patient was hospitalized for 45 days and treated with herbal medicine (mainly Bohyulanshin-tang [補血安神湯], pharmacopuncture, acupuncture, electroacupuncture, and moxibustion). We used the International Restless Legs Scale (IRLS) and Numerical Rating Scale (NRS) and checked the duration of the symptoms to evaluate the clinical effects of the treatment. Results: After 45 days of hospitalization, the IRLS score decreased from 33 to 19, with the lowest score of 8 recorded on the twenty-seventh day after discharge. The NRS score and the duration of symptoms also improved, and the treatment effect continued following discharge. Conclusions: This study suggests that Korean medical treatment may be effective for the treatment of restless legs.

다한증 및 좌측 상지 감각이상을 호소하는 중대뇌동맥 경색 환자에 대한 치험 1례 (A Case Report of a Middle Cerebral Artery Infarction Patient Complaining of Hyperhidrosis and Left Upper Extremity Paresthesia)

  • This work reports the case of a middle cerebral artery infarction patient with hyperhidrosis and left upper extremity paresthesia whose condition improved following Korean medicine treatment. The patient was hospitalized for 10 days and treated with herbal medicine, mainly Hwanggigyeji-tang, and acupuncture, electroacupuncture, and moxibustion. The hyperhidrosis was clinically classified and the patient's subjective discomfort in this regard was also checked. A subjective percentage of strength and sensation in the left extremity was evaluated to assess the clinical effects of the treatment. After treatment, the hyperhidrosis classification improved from Grade 3 to Grade 0, and the patient's subjective discomfort was greatly improved and continued for the two weeks after discharge. In addition, subjective strength and sensation improved by 20% and 5%, respectively, compared to time of admission. These results suggest that Korean medicine is effective in treating paresthesia after stroke and long-lasting hyperhidrosis.

  • ■ Objectives The purpose of this case is to report the effectiveness of Korean medicine in the treatment of early stage multiple system atrophy-cerebellar ataxia(MSA-C) ■ Methods The patient was treated with acupuncture, bee venom, and herbal medicine. The clinical symptoms was assessed using unified multiple system atrophy rating scale(UMSARS) and 20M gait time and steps. ■ Results After treatment, UMSARS score and 20M gait time and steps showed improvement. ■ Conclusion This study suggested that Korean medical treatment could be an effective option for treating early stage MSA-C.

  • Electrodiagnostic studies such as nerve conduction studies (NCS) and needle electromyography (EMG) provide important and complementary information for evaluating patients with suspected neuromuscular disorders. NCS and needle EMG are reasonably safe diagnostic investigations and are generally associated with only mild transient discomfort when performed by experienced physicians. However, there is the risk of complications in some patients, because NCS involve the administration of electric current and EMG involves inserting a needle percutaneously into muscle tissue. This article reviews the potential risks of NCS and needle EMG.

  • Myasthenia gravis (MG) and amyotrophic lateral sclerosis (ALS) are distinct disorders. ALS affects motor neurons that control muscle movement, while MG controls communication between neurons and muscles, which occurs at neuromuscular junctions. However, on rare occasions, ALS develops after MG and vice versa. The coexistence of the two diseases represents a diagnostic challenge and requires thoughtful interpretation of clinical features. We present the case of a 53-year-old Korean male who developed ALS after MG, confirmed by clinical and electrophysiological follow-up.

  • ■ Objectives The purpose of this case report is to show the effect of Bojungiki-tang on a patient complaining dysuria and urinary pain caused by repeated urinary tract infection(UTI) occurred after onset of cerebral infarction. ■ Methods A cerebral infarction patient with repeated UTI was treated with herbal medication, Bojungiki-tang. Then we evaluated the improvement with amount of total urine output, nelaton catheterization urine output and self voiding urine output. ■ Results Increase in amount of total and self voiding urine output and decrease in amount of nelaton urine output were observed after taking Bojungiki-tang. ■ Conclusion This case showed the effect of Bojungiki-tang on dysuria and urinary pain caused by repeated UTI.

  • This case study is to report the effectiveness of Hyungbangdojeock-san in the treatment of daytime sleepiness. The patient was diagnosed as a Soyangin, one of the 4 constitution types in Korean medicine, and treated with herbal medicine, Hyungbangdojeock-san and acupuncture treatment for 14 days. The severity of symptom was assessed using Epworth Sleepiness Scale (ESS). After treatment, the severity of daytime sleepiness was reduced. ESS score was decreased from 11 to 2. As a result this case showed the effect of Hyungbangdojeock-san on daytime sleepiness.
