GERD-unrelated Non-cardiac Chest Pain may be Associated with Depression and Anxiety (위식도역류질환과 관련 없는 비심인성 흉통 환자의 우울 및 불안)

  • Objectives : Non-cardiac chest pain (NCCP) can be divided into gastroesophageal reflux disease (GERD) related NCCP and non-GERD related NCCP. Our study was designed to examine the differences in clinical characteristics and psychological mood states between the two clinical syndromes. Methods : After some cardiologic evaluations such as treadmill exercise, coronary angiography, and echocardiography, 27 patients with NCCP were enrolled in this study. They were divided into patients with GERD related NCCP (12 patients) and those with non-GERD related NCCP (15 patients) using the upper gastrointestinal endoscopy and the ambulatory 24 hour esophageal pH monitoring. Clinical characteristics such as typical reflux symptoms and psychological mood states were measured. Patients who showed scores more than 10 on the Beck Depression Inventory (BDI) or Beck Anxiety Inventory (BAI) were defined as depressed or anxious group. Anxiety sensitivity Index (ASI) was also measured in all patients. All parameters were compared between patients with GERD related NCCP and those with non-GERD related NCCP. Results : The two groups showed a difference in typical reflux symptoms. Patients with non-GERD related NCCP had higher scores on the BDI, BAI and ASI than those with GERD related NCCP. Among all NCCP patients, 14 patients (51.9%) were suggested to have possible depression or anxiety disorders. Conclusion : The non-GERD related NCCP was shown to be associated with psychological mood states such as anxiety and depression. Thus, we suggest that routine measurement of psychological mood states should be necessary in the evaluation and treatment of NCCP.

Interim Report about The Comparative Clinical Study of Efficacy of Gamisoyo-san(Jiaweixiaoyaosan) on Anxiety of Generalized Anxiety disorder (범불안장애의 불안증상에 대한 가미소요산의 제형별 효능 비교 임상연구의 중간보고)

  • Objectives : The purpose of this research is to examine the efficacy of Gamisoyosan on anxiety of generalized anxiety disorder according to dosage form. Methods : In this randomized, double blinded, placebo-controlled study, we planned to give Gamisoyosan simple extract mixture or Gamisoyosan compound extract or controlled medication for major symptoms of generalized anxiety disorder. As preparatory research, Hamilton rating Scale for Anxiety(HAM-A) was measured as the 1st evaluative instrument, and Korean State-Trait Anxiety Inventory(STAI-K), Penn State Worry Questionnaire(PSWQ) Korean Beck's Depression Inventory (BDI-K), Symptom Checklist-90-Revised(SCL-90R), WHO Quality of Life Abbreviated(WHOQOL-BREF) and Heart Rate Variability(HRV) were also measured as the 2nd evaluative instrument at the before treatment. Results : Demographic characteristics showed that there are Clinical characteristics-vital signs are within the normal range. The characteristics of disease-chief complaint, pattern Identification and etiological factor of the highest frequency number were worry, heart deficiency with timidity(心膽虛怯), family matters. The average period of disease in subjects is 6.31years. The results of Chest PA, EKG and clinicopathologic examination are within the reference range. The Scores of HAM-A, STAI-K, PSWQ were measured above the cutoff point. There are significant positive correlations among HAM-A, STAI-K, BDI-K and among HAM-A, STAI-K, PSWQ. There are no significant correlations between PSWQ and BDI-K. Conclusions : We considered that selection of subjects in this research is appropriately accomplished And this methodology is expected to be applied to the subsequent research. And also, we hope to make up for this study through various study and discussion.

Effects of Anxiety and Infection Control Job Stress on Depression in Nurses (임상간호사의 불안과 감염관리 직무스트레스가 우울에 미치는 영향)

  • Purpose: This study aims to identify the degree of anxiety and depression related to infection control and job stress in clinical nurses during the COVID-19 pandemic. Furthermore, it seeks to identify the effects of depression. Finally, it intends to develop intervention measures to prevent depression in clinical nurses during epidemics of new infectious diseases and prepare basic data for developing educational programs. Methods: Data were collected using a self-report questionnaire survey administered to 208 nurses. The data were analyzed using the IBM SPSS (Version 26.0) statistics program. Results: The correlation analysis between participant depression, anxiety, and infection control job stress found that depression had a positive correlation with anxiety (r=.63, p<.001) and infection control work stress (r=.26, p<.001). Anxiety (β=0.60, p<.001) was found to significantly affect participant depression with an explanatory power of 40.2%, confirming that higher anxiety levels induce increased depression. Conclusion: Based on the above results, anxiety is confirmed to be a factor that affects depression. Accordingly, education is required to strengthen individuals' physical and mental capabilities and promote a safe working environment, including providing sufficient personal protective devices and other appropriate gear to defend clinical nurses against outbreaks of emerging infectious diseases at medical institutions and through healthcare policies. Other considerations should include sufficient rest hours and paying additional benefits for nurses caring for patients with severe confirmed COVID-19.

  • Objectives : This study was conducted to examine whether there are qualitative differences in attention problem among children with various psychiatric disorders, including attention-deficit hyperactivity disorder (ADHD), depressive disorder, anxiety disorder, and tic disorder using clinical ADHD diagnostic system (ADS) T-scores. Methods : The subjects were 794 outpatient children aged from 5 to 15 years, including 540 children with ADHD, 95 children with depressive disorder, 86 children with anxiety disorder, and 73 children with tic disorder. Clinical T-scores on the ADS were calculated using the mean and standard deviations of four ADS variables for the ADHD group. Results : All four groups had T-scores on the ADS in the abnormal range. However, when comparing the clinical T-scores, the children with depressive and anxiety disorders performed better than the children with ADHD. We also found that although the four groups seemed to be similar in terms of clinical T-scores for omission and commission errors, there were significant differences in clinical T-scores for reaction time and the standard deviation of response time (RT) between the ADHD and other groups. Conclusions : We concluded that inattention and impulsivity might not be specific only to ADHD and that the clinical T-scores of RT and standard deviation of RT on the ADS could be used to discriminate between ADHD and other clinical groups.

Comparison of Clinical Characteristics Between Respiratory and Non-Respiratory Subtypes of Panic Disorder (공황장애 호흡기 아형과 비호흡기 아형의 임상 특성 비교)

  • Objectives : The goal of this study was to compare the clinical characteristics of panic disorder respiratory subtype(PD-R) and non-respiratory subtype(PD-NR). Methods : 84 patients with panic disorder were enrolled and divided into 2 groups, 29 PD-R and 55 PD-NR. Diagnosis of panic disorder was evaluated using Diagnostic and Statistical Manual of Mental Disorders $4^{th}$ edition and Mini International Neuropsychiatric Interview. They were also measured with Hamilton Rating Scale for Anxiety(HAM-A), Hamilton Rating Scale for Depression(HAM-D), and Panic Disorder Severity Scale (PDSS). Results : PD-R group showed significantly higher scores in PDSS than those of PD-NR group(p=.027). After controlling for the severity of panic disorder and gender, PD-R group showed higher HAM-D and somatic anxiety subscale of HAM-A than those of PD-NR group. Furthermore, results of logistic regression analysis suggested that the somatic anxiety was a possible risk factor of PD-R(OR=1.404,p=0.009). Conclusion : These results suggest that somatic anxiety and depressive symptom would be important clinical characteristics of PD-R.

Effects of Classic Music Listening on Anxiety and Psychological Well-Being in Undergraduate Nursing Students during Clinical Practice (고전음악청취가 임상실습 간호대학생의 불안과 심리적 안녕감에 미치는 효과)

  • The aim of this study was to examine the effectiveness of classic music listening for the release anxiety and improvement psychological well being in undergraduate nursing students during clinical practicer. 56 participants received classic music listening weekly for a period of overall 2 weeks program. In experimental group, anxiety (t=2.49, p=.013) have decreased significantly comparing to the control group. Psychological well being (t=4.18, p=.008) have increased significantly comparing to the control group. Conclusions: Classic music listening may be effective at reducing anxiety and improving psychological well being in undergraduate nursing students during clinical practice.

  • Objectives : The purpose of this study was to explore comparison of clinical characteristics among Anxiety disorder, Depressive disorder, and Co-morbid of Anxiety disorder and Depressive disorder. Methods : Since January, 2017 to May, 2019, patients were recruited from the department of psychiatry of Hanyang University Guri Hospital. Participants were diagnosed Anxiety disorder, Depressive disorder, and Co-morbid of Anxiety disorder and Depressive disorder using DSM-5 criteria by board certified psychiatrists or clinical psychologists. And each of participants tested with MMPI-2 and clinical psychologists were converted MMPI-2 to MMPI-2-RF. The data were analyzed using SPSS 18.0. Results : The results indicated EID, RC2, HLP, SAV, and INTR-r were the highest in Co-morbid of Anxiety disorder and Depressive disorder group. And BXD, RC4, JCP, and AGGR-r were the highest in Depressive disorder group. Conclusions : The results indicated that Co-morbid of Anxiety disorder and Depressive disorder group were more severe internalizing dysfunction like lower positive emotion, helplessness, social avoidance and discomfort than single-diagnosed group. Single Depressive disorder group were more serious externalizing dysfunction like anger and aggression than single Anxiety disorder and Co-morbid of Anxiety disorder and Depressive disorder. The implications and limitations were discussed.

  • There appears to be an increased incidence of anxiety symptoms in association with certain medical illness. It is usually assumed that certain medical disorders are directly associated with anxiety symptoms in that the pathophysilolgy of the medical illness somehow causes the anxiety symptoms. However, the pathophysilolgies of many such medical illnesses as well as anxiety are not known and no discrete group of medical illnesses is clearly more likely to produce anxiety symptoms than any other Moreover, anxiety symptoms may be the representation of other psychiatric symptoms, like depression and delirium. Though it is important to differentiate primary and other causes(secondary) of anxiety clinically, the clear differentiation is somewhat arbitrary, as the confusion of mind-body dichotomy. for the purpose of accurate treatment of anxiety symptoms relating medical illness, this review article focuses on those clinical features and mechanisms of anxiety that are potentially useful in differential diagnosis.

The Relations among Mobile internet addiction, Sleep quality, Anxiety and Interpersonal relationship for University students (대학생의 모바일 인터넷 중독 정도에 따른 수면의 질, 불안 및 대인관계 간의 관련성)

  • Purpose. This study was to investigate the relationships among mobile internet addiction, sleep quality, anxiety, and interpersonal relationship in university students. Method. A descriptive study design was used. Between 10 and 31 March 2016, 213 subjects completed a questionnaire consisting of questions on mobile internet addiction, anxiety, sleep quality, and interpersonal relationship. The data were analyzed using descriptive statistics, One way-ANOVA, and Pearson's correlation coefficient SPSS/WIN 21.0 programs. Results. Among mobile internet addiction there were significant differences between gender (t=3.40, p<.001), grade (F= 2.31, p<.001), majority field (F=5.34, p=.005), and take exercise (t=3.50, p<.001). This study has shown that mobile internet addiction were associated with sleep quality (r=-.40, p<.001) and anxiety (r=.14, p=.037). In addition, a correlation has been found significant between anxiety, and sleep quality (r=.48, p<.001), and sleep quality has associated with interpersonal relationship (r=-.46, p<.001). Conclusion. In this study, we revealed the observed a meaningful significant correlation between mobile internet addiction, sleep quality and anxiety. Therefore, it is important to emphasize control of mobile internet addiction for university students. The results of this study can be used as a basis on which to develop a mobile internet addiction intervention program.

  • Contemporary societal and environmental transformations coincide with the emergence of novel mental health challenges. anxiety disorder, a chronic and highly debilitating illness, presents with diverse clinical manifestations. Epidemiological investigations indicate a global prevalence of 5%, with an additional 10% exhibiting subclinical symptoms. Notably, 9% of adolescents demonstrate clinical features. Untreated, anxiety disorder exerts profound detrimental effects on individuals, families, and the broader community. Therefore, it is very meaningful to predict anxiety disorder through machine learning algorithm analysis model. The main research content of this paper is the analysis of the prediction model of anxiety disorder by machine learning algorithms. The research purpose of machine learning algorithms is to use computers to simulate human learning activities. It is a method to locate existing knowledge, acquire new knowledge, continuously improve performance, and achieve self-improvement by learning computers. This article analyzes the relevant theories and characteristics of machine learning algorithms and integrates them into anxiety disorder prediction analysis. The final results of the study show that the AUC of the artificial neural network model is the largest, reaching 0.8255, indicating that it is better than the other two models in prediction accuracy. In terms of running time, the time of the three models is less than 1 second, which is within the acceptable range.
