Understanding of Clinical Trials and Application to the Real Practice (임상시험의 단계별 이해 및 실제)

  • Understanding of a clinical trial is essential in developing clinical guideline and adopting evidence based practice. In designing and executing clinical trials, following ethical requirements should be considered : social value, scientific validity, fair subject selection, informed consent, favorable risk-benefit ratio, institutional review board, and respect for human subjects. According to the stage of drug development, purpose of trials, accumulated scientific data, clinical trials for drug development are classified as phase 1, 2, 3, and 4. Phases of clinical trials can be overlapped and the judgment of entering into the next phase should be considered highly strategically. In reading, evaluating and interpreting clinical trial reports, various skills and challenges exist. Patient sample composition, trial duration, selection of endpoints, responders and non-responders, placebo effect, patient recruitment, and extrapolation to the real world are the examples of those challenges. Treatment success will come from the well balanced approach of evidence based decision making and consideration of specific single case.

Assessing the Validity of the Preclinical Objective Structured Clinical Examination Using Messick's Validity Framework (Messick의 타당도 틀을 활용한 임상실습 전 실기시험의 타당도 평가)

  • Students must be familiar with clinical skills before starting clinical practice to ensure patients' safety and enable efficient learning. However, performance is mainly tested in the third or fourth years of medical school, and studies using the validity framework have not been reported in Korea. We analyzed the validity of a performance test conducted among second-year students classified into content, response process, internal structure, relationships with other variables, and consequences according to Messick's framework. As results of the analysis, content validity was secured by developing cases according to a pre-determined blueprint. The quality of the response process was controlled by training and calibrating raters. The internal structure showed that (1) reliability by generalizability theory was acceptable (coefficients of 0.724 and 0.786, respectively, for day 1 and day 2), and (2) the relevant domains had proper correlations, while the clinical performance examination (CPX) and objective structured clinical examination (OSCE) showed weaker relationships. OSCE/CPX scores were correlated with other variables, especially grade point average and oral structured exam scores. The consequences of this assessment were (1) making students learn clinical skills and study themselves, while causing too much stress for students due to lack of motivation; (2) reminding educators of the need to apply practical teaching methods and to give feedback on the test results; and (3) providing an opportunity for faculty to consider developing support programs. It is necessary to develop the blueprint more precisely according to students' level and to verify the validity of the response process with statistical methods.

A Study on the Efficiency of Predicting Variables of Nurses ′ Clinical Performance flow College Achievements (간호원의 근무성적 예측변인의 효율성에 관한 연구)

  • This study attempts to predict nurses'clinical performance from entrance examination of university and college achievements after graduation. This is based upon a survey study, 49 nurses in Yon Sei Medical Center after graduation of college of nursing. Correlation of college achievements, clinical performance and variables of nurses'clinical performance are produced using Pearson's Product moment correlation coefficient significance of correlation tested by T- test. The result of the study were as follows; 1. Clinical performance of the college achievements is the best efficient variables in predicting the nurses'clinical performance after graduation and next theory scholastic total achievements and entrance examination the last. 2. There are significant correlation among nurses'clinical performance rating variables as nursing ethics, nursing skills, achievements, attitude, the state of health.

Predictive Factors Influencing Clinical Competence in Nursing Students (간호학생의 임상수행능력에 영향을 미치는 예측변인)

  • This study examined the influence of problem solving ability, communication skill and self-efficacy on clinical competence of nursing students. The research subjects were 184 grade 4 nursing students in D and G city. Data were collected from September 1 to September 10, 2016 and analyzed by independent t-test, one-way ANOVA, Pearson's correlation, and stepwise multiple regression using the IBM SPSS/WIN 20.0 program. The results showed that the problem solving ability was 3.60, communication skill was 3.79, and self-efficacy was 3.45. The clinical competence was positively correlated with problem solving ability, communication skill and self-efficacy. Problem solving ability (${\beta}=0.283$, p<0.001), self-efficacy (${\beta}=0.249$, p<0.001), motivation for nursing (${\beta}=0.182$, p=0.002), communication skills (${\beta}=0.176$, p=0.016), and interpersonal relationships (${\beta}=0.101$, p=0.082) explained 42.8% of the total variance in clinical competence of nursing students. The most significant predictors of clinical competence were problem solving ability, followed by self-efficacy and communication skills. Therefore, it is necessary to develop an empowerment program to improve problem solving ability, self-efficacy and communication skills of nursing students.

Critical thinking disposition, stress of clinical practice and clinical competence of nursing students (간호학생의 비판적 사고성향과 임상실습 스트레스 및 임상실습수행능력)

  • Purpose: This study was done to identify the relationship among Critical Thinking Disposition (CTD), stress of clinical practice and clinical competence in Korean nursing students. Methods: Participants were 407 baccalaureate nursing students (3rd and 4th grades) in Korea. Variables included CTD, stress of clinical practice, clinical competence, and demographic variables. Data was analyzed by frequencies, t-test, ANOVA, Pearson's correlation and multiple regression. Results: Clinical competence positively correlated with CTD, but negatively correlated with stress of clinical practice. The regression model explained 25.6% of clinical competence. The significant predictors of clinical competence were intellectual eagerness, intellectual fairness in CTD, conflict with patients, and clinical environment in stress of clinical practice. Conclusions: CTD and stress of clinical practice contribute to nursing student's clinical competence. Therefore, efforts to encourage nursing student's CTD, increase stress management skills, especially in conflict with patients, and build a supportive clinical environment should be made to strengthen clinical competence.

  • The use of resin-bonded fixed partial dentures described in the early 1980s caused an conservative way to preserve tooth structure in the restorative dental community. The treatment of patients with requires long term analysis of clinical application and basic research. Failure rates of these prosthesis ranged from 3% to 55%. These varieties were orginated by different techniques, materials, tooth preparation methods and diverse clinical situations. This article review was focused on the standard long term results and in vitro studies on bond strength between metal and teeth. From this, many useful clinical guidelines to RBFPD could be adopted to clinical dentistry. For successful results, careful case selection and good clinical skills are needed. And appropriate techniques for each situations should be adopted. Also, RBFPD using new materials like all-ceramics, FRC/Ceromer was introduced.

Clinical Competency-Centered Learning Outcomes in Basic Medical Education (진료역량 중심의 기본의학교육 학습성과)

  • Outcome-based learning is a global trend in medical education. The Korean Association of Medical Colleges (KAMC) has been developing learning objectives for basic medical education (BME) in Korea. In 2012, KAMC published the 1st edition of "Learning outcomes of basic medical education: Clinical competency-centered" in order to promote outcome-based medical education. KAMC has recently revised and updated the learning outcomes of basic medical education in the clinical competency-centered 2nd edition to reflect the suggestions of all medical schools in Korea and improve application of the published learning outcomes for BME in the field of medical education. KAMC has been making efforts to integrate clinical competency-centered learning outcomes with scientific concepts and principle-centered learning outcomes in addition to basic clinical skills and performance in BME.

Influence of Major Satisfaction, Communication Skills, Image of Nurses on Nursing Professionalism of Nursing Students (간호대학생의 전공만족도, 의사소통능력, 간호사 이미지가 간호전문직관에 미치는 영향)

  • The purpose of this study was to investigate major satisfaction, communication skills, image of nurses and nursing professionalism, and to identify the influencing factors on nursing professionalism of nursing students. Data were collected using questionnaires from 214 students who were 3rd years in the nursing university in Gwangju, June 2019. The collected data was analyzed by descriptive statistics, pearson's correlation coefficient and stepwise multiple regression using IBM SPSS 21.0 program. The mean scores for the major satisfaction(3.71±0.53), communication skills(3.50±0.38), image of nurses(3.80±0.52), nursing professionalism(3.70±0.56) were above average. It was found that there were significant differences in the nursing professionalism depending on age, motivation for nursing clinical practice. Significant relationships were found among major satisfaction(r=.531, p=.000), communication skills(r=.531, p=.000), image of nurses(r=.836, p=.000) and nursing professionalism in nursing students. The results of stepwise multiple regression indicates that image of nurses(β=.705, p=.000) and communication skill(β=.136, p=.001) and major satisfaction(β=.133, p=.002) and predicts 72.8% in clinical practice competency(F=191.248, p<.001). In conclusion, to enhance nursing professionalism for nursing students, it is necessary to develop and utilize a program that enhances image of nurses, communication skills and major satisfaction.

A Study on the Effectiveness of a Host Family Experience Program on the Adaptive Behavior and Self-Management Skills of Children with Severe Disabilities at a Facility (시설 중증장애아동의 결연가정체험 프로그램을 통한 적응행동과 자기관리기술 효과성 연구)

  • The purpose of this study is to analyze the effectiveness of a host family experience program on the adaptive behavior and self-management skills of children with severe disabilities and to suggest its applicability into clinical practice. The investigators developed a host family experience program and researched the pre-test and post-test designs with nonequivalent groups. The experiment and comparison groups were comprised of 34 children with severe disabilities that lived at S, a care facility for the severely disabled. They were examined for the program's effects on them before and after the application. After analyzing 17 cases for differences in the subjects' quantitative scores, a qualitative analysis was carried out on one representative case. The quantitative analysis results reveal that the experiment group showed a significant increase in their scores of adaptive behavior and self-management skills compared to the comparison group. Positive changes were also observed among the former group in the qualitative analysis. Thus the host family experience program was effective in improving the adaptive behavior and self-management skills of the children with severe disabilities at the facility. These findings indicate that the host family experience program played a positive role in enhancing the adaptive behavior and self-management skills of the children with severe disabilities at the facility and that such host family experience programs should be applied and reinforced in the field of social welfare practice.

Association between indirect vision skills and musculoskeletal pain in dental hygienists (임상치과위생사에서 간접시진 능력과 근골격계 통증과의 연관성)

  • Objectives: The purpose of the study was to analyze the association between indirect vision skills and neck, shoulder, and back pain in dental hygienists. Methods: This cross-sectional study included 85 dental hygienists working full-time at dental clinics. A survey was performed for musculoskeletal symptoms in the neck, shoulders, and back of the subjects. The O'Connor tweezer dexterity test was performed using a mirror to evaluate their indirect vision skills. For statistical analyses, the t-test and one-way analysis of variance were performed. Results: The neck was the most common region of musculoskeletal pain (89.4%). The most intense pain was experienced in the neck (70.6%) and right shoulder (60.0%). Neck pain (20.0%) caused the most interference with clinical practice. Indirect vision skills were lower in the group with severe pain in the left shoulder or with high interference in work due to the left shoulder pain (p=0.026 and p= 0.017, respectively) or right shoulder pain (p=0.004). Conclusions: In this study, neck pain was a major musculoskeletal symptom among dental hygienists, and poor indirect vision skills were associated with the development of shoulder pain. Therefore, to prevent such musculoskeletal symptoms, dental hygienists should become proficient in indirect visualization, which enables a balanced and neutral posture.
