문제중심학습 연계 시뮬레이션 기반 통합간호교육과정을 이수한 신졸업간호사의 간호수행능력과 자신감 (Clinical Competence and Self-confidence of New Graduate Nurses with an Integrated Nursing Curriculum of Simulation with Problem-Based Learning)

  • 본 연구의 목적은 PBL 연계 시뮬레이션 기반 통합간호교육과정을 이수한 신졸업간호사와 교과목 중심의 전통적 간호교육과정을 이수한 신졸업간호사간의 간호수행능력정도와 간호수행능력 자신감의 차이를 표준화환자 기반 간호수행평가를 활용하여 비교분석하는 것이다. 연구대상자는 전통적 간호교육과정을 이수한 신졸업간호사 39명과 통합간호교육과정을 이수한 신졸업간호사 35명, 총 74명이었다. 연구방법은 두 대상자 그룹간에 표준화환자 기반 간호실무능력 평가 시험에서 간호수행능력 평가점수와 간호수행능력 자신감을 비교하기 위한 비동등성 대조군 사후 시차설계를 적용한 유사 실험 연구로 수집된 자료는 SPSS 19.0 프로그램을 사용하여, Fisher's exact test, t-test, Pearson's correlation coefficient, ANCOVA를 이용하여 분석하였다. 표준화환자를 활용한 간호실무수행능력 평가시험을 실시한 결과 통합간호교육과정 이수 신졸업간호사와 교과목 중심의 전통적 간호교육과정 이수 신졸업간호사 간에 표준화 환자가 채점한 간호수행능력 점수는 차이가 없었으나, 통합간호교육과정 이수 신졸업간호사의 교수자가 평가한 기술적 술기 점수, 비기술적 술기 점수와 간호수행능력에 대한 자신감이 대조군에 비해 유의하게 높았다. 본 연구의 결과를 바탕으로 간호교육과정에 PBL 연계 시뮬레이션 기반 교과를 적용할 것을 제언하며, 통합간호교육과정을 이수한 신졸업간호사의 임상간호 현장에서의 실무능력과 환자 성과 변화를 장기추적 관찰하는 추후 연구가 필요하다.

  • 본 연구의 목적은 간호대학생의 비판적 사고성향과 임상의사결정능력 및 환자안전 지식, 태도가 임상실습 시 환자안전수행능력에 미치는 영향을 조사하고 이들 변수간의 관련성을 파악하여 환자안전수행능력의 향상을 위한 중재 프로그램 개발의 기초자료를 마련하기 위함이다. 자료분석은 SPSS 21.0 프로그램을 이용하여 t-tests, one-way ANOVA 등을 실시하였다. 연구결과, 대상자의 환자안전 수행능력은 비판적 사고성향(r=.278, p=.001)과 양적 상관관계,. 임상의사결정능력(r=.202, p=.014)과 양적 상관관계, 환자안전 태도(r=.421, p<.001)와 양적 상관관계가 있었다. 환자안전 수행능력에 미치는 영향요인은 성별(${\beta}=.175$, p=.031), 환자안전 교육(${\beta}=-2.266$, p=.025), 비판적 사고성향(${\beta}=3.354$, p=.001), 환자안전 태도(${\beta}=.368$, p<.001)로서 설명력은 31.5%의 영향력을 보였다(R2=.561, F=4.56 p<.001). 간호대학생의 임상실습 교육에 필요한 환자안전 수행능력을 증진하기 위해서 비판적 사고성향, 임상의사결정능력, 환자안전교육에 대한 교과 과정의 수립이 필요하다.

간호사의 핵심역량과 상사의 직무역량 인식이 간호업무성과에 미치는 영향 (The Effect of Core Competencies and Cognition of Job Competency of Their Senior Nurses on Work Performance in Hospital Nurses)

  • Purpose: This study was to investigate the effect of nurses' core competencies and cognition of job competency of senior nurses on work performance. Methods: Three hundred two nurses participated in the study. The collected data were analyzed with SPSS/WIN 20.0. Frequency, t-test, ANOVA and stepwise multiple regression were utilized. Results: The level of nurses' core competencies was 3.65 (${\pm}.40$) points, cognition of job competency of senior nurses was 4.02 (${\pm}.60$) points, and work performance was 4.04 (${\pm}.55$) points. In accordance with the participant's general characteristics, work performance was found to be significant in their age, marital status, educational level, total clinical experience, length of time on the present ward, and their income. Cognition of job competency of senior nurses was found to be the most effective factor in work performance, followed by core competencies and total clinical experience, and these factors accounted for 63.4%. Conclusion: The study suggested that work performance was highly affected by the nurses' core competencies and cognition of job competency of senior nurses. Therefore, effective education programs are required to provide an appropriate experience to both entry-level and experienced nurses, as well as to help in enhancing nurses' and senior nurses competency at the same time.

임상간호사의 감정노동 경험 (Clinical Nurses' Experience of Emotional Labor)

  • Purpose: The purpose of this study was to understand the experience of emotional labor of clinical nurses in medical institutes. Methods: A total of 26 nurses from 11 hospitals participated in the study. Six focus groups were organized and 4 to 5 nurses took part in each group. The compositional factors of groups included clinical experience, age, gender, work place and position. Data collection was conducted through focus group interview and it was proceeded by the time of data saturation. In this qualitative study, content analysis was conducted. Results: Five themes, 14 categories, and 33 subcategories, were emerged. The themes were 'Restrain themselves', 'Communion to the patients', 'Working environment provoking emotional tension', 'Respond to emotional events', 'Recovery of emotional energy'. Conclusion: Results indicated that surface acting of emotional labor such as, repression of personal desire and presenting the emotions that the organization ask nurse to express was related to psychosomatic symptoms, depression, burnout, poor job performance, increased mistakes, and low job satisfaction which eventually leads to nurses' turnover. In order to reduce negative influence of emotional labor, it is necessary to build positive organizational culture, to provide support from managers and co-workers. It is also important to improve work environment in order to do more deep acting since sharing emotions with patients can reduce the negative influence of emotional labor.

보드게임요법이 만성 정신분열병 환자의 실행능력에 미치는 효과 (Effects of Board Game Therapy on Executive Function in Hospitalized Patients with Chronic Schizophrenia)

  • Purpose: This study investigated changes in neurocognitive function over a 2-month period of board game therapy in patients with schizophrenia. Method: Twenty-one schizophrenic patients treated with board game therapy and nineteen control schizophrenic patients were evaluated with neuropsychological and clinical tests, such as the Wisconsin Card Shorting Test (WCST). The same tests were re-administered after 2 months of board game therapy. Results: At the first series of neuropsychological tests, no difference was seen in performance, demographical aspects, or clinical severity among both patient groups. After 2 months, the group receiving board game therapy showed significant improvement of WCST performances compared to the controls. However, no difference was observed in clinical symptoms between the groups. Conclusion: The results of cognitive enhancement in patients playing board games indicates that board game play, easily used in an inpatient setting, is a promising tool for executive function improvement in chronic schizophrenic patients.

혈액투석실에서 근무하는 간호사의 특성이 근거기반 혈액투석간호지식과 수행정도에 미치는 영향 (The Effects of Characteristics of Nurses on Knowledge and Nursing Performance Evaluation of Evidence Based Hemodialysis Nursing Practice in Hemodialysis Unit Nurses)

  • Purpose: The purpose of this study was to identify the effects of characteristics of nurses on knowledge and nursing performance of evidence based hemodialysis nursing practice among hemodialysis unit nurses. Methods: The participants were 180 nurses working in hemodialysis unit for more than 6 months in 23 private and general hospitals in Seoul and Gyeonggido. Data were collected from March 30th to April 15t in 2016 and were analyzed using stepwise regression analysis, descriptive statistics, t-test and ANOVA. Results: Nurses' knowledge on evidence based hemodialysis nursing practice was $15.87{\pm}4.52$ out of 23 points. Type of hospitals working in and adherence to evidence based hemodialysis guidelines were significant factors to knowledge and these two factors explained 30.0%. Nurses' nursing performance on evidence-based hemodialysis nursing practice was 4.52 out of 5 points. The performace level was significantly related to total nurisng career and necessity of hemodialysis nursing education and these two factors explained 8.0%. Conclusion: A development of guideline and continuing education is necessary for improving knowledge and performance of evidence based hemodialysis nursing practice.

간호과정 적용 평가도구 개발 -산소요법과 흡인간호를 중심으로- (Development of Performance Measures Based on Nursing Process for Oxygen Therapy and Airway Suctioning)

  • Purpose: This study was conducted to develop standards to ensure nursing process-based care of oxygen therapy and airway suctioning and to develop a performance measurement tool to evaluate the care applied according to the standards, and finally to determine validity of the standards and the tool. Methods: The standards and the tool were reviewed by a panel of experts and refined based on the panel's suggestions. Validity of the standards and the tool were examined through surveying a total of 366 hospital nurses. Results: The mean validity scores of the performance measurement standards and the tool were 3.58 and 3.55, respectively, out of 4.00. So the performance measurement standards and the tool in this study were found to be acceptable in evaluating quality of nursing care provided at patient admission and discharge. Conclusion: This result indicates that the performance measurement standards and the tool developed in this study are valid instruments to monitor and improve quality of nursing care for oxygen therapy and airway suctioning.

신규간호사의 실무적응지원 교육 만족도 및 역할 이행 경험에 관한 조사 (New Graduate Nurses' Satisfaction with Transition Programs and Experiences in Role Transition)

  • Purpose: The aim of this study was to investigate the satisfaction of newly graduated nurses with educational programs and their experiences in role transition. Methods: Data were collected from November 1 to December 15, 2018 and 483 new graduate nurses working at 15 tertiary hospitals and 10 general hospitals participated. For data collection, self-report questionnaires including the Casey-Fink Graduate Nurse Experience Survey tool and satisfaction with education were used. Data were analyzed using descriptive statistics, t-test, and one-way analysis of variance. Results: Satisfaction with education ranged from 3.09 to 3.27, and satisfaction with preceptors was 3.45(maximum 4). The skill that new nurses ranked as most difficult during the first 3 months was charting/documentation, and throughout a whole year, the top 4 difficult skills were cardiopulmonary resuscitation/emergency response, ventilator care, end-of-life care, and prioritization/time management. In comfort/confidence, new graduates felt most comfortable with support and least comfortable with patient safety. More than 50 percent of new graduates experienced stress during role transition, and the most frequently experienced stressors were related to job performance and personal life. Levels of satisfaction with education and comfort/confidence differed according to the hospital type and number of preceptors for new nurses. Conclusion: In order to facilitate the transition of new graduate nurses to professional nurse, an extended period of education, systematic and standardized transition programs, and continuous support during the first year of practice are required.

간호학 만족도에 따른 비판적 사고성향과 임상수행능력의 변화 (Differences between Critical thinking Disposition and Clinical practice according to Satisfaction for Nursinga Practice)

  • 본 연구의 목적은 간호대학생에게 수술환자 간호에 대한 교육을 적용하여 간호학 만족도에 따른 비판적 사고성향과 임상수행능력에서 어떤 차이가 있는지 확인하여, 추후 수술환자 간호에 대한 효과적인 교육법 개발의 기틀을 마련하기 위함이다. 연구방법으로 대상자는 Y대학의 간호학과 3학년 155명이었으며, 대상자들은 3학년 첫 수업시간에 교육과정에 대한 전반적인 설명과 함께 실험참여에 대한 동의서와 사전 자가 설문지를 작성하였다. 이 후, 1학기와 2학기 마지막 수업에서도 자가 설문지를 작성하였다. 자료 분석은 SPSS/WIN 23.0 Program을 이용하여 실수, 백분율, 평균, 표준 편차, 𝑥2-test, t-test, repeated measure ANOVA로 분석하였다. 연구 결과, 만족도가 높은 간호학생들과 중간이하로 낮은 학생들의 일반적 특성 중 연령을 제외하고는 두 군 간에 동질성이 검증되었다. 비판적 사고성향은 만족도가 중간 이하인 간호학생들의 비판적 사고성향에 비하여 유의하게 높은 것으로 나타났으며(F=4.57 p=.012). 시간적 변화에 따라서는 유의한 차이가 없는 것으로 나타났다(F=0.20, p=.821). 이에 반해, 임상수행능력은 간호학에 대한 만족도와(F=1.08, p=.343) 시간에 따라서 모두 유의한 차이가 없는 것으로 나타났다(F=1.58, p=.210). 즉, 간호학생들의 만족도를 향상시키는 것이 간호사로서의 핵심역량 중 하나인 비판적 사고성향을 향상시킬 수 있는 방안이 될 수 있음을 유추할 수 있었다. 그러므로 추후 비판적 사고성향을 높여 간호대학생의 문제해결능력을 향상시키기 위해서는 간호학 만족도를 높일 수 있는 노력이 필요할 것이다.

간호근무환경과 간호결과와의 관련성 및 간호사 업무 부담에 대한 내용분석 (The Relationship of the Nursing Work Environment and Nursing Outcome among it's Nurses and Content Analysis of Nurses' Workload)

  • Purpose: The aim of this study was to explore the relationship between nursing work environment and nursing outcome of clinical nurses. Also, the purpose of this study was to identify the nurses' workload. Method: The participants in this study were 246 nurses working at one hospital in a province. Data was collected using a structured questionnaire from October 2011 to November 2011. Open questions were subject to content analysis; closed questions were subject to descriptive statistics and Pearson correlation coefficient with SPSS 18.0 program were used. Result: The number of patients per nurse in day-duty and overtime work had a positive correlation. Nursing performance and the overtime work in night duty nurses had a negative correlation. The results of this study, with a much higher number of patients per nurse, showed an increase in overtime hours. In the nursing delivery system experience of nurses, 13 themes were identified and they were divided into five categories. Conclusion: Therefore, in order to improve nursing performance and increase job satisfaction, efforts must be made to reduce overtime, and nursing personnel placement to secure a substantial nursing staff.
