Correlations of education experience, awareness and practice of infection control during the prosthodontic treatment in clinical dental hygienists (일부 임상치과위생사의 치과보철물 제작 과정에서의 감염관리에 대한 교육경험, 인식 및 실천도)

  • Objectives: The purpose of this study is to investigate the relationship between education experience, awareness and practice of infection control during the prosthodontic treatment in clinical dental hygienists. Methods: A self-reported questionnaire was answered by 255 clinical dental hygienists in Seoul and Gyeonggi areas from October 10 to December 30, 2016. The questionnaire consisted of general characteristics of the subjects (8 items), dental prosthesis infection education experience (5 items), dental prosthesis infection awareness (5 items) and dental prosthesis infection practice (14 items) based on Likert 5 point scale. For statistical analysis, SPSS Statistic 22.0 was used. Results: A significant association was shown among education experience, awareness and practice of infection control during the prosthodontic treatment of clinical dental hygienists (p<0.001). Dental hygienists count, dental prosthesis infection education experience and awareness had positive influences, but the age group from 26 to 30 had negative influence on dental prosthesis infection practice. Conclusions: The study confimed that the dental prosthesis infection education program and continued education is necessary for the safety and health of patients, and to prevent the cross-infections of the clinical dental hygienists.

The Differences of Nurses' Perception of the Code of Ethics, Degree of Application of Nursing Ethics and Biomedical Ethical Consciousness according to Nursing Students' Clinical Practice Experience, Ethical Values and Biomedical Ethics Education (간호대학생의 임상실습, 윤리적 가치관 및 생명의료윤리교육에 따른 간호사 윤리강령인식, 윤리강령 적용정도 및 생명의료윤리의식의 차이)

  • Purpose: This study was conducted to investigate the differences of the Code of Ethics, level of application of the Code of Ethics and consciousness of biomedical ethics of nurses about clinical practice experience, ethical values and biomedical ethics education experience of nursing students. Methods: A descriptive research design, t-test and one-way ANOVA were used. The data were collected 1st June to 31st August 2016 from two large cities. The participants were 246 nursing students with a mean age of 22.57 years. Among them, 209 (84.6%) were female, 94 (38.2%) had clinical practice experience, 155 (63.0%) listed their ethical value as different depending on the situation, and 127 (51.6%) had biomedical ethics education experience. Results: The scores were: awareness of the Code of Ethics of Nurses $4.29{\pm}0.60$; application of the Code of Ethics of Nurses $4.24{\pm}0.6$4; and consciousness of biomedical ethics $2.91{\pm}0.19$. There were significant differences in application of the Code of Ethics of Nurses (t=-1.97, p=.050); consciousness of biomedical ethics (t=-2.25, p=.025) related to clinical practice experience; and consciousness of biomedical ethics related to biomedical education experience (t=2.67, p=.007). Conclusion: Clinical practice and biomedical ethics education experience contributed to enhance the level of application of the Code of Ethics of Nurses and consciousness of biomedical ethics of nursing students.

Clinical Application of the Adenosine Triphosphate-based Response Assay in Intravesical Chemotherapy for Superficial Bladder Cancer

  • Objective: To investigate correlations between adenosine triphosphate chemotherapy response assay (ATP-CRA) and clinical outcomes after ATP-CRA-based chemotherapy for drug selection in patients receiving intravesical chemotherapy to prevent recurrence of superficial bladder cancer after surgery. Methods: The chemosensitivities of 12 anticancer drugs were evaluated, including 5-Fu ADM, and EPI, using ATP-CRA and primary tumor cell culture in 54 patients. In addition, a further 58 patients were treated according to clinical experience. Differences in post-chemotherapeutical effects between drug sensitivity assay and experience groups were compared. Results: The evaluable rate of the test was 96.3%, the clinical effective rate was 80.8%, the sensitivity rate was 97.6% (41/42), the specificity was 20%, the total predicting accuracy was 74.3%, the positive predictive value was 83.7% (41/49), the negative predictive value was 66.7% (2/3); in the drug sensitivity test group, the clinical effective rate was 80.8%, the experience group response rate was 63.8%, with a significant difference in clinical effects between the ATP-based sensitivity and experience groups (${\chi}^2$=7.0153, P<0.01). Conclusion: ATP-CRA is a stable, accurate and potentially practical chemosensitivity test providing a predictor of chemotherapeutic response in patients with superficial bladder cancer.

Nursing Students' Practice Experience in Neonatal Intensive Care Units (간호대학생의 신생아집중치료실 실습경험)

  • Purpose: This study was done to identify the meaning and substantial nature of NICU practice among nursing students by applying a research method based on hermeneutic phenomenology in an attempt to lay a foundation for enhancing theoretical and practical child nursing education. Methods: The participants were eight nursing student who had experience in clinical practicing in NICU. In-depth interviews were performed from April to December, 2014 until data were fully saturated. Collected data were analyzed using Colaizzi's method. Results: Seven theme clusters were extracted from fourteen themes. The seven theme clusters of the nursing students' clinical practice experience in NICU were: 1) Excitement about the clinical practice setting 2) Afraid of practice environment 3) Babies have priority over clinical practice 4) Comparing with adult nursing clinical practice 5) Feeling pity for babies and their parents 6) New awareness about the role of nurses 7) Demands of enhancing clinical practice. Conclusion: The results of the study provide useful information in understanding nursing students' practice experience in NICU and establishing effective strategies to support these nursing students.

Factors Influencing Clinical Competence for General Hospital Nurses (일 종합병원 간호사의 간호수행능력 영향 요인)

  • This study was conducted to investigate factors contributing to clinical competence for nurses. A descriptive correlational study design was used. Participants were 163 nurses from a general hospital. Data were collected using self-administerd questionnaire. There were significant differences in clinical competence according to scholarship, department type, preceptor experience, future plan, continuing education participation. The clinical competence had significant correlations with the clinical experience, critical thinking disposition and clinical decision making. Factors influencing clinical competence were critical thinking disposition, department type, and clinical experience, which explained about 50.3% of total variance. we suggest that continuing education program to enhance critical thinking disposition warranted for development of clinical competence for nurses.

Lived Experience of Clinical Nursing Instructors' Role in Clinical Practicum (간호대학 임상실습 교육자의 역할 경험)

  • Purpose: The purpose of this study was to explore the role of clinical nursing instructors' lived experience in clinical practicum. Methods: Data were collected from 11 clinical nursing instructors by in-depth interviews. The data were analyzed using content analysis of Downe-Wamboldt (1992). Results: Four themes and twelve subthemes were extracted. 1) Recognizing and conducting the roles of clinical nursing instructor: 'Helping and providing support for successful clinical practicum', 'Coordinating clinical activities in daily practice', 'Providing mentoring as an elder in life'; 2) Participating in improving integrative nursing competency: 'Helping to improve cognitive competency', 'Helping to improve functional competency', 'Contributing to form desirable professionalism', 'Helping to deliberate the nature of nursing'; 3) Experiencing difficulties in performing the role of clinical nursing instructor: 'Facing with difficulty from institutional limits', 'Recognizing difficulty from lack of personal knowledge and experience'; 4) Experiencing value of clinical nursing instructor and accomplishing personal growth: 'Making efforts to widen personal knowledge and experience', 'Developing one's own educational competency', 'Making a chance to reflect oneself'. Conclusion: Despite the distinctive features of clinical nursing instructors, little is known of the characteristics. The results of this study could be used as a reference to improve the quality of clinical nursing education.

A Comparison between Importance of Nursing Management Clinical Practicum and Practical Experience recognized by Nursing Students (간호대학생이 인식한 간호관리학 실습 중요도와 실습경험도 비교)

  • Importance recognition of the nursing management(N. M.) clinical practice contents and the actual practical experience were studied against 289 students of 3 nursing schools who had undergone the senior N. M clinical practice. The most important item they considered from the practical evaluation was responsibility followed by interpersonal relationship, trust, communication, and ethic. For the importance of the N. M. contents, nursing unit management was considered as the most important(4.37/5). Abilities they could actually experience from the clinical practice were nursing unit management(22.0%), understanding of nursing management(20.7%), and human resource management(19.8%). And, abilities they could never experience were marketing(52.4%) and financial management(44.0%). Since there was an inconsistency between the importance of contents students recognized and the actual experience, I suggest a completion of standardized protocol of N. M. clinical training and implementation of management training exclusive preceptor system.

Predicting patient experience of Invisalign treatment: An analysis using artificial neural network

  • Objective: Poor experience with Invisalign treatment affects patient compliance and, thus, treatment outcome. Knowing the potential discomfort level in advance can help orthodontists better prepare the patient to overcome the difficult stage. This study aimed to construct artificial neural networks (ANNs) to predict patient experience in the early stages of Invisalign treatment. Methods: In total, 196 patients were enrolled. Data collection included questionnaires on pain, anxiety, and quality of life (QoL). A four-layer fully connected multilayer perception with three backpropagations was constructed to predict patient experience of the treatment. The input data comprised 17 clinical features. The partial derivative method was used to calculate the relative contributions of each input in the ANNs. Results: The predictive success rates for pain, anxiety, and QoL were 87.7%, 93.4%, and 92.4%, respectively. ANNs for predicting pain, anxiety, and QoL yielded areas under the curve of 0.963, 0.992, and 0.982, respectively. The number of teeth with lingual attachments was the most important factor affecting the outcome of negative experience, followed by the number of lingual buttons and upper incisors with attachments. Conclusions: The constructed ANNs in this preliminary study show good accuracy in predicting patient experience (i.e., pain, anxiety, and QoL) of Invisalign treatment. Artificial intelligence system developed for predicting patient comfort has potential for clinical application to enhance patient compliance.

A Convergence Study on the Changes of Awareness and Preference according to the Clinical Application Experience of Digital Intraoral Scanners in Dental Hygienists (디지털 구강스캐너 임상적용 경험에 따른 치과위생사의 인지도 및 선호도 변화에 관한 융합연구)

  • This study aimed to determine the changes of awareness and preference of dental hygienists according to the experience of using a digital intraoral scanner through a convergence study. Data collected by an online survey for dental hygienists in Busan, Gyeongnam and Gyeongbuk were analyzed by SPSS 24.0 program. When dental hygienists had an experience of using a digital intraoral scanner, the awareness and preference of digital intraoral scanners were significantly higher. The experience of using a digital intraoral scanner showed a positive correlation with the clinical application experience of digital intraoral scanners; the clinical application experience of digital intraoral scanners, with the awareness of digital intraoral scanners; and the awareness of digital intraoral scanners, with the preference of digital intraoral scanners. In conclusion, the dental hygienist's experience in clinical application of digital intraoral scanners is expected to increase the awareness and preference, resulting in the improvement of dental hygienists' work ability. Therefore, it is believed that continuous education and learning about digital oral scanners are needed.
