Clinical Characteristics of Pediatric Constipation in South Jordan

  • Purpose: Constipation is a common pediatric problem worldwide. This study aims to describe the clinical characteristics of pediatric constipation in south Jordan according to gender and age group. Methods: All patients with constipation managed at our pediatric gastroenterology service between September 2009 and December 2012 were included. Hospital charts were reviewed. Demographic data, clinical characteristics, and final diagnosis were recorded. Data were analyzed according to gender and the following age groups: infants, pre-school, school age, and adolescents. Results: During the study period, 126 patients were enrolled. The number (percentage) of patients according to age were the following infants: 43 (34.1%), pre-school: 55 (43.7%), school age: 25 (19.8%), and adolescents: 3 (2.4%). Males made up 54.8% of the study population. There were no statistical gender differences in any age group. The most common symptom in all age groups was dry, hard stool. Infrequent defecation was found in almost one-half of the patients. Fecal incontinence was more common in school-aged children compared to pre-school-aged children and adolescents. Abdominal pain was seen in almost 40% of the constipated children. Abdominal pain was more prevalent in girls and older children. Fecal mass in the rectum was the most common physical finding, with constipated boys exhibiting higher rates. Functional constipation was the most common etiology. Conclusion: Clinical characteristics of constipation in children vary according to age group and gender. Older children had less frequent bowel motions, a longer duration of symptoms, and a higher prevalence of long-standing constipation compilations (fecal incontinence and abdominal pain).

  • Postural balance of temporomandibular joint (TMJ) reflects the neuromuscular balance. TMJ posture treatment for meridian yinyang balance has been asserted to have an impact upon cervical spine alignment with an episodic good clinical result for refractory torticollis cases. A retrospective study with one-group pretest-posttest design was conducted to review cervical spine imaging data and clinical symptom changes in cervical dystonia cases to explore the effect of TMJ balance treatment. Review of clinical symptoms and cervical spine imaging data including computed tomography and simple x-ray of 9 cervical dystonia cases revealed significant improvement of cervical spine alignment and clinical symptoms. Improvement of cervical spine alignment may be an underlying mechanism of TMJ balance treatment, as a way of balancing the whole-body meridian system.

The Study of the Knowledge and Performance of Nursing Students for the Nosocomial Infection Control (병원감염관리에 관한 간호대학생의 지식 및 실천에 관한 연구)

  • Purpose: This study was designed to investigate nursing students' exposure experience to infectious diseases and their knowledge and performance for the nosocomial infection control. Method: The data was obtained from 292 nursing students having clinical experience at two university hospitals in Seoul from November 2005 to February 2006. Specific questionnaire was constructed and used for this study. The obtained data was analyzed by using the SPSS window program. Result: The results were as follow : 1. Among the evaluation scores of knowledge for the nosocomial infection control, the score of sterilization and the score of the intravenous therapy management were low(.14). 2. Among the evaluation scores of performance for the nosocomial infection control, the score of hand washing is low(1.63). 3. 86.3 percent of the nursing students had exposure experience to infectious disease. Many of them did not use the protective equipments in clinical experience nor took any treatment after the exposure to infectious diseases. Conclusion: Based on these results, we suggest that the programs for the nosocomial infection control should be developed for nursing students. It is also suggested that nursing students should be educated before their clinical experience to keep protected from the infection. In particular, the hand washing, sterilization and intravenous therapy management should be emphasized in the educational programs.

The Moderating Effects of Social Support between Emotional Labor and Job Satisfaction in Clinical Nurses (임상간호사의 감정노동과 직무만족 간의 관계에서 사회적 지지의 조절효과)

  • Purpose: The purpose of this study was to investigate the moderating effects of social support between emotional labor and job satisfaction in clinical nurses. Methods: Participants were 311 clinical nurses and data were collected from July 11th to 18th, 2014. The data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient and hierarchical multiple regression with SPSS 21.0. Results: The job satisfaction indicated a significant relationship to surface-acting (r=-.191, p=.001), deep-acting (r=.179, p=.002) and social support (r=.342, p<.001) respectively. Emotional labor significantly affected job satisfaction (F=11.592, p<.001), and explained 5.4% of the variance in job satisfaction. The social support acted as a moderator on the relationship between emotional labor and job satisfaction at significant level (F=11.416, p<.001). Conclusion: The results of this study suggest that it is important to develop social support promoting and stress relief program for clinical nurses to improve job satisfaction.

Development of Vancomycin Dosing Nomogram Based on Clinical Pharmacokinetic Data of Korean Adult Patients (한국성인환자의 임상약동학 자료를 이용한 반코마이신 용량설정표 (nomogram)의 개발)

  • This research developed an intravenous (IV) vancomycin dosing nomogram based on the clinical pharmacokinetic data of Korean adult patients. Total 99 pairs of steady-state peak and trough serum concentrations of vancomycin were obtained from 73 adult patients in a tertiary general hospital. Serum vancomycin concentrations were determined to assess the appropriateness of initial vancomycin dosing. Only 47.2% of the cases were within therapeutic range. To characterize the clinical pharmacokinetics (PK) of vancomycin, PK parameters including elimination rate constant ( $K_{e}$) half-life( $T_{1}$2/), clearance (C $l_{van}$), volume of distribution ( $V_{d}$) were calculated by using one-compartment, first order pharmacokinetic equations. PK parameters were evaluated based on the differences of patients'renal function and age. Regression analysis showed a significant correlation between C $l_{van}$ and $C_{cr}$ (C $l_{van}$ = -1.89+0.914 $C_{cr}$ , r=0.763) and between $K_{e}$ and $C_{cr}$ , ( $K_{e}$=-0.0037+0.00139 $C_{cr}$ =0.724). The relationship between $K_{e}$ and $C_{cr}$ , and the mean $V_{d}$ were utilized for developing the nomogram to individualize the initial dosing regimen of vancomycin for the patients with various degrees of renal functions. The nomogram may be used as an efficient tool to determine safe and effective doses of vancomycin for the Korean adult patients.nts.nts.nts.s.nts.

Development of a Patient Monitoring System Overall Architecture and Specifications (환자모니터링시스템의 개발 : 전체구조 및 기본사양)

  • We have developed a patient monitoring system including module-based bedside monitors, interbed network, central stations, clinical workstations, and DB servers. A bedside monitor with a color LCD can accommodate up to 3 module cases and 21 different modules. Six different physiological parameters of ECG, respiration, invasive blood pressure, noninvasive blood pressure, body temperature, and arterial pulse oximetry with plethysmoyaph are provided as parameter modules. In a single bedside monitor, modules and a module controller communicate with IMbps data rate through an intrabed network based on RS-485 and HDU protocol. At the same time, it communicates with other bedside monitors and central stations through interbed network based on 1 OMbps Ethernet and TCP/IP protocol. Central stations using 20" color CRT monitors can be connected with many bedside monitors and they display 18 channels of waveforms simultaneously. Clinical workstations are used mainly for the review of patient datE In order to accommodate more advanced data management capabilities such as 24-hour full disclosure, we have developed a relational database server dedicated to the patient monitoring system. Software for bedside monitor, central station, and clinical workstation fully utilizes graphical user interface techniques and all functions are controlled by a rotate/push button on the bedside monitor arid a mouse on the central station and clinical workstation. The entire system satisfies the requirements of AAMI and ANSI standards in terms of electrical safety and performances.nces.

Violence Episodes and Turnover Among Clinical Nurses (임상 간호사가 경험하는 폭력과 이직에 관한 실태조사)

  • Purpose: The purposes of the study were to describe the prevalence, sources and feelings of verbal and physical violence experienced by nurses and to identify the association of violence episodes and their intent and attempt to turnover. Method: Data were collected from the self reported survey of 589 nurses working in various clinical settings in three different hospitals. SPSSWIN 11.0 program was utilized for data analysis using descriptive statistics and Chi-square test. Result: The findings revealed that the majority of nurses had experienced verbal violence(93.7%) and physical violence(23.4%) at work. The most common sources of violence were by patient's families(61.6%), patients(60.3%), and physicians(48.4%). The most common feeling experienced after an episode of violence were anger(56.6%) and resentment(50.9%). 78.1% of those experienced verbal violence and 86.2% of those experienced physical violence reported that they had intent to turnover after violence episodes. Conclusion: Considering the high prevalence of physical and verbal violence experienced by clinical nurses, organizations must develop administrative support system, preventive education and policy to deal with various causes of violence. This will eventually prevent high turnover rates among clinical nurses related to the violence experiences.

Comparative Study of Emotional Intelligence, Organizational Citizenship Behavior, Organizational Commitment and Organizational Performance of Clinical Nurses in Korea (임상간호사의 감성지능, 조직시민행동, 조직몰입 및 조직성과의 관계)

  • Purpose: The purpose of this study was to identify the relationships between emotional intelligence, organizational citizenship behavior, organizational commitment and organizational performance of clinical nurses in Korea. Methods: The participants were 453 nurses, working in three general hospitals in Seoul, and data were collected from December 15, 2012 to February 2, 2013. Data were analyzed using PASW (SPSS ver. 20.0) and SPSS/WIN 19.0 (SPSS INC, Chicago, IL, USA), and AMOS ver. 20.0. Results: The results were as follows: organizational performance is positively influenced by emotional intelligence. Organizational citizenship behavior is positively influenced by emotional intelligence, organizational commitment is positively influenced by organizational citizenship behavior, and organizational performance is positively influenced by organizational commitment. Conclusion: The results of this study demonstrate and help in understanding the relationship between the research concepts and the direction of nursing organizations. We recommend that various educational programs should be developed to improve clinical nurses' emotional intelligence.

Effects of Empowerment and Job Satisfaction on Nursing Performance of Clinical Nurses (간호사의 임파워먼트와 직무만족이 간호업무성과에 미치는 영향)

  • Purpose: This study was conducted to identify factors that affect nursing performance; Those factors are personal characteristics, sub-categories of empowerment and job satisfaction of clinical nurses. Methods: Data were collected from November 24, 2012 to March 11, 2013, and participants were 451 hospital nurses in 6 hospitals. Descriptive statistics, t-test, one-way ANOVA, Pearson correlation coefficient, and stepwise multiple regression were used for data analysis. Results: Nurses who were older, married, highly educated, in the position of charge nurse, and with longer clinical careers showed relatively higher empowerment, higher job satisfaction, and higher nursing performance than others. Also, there were strong correlations between nursing performance and empowerment (r=.576)/job satisfaction (r=.617). Factors predicted nursing performance were relationship with coworkers (${\beta}=.398$), duty (${\beta}=.181$), promotion system (${\beta}=.134$), and turnover intention (${\beta}=-.109$). Factors predicting job satisfaction were competency of empowerment (${\beta}=.249$), and clinical career (${\beta}=.151$). These 6 factors explained 55.2% of the variance in nursing performance (F=93.37, <.001). Conclusion: Regarding human resource management, relationship with co-workers is a changeable factor. Therefore nursing organizations should apply these factors in human resource management to enhance nursing performance and achieve organizational goals.

The Effects of Time Management on the Clinical Nurse's Organizational Commitment and Job Satisfaction (임상간호사의 시간관리 요인이 조직몰입 및 직무만족에 미치는 영향)

  • Purpose: The aim of this study was to analyze the effects of time management on the clinical nurse's organizational commitment and job satisfaction. Methods: Subjects were recruited in two general hospitals in Seoul and Incheon. Data collection was done using a self-report questionnaire. Time management was measured using the questionnaire developed by Han (1992). Organizational commitment and job satisfaction were measured using the questionnaire developed by Yoon (2000), based on Mowday et al. (1979) and Stamps et al. (1978). The data were analyzed using the SAS statistical package program, version 10.0. Specifically, descriptive statistics and stepwise multiple regression were performed. Results: The predictive time management factors for organizational commitment included deadline decision, simplification, and goal-setting. The predictive time management factors for job satisfaction included planning/making the priority order, deadline decision, simplification, asking for help, and responsibility reduction. Conclusion: Time management factors are highly correlated with organizational commitment and job satisfaction in clinical nurses. Deadline decision and simplification are common predictive factors for organizational commitment and job satisfaction. These results can be used to develop more effective time management strategies for increasing organizational effectiveness in clinical nurses.
