Kim's analysis에 의한 III급 부정교합아동의 측모두부방사선 계측학적 연구 (THE LATERAL CEPHALOMETRIC STUDY OF THE GROWING CHILDREN WITH CLASS III MALOCCLUSION BY KIM'S ANALYSIS)

  • 측모두부방사선 사진은 임상적으로 쉽게 드러나지 않은 치아와 골격간의 상세한 관계를 나타내주며, 이것을 계측 분석하는 다양한 방법들이 소개되어 임상에 적용되고 있다. 이 중 Kim's analysis는 수직피개(overbite)의 지표로 ODI(Overbite Depth Indicator), 전후방적 부조화의 지표로 APDI(Anteroposterior Dysplasia Indicator). 그리고 이들의 합인CF(Combination Factor)를 제시하였다. 또 CF에 상하악의 절치각과 안모의 돌출도를 고려하여 EI(Extraction Index)를 산출하였다. 이러한 분석법에 이용되는 각 계측치들이 정상교합자와 III급 부정교합자에서 어떻게 분포하며 어떤 차이를 보이는가를 조사하기 위해 본 연구를 시행하였다. 골격이 조화로우며 안모가 단정한 7세부터 9세의 정상교합자와 동일한 연령의 III급 부정교합자 141명의 아동을 대상으로 하여 Kim's analysis에 이용되는 계측항목들을 통계적으로 분석하여 다음과 같은 결과를 얻었다. 1. 정상교합자와 III급부정교합자의 비교에서 ODI, APDI, IIA, UL은 유의한 차이를 보였으나, CF와 EI는 통계적인 유의차가 없었다(P<0.01). 2. 정상교합자에서 ODI, APDI, IIA의 평균은 각각 72.63, 80.47, 121.37이었다. 3. III급 부정교합자 ODI, APDI, IIA의 평균은 각각 64.46, 87.31, 129.80이었다. 4. ODI와 APDI의 Pearson 상관계수는 -0.576으로 역상관관계를 나타내었다. 5. EI와 다른 계측치들간의 상관관계는 CF(0.777), LL(-0.607), UL(-0.588), IIA(0.485), APDI(0.444), ODI(0.304)의 순서였다. 이것은 발치분석을 하는데 있어 상순과 하순의 돌출도가 가장 민감하게 반영된다는 것을 암시한다.

Three-dimensional evaluation of the mandibular condyle in adults with various skeletal patterns

  • Objective: Morphometric and morphological evaluation of the mandibular condyle in adults and to identify its correlation with skeletal malocclusion patterns. Methods: Cone-beam computed tomography scans of 135 adult patients were used in this study and classified into groups according to four criteria: (1) sex (male and female); (2) sagittal skeletal discrepancy (Class I, Class II, and Class III); (3) vertical skeletal discrepancy (hyperdivergent, normodivergent, and hypodivergent); and age (group 1 ≤ 20 years, 21 ≤ group 2 < 30, and group 3 ≥ 30 years). The morphometrical variables were mandibular condyle height and width, and the morphological variable was the mandibular condyle shape in coronal and sagittal sections. Three-dimensional standard tessellation language files were created using itk-snap (open-source software), and measurements were performed using Meshmixer (open-source software). Results: The mandibular condyle height was significantly greater (p < 0.05) in patients with class III malocclusion than in those with class I or II malocclusion; the mandibular condyle width was not significantly different among different sexes, age groups, and sagittal and vertical malocclusions. There were no statistical associations between various mandibular condyle shapes and the sexes, age groups, and skeletal malocclusions. Conclusions: The condylar height was greatest in patients with class III malocclusion. The condylar height and width were greater among males than in females. The mandibular condyle shapes observed in sagittal and coronal sections did not affect the skeletal malocclusion patterns.

Face-Mask를 이용한 골격성 III급 부정교합 환아의 치험례 (CASE REPORTS ON TREATMENT OF SKELETAL CLASS III MALOCCLUSION WITH FACE-MASK)

  • The conventional treatment of skeletal Class III malocclusion has been focused on application of orthopedic force primarily to the mandible. However, In Class III malocclusion with retrograde position or underdevelopment of Maxilla, this approach is not suitable treatment. These patients need an application of orthopedic forces via face-mask to the Maxilla to stimulate its growth and to change the direction of growth. In skeletal Class III patients who were treated by Face-Mask, the following results were obtained. 1. Forward growth of Maxilla was enhanced. 2. Labioversion of upper incisors and linguoversion of lower incisors were observed. 3. Mandible was rotated to clockwise direction and remodeling of B point was observed. 4. Anterior crossbite was corrected by combining of the above results.

구치(臼齒)의 근원심(近遠心) 경사도(傾斜度)에 관(關)한 두부방사선계측학적(頭部放射線計測學的) 연구(硏究)

  • This study was designed to get the informations of the mesiodistal axial inclinations of the posterior teeth and of the relationships between these and other angular measurements of facial bony structures in normal occlusion and malocclusion groups using lateral roentgenocephalograms. The subjects consisted of 73 normal occlusions (31 males 42 females), 38 Class II Division 1 malocclusions (17 males 21 females) and 47 Class III malocclusions (19 males 28 females). The findings of this study are as follows : 1. In mandible, the posterior teeth axes of Class II Division 1 malocclusion group were inclined more mesially and those of Class III malocclusion group were inclined more distally than normal occlusion group. In maxilla, Class II Division 1 malocclusion group showed more distal inclination and Class III malocclusion group showed more mesial inclinaton of 1st, 2nd premolars and more distal inclination of 1st, 2nd molars than those of the normal occlusion group. 2. There was a tendency for teeth axes to maintain nearly the same inclination in relation to occlusal plane irrespective of various OMA and OPA in each group. 3. F M A, P M A and O P A were the largest in Class II Division 1 malocclusion group and O M A, GoA were the largest in Class 111 malocclusion group. 4. There were high correlationships between mandibular teeth inclinations related to mandibular plane and 4 angular measurements except OPA, and between maxillary teeth inclinations related to palatal plane and OPA.

정상교합자와 골격성 III급 부정교합자의 경추골성숙도에 관한 연구 (THE SKELETAL MATURITY OF CERVICAL VERTEBRAE OF CHILDREN WITH NORMAL OCCLUSION AND SKELETAL CLASS III MALOCCLUSION)

  • 정상교합자와 골격성 III급 부정교합자간의 성장 정도에 차이가 있는지를 평가하기 위해 8-12세 여자를 대상으로 하여 진단 석고 모형과 측모두부방사선사진을 기준으로 정상교합자 172명, 골격성 III급 부정교합자 191명 총 363명을 분류하여 연구를 시행하였다. 경추의 하연의 만곡도와 수직 수평 길이의 비를 측정해서 연령이 증가함에 따른 변화를 분석하여 다음과 같은 결론을 얻었다. 1. 연령이 증가함에 따라 제$2{\sim}6$경추의 하연의 만곡도는 정상교합자와 골격성 III급 부정교합자 모두에서 일정하게 증가하였다. 2. 연령이 증가함에 따라 제$3{\sim}6$ 경추의 수직 수평 길이의 비도 정상교합자와 골격성 III급 부정교합자 모두에서 일정하게 증가하였다. 3. 제$2{\sim}6$경추의 하연의 만곡도와 제$3{\sim}6$경추의 수직 수평 길이의 비에서 정상교합자와 골격성 III급 부정교합자간의 경추골성숙도 차이를 조사한 결과 통계적으로 유의한 차이를 보이지 않았다. 이상의 결과에서 정상교합자와 골격성 IIII급 부정교합자간의 경추골성숙도를 비교했을 때 유의한 차이를 보이지 않았으므로 성장에 차이가 없음을 시사하였다.

Angle III급 부정교합자의 악안면골격 특성에 관한 연구 (STUDY ON THE CHARACTERISTICS OF CRANIOFACIAL SKELETON OF ANGLE'S CLASS III MALOCCLUSION CASES)

  • This investigation was designed to categorize Angle's class III malocclusion groups through analyzing horizontal and vertical components of craniofacial skeleton in Angle's class III malocclusion. The material selected for this study consisted in standard lateral cephalogram of one hundred and fifty five children, seventy four boys and eighty one girls, aged 6 through 15 years, having Angle's class III malocclusion. On the basis of findings of this study, the following results were obtained. 1. In horizontal skeletal classifications,16 groups were classified FMN-A-B, SE-FMN-A according to the Ba-SE-Me, Ba-SE/R. 2. The sequences that have relatively high frequency are as follow; a) Horizontal Group l b) Horizontal Group 3 c) Horizontal Group 5 d) Horizontal Group 9 e) Horizontal Group 4 3. In vertical skeletal classification, 8 groups were classified according to the PMV/PP, PMV/OP, PMV/MP. 4. The sequences that relatively high are as follows; a) Vertical Group 1 b) Vertical Group 3 c) Vertical Group 4 d) Vertical Group 8.

혀의 자세 및 크기와 치아치조골 패턴의 관계에 관한 연구 (RELATIONS BETWEEN POSTURE AND SIZE OF THE TONGUE AND DENTOALVEOLAR PATTERN)

  • This study was designed to examine relations between posture and size of the tongue and dentoalveolar pattern. The sample was consisted of three groups, the 34 subjects of Normal occlusion, the 31 subjects of Bimaxillary protrusion and the 31 subjects of Class III malocclusion. On the cephalograms, lengths, heights and areas of the tongue and intermaxillary space and on the study model, arch length, intercanine width, intermolar width and palatal height were measured. These data from measuring cephalograms and models were statistically analyzed. The results of the study were as follows; 1. Length of the tongue was the greatest in Bimaxillary protrusion and in order of Normal occlusion and Class III malocclusion. 2. Posture of the tongue was the lowest in Class III malocclusion and in order of Bimaxillary protrusion and Normal occlusion. 3. There were a tendency to be larger area of tongue and intermaxillary space in Class III malocclusion compared to Normal occlusion and Bimaxillary protrusion. 4. Size of the tongue and intermaxillary space showed low correlations with the dentoalveolar pattern.

Correction of Class III malocclusion with alternate rapid maxillary expansions and constrictions using a hybrid hyrax-mandibular miniplate combination and simultaneous orthodontic treatment: A case report

  • In this report, we describe the successful use of alternate rapid maxillary expansions and constrictions with a hybrid hyrax-mandibular miniplate combination and simultaneous orthodontic treatment for the management of severe Class III malocclusion due to maxillary hypoplasia in an 11-year-old girl. The devices were removed after 20 months of treatment, and the family was instructed about a careful control and retention program that should be followed in accordance with the patient's growth. The final result included the correction of Class III malocclusion with adequate function and excellent facial esthetics, which restored the patient's self-esteem and provided personal motivation. The outcomes showed good stability after 24 months of retention. The decrease in the duration of active treatment is the most important finding from the present case. Considering that facial esthetics in adolescence is a determining factor for the development of a personality and interpersonal relationships, we recommend the use of this protocol for growing patients, who will exhibit not only an improved physical appearance but also a better quality of life.

한국인과 일본인 부정교합자의 하악 치열궁 헝태 비교 (Comparison of arch forms between Korean and Japanese in Class I, II, and III malocclusion)

  • 본 연구의 목적은 한국인과 일본인의 Angle씨 I급, II급, III급 부정교합군에서 하악치열궁의 크기와 형태적 특성을 상호 비교 분석하는데 있었다. 한국인의 부정교합자 368명(I급 114명, II급 119명, III급 135명)과 일본인의 부정교합자 160명(I급 60명, II급 50명, III급 50명)을 대상으로 하였다. 치료 전 하악 치열 모형을 복사한 사진에서 13개의 인접한 접촉면에서 가장 협측에 위치한 점들을 좌표치화한 후 하악 치아의 두께 자료에 근거하여 임상 브라켓 점들을 설정하였고 4개의 선 계측과 이들을 이용하여 2개의 비율을 측정하였다. 각 부정교합군에서 측정 항목들에 관하여 t-test를 하였고 치열궁 형태를 square, ovoid, tapered 의 3가지로 분류한 후 분포 특성에 관하여 카이제곱 검정하였다. 그 결과 I급과 II급 부정교합군에서 일본 여성의 하악 치열궁 폭경이 일본남성, 한국 남성 및 한국 여성에 비해 작았으며, III급 부정교합군의 하악 치열궁 크기는 인종이나 성별에 따른 차이가 없었다. 그리고 모든 부정교합군에서 한국인과 일본인의 대부분이 square와 ovoid한 치열궁 형태를 가졌으며 II급 부정교합군에서는 일본인은 ovoid 형태가 52.%으로 가장 많고 한국인은 square 형태가 40%로 가장 많게 나타났다.

골격성 III 급 부정 교합자에서 양악 회전 수술 후 연조직 변화에 대한 연구 (SOFT TISSUE CHANGES AFTER DOUBLE JAW ROTATION SURGERY IN SKELETAL CLASS III MALOCCLUSION)

  • The aim of this study was to evaluate the amount and interrelationship of the soft and hard tissue changes after simultaneous maxillary clockwise rotation and mandibular setback surgery in skeletal class III malocclusion. The sample comprised of 16 adult patients who had anteroposterior skeletal discrepancy. These patients had received presurgical orthodontic treatment and surgical treatment which consisted of Le fort I Osteotomy and bilateral saggital split ramus osteotomy. The presurgical (T1) and postsurgical (T2) lateral cephalograms were evaluated. The computerized statistical analysis was carried out with SPSS/PC program. The results demonstrated a decrease in the vertical dimension in the soft and hard tissue. The nasolabial angle was increased and the mentolabial angle was decreased. The results showed also many statistically significant correlations(p<0.05). The lower lip closely followed the skeletal movement of the B- point in the horizontal plane. The double jaw rotation surgery can afford a good solution to solve the problems of class III malocclusion cases.
