Four Treatment Cases of Psoriasis of Soyangins Demonstrated by Yang Poison Exanthema (양독발반(陽毒發斑)으로 변증(辨證)한 소양인 건선 치험 4례)

  • Psoriasis is a chronic, recurrent skin disease without any cure so far. The symptoms of the guttate psoriasis of Soyangins going through tonsillitis and of the psoriasis under the steroid rebound phenomenon are identical to one of yang poison exanthema of a Soyangin. The study administered Yangdokbagho-tang, which is used to cure yang poison exanthema of Soyangins, and, as a result, showed significant treatment results from these two types of four clinical cases. Sasang medicine is considered an effective treatment method to cure psoriasis, an obstinate skin disease.

  • The purpose of this study is to give an account of the effects of the Korean herbal medicine therapy which is based on the Sasang constitutional medicine for the patient who has suffered from scale of both foot caused by chronic psoriasis. The subject is a 81-year old man who has the symptoms caused by lacunar infarction, which are both leg weakness, gait disturbance, dizziness, dysarthria, frequency voiding, and scale of both foot caused by chronic psoriasis. We have diagnosed him as Shaoyangin(少陽人), and have prescribed Yukmi jiwhangtang(六味地黃湯). The consequence is the accompanied scale of both foot, both leg weakness, gait disturbance, dizziness, frequency voiding and general condition has improved.

Brazilin downregulates CCL20 expression via regulation of STAT3 phosphorylation in TNF-α/IL-17A/IFN-γ-induced HaCaT cells (TNF-α/IL-17A/IFN-γ 유도된 HaCaT 세포에서 브라질린의 STAT3 인산화 억제를 통한 CCL20 저해 효과)

  • Psoriasis is a chronic intractable skin disease caused by various inflammatory cytokines such as IL-6, CXCL8, TNF-α, and IFN-γ, as well as IL-17A secreted from Th17 cells and is characterized by hyperkeratosis and chronic inflammation of the epidermis. Brazilin, an active ingredient of Caesalpinia sappan L., is known to exert antioxidant and anti-inflammatory activity, and function in skin barrier improvement. In particular, it was shown as a potential material for treating psoriasis in a tumor necrosis factor (TNF)-α-stimulated HaCaT keratinocyte model. However, the direct regulation of the C-C motif chemokine ligand (CCL) 20, a psoriasis-inducing factor, by brazilin has not been reported. Therefore, in this study, we investigated the suppression of CCL20 and the regulatory mechanism by brazilin using a psoriasis-like model. First, brazilin downregulated CCL20 and CXCL8 in IL-17A-stimulated HaCaT cells in a concentration-dependent manner by inhibiting signal transducer and transcription (STAT)3 phosphorylation. In addition, brazilin significantly inhibited the expression of psoriasis-related genes CXCL8, CCL20, IL-1, IL-6, and TNF-α in TNF-α/IL-17A/IFN-γ-stimulated HaCaT cells. Moreover, brazilin also had a positive effect on improving the skin barrier in TNF-α/IL-17A/IFN-γ-stimulated HaCaT cells. The above results indicated that brazilin ultimately downregulated CCL20 expression by inhibiting STAT3 phosphorylation, and also suppressed the expression of psoriasis-induced cytokines. If the efficacy of brazilin in improving psoriasis is verified through animal models and clinical trials in the future, it may represent a potentially therapeutic substance for psoriasis patients.

  • Objective : We investigated the effects of Oriental medical treatment on psoriasis which is a chronic disease. Methods : We applied acupuncture and herbal medicine to psoriasis patients. The progress of symptom is calculated using PASI(psoriasis area and severity) score and degree of itching is checked 0 to 3. Results : After oriental medical treatment, patients recovered from psoriasis without side effect. Conclusion : Oriental medical treatment can be a very effective way to treat psoriasis. The more patients we treat, the more clinical report is accumulated. Then it would be helpful to map out a systematic treatment on psoriasis.

  • Psoriatic arthritis is a chronic inflammatory form of arthritis that is associated with psoriasis. A 54-yr-old male with chronic psoriatic temporomandibular joint arthritis and myofascial pain was treated using methotrexate and a myofascial pain protocol. Jaw pain improved after 3 weeks, however, tenderness to palpation of muscles remained. Comprehensive evaluation and multidisciplinary clinical treatment is required for the treatment of patients with psoriatic temporomandibular joint arthritis.

  • Objectives : The purpose of this study is to report the Sik-yang-seng(食養生) therapy with Korean Medicine and acupuncture on a chronic psoriasis patient who is occurred to latrogenic cushing's syndrome. Methods : The patient who has been diagnosed psoriasis had symptoms with latrogenic Cushing's syndrome. The patient was treated with Sik-yang-seng(食養生) therapy including Korean Medicine and acupuncture. Evaluation was done by PASI score, itching score, statistics and pictures. Results : After treatment, the PASI score of patients with Sik-yang-seng(食養生) therapy including Korean Medicine and acupuncture was 1.8 which means 95.5% improvement. Conclusions : Korean medical treatment performed with Sik-yang-seng(食養生) therapy can help recovery of Biwi(脾胃)function therefore can increase effect of the treatment of psoriasis and latrogenic Cushing's syndrome.

  • Psoriasis is a common genetically determined, chronic, inflammatory skin disease characterized by rounded crythematous, dry, scaling patches. It is caused to blood heat(血熱), shortage of blood(血燥) and blood stasis(血瘀) in oriental medicine. We treated a patient who suffered from chroinc and recurred psoriasis, and who was differenciated with endogenous wind due to shortage blood(血燥生風) with Danguiumjagagam(當歸飮子加減). After 10 days of treatment with Danguiumjagagam(當歸飮子加減) and acupunture, we observed an improvement. Based on this experience, it is considered that oriental medicine can be applied to the treatment of psoriasis.

Investigation of Effective Korean Herbal Medicine for Psoriasis - Focusing on Lipid Metabolism - (건선에 효과적인 한약 처방 탐색 - 지질 대사를 중심으로)

  • Objectives : The purpose of this study is to investigate the possibility of using herbal medicine for the management of psoriasis focusing on lipid metabolism. Methods : We reviewed studies about pathophysiology, and medical treatment of psoriasis, the relationship between psoriasis and metabolic syndrome and lipid metabolism, and herbal medicine on Pubmed and Google scholar. Results : Psoriasis is a chronic multi-organ inflammatory disease not limited to skin, and steroids, immuno-suppressants, and biological agents are used. It is known that psoriasis and metabolic syndrome act as mutual risk factors, and lipid metabolism are involved in psoriasis. The effects of various single herbal preparations and complex herbal extract, decoction on improving lipid metabolism have been consistently reported, and there was an improvement of psoriatic skin lesions and improvement of blood lipid levels through herbal medicine. Conclusions : Herbal medicine research in psoriasis has focused on the anti-inflammatory effect and the suppression effect of certain immune mediators. However, considering that psoriasis is affected by lipid metabolism and side effects of Western medicines, the use of herbal medicines for the purpose of controlling lipid metabolism in psoriasis is useful in aspects of reducing side effects of concurrent Western medicine, improving the severity of psoriasis, and managing metabolic risk factors.

  • The Dryness and the Dampness is the congential nature. the Water and Fire is the acquired action. the skin is related to the physiological function of the lung, so the skin is the domain of the lung. the lung belongs to the dryness-metal(燥金). And the Dryness and the Dampness in the six pathogenic factors exert a important pathogenic influence upon the skin. Psoriasis is a chronic skin condition charactered by red, silver scaly patches in the affected areas, it is caused by excessive growth of the top layer of the skin(epidermis). Scalp psoriasis affects at least two thirds people who have psoriasis. Generally, the scalp will have plaque psoriasis characterized by elevated, inflamed, lesions covered with silvery white scales. The lesion of psoriasis is head, elbow and knee that are yang-channels in Oriental medicine. The syndrome falls with in the purview of the dryness-metal(燥金). In time, psoriasis takes a turn for the worse in the winter season. In space, many of people in comparison to the average in the world have psoriasis in high latitude that is dry and cold, for instance, the Scandinavian peninsula. Deficiency of yin-fluid and blood course often the dryness-syndrome, for instance, psoriasis, etc. So psoriasis is one of the dryness-syndrome in Oriental medicine.

Case Report of the Patient with the Atopic Dermatitis, Psoriasis Treated with Mahaenggamseog-tang (마행감석탕(麻杏甘石湯)으로 호전을 보인 아토피 피부염, 건선 증례보고)

  • This report describes the effectiveness of mahaenggamseog-tang(麻杏甘石湯) on the patients who suffered from atopic dermatitis and psoriasis. We administered mahaenggamseog-tang with heat-clearing and lung-dispersing(淸熱宣肺) effect to two dermatitis patients who showed sweating with panting(汗出而喘), vexation and thirst(煩渴), aversion to heat(惡熱), agitation(煩燥), swelling of the face and eyes(面目浮腫). The severity of dermatitis was evaluated by visual analogue scale(VAS), pictures, SCORAD index(case 1), PASI(case 2). After the treatment, VAS/SCORAD index of case 1 changed from 9/71.5 to 0/6.1; VAS/PASI of case 2 from 8/36.8 to 1/6.8. Mahaenggamseog-tang could improved the symptoms of atopic dermatitis and psoriasis in this study. This study offers an approach to managing chronic dermatitis more effectively using Mahaenggamseog-tang which used mainly for bronchial diseases and obesity.
