Investigation on radiation shielding parameters of cerrobend alloys

  • In this study, to determine the most effective alloy for shielding against gamma-rays, the gamma-ray shielding parameters of six types of cerrobend alloys have been investigated. Gamma-ray interaction with the cerrobend alloys has been discussed mainly in terms of total mass attenuation coefficient (${\mu}_t$), half value layer (HVL), tenth value layer (TVL), effective atomic number ($Z_{eff}$), and effective electron density ($N_{eff}$). These parameters have been calculated by theoretical approach using the ParShield program in a photon energy range between 0.1 MeV and 100 GeV. The dependence of these parameters on the incident photon energy and chemical composition of the cerrobend alloys has been studied.

The Analysis of a Cerrobend Compensator and a Electronic Compensator Designed by a Radiation Treatment Planning System (방사선치료계획장치로 설계된 Cerrobend 선량보상체와 전자 선량보상체의 제작 및 특성 분석)

  • In this study, the physical compensator made with the high density material, Cerrobend, and the electronic compensator realized by the movement of a dynamic multileaf collimator were analyzed in order to verify the properness of a design function in the commercial RTP (radiation treatment planning) system, Eclipse. The CT images of a phantom composed of the regions of five different thickness were acquired and the proper compensator which can make homogeneous dose distribution at the reference depth was designed in the RTP. The frame for the casting of Cerrobend compensator was made with a computerized automatic styrofoam cutting device and the Millennium MLC-120 was used for the electronic compensator. All the dose values and isodose distributions were measured with a radiographic EDR2 film. The deviation of a dose distribution was $\pm0.99 cGy\;and\;\pm1.82cGy$ in each case of a Cerrobend compensator and a electronic compensator compared with a $\pm13.93 cGy$ deviation in an open beam condition. Which showed the proper function of the designed compensators in the view point of a homogeneous dose distribution. When the absolute dose value was analyzed, the Cerrobend compensator showed a $+3.83\%$ error and the electronic compensator showed a $-4.37\%$ error in comparison with a dose value which was calculated in the RTP. These errors can be admtted as an reasonable results that approve the accuracy of the compensator design in the RTP considering the error in the process of the manufacturing of the Cerrobend compensator and the limitation of a film in the absolute dosimetry.

  • The skin cancer is a highly curable disease which frequently occurs in the head and neck region exposed to the sun. When the eyelid is treated usually eye shield made of lead is used to protect the eyeball as a internal shield. For the same reason on internal shield should be used when the nose is treated when electron to protect the nasal mucosa. Our hospital made an internal shield for the treatment of the skin cancer on the nose using provil and cerrobend. The characteristics of the internal shield were examined.

  • The eye lens is known to be radiosensitive organ and catarat can be induced by relatively low dose of radiation. In the treatment of head and neck tumors, shielding blocks are frequently used to minimize dose on sensitive organs. The shielding block, which is made of high atomic number materials (cerrobend), produce significant dose perturbations in megavoltage photon beams. The effects of these perturbations of eye shielding blocks are measured with film and ion chambers for the treatment of head and neck malignancies. Optimum parameters for the treatment are suggested.

  • Multileaf Collimator is an essential tool for conformal therapy. We have measured the scallop penumbars of a poligon-shape field produced by Multileaf Collimator(MLC) with various leaf widths and the penumbra from conventional block, and compared the difference between them for the energy of photons, leaf widths, and angle of filed edges in order to find out the optimum leaf width. In addition, we have simulated a treatment of 20 factionations with errors of patient positioning. Optimum cost-benefit balanced leaf width is 8 mm.

  • Purpose : A new fiduciary plate and orientation marker have been devised to assist the quality assurance (QA) procedures for port films in radiation therapy department. The plate is used in conjunction with the film/cassette combination during weekly QA procedures, at Seoul National University Hospital (SNUH), in order to verify treatment fields in high radiation therapy. Materials and Methods : A new fiduciary plate was fabricated using an acrylic plate, cerrobend, standard blocking tray and mercury. The acrylic plate had the dimension of $1{\times}25{\times}25$ cm, with two fiduciary markers. The plate was rigidly attached onto the standard blocking tray, thus making it easier to set the fiduciary plate to the center on the radiation field on the linear accelerator. The plate had two 2-mm vertical and horizontal lines, with the minor scales in 2-cm steps. The orientation marker was a small mercury filled disk, which was inserted into the plate. Results : The geometrical structure of the lines in the plate makes it easier to correlate two different images between the simulation and port films. The marker clearly indicated the orientation of the film, for example, the anterior, posterior, left, right and various oblique orientations, without the placement of a conventional orientation marker. Also, the new orientation marker could easily be applied to the simulator by placing the small orientation marker onto the image intensifier or in front of the film/cassette holder. Conclusions : The new fiduciary plate appears to be useful in verifying the treatment fields, and the new orientation marker makes the film orientation simple, which is expected to lower the block fabrication errors.

The Use of Lens Shielding Device(L.S.D.) for a Conjunctival Lymphoma

  • When therapeutic irradiation is indicated for the orbital tumors, the greatest concern is the risk of radiation-induced cataract. Conjunctival lymphoma is one of the good examples. We would like to report the procedure of the lens shielding device(L.S.D) and the result of irradiated dose to the lens. L.S.D. consistes of two parts : load alloy to attenuate electron beam, and dental acryl which completely covers the lead alloy to avoid discomfort of cornea from contacting directly with cerrobend and side scattering by cerrobend. And for easy location and removal, side bars were made on each side. Radiation doses were meaured with TLD(TLD 3500 Hawshaw). Markus chamber in a polystyrene phantom. The phantom was irradiated with 9MeV electron beams from Clinac 2100C with $6{\times}6cm$ electron cone. The relative dose at 6mm depth where the lens is located was $4.2\%$ with TLD and $5.1\%$ with Markus chamber clinically when 2600 cGy are irradiated to the eyeball, the mapinary dose to the lens will be 109 cGy or 132 cGy, which will significently reduce the cataract.

The Dosimetric Effects on Scallop Penumbra from Multi-leaf Collimator by Daily Patient Setup Error in Radiation Therapy with Photon (광자선 치료시 Setup 오차에 따르는 Multi-leaf Collimator의 Scallop Penumbra 변화 효과)

  • Purpose : To evaluate the clinical implications of scallop penumbra width that comes from multileaf collimator(MLC) effect by the daily routine patient setup error. Materials and Methods : The anales of $0^{circ},{\;}15^{circ},{\;}30^{circ},{\;}45^{circ},{\;}60^{circ},{\;}and{\;}75^{circ}$ inclined -radiation blocked fields were generated using the both conventional cerrobend block and the MLC. Film dosimetry in the phantom were performed to measure penumbral widths of differences between the dose distributions from the cerrobend block and those of respect the MLC. The patient setup error effect on scallop penumbra was simulated with respect to the table of setup error distribution. Same procedures are repeated for the cerrobend block generated field. Results : There are penumbral widths of to 3mm difference between the dose distributioins from two kinds of field shaping tools, the conventional block and the MLC with 4mm setup error model and resolution of 1cm leaf at the isocenter. Conclusion : We need not additive margin for MLC, if planning target volume is selected according to the recommendation of ICRU 50. For particular cases, we can include the target volume with less than 3mm additive margin.

  • The quasi-conformation therapy was performed to get a homogeneous dose distributions for irregeular shaped tumor lesion by using the arc moving beam and beam modifying filter which was made by cerrobend alloy($\rho$=9.4 g/cc) metal. In our dose calcuation programme, it was fundmentally based on Clarkson's method to calcuate the irregular multi-step block field in rotation therapy. In this study, the expected relative depth doses under multipartial attenuator agree well with measured data at same plane. The results of comparison the dose computation with that of TLD measurement are very closed within ${\pm}5\%$ uncertainties in the irradiation to phantom with quasi-comformation method. And it has shown that irregular typed multi-step filter can be applied to quasi-conformation therapy in high energy radiation plannings.

Characteristics of 15 MV Photon Beam from a Varian Clinac 1800 Dual Energy Linear Accelerator (CLINAC 1800 선형가속기의 15 MV X-선의 특성)

  • A comprehensive set of dosimetric measurements has been made on the Varian Clinac 1800 15 MV photon beam. Beam quality, percentage depth dose, dose in the build up region, output, symmetry and flatness, transmission through iead (Cerrobend), tray attenuation, isodose curves for the open and wedged fields were measured using 3 dimensional water phantom dosimetry system (including film densitometer system) and polystyrene phantoms. These dosimetric measurements sufficiently characterized the beam to permit clinical use. The depth dose characteristics of photon beam is $d_{max}$ of 3.0 cm and percentage depth dose of $76.8\%$ at 10 cm,100 cm source-surface distance, field size of $10\times10\;cm^2$ for 15 MV X-ray beam. The Output factors ranged 0.927 for $4\times4\;cm^2$ field to 1,087 for $35\times35\;cm^2$ field. The build-up level of maximum dose was at 3.0 cm and surface dose was approximately $15.5\%$ for a field size $10\times10\;cm^2$ The stability of output is $within\pm1\%$ and flatness and symmetry are $within\pm3\%$. The half value thickness (HVL) of lead is 13 mm, which corresponds to an attenuation coefficient of $0.053\;mm^{-1}$. These figures compare facorably with the manufacturesr`s specifications.
