Background: The role of totally implantable central venous port (TICVP) system is increasing. Implantation performed by radiologist with ultrasound-guided access of vein and fluoroscope-guided positioning of catheter is widely accepted nowadays. In this article, we summarized our experience of TICVP system by surgeon and present the success and complication rate of this surgical method. Materials and Methods: Between March 2009 and December 2010, 245 ports were implanted in 242 patients by surgeon. These procedures were performed with one small skin incision and subcutaneous puncture of subclavian vein. Patient's profiles, indications of port system, early and delayed complications, and implanted period were evaluated. Results: There were 82 men and 160 women with mean age of 55.74. Port system was implanted on right chest in 203, and left chest in 42 patients. There was no intraoperative complication. Early complications occurred in 11 patients (4.49%) including malposition of catheter tip in 6, malfunction of catheter in 3, and port site infection in 2. Late complication occurred in 12 patients (4.90%). Conclusion: Surgical insertion of TICVP system with percutaneous subclavian venous access is safe procedures with lower complications. Careful insertion of system and skilled management would decrease complication incidence.
Central venous stenosis is a rare cause of unilateral breast edema occurring in hemodialysis patients that needs to be differentiated from other differential diagnoses, including, but not limited to, inflammatory breast carcinoma, mastitis, lymphedema, and congestive heart failure. All reports of similar cases in the available literature have described improvement or resolution of the edema after treatment. Herein, we report and discuss the pathophysiology of breast edema formation in a patient who presented with massive left-sided breast edema 7 years after being diagnosed with central venous stenosis. Medical and minimally invasive therapy had not been successful, so she underwent reduction mammoplasty to relieve the symptoms.
항암치료를 위해 흉강 내 속목정맥의 천자를 통한 피하매몰 중심정맥 케모포트(implantable central venous chemoport) 도관의 설치 중 발생할 수 있는 의인성 속목정맥 천공은 매운 드문 합병증 중의 하나로 혈흉이나 출혈성 쇼크를 일으킬 수 있으며, 부적절한 항암제 주입으로 인한 늑막삼출이 발생할 수 있다. 따라서 항암제 주입 전 조기에 진단하여 응급 개흉술을 통해 천공된 속목정맥을 봉합하는 것이 치료 원칙이다. 저자들은 우측 속목정맥을 통한 피하매몰 중심정맥 케모포트의 설치 후 발생한 속목정맥 천공 환자에서 부적절한 항암제 주입으로 인해 발생한 늑막삼출과 혈흉을 개흉술을 시행하지 않고 경피적 배액술 후 코일과 N-butyl cyanoacrylate를 이용한 색전술을 통해 성공적으로 치료한 1예를 경험하였기에 이를 보고하고자 한다.
This study was performed to investigate the effects of renal denervation and cromakalim, a K+ Channel opener, on central diuretic action of glibenclamide, an ATP-dependent K+ Channel blocker, in dog. Diuretic action of glibenclamide administered into the vein was weakened markedly by renal denervation and pretreatment of of cromakalim. Above results suggest that central diuretic action of glibenclamide is mediated by renal nerves and K+ Channel localized in kidney.
중심 정맥 도관술은 미숙아들의 출생과 생존율의 증가로 현재 신생아실에서 많이 시행 되고 있는 수기이며, 최근 중심 정맥 카테터 사용이 증가함에 따라 혈전이나 색전, 혈관 손상과 파열, 흉막 삼출, 화학적 폐렴과 같은 합병증이 보고되고 있다. 이에 저자들은 경피 중심 정맥 카테터 삽입 후 우측성 흉막 삼출액이 발생하여 시행한 혈관조영술에서 카테터가 홀정맥에 위치하였던 초극소 저출생 체중아에서 경피 중심 정맥 카테터를 제거한 후 흉막 삼출액이 호전되었던 1례를 경험하였기에 보고하는 바이다.
투석 환자에서 중심정맥협착은 비교적 흔한 합병증이나, 이로 인한 경정맥역류 및 두개내압 상승은 드물며, 진행성 시력 저하를 보이는 경우는 몇 개의 증례만 보고되고 있다. 저자들은 경정맥역류로 인한 두개내압 상승, 시력 저하 그리고 뇌 MRI에서의 상안정맥 확장에 대해 해면경막 동정맥루로 오인하였던 증례를 보고하고자 한다. 환자는 time-of-flight MR angiography (이하 TOF-MRA)에서 경정맥역류 소견이 있었고, 혈관조영술에서 좌측 완두정맥의 협착이 확인되었다. 중심정맥협착에 대해 풍선혈관성형술을 시행하였고 증상이 호전되어 퇴원하였다. 중심정맥협착에 의한 경정맥역류와 해면경막 동정맥루는 유사한 증상을 보일 수 있으나 치료법이 다르므로, MRI와 TOF-MRA의 면밀한 검토를 토대로 영상의학적으로 감별하는 것이 중요하며, 뇌혈관조영술을 통해 중심정맥협착 유무를 확인하는 것이 필요하다.
In this paper we present 3D image analysis of liver and blood vessels using MDCT. The purpose is to enhance the performance of clinician in assessing anatomical information of liver and blood vessels. The system consists of two parts: 3D image reconstruction and analysis of the 3D liver and blood vessel image. The central vein of the liver is the most important blood vessel for the liver transplantation. We will find the central vein's location and characteristic, and will scheme out a computer assistant liver transplantation planning. It will be an effective tool for interventional radiology, surgical planning, and quantitative diagnosis.
Objective: Central venous catheter (CVC) misplacement can result in incorrect readings of the central venous pressure, vascular erosion, and intravascular thrombosis. Several studies have examined the correlation between the guidewire J-tip direction and misplacement rate. This study examined whether the guidewire J-tip direction (cephalad vs. caudad) affects the misplacement rate in right subclavian venous catheterization. Methods: This prospective randomized controlled study was conducted between February 2016 and February 2017. The subjects were divided into two groups (cephalad group vs. caudad group) and the misplacement rate was compared according to guidewire J-tip direction in each group. Results: Of 100 patients, the cephalad and caudad groups contained 50 patients each. The age, sex, and operator experience were similar in the two groups. In the cephalad group, misplacement of CVC insertion into the ipsilateral internal jugular vein occurred in two cases. In the caudad group, misplacement of CVC insertion into the contralateral subclavian vein occurred in one case, with loop formation in the brachiocephalic trunk in one case. Guidewire J-tip direction showed no significant correlation with CVC misplacement. Conclusion: The guidewire J-tip direction does not influence the rate of misplacement.
A mutant of Potato vims Y (PVY) was occurred in PVY resistance flue-cured tobacco breeding line KF0402 $(TC1146{\times}KF117)$ showing vein necrosis at Suwon in Korea. This isolate, PVY-SWM, was differentiated from other PVY based on biological properties and nucleotide sequence analyses of coat protein gene. PVY-SWM caused typical symptoms on 21 indicator plants as compared to the PVY-TOJC37. Remarkably, the PVY-SWM induced distinctly different symptom of systemic vein necrosis on tobacco cultivars V.SCR, PBD6, TN86, TN90, Virgin A Mutant (VAM), Wislica, NC744, KB108 and KB111, which were reported to have the recessive potyvirus resistance gene va. In RT-PCR assays with specific primers for detection of PVY, a single band of about 800bp in length was produced. The amplified DNA was cloned and the nucleotide sequence was determined. The coat protein gene of PVY-SWM showed 88.4%-99.0% and 92.5%-98.5% identities to the 12 different PVY isolates of Genbank Database at the nucleotide and amino acidi respectively. Multiple alignments as well as cluster dendrograms of PVY-SWM isolate revealed close phylogenetic relationship to the $PVY^{NTN}$ subgroup.
5-Hydroxytryptamine(5-HT, serotonin), when given into the vein, produced antidiuretic action accompanied with reduction of glomerular filtration(GFR), renal plasma flow(RPF), osmolar clearance(Cosm) and amounts of sodium or potassium excreted in urine( $E_{Na}$ , $E_{K}$), with the augmented reabsorption rates of sodium and potassium in renal tubules. 5-HT, when infused into a renal artery, exhibited diuretic action accompanied with the augmented RPF and increased $E_{Na}$ and $E_{K}$ in only infused kidney. Antidiuretic action of 5-HT infused into the vein was not influenced by ketanserin, 5-H $T_2$receptor blockade, given into a renal artery, vein or carotid artery, by methysergide, 5-H $T_1$receptor blockade, given into a renal artery, whereas above antidiuretic action was inhibited by methysergide given into vein or carotid artery. Diuretic action of 5-HT infused into a renal artery in only experimental kidney was blocked by ketanserin injected into a renal artery, was not influenced by methysergide administered into a renal artery. Above results suggest that 5-hydroxytryptamine(5-HT) produced the antidiuretic action through central 5-H $T_1$receptor and the diuretic action through 5-H $T_2$receptor located in renal tubules of kidney.ney.
