Lateral Supramalleolar Flap for Reconstruction of Soft Tissue Defect around the Ankle Joint

  • Purpose: Soft tissue defect on foot and ankle is vulnerable and requires a thin flap for improvement of aesthetic and functional results. Lateral supramalleolar flap is a simple and fast procedure, which can preserve and supply reliable constant blood flow, and causes fewer donor site complications. The authors reviewed our cases and report the clinical results. Materials and Methods: Ten cases of soft tissue defects on the lower leg, around the ankle were treated with lateral supramalleolar flap. There were seven males and three females with a mean age of 54.8 years. The mean size of flaps was $5.9{\times}6.3$ cm and the mean follow-up period was 23 months. Flap survival and postoperative complications were evaluated. Results: Nine flaps survived completely without loss of flap. There was one case of partial wound dehiscence requiring debridement and repair, and another case of necrotic flap change requiring partial bone resection and closure. All patients were capable of weight bearing ambulation at the last follow up. Conclusion: The authors suggest that the lateral supramalleolar flap could be a useful option for treatment of soft tissue defect around the ankle joint.

  • The facial cleft and duplicated maxilla are lire congenital anomaly. After Rushton and Walker had reported a unilateral facial cleft with excess tooth and bone formation in 1937, few authors described similar cases. The etiology of this anomaly is not well understood, but considered embryologically as a neurocristopathy. A neurocristopathy is defined as a condition arising from aberrations in early migration, growth and differentiation of neural crest cells. This aberrations result in facial malformation such as facial clefts and loss or duplication of facial structures. We experienced a male newborn baby with bilateral facial cleft and duplicated maxilla. The cleft was surgically corrected when he was 5 months old. The function and appearance of lip are improved. Duplicated maxilla will be surgically removed. We report this case with review of literatures.

High Selectivity Coupled Line Impedance Transformer with Second Harmonic Suppression

  • This paper presents a design of an impedance transformer (IT) with high frequency selectivity characteristics. The frequency selectivity can be controlled by even- and odd-mode impedance of a shunt coupled transmission line (TL). For experimental validation, a 50- to $20-{\Omega}$ IT was implemented at a center frequency ($f_0$) of 2.6 GHz for the long-term evolution signal. The measured results were in good agreement with the simulations, showing a return loss higher than 19 dB over a passband bandwidth of 0.63 GHz (2.28-2.91 GHz) and good sharp frequency selectivity characteristic near to the passband. The series coupled TL provides a transmission zero at 5.75 GHz, whereas the shunt coupled TL provides three transmission zeros located at 2 GHz, 3.1 GHz, and 7.14 GHz.

The Performance of Ultra-Wideband Filter with regard to dielectric materials (유전체 특성에 따른 UWB용 필터 특성에 관한 연구)

  • In this paper, we propose an ultra wideband pass filter of ring type with embedded stripline stub. The fractional bandwidth was 98 % at the center frequency of 8 GHz and insertion loss was below 1 dB in passband. Two kinds of dielectric materials with the permittivity of 7.8 and 40, respectively were adopted in evaluating the suggested filter structure. As the permittivity of material became larger, the size of filter smaller as expected without any sacrifice in filter performance. In summary, the suggested filter structure has smaller size due to the embedded stripline stub and can be minimized by adopting dielectric material with higher permittivity again.

The Progress Report of Weight and Waist Circumference through the Oriental Obesity Treatment (한방비만프로그램을 통한 체중과 부위별 복부둘레의 경과 관찰 및 상관성 연구)

  • Objectives The purpose of this study was to investigate the progress of weight and waist circumference(WC) through oriental obesity treatment and clarify the correlation between weight loss and WC change. Methods The subjects were treated from January 2007 to September 2009 in J Oriental Medical Center. Herbal medicine, electroacupuncture, oriental obesity physiotherapy were performed for a month. We checked the change of weight and WC at every treatment. We classified the WC into upper, middle, lower parts specifically and measured them individually. Measurements were analyzed by using one-way repeated measures ANOVA and we also investigated correlation between weight loss and WC change. Results (1) During the first and second treatment sessions, the weight loss was the highest, $0.96{\pm}0.86\;kg$($1.46{\pm}1.32\;%$) and during the 5th and 6th treatment sessions, the lowest $0.37{\pm}0.616\;kg$. The final weight loss was $3.71{\pm}1.476\;kg$($5.51{\pm}2.12\;%$). (2) After 8 treatment sessions, the reductions of upper, middle and lower WC were $3.59{\pm}2.26\;%$, $5.93{\pm}2.75\;%$, $5.51{\pm}3.22\;%$, respectively. As the patients received more treatment, there was a progressive decline in the variation of weight and middle WC. However, the decline rate of upper and middle WC fluctuated during the treatments, forming a W-shaped curve. (3) Analysis of the correlation between the weight loss and WC change shows that the reduction of middle WC was most closely associated with the weight loss. Conclusions This study indicated that the rate of weight loss and WC reduction showed stagnation during the 5th and 6th treatment sessions, and the reduction of middle WC was most closely associated with the weight loss. Since the progress of weight and WC change is an important field of research on obesity, further systemic studies would be needed for the foundation of a clinical guideline.

  • This study estimates how soil loss in a basin has been occurred according to the change of land cover, and analyzes which type of land cover has the largest soil loss by classifying the land-cover type into each area and a whole basin. Musimcheon, the second branch stream of GeumGang, is chosen as a research area. The result of analysis shows that the average soil loss occurs most largely in a crop land and a paddy field. The yearly soil loss of watershed estimates approximately 14,000 ton/yr in case of using 100-year-frequency rainfall data. A forest area, which takes the largest area in watershed, shows the soil loss occurs approximately 1,000ton/yr. A crop field shows that soil loss increased most largely 4,900 ton/yr (34.6%) in 1985 to 8,100 ton/yr (56.1%) in 2000. The change of land cover in a crop land increased 8% to 14%, and this change influences on the increase of soil loss. As a result of analyzing the area over $200ton/km^2/yr$, the soil loss in a crop field accounts for 74% to 96%.

A Study on the Effect of Life Behavior and Socio-environmental Satisfaction on Life Satisfaction of the Elderly People with or without Hearing Loss (난청 여부에 따른 노인의 생활행태와 사회환경만족도가 삶의 만족도에 미치는 영향 연구)

  • This study was conducted to empirically analyze the effect of life behavior and socio-environmental satisfaction on life satisfaction of the elderly with or without hearing loss. To this end, 3,071 elderly people of 「the Survey on the Actual Condition of the Elderly」 in 2020 were set as the subjects. After controlling for their demographic variables, the multiple regression model showed that the elderly with hearing loss had different factors of life behavior and socio-environment satisfaction that affected their life satisfaction compared to those without. As a result, the sub-factors of life behavior in the elderly with hearing loss, such as economic activities, social group activities, the use of the center, and the sub-factors of social environment satisfaction, such as family satisfaction and environmental satisfaction, had a positive (+) effect on life satisfaction. Especially, the elderly with hearing loss had more limitations in both the area of living behavior and the area of social environment satisfaction than the elderly without hearing loss. Therefore, in order to improve the life satisfaction of the elderly with hearing loss, it is suggested that the government and local governments should simultaneously supplement welfare policies and facilities for the elderly with hearing loss.

Analysis Evaluation of Impact Behavior of 270,000kL LNG Storage Outer Tank from Prestress Force Loss (프리스트레스 손실량을 고려한 270,000kL급 LNG 저장탱크 외조의 비산물체 속도에 따른 충돌 거동 해석)

  • LNG storage outer tank is a vertically and horizontally prestressed concrete wall structure. Therefore, when the storage tanks become larger, prestressing tendons become longer and eventually the prestressing loss becomes larger. Also, recently, bomb terrors and accidental crashes have occurred frequently on important infrastructures. Therefore, LNG storage tanks are also exposed to these dangerous scenarios, where they need to be evaluated and protected from these threats. Therefore, in this study, the behavior of 270,000 kL LNG storage outer tank impacted by a flying object is evaluated using implicit FEM code, LS-DYNA. In the analysis, the prestress loss due to the increased length of prestressing tendons from enlargement of outer tank is considered. A comparison study between the LNG tanks with and without prestress loss is performed to investigate the impact behavior and the effect of prestressing force change on the safety and serviceability prestressed concrete containment.

Correction of TDC Position for Engine Output Measuring in Marine Diesel Engines (선박용 디젤엔진의 출력산정을 위한 TDC 위치보정에 관한 연구)

  • The accurate engine output is basically one of important factors for the analysis of engine performance. Nowadays in-cylinder pressure analysis in internal combustion engine is also an indispensable tool for engine research and development, environment regulation and maintenance of engine. Here, it is essential more than anything else to find the correct TDC(Top Dead Center) position for the accuracy of engine output for diesel engine. Therefore this study is to analyze affecting factors to TDC position in 2-stroke large low speed engine and to suggest new method for determining correct TDC position. In the previous paper, it was mentioned that the accuracy of engine output is influenced by the determination of exact TDC position, and that 'Angle based sampling' method is better than 'Time based sampling' method in terms of precision. It was confirmed that there is 'Loss of angle', which is a difference between compression pressure peak and real TDC caused by heat loss and blow by of gas leakage. Consequently we invented new method, called "An improved method of time based sampling", which can obtain the correct engine output. The results by this method with compensating loss of angle was shown the same result by the 'Angle based sampling' method in encoder setting cylinder. This study is to suggest the new measuring method of exact engine output, and to examnine the reliance on the outcome.

Variation of Magnetic Properties of Fe-Si Compressed Cores with Si Content (Si 함량에 따른 Fe-Si 압분코어의 자기적 특성)

  • Fe-3, 4.2 and 6.8% Si compressed cores were fabricated, and then electrical resistivity, AC and DC magnetic properties, microhardness, and other properties were analyzed in order to know whether best soft magnetic properties could be also obtained in an Fe-Si compressed core with the well-known composition of Fe-6.5% Si. With increasing the silicon content, eddy current loss and hysteresis loss decreased and increased, respectively, so that a minimum total loss was not obtained in the well-known Fe-6.8 % Si cores, but obtained in the Fe-4.2 % Si cores. Also electrical resistivity of the cores and hardness of the particles increased monotonously with silicon content so that compaction ratio of the cores decreased. B2 and $DO_3$ ordered phase could be observed only in Fe-6.8% Si powder. A minimum loss and highest permeability of the Fe-4.2 % Si cores can be explained by the ratio of specific electrical resistivity of insulator to that of magnetic particles, micro-hardness, compaction ratio and demagnetization coefficient of the Fe-Si powder particles with silicon content.
