Superficial Circumflex Iliac Artery Perforator Free Flap for Reconstruction of Small or Medium Sized Defect on Lower Extremities (하지의 작거나 중등도의 결손 부위 재건을 위한 얕은엉덩휘돌이동맥 천공지 유리 피판술)

  • Purpose: For reconstruction of lower extremity defects, various flaps can be used and the appropriate flap must be selected and applied according to the size of the defect. In particular, in cases where the defect size is small to moderate, thinner or smaller volume flaps are useful. The authors performed reconstruction of small to moderate defects on the lower extremities using superficial circumflex iliac artery perforator free flaps and are reporting the results. Materials and Methods: Fifteen patients underwent reconstruction of defects on lower extremity areas using superficial circumflex iliac artery perforator free flaps from July 2011 to July 2012 at this hospital. The flaps were elevated from above the deep fat layer, and, in all cases, the vessel diameter of the flaps was less than 1mm, with the exception of superficial vein that accompanied it. Results: The mean follow up period was 4.46 months, and, despite a partial loss in the flap in two cases, there were no total losses. All donor sites were closed with primary closure, and there was no occurrence of complications, such as hematomas, seromas, or lymphorrheas. The patients were highly satisfied with the donor site scar since it could be masked by underwear. Conclusion: Compared to other flaps, superficial circumflex iliac artery perforator free flaps are thinner in thickness and smaller in volume, which results in a more natural contour of the recipient site after the operation. In addition, since the flap can be elevated from supra-deep fat layer, the operation time can be shortened, and lymphorrhea can be prevented, which in turn lessens donor-site morbidity.

Evaluation of Flow Characteristics in Water Supply Pipes Shielding Electromagnetic Pulse of 100 dB with Concentric and Eccentric Reducers (Concentric Reducer와 Eccentric Reducer를 사용한 EMP 차폐 100dB급 급수관의 유동특성 평가)

  • In this paper, the flow characteristics of water in the water supply pipes of a WBC array were evaluated. We simulated the flow velocities and pressures for a standard pipe, an expansion pipe with a concentric reducer, and an expansion pipe with an eccentric reducer using computational fluid dynamics. In the case of the standard pipe, when the inlet flow velocities were 0.5 m/s and 2.0 m/s, the maximum flow velocities at the center of the WBC array were 0.54 m/s and 2.74 m/s, respectively, which were the greatest values among those of all the pipe models considered. In the case of the expansion pipe, the maximum flow velocities at the center of the WBC array were almost the same under the same conditions regardless of the type of reducer. The pressure losses in the pipe due to the concentric and eccentric reducers were found to be (165.09 ${\times}$ inlet $velocity^{1.6677}$) and (210.98 ${\times}$ inlet $velocity^{1.6478}$), respectively. The coefficient of determination at this time was greater than 0.99 and was the same for both the models. As a simulation result, it was found that in order to reduce the pressure loss when pipe with WBC array is connected with a conventional pipe, diameter of the pipe with WBC array at that section should be enlarged by one step, and then connected to the conventional pipe with a concentric reducer.

  • The side effects of head and neck radiation therapy include mucositis, xerostomia, loss of taste, radiation caries, oral infection, osteoradionecrosis and trismus. When a patient is arranged to begin head & neck radiotherapy, oral pathologic lesions are examined and managed for the prevention of oral complications. The advanced odontogenic infection should be especially controlled before the radiotherapy and the patient must be instructed for proper oral prophylaxis. Generally the more conservative treatments, such as, scaling, restoration, endodontic treatment, are the care of choice and dental extraction is performed in advanced periapical and periodontal pathologic conditions. If the dental extraction should be done, the radiotherapy consequently will be delayed until there is epithelium covering the extraction socket, leaving no exposed bone. The cancer patient with severe emotional stress pray for the early radiation therapy, in spite of possibility of the recurrent odontogenic infectious lesions. So, the authors attempted to do the early radiation therapy by the conservative endodontic drainage and surgical incision & drainage without extraction of the infected teeth, and resulted in relatively good prognosis without the severe side effects of head and neck radiotherapy.

  • The prosthodontic treatment of severely resorbed edentulous patients has been one of the frustrating areas due to extensive loss of tissues. The integrated neuromuscular balance among tongue, lip, and cheek is compromised. The retention, stability, and support are the three major factors to influence the clinical outcome. Fish described a denture as having three surface, with each surface playing an independent and important role in the over all fit, stability, and comfort of the denture. He recommended that the polished surface should be a series of inclines so that pressure from muscular activity will retain dentures. Within the denture space there is an area that has been termed the neutral zone. The neutral zone is that area in the mouth where, during function, the forces of the tongue pressing outward are neutralized by the forces of the cheeks and lips pressing inward. According to Jacobson and Krol, neuromuscular control interacts to provide retention and the relationship of polished surface of denture base to the surrounding muscular structure of orofacial capsule facilitates the stability and retention. This neutral zone concept has been demonstrated with various modification by a number of authors. The theory used to develop the denture base contours is based on the belief that the muscle should functionally mold not only the border but the entire polished surface. Lott and Walsh reported the clinical success on complete mandibular dentures with application of neutral zone concept. A number of studies demonstrated that denture stability and retention are more dependent on correct position of the teeth and correct contour of external surfaces of the denture in a severely resorbed alveolar ridge. This article presents a prosthodontic approach to treatment of a edentulous patient using neutral zone technique to improve the retention and stability of the prosthesis.

  • This study was conducted for investigating the status of management of preemployment health examination and to have an effect on the worker's employment. Health managers of 103 companies in Incheon metropolitan city and Gyeonggi were interviewed by telephone. Of 103 companies, 67(65.1%) said they don't hire the applicants who have an active pulmonary tuberculosis, 80(77.7%) companies said they health HBV carrier is acceptable but active HBV carrier is not 29(28.2%) companies said they don't hire the applicants who have a hypertension or diabetes mellitus, 42(40.8%) companies said they don't hire the applicants who have a hearing disturbance. If HIVD is suspicious in X-ray lumbar-sacral region, 37(78.7% of 47 companies) said they do not hire the applicants. 29(35% of 83 companies) said they cancel the employment of the applicants who are suspicious of noise induced hearing loss on preplacement health examination. From our survey, preemployment health examination was utilizing mainly as a tool for the selection of health employees who don't have a disease. Furthermore, in many companies, additional test items are being included and getting more strict the selection criteria for preemployment health examination. For the right use of preemployment health examination, author suggested that further studies were needed to select the adequate test items and establish the reasonable criteria for preemployment health examination.

  • This paper presents the InGaP/GaAs HBT differential LC VCO with low phase noise performance for adaptive feedback interference cancellation system(AF-lCS). The VCO is verified with enhanced tank structure including filtering technique. The output tuning range for proposed VCO using asymmetric inductor and symmetric capacitors withlow pass filtering technique is 207 MHz. The output powers are -6.68 including balun and cable loss. The phase noise of this VCO at 10 kHz, 100 kHz and 1 MHz are -102.02 dBc/Hz, -112.04 dBc/Hz and -130.40 dBc/Hz. The VCO is designed within total size of $0.9{\times}0.9mm^2$.

Temperature Dependent Terahertz Generation at Periodically Poled Stoichiometric Lithium Tantalate Crystal Using Femtosecond Laser Pulses

  • Coherent tunable terahertz generation was demonstrated in periodically poled stoichiometric lithium tantalate crystal via difference frequency generation of femtosecond laser pulses. Simultaneous forward and backward terahertz radiations were obtained around 1.35 and 0.63 THz, respectively at low temperature. By cooling the crystal to reduce losses caused by phonon absorptions, the generated THz bandwidth was as narrow as 23GHz at the center frequency of 0.63 THz. The measurement result of temperature-dependent showed gradual intensity increase of the generated terahertz pulse and red shift of the center frequency as the temperature decrease from 291 to 143 K, but insignificant reduction of the spectral bandwidth. Furthermore, the stoichiometric crystal was very suitable for the suppression of THz loss at low temperature compared to the congruent $LiNbO_3$ crystal.

Colorless Amplified WDM-PON Employing Broadband Light Source Seeded Optical Sources and Channel-by-Channel Dispersion Compensators for >100 km Reach

  • We have demonstrated an amplified wavelength-division multiplexed (WDM) passive optical network (PON) by using broadband light source (BLS) seeded optical sources and chirped fiber Bragg gratings (FBGs) based dispersion compensators. Chirped FBGs located at central office (CO) were fabricated and used as channel-by-channel dispersion compensators in order to mitigate the dispersion-induced distortion of both downstream and upstream signals. Owing to a low insertion loss of chirped FBG based dispersion compensator, the optical signal-to-noise ratio (OSNR) of the downstream signal could be improved to be ~28 dB. Thus, we re-confirmed that an error-free transmission of 1.25 Gb/s signals over a 100 km single-mode fiber (SMF) link could be achieved with a proposed amplified WDM-PON architecture. We have also evaluated the impact of various noises on the system's performance, and found that the low OSNR of the downstream signal would be a main limiting factor on the maximum reach of the proposed amplified WDM-PON architecture. From the measured ~13 dB improvement in OSNR of the downstream signal compared to our previously-proposed dispersion compensating module based scheme, we believe that the proposed architecture can accommodate a reach of longer than 100 km SMF link easily.

Reduced Burst Release from ePTFE Grafts: A New Coating Method for Controlled Drug Release

  • Hemodialysis graft coated with paclitaxel prevents stenosis; however, large initial burst release of paclitaxel causes many negative effects such as drug toxicity and inefficient drug loss. Therefore we developed and tested a novel coating method, double dipping, to provide controlled and sustained release of paclitaxel locally. Expanded polytetrafluoroethylene (ePTFE) grafts were dipped twice into a solution of several different paclitaxel concentrations. In vitro release tests of the double dipping method showed that early burst release could be somewhat retarded and followed by sustained release for a long time. We observed the effect of paclitaxel coating by double dipping in porcine model of arterio-venous (AV) grafts between the common carotid artery and the external jugular vein. 12 weeks after constructing AV grafts, cross sections of the graft venous anastomosis were obtained and analyzed. Paclitaxel coated ePTFE grafts by double dipping were observed to prevent neointimal hyperplasia and therefore reduced stenosis of the arteriovenous hemodialysis grafts, especially at the graft venous anastomosis sites. Our results demonstrate that second dipping of ePTFE graft, which was already coated once with paclitaxel, washes off the drug on a surface of the graft and affects the ratio of paclitaxel on the surface to that of the inner space, possibly by diffusion: thus the early burst of drug can be somewhat reduced.

Evaluation on Dielectric Properties of Epoxy/Montmorillonite Nanocomposites (에폭시/몬모릴로나이트 나노복합재료의 유전특성 평가)

  • The epoxy composites are prepared with mixing temperature of epoxy/montmorillonite (MMT) melt master batch and the dielectric properties of the composites are also compared with intercalation of MMT. The exfoliation mainly occurrs iii the low content of MMT composites, while in the composites with high content of MMT the interspacing distance increases as the mixing temperature of epoxy/MMT master batch is increased. Class transition temperature of the composite which the MMT are effectively exfoliated is increased with the appropriate postcuring condition. Since the orientation polarization of dipoles in the epoxy molecules is restricted by the clay nanolayers exfoliated, the dielectric constant and dielectric loss of the composites are reduced. Furthermore, the dielectric properties could be improved by controling the mixing temperature and time of epoxy/MMT master batch as well as postcuring condition.
