Bloody nipple discharge in an infant

  • Although milky nipple discharge appears frequently in infants, bloody nipple discharge is a very rare finding. We experienced a 4-month-old, breast-fed infant who showed bilateral bloody nipple discharge with no signs of infection, engorgement, or hypertrophy. The infant's hormonal examination and coagulation tests were normal, and an ultrasound examination revealed mammary duct ectasia. The symptoms resolved spontaneously within 6 weeks without any specific treatment, except that we advised the mother to refrain from taking herbal medicine. Since no such case has been previously reported in Korea, we present this case with a brief review of the literature.

  • Abdominal epilepsy is an uncommon disorder and a rare cause of recurrent abdominal pain of children. Diagnostic criteria of this disorder include otherwise unexplained, paroxysmal gastrointestinal complaints, symptoms of a central nerve system disturbance, an abnormal electroencephalogram with a finding specific for a seizure disorder, and improvement with anticonvulsant medication. We present a case of a 6-year-old boy with abdominal epilepsy presenting with recurrent, paroxysmal abdominal pain for 4 years. This patient had definite electroencephalogram abnormalities and a striking response to administration of an anticonvulsant.

Evaluating Efficacy of Hilbert-Huang Transform in Analyzing Manufacturing Time Series Data with Periodic Components (제조업의 주기성 시계열분석에서 힐버트 황 변환의 효용성 평가)

  • Real-life time series characteristic data has significant amount of non-stationary components, especially periodic components in nature. Extracting such components has required many ad-hoc techniques with external parameters set by users in case-by-case manner. In our study, we evaluate whether Hilbert-Huang Transform, a new tool of time-series analysis can be used for effective analysis of such data. It is divided into two points : 1) how effective it is in finding periodic components, 2) whether we can use its results directly in detecting values outside control limits, for which a traditional method such as ARIMA had been used. We use glass furnace temperature data to illustrate the method.

  • Cytologic features of inflammatory pseudotumor of the lung have not been described frequently. We report fine needle aspiration cytologic(FNAC) finding of a case of inflammatory pseudotumor misdiagnosed as adenocarcinoma in a 63-year-old man. The FNAC displayed a mixture of histiocytes, myofibroblasts, pneumocytes, and plasma cells. Some histiocytes and myofibroblasts had large nuclei with irregular nuclear membrane and prominent nucleoli, which mislead the diagnosis on adenocarcinoma on FNAC. The heterogeneous cell population is the unique cytologic features of inflammatory pseudotumor, which are helpful to distinguish it from other circumscribed benign and malignant lesions. Familiarity with these features is essential to avoid misdiagnosis and possible overtreatment.

  • An extremely unusual case of epidermoid cyst showing diffuse parakeratosis and aggressive clinical behavior is presented. A destructive bone lesion with surrounding ill-defined soft tissue lesion was found by computed tomography in a 63 year-old man complaining of painful swelling of the right buttock. He had a history of surgical excision twice for epidermoid cysts of soft tissue of the right hip during recent one year On aspiration cytology, the aspirate was highly cellular and mostly composed of desquamated nucleated squamous cells. Operation finding revealed that the iliac bone was Irregularly destroyed and filled with gray-white cheesy material and necrotic bone bedris. Adjacent gluteus muscle showed scattered gray-white lesions. The curettage specimen showed bone necrosis and desquamated squamous cells filling the marrow spaces. The lesion within muscle revealed epidermoid cyst with diffuse parakeratosis.

  • Pleural metastasis from breast cancer is a common manifestation. While pleural effusion is the most frequent finding, it is relatively rare for pleural nodularity and plaque that do not accompany pleural effusion. We report a patient with a rapidly growing huge pleural mass without pleural effusion caused by breast cancer. The patient was treated for severe dyspnea caused by the pleural mass. Along with the case report, we performed a systematic review of management of dyspnea in terminal cancer patients.

A Study on the Efficient Applicability of Fenton Oxidation for the Wastewater Containing Non-biodegradable Organics (생물난분해성 유기물질 함유 폐수처리를 위한 Fenton 산화법의 효율적 적용방안에 관한 연구)

  • This research is about wastewater containing non-biodegradable TDI(Toluene Diisocyanate) that is treated by the activated carbon adsorption method. In the case of the Fenton oxidation process being applied to the existing process, optimal pH, reaction time, chemical dosing amount, removal rate, and cost were investigated. A pilot plant test was applied after finding optimal conditions with lab experiments. The optimal conditions were pH 3~5(COD removal rate 84~88%) and reaction time 30min~1hr. In higher $H_2O_2$ dosing amount, COD removal rate was a little higher. But there was little difference in the removal rate according to $FeSO_4{\cdot}7H_2O$ dosing amount. Treatment cost was economical in the case of the Fenton oxidation process being operated earlier than activated carbon adsorption system. But chemical dosing point, chemical mixing effect, chemical dosing amount, removal rate, and the cost of facility and others must be considered in practical process.

A clinical evaluation of surgical treatment of esophageal diverticulum (식도게실의 외과적 치료에 대한 임상적 고찰)

  • This is a report of 12 cases of esophageal diverticulum treated surgically at the Department of Thoracic and Cardiovascular Surgery, Hanyang University Hospital during the 16 years. 10 patients were female and 2 were male and the age distribution was between 31 and 61 years. Various subjective symptoms were noticed preoperatively. Diagnosis was confirmed by esophagography. The type of esophageal diverticulum were pulsion type in 11 cases and traction type in one case, and they were all located on the mid-thoracic esophagus, but one case was associated with epiphrenic pulsion diverticulum After diverticulectomy with or without myotomy, subjective symptoms disappeared and there were no operative complications and mortality. Indication of myotomy evaluated with motilities status by fluoroscopic finding of X-ray barium study. Recurrence of symptoms was not noticed during follow-up.

Parachordoma of the Chest Wall -1 case report- (흉벽에 발생한 유척삭종(Parachordoma) -1예 보고-)

  • Parachordoma is a very rare, slow-growing, and low-grade malignant tumor that occurs in the extremities and trunk. The differential diagnosis includes extraskeletal myxoid chondrosarcoma and chordoma in the histologic finding. Thus, histologic findings with immunohistochemistry may be helpful in distinguishing parachordoma from extraskeletal myxoid chondrosarcoma and chordoma. I report with a brief review of literatures one case of parachordoma of the chest wall which was successfully treated by en-bloc resection and chest wall reconstruction using 2 mm Gore-Tex$^{\circledR}$ soft tissue patch and free from recurrence for 16 months.

  • The Authors report a case of sleep paralysis patient who is 25-years old man. He complained that he has been experiencing episode "being suffocated" during sleep and could not move by himself while semiconsciousness state. He was a only son of divorced parents and could not attach anybody from yong childhood. His symptoms was began intermittently after his early teens. Medical history and physical examination revealed no significant finding. A nocturnal polysomnogram showed loss of muscle tone during terminal REM period and sudden hyperventilation attack followed by wake-up. Psychosocial stress and insufficient sleep were presumed to be an etiological factors. Imipramine 50mg per day almost completely relieved symptoms.
