A Case of Subclinical Hyperthyroidism Treatment with Herbal Medicine (한약 단독치료로 호전된 불현성 갑상선기능항진증 증례 보고)

  • Objective: This study was performed to evaluate the effects of herbal medicine on an elderly patient with subclinical hyperthyroidism who could not use antithyroid medication due to an adverse reaction to methimazole. Methods: Herbal medicine was administered and a thyroid function test was used to evaluate the effects of the treatment. Results: After treatment with herbal medicine and therapy, the patient showed improvements in TSH levels. Conclusion: This case suggested that herbal medicine can be used for patients who have adverse reactions to antithyroid medication; however, additional studies with more patients are required for verification of this finding.

Infantile Lobar Emphysema -A Case Report- (유아 엽성 폐기종 -1례 보고-)

  • Infantile lobar emphysema is a pulmonary hyperinflation state that has the clinical features of an air block syndrome characterized by bronchial cartilaginous abnormalities or unknown origin. Left upper lobe was affected in most of the reported infantile lobar emphysema. Infantile lobar emphyema is divided into two categories. e.g., congenital and acquired. We have experienced a case of left lower lobe involved infantile lobar emphysema which had undergone left pneumonectomy. She had progressive signs of tension accompanied by mediastinal displacement, ventilatory and circulatory failure in infant period. Because of the combined left upper lobe hypoplasia, left pneumonectomy was performed. And there was no cartiliginous abnormality in pathologic finding. This is the first domestic case which was affected in the lower lobe and successful surgical repaired.

  • The problems of mixed $H_2/H_{\infty}$ filtering design fer continuous and discrete time linear systems with time delay are investigated. The main purpose is to design a stable mixed $H_2/H_{\infty}$ filter which minimizes the H$_2$Performance measure satisfying a prescribed H$_{\infty}$ norm bound on the closed loop system in continuous-time case and discrete-time case, respectively. The sufficient conditions of existence of filter, the mixed $H_2/H_{\infty}$ filter design method, and the upper bound of performance measure are proposed by LMI(linear matrix inequality) techniques in terms of all finding variables. Also, we present optimization problems in order to get the optimal mixed $H_2/H_{\infty}$ filter in continuous and discrete time case, respectively.

  • Congenital hepatic fibrosis is an inherited, congenital disorder of the liver characterized by portal hypertension and hepatic fibrosis. We experienced a case of congenital hepatic fibrosis with esophageal varix in a 9-year-old male. He complained hematemesis, hematochezia, dizziness. In laboratory examination, AST/ALT was slightly increased. Esophageal varix was noted by an endoscopic examination. Hepatosplenomegaly and hypoechoic lesion of periportal area were seen by abdominal CT scanning. Histologic finding of liver biopsy showed fibrous tracts containing dilated bile ductules connecting adjacent portal spaces that were widened by mature fibrosis. Endocopic sclerotherpy and ligation was done. We summarized a case with review of literatures

  • The purposes of study were to analyze the characteristics of case management network for the homebound elderly in Puchon and to identify major factors that affect the number of network member. The population of this study was social workers who provide services to the elderly. The results show that license of social work and information closeness have a significant effect on the number of network member among the independent variables. This finding is important for case management in the network. As observed in relationship characteristics, the community welfare center established by the university and city government roles in promoting service coordination.

  • An economic design of specification limits for production process of ham is considered for a given process mean in a complete inspection plan. Each ham is inspected, and if it meets the specification, it is accepted. The ham less than the lower specification limit are changed another products or at a discounted price, and those greater than the upper specification limit are reworked. A profit model is developed which involves selling price, production cost, rework cost and the cost which is incurred by imperfect quality. Methods for finding the optimal specification limits are derived for the case of piecewise linear loss function with an industrial case study.

Endobronchial Hamartoma -One Case Report- (기관지내 발생한 과오종 -1례 보고-)

  • We experienced one case of endobronchial hamartoma on left lower lobar bronchus in sixty year old male patient. Less than 1 % of lung tumors are benign, and the prevalence of endobronchial hamartoma is reported to be from 3 % to 40 %. The mean age was 52.9 years, and equal gender prevalence.Symptoms were related to intraluminal growth, including fever, chill, productive cough, hemoptysis, exertional dyspnea, recurrent pneumonia and so on. Bronchoscopic finding was tumor present as polypoid mass in the lumen of a left main stem bronchus,distal to 3 cm from carina. Biopsy was done. The histopathologic pattern showed several nodules of loose myxoid tissue and islands of cartilage. We performed partial resection of the affected bronchus,1cm anterior to the superior segmental bronchial opening to just distal from superior segmental orifice. Including superior segmentectomy, partial resection of the left lower lobar bronchial resection and end-to-end anastomosis with 4-0 Poly dioxanone sutere materials interruptedly. We report this case with the brief review of literatures.

  • Broncholithiasis is defined as a condition in which a concretion is present within a bronchus or a cavity in the lung communicating with a bronchus. Broncholithiasis, although recognized in antiquity by Aristotle and well known to accompany the mediastinal calcifications of tuberculosis and histoplasmosis frequently seen in our society, is rarely mentioned in recent medical literature. Esophagobronchial fistula is an uncommon finding in broncholithiasis. This report is a case of esophagobronchial fistula with broncholithiasis in 58-year-old male patient who complained paroxysmal coughing after ingestion of fluids. The fistulous tract was successfully resected and reinforced by mediastinal pleura. Broncholithiasis was confirmed by lithoptysis as the cause of previous esophagobronchial fistula.

Intramural esophageal dissection after endoscopy: A case report in a hypopharyngeal cancer patient treated with concurrent chemoradiotherapy

  • Intramural esophageal dissection is a rare disorder characterized by a separation of the mucosa and/or submucosa from deeper muscular layers of the esophagus, with or without perforation. Iatrogenic instrumentation such as endoscopy is one of the major causes of IED. We report a case of IED after endoscopy in a patient with hypopharyngeal cancer treated with concurrent chemoradiotherapy, and suggest that a history of chemoradiotherapy can be a risk factor of IED on endoscopy. In this case, chest computed tomography scans show not only typical esophageal double lumen but also eccentric esophageal wall thickening and abnormally thin the other side esophageal wall, and this CT finding may also be important to diagnose IED.

A Case of Rapidly Growing Extraocular Sebaceous Carcinoma

  • Sebaceous carcinoma is a rare malignant tumor differentiated from the adnexal epithelium of sebaceous glands and forms less than 1% of all cutaneous malignancies. We present a case of a 93-year-old woman with a rapidly growing mass on the right cheek. Initial histiopathologic finding was basal cell carcinoma. The mass was widely excised and superficial parotidectomy was performed while preserving the facial nerve branches. The resulting defect was covered with a transposition flap from the ipsilateral posterior auricular area and the donor site was closed primarily. However, histopathologic examination of the excised mass showed a poorly differentiated sebaceous carcinoma with a clear resection margin. The diagnosis of sebaceous carcinoma can be difficult to make at initial presentation. This report describes a rare case of a rapidly growing extraocular sebaceous carcinoma, which resulted in a good treatment outcome, and provides a review of relevant literature.
