Carotid Cavernous Sinus Fistula with Abducens Nerve Palsy after Le Fort I Osteotomy : A Case Report (상악골절단술 후 외전신경마비를 동반한 경동맥 해면정맥동루)

  • Carotid cavernous sinus fistula(CCSF) is an abnormal communication at the base of the skull between the internal carotid artery and the cavernous sinus. Fistula is almost associated with extensive facial trauma as a result of direct or indirect forces. Most fistulas of traumatic origin develop as a result of fractures through the base of the skull, which cause the laceration of the internal carotid artery near the cavernous sinus. The signs and symptoms of CCSF are pulsating exophthalmosis, orbital headache, pain, orbital or frontal bruit, loss of visual acuity, diplopia and ophthalmoplegia. Angiography reveals a definite CCSF and a detachable balloon embolization is known to be the treatment of choice. Even though carotid cavernous sinus fistula is an uncommon complication after orthognathic surgery, several cases of CCSF due to congenital anomalies, pre-existing aneurysms and abnormally thickened maxillary posterior wall have been reported in the literature. We have experienced a case of CCSF after Le Fort I osteotomy for maxillary advancement in skeletal class III patient and the cause, pathogenesis, diagnosis and treatment of this case.

  • This case analyze the point of trouble and reform processing construction on input design to resolve the problem for setting tower in front of narrow base at the 154kV OO Transmission Line substaion. Hereafter the input design are presented for the basic reference data which the case a point of trouble and reform contents, input design of special shape tower and the foundation on substation input design cannot be setted by standard type of tower because of narrow base and cannot keep clerance overhead transmission conductor input to the substion.

Variation of Wind Field over the Leeward Area According to the Local-scale Geographical Variation under Strong Wind Condition (강풍조건에서 국지규모 지형 변화에 따른 풍하측 바람장 변화)

  • We have investigated the wind speed variations over the leeward region when the strong wind blows. In this study we employ Envi-met numerical model to simulate the effect of surface boundary conditions. This model is applied for three cases which are characterized by land use and terrain height. The base case having natural geographical condition shows the weakest wind speed around lee side of Chunsudae. The others which remove the vegetation and cut off the terrain above 20 m ASL represent the stronger wind speed than base case. The main factor of this result is the surface friction. The distinct variation of wind is found at offshore area between Chunsudae and the southern part of village, but the northern part where is apart from Chunsudae shows a small variation of wind pattern. The weakening of wind speed around residential area is a maximum of 4~10 m/s when the wind blows in the village as strong as 55 m/s. The gust wind speed is weakened about 7~17 m/s in this case if the coefficient of gust wind adapted as 1.75.

Expert System for Emergency Decision Making for Metro Water Supply Systems (광역상수도 시설의 비상시 의사결정을 위한 전문가시스템)

  • An efficient operational strategy using expert system for metro water supply systems in case of emergency situations is developed in this study. The emergency situations of the water supply systems are classified into three categories : pipeline system accident, machinery and electric facility accident and water quality accident. A PC-based expert system is developed using CLIPS for Seoul metro water supply system, Phase 1 & 2 system and Phase 3 & 4 system. Broad professional knowledges and experiences from the experts in the water supply systems have been collected systematically to construct the knowledge base. Decision-making in case of an emergency is based upon the professional knowledge so that a rational and efficient operational management can be available even in the absence of experienced expert. Especially the expert model developed in this study also provides a guide for pumping operation in case of pipeline accident to confirm that the proper pressure to all nodes in the system is supplied. The pipe network simulator KYPIPE has been consecutively executed by trial and error fashion for each pipeline in the system. The results from KYPIPE were included in the knowledge base to supplement the knowledge of the field engineers.

Heart-base Tumors in Three Yorkshire Terriers (세 마리 개에서 발생한 심저부 종양 증례)

  • Three Yorkshire Terriers (12-year-old, 13-year-old, and 15-year-old castrated males) with respiratory distress, coughing and anorexia were the subjects of this report. In laboratory examinations, there were no remarkable findings. However, the thoracic radiographic findings included a large mass of soft tissue density in the cardiac base region, tracheal elevation, and aortic bulging in all three Yorkshire Terriers. There were no remarkable findings in the abdominal radiographs. In echocardiography, a homogeneous hyperechoic mass around the aorta and bicuspid valve regurgitation were found in all three dogs. There were no remarkable findings in abdominal ultrasonography. Computed tomographic findings showed a large well -defined heterogeneous mass in the cranial vena cava, which was dominant in the left side in all three Yorkshire Terriers. The mass sizes were about $3{\times}4cm$. In post-contrast scanning, contrast enhancement was evident. These cases were diagnosed as heart-base tumor. Treatments provided to the three dogs were based on symptomatic medical management of cardiac failure and tracheal collapse. Case 1 (12-year-old) survived for 3 months, case 2 (13-year-old) for 5 months, and case 3 (15-year-old) for 32 months after the diagnosis. Our results show that the clinical findings, thoracic radiography, echocardiography, computed tomography (CT) and symptomatic medical management in dogs suspected to have heart base tumor.

The diameter and base curve changes of soft contact Lens by protein deposition (단백질 침착에 의한 소프트콘택트렌즈의 직경 및 곡률반경 변화)

  • The aim of the study was to investigate the diameter and base curve changes of soft contact lens by protein deposition. Soft contact lenses were soaked in artificial tear or protein solution which had the same composition with tear for 2min, 10min, 30min, 1hr, 3hr, 6hr, 12hr, and 24hr. Diameter and base curve changes of soft contact lenses were examined by using the high speed camera(Fastcam ultima 1024). The longer the soaking time of soft contact lenses in the artificial tear, the diameter and base curve changes of soft contact lenses was more increased. In the case of soft contact lenses adsorbed only protein, the similar pattern was shown and the diameter and base curve were decreased. However, the influence of calcium ion was found to be less than that of protein. These results suggest that the tear protein causes the diameter and base curve decrease of soft contact lens, which might be related to the discomfort after soft contact lens wearing.

Long Term Follow-Up after Skull Base Reconstrucion (두개저부 종양 절제 및 재건 후 장기 추적관찰)

  • Skull base tumors have been determined inoperable because it is difficult to accurately diagnose the extent of the involvement and to approach and excise the tumor safely. However, recently, the advent of sophisticated diagnostic tools such as computed tomography and magnetic resonance imaging as well as the craniofacial and neurosurgical advanced techniques enabled an accurate determination of operative plans and safe approach for tumor excision. Resection of these tumors may sometimes result in massive and complex extirpation defects that are not amenable to local tissue closure. The purpose of this study is to analyze experiences of skull base reconstruction and to evaluate long term survival rate and complications. All cranial base reconstructions performed from July 1993 to September 2000 at Department of Plastic and Reconstructive Surgery of the Seoul National University Hospital were observed. The medical records were reviewed and analysed to assess the location of defects, reconstruction method, existence of the dural repair, history of preoperative radiotherapy and chemotherapy, complications and causes of death of the expired patients. There were 12 cases in region II, 8 cases in region I and 1 case in region III according to the Irish classification of skull base. Cranioplasty was performed in 4 patients with a bone graft and microvascular free tissue transfer was selected in 17 patients to reconstruct the cranial base and/or mid-facial defects. Among them, 11 cases were reconstructed with a rectus abdominis musculocutaneous free flap, 2 with a latissimus dorsi muscluocutaneous free flap, 1 with a fibular osteocutaneous free flap, 2 with a scapular osteocutaneous free flap, and 1 with a forearm fasciocutaneous free flap, respectively. During over 3 years follow-up, 5 patients were expired and 8 lesions were relapsed. Infection(3 cases) and partial flap loss(2 cases) were the main complications and multiorgan failure(3 cases) by cancer metastasis and sepsis(2 cases) were causes of death. Statistically 4-years survival rate was 68%. A large complex defects were successfully reconstructed by one-stage operation and, the functional results were also satisfactory with acceptable survival rates.

  • The clustered regularly interspaced short palindromic repeats (CRISPR) system is a family of DNA sequences originally discovered as a type of acquired immunity in prokaryotes such as bacteria and archaea. In many CRISPR systems, the functional ribonucleoproteins (RNPs) are composed of CRISPR protein and guide RNAs. They selectively bind and cleave specific target DNAs or RNAs, based on sequences complementary to the guide RNA. The specific targeted cleavage of the nucleic acids by CRISPR has been broadly utilized in genome editing methods. In the process of genome editing of eukaryotic cells, CRISPR-mediated DNA double-strand breaks (DSB) at specific genomic loci activate the endogenous DNA repair systems and induce mutations at the target sites with high efficiencies. Two of the major endogenous DNA repair machineries are non-homologous end joining (NHEJ) and homology-directed repair (HDR). In case of DSB, the two repair pathways operate in competition, resulting in several possible outcomes including deletions, insertions, and substitutions. Due to the inherent stochasticity of DSB-based genome editing methods, it was difficult to achieve defined single-base changes without unanticipated random mutation patterns. In order to overcome the heterogeneity in DSB-mediated genome editing, novel methods have been developed to incorporate precise single-base level changes without inducing DSB. The approaches utilized catalytically compromised CRISPR in conjunction with base-modifying enzymes and DNA polymerases, to accomplish highly efficient and precise genome editing of single and multiple bases. In this review, we introduce some of the advances in single-base level CRISPR genome editing methods and their applications.

Analytical Delay-Time Modeling of BICMOS Buffere (BICMOS 버퍼의 해석적 지연시간 모델링)

  • A model for BICMOS buffer switching operation is presented, including the influence of bipolar base transit time and collector-base capacitances. A closed-form solution for the propagation delay-time is obtained assuming low level injection and channel velocity limitation. For the high level injection case, the delay-times are numerically obtained using effective current gain. These results are compared with those by HSPICE simulation, which shows good agreement. It is noted that the collector-base capacitance strongly affects the delay-time. The effects of voltage scaling are also investigated, which shows the model can be applied for the reduced supply voltages.

Growth and Impact Analysis of Nonemployment Income as an Urban Economic Base -The case study of U.S. Arizona State- (도시경제기반으로서의 비고용소득 성장과 영향분석 - 미국 아리조나주의 경우 -)

  • Most studies on regional economic impact have utilized employment or employment income data. Recently, a few scholars have noticed the importance of nonemployment income sources in urban economies. Using decennial census data on Arizona towns from 1970 to 1990, this paper first examines the increasing importance of nonempolyment income sources in urban economies and the associations of nonemployment income sources with elderly population and metropolitan location. Then, this paper investigates the impact of nonemployment income on urban growth in the framework of economic base model. The regression results show that the impact of nonemployment income is significant in the increase of nonbasic income and becomes greater over time, and that the impact of transfer over time, and that the impact of transfer income on nonbasic income of transfer income on nonbasic income is stronger in smaller towns and the impact of investment income is stronger in larger towns.
