폐 aspergillosis의 외과적 치료 - 5예 보고 - (Surgical Treatment of Pulmonary Aspergillosis - 5 Cases -)

  • Pulmonary aspergillosis is a rare disease, most commonly presenting pre-existing cavitary disease. In Department of thoracic and cardiovascular surgery, Chonbuk National University, 5 patients have been recognized as having this disorder from 1988 to 1990, June. Of the 5 patients, three were female and two were male. Age ranged from 30 to 53 years, the average age was 38.4 years. The most common presenting symptoms were blood-tinged sputum, hemoptysis, coughing, and chest pain Pulmonary tuberculosis occupied 8-% of underlying pulmonary disease. The locations of lesion were right upper lobe in 3 cases, left upper lobe in 1, and right lower lobe in l. All of these patients were treated by surgical resection. The operative procedures were as follows: lobectomy, 3 cases; segmentectomy, 1 case; lobectomy and segmentectomy, 1 case. There was no death in early and late postoperative period. Empyema and dead space developed in two cases, respectively. The postoperative empyema was treated with open thoracostomy and the dead space was carefully observed. During follow-up, there was no recurrence.

  • Different lifestyles have an impact on useful metabolic functions, causing disorders. Different lipids are involved in the metabolic functions that play various vital roles in the body, such as structural components, storage of energy, in signaling, as biomarkers, in energy metabolism, and as hormones. Inter-related disorders are caused when these functions are affected, like diabetes, cancer, infections, and inflammatory and neurodegenerative conditions in humans. During the Covid-19 period, there has been a lot of focus on the effects of metabolic disorders all over the world. Hence, this review collectively reports on research concerning metabolic disorders, mainly cardiovascular and diabetes mellitus. In addition, drug research in lipid metabolism disorders have also been considered. This review explores lipids, metabolism, lipid metabolism disorders, and drugs used for these disorders.

방풍산(防風散)이 실험동물(實驗動物)의 심혈관계(心血管系)에 미치는 영향(影響) (Effects of Bangpoongsan on the Cardiovascular System in the Experimental Animals)

  • The present experiments were designed to investigate the effects of BangPoongSan on the cardiovascular system in the experimental Animals. And thus the change of blood pressure, auricular blood flow, artery contraction, death rate, platelet aggregation repression, plasma coagulation factor activity, plasma antithrombin activity, whole blood viscosity and plasma viscosity were studied. The result were summarized as the followings: 1. BangPoongSan dropped the blood pressure in the spontaneous hypertensive rat. 2. The drug increased the auricular blood flow in rabbit. 3. The drug relaxed the artery contraction by pretreated norepinephrine in white rat. 4. The drug inhibited the death rate of mouse which was led to thromboembolism by serotonin and collagen. 5. The drug inhibited the platelet aggregation in rat. 6. The drug prolonged the prothrombin time and activated partial thromboplastin time on the test of plasma coagulation factor activity in rat, but was not valuable. 7. The drug presented the antithrombin activity in rat. 8. The drug reduced the whole blood viscosity and plasma viscosity in rat, but the latter was not valuable. According to the results, Bangpoongsan increased the blood flow and dropped the blood pressure by dilatation of blood vessel smooth muscle. And the drug presented the antithrombin acivity, inhibited the platelet aggregation and reduced blood viscosity. Therefore these effects are assumed to improve the cardiovascular circulation disorder and prevent thrombosis.

  • Hyperimmunoglobulin E 증후군, 혹은 Job's 증후군은 비정상적으로 높은 Immunoglobulin E 수치와 더불어 면역기능의 이상으로 인해 반복적인 피부감염과 호흡기계 감염을 특정으로 하는 질환이다. Job's 증후군에서 나타나는 호흡기계 발현 양상은 폐렴, 기류, 기흉, 폐농양, 농흉 등 여러 가지 형태로 나타나며 각각에 대한 치료도 다양하다. 치료원칙은 감염에 관한 예방적 약물치료 및 감염균에 관한 항생제투여와 수술의 적응증이 될 때, 폐실질의 손실을 최소화하는 폐절제술을 시행하는 것이 중요하다.

Cardiometabolic Effects of Obstructive Sleep Apnea and Treatment Effects of Oral Appliance: An Updated Review for Dentists

  • Obstructive sleep apnea (OSA) is a relatively common, but greatly underdiagnosed sleep-related breathing disorder, characterized by recurrent collapse of the upper airway during sleep. OSA has been associated with a variety of cardiometabolic disease, such as hypertension, coronary artery disease, cardiac arrhythmia, cerebrovascular disease and metabolic dysfunction. Neurocognitive impairment, including excessive daytime sleepiness, increased risk of motor vehicle accidents, is also related to OSA. Sleep fragmentation and related arousals during sleep lead to intermittent hypoxia, sympathetic activation, oxidative stress, systemic inflammation and metabolic dysregulation which provide biological plausibility to this pathologic mechanism. Extensive studies demonstrated that OSA is a modifiable risk factor for the above mentioned diseases and oral appliances (OAs), although continuous positive air pressure (CPAP) is a first-line therapy of OSA, are not inferior to CPAP at least in mild OSA, and may be an alternative to CPAP in CPAP-intolerant subjects with OSA. The goal of this article is to provide a current knowledge of pathologic link between OSA and cardiovascular disease, focusing on intermittent hypoxia, sympathetic activation, oxidative stress and metabolic dysregulation. Then, previous epidemiologic studies will be reviewed to understand the causal relationship between OSA and cardiovascular disease. Finally, the effects of OAs will be updated via recent metaanalyses compared to CPAP.

민간 항공기 운항승무원의 건강문제 분석(2016~2018) (Analysis of Health Problems among Airline Pilots in Korea (2016~2018))

  • The medical fitness of pilots is part of the civil aviation safety scenery. This study aimed to analyze the health problems occurring among Korean commercial pilots. Three data sources were used to identify the health problems: 7,574 aviation medical examination data, 5,400 sick leaves and 39 cases of a decrease in medical fitness during flight duty period and layover of Korean commercial pilots who have been working for K airline from Jan. 1, 2016 to Dec. 31, 2018. This study was retrospectively analyzed using SPSS 22 statistical program. Waiver of medical certificates was an average 11.9% of total issuance for 3 years, with a denial of 0.1%. The leading cause of denial of medical certificates was predominantly of cardiovascular cause (55.6%). Mild respiratory and digestive disease accounted for 82% of total sick prevalence and 68% of total sick days. The psychiatric and cardiovascular disease were ranked high according to the number of days lost per case. The most common cause of decrease in medical fitness during flight duty period was acute abdominal pain (36.4%) and musculoskeletal disorder (40%) when staying abroad. Aeromedical emphasis on minimizing cardiovascular risk remains appropriate. Major pilot health problems identified in this study should be considered in establishing pilot health promotion programs for aviation safety.

식도 Achalasia 의 외과적 치료: Modified Heller`s method 에 의한 수술증예 (Surgical Treatment of Achalasia of the Esophagus: One Case Performed Modified Heller`s Operation)

  • Achalasia is a functional disorder of a short segment of the lower esophagus showing obstruction of the esophagogastric junction. Dysphagia, regurgitation, and weight loss are outstanding features and are some times more apparent than the disease entity in children though achalasia is rarely encountered in children. A 6 years male had complained of dysphagia, regurgitation and severe weight loss and was operated by modified Heller`s operation. Before he was operated, he could swallowed only milk but swallowed all regular diet after being operated.

  • Thoracic outlet syndrome is very rare thoracic surgical neurovascular disorder which is subject to compression by bones and muscular structures impinging upon the subclavian artery, vein k brachial plexus. The operative therapy is applied to remove the anatomical abnormal structures which leads to the compression to develop symptoms. We have operated one patient with thoracic outlet syndrome by excision of cervical ribs & first ribs with using transaxillary & posterior parascapular approaches in the thoracic surgical department, Yonsei University College of medicine, The post-operative courses has been uneventful for 7 months to now.

Achalasia 의 외과적 치료 (Result of modified Heller operation with gastric fundoplication in esophageal achalasia)

  • For recently 2 years, 3 cases of esophageal achalasia were surgically treated by modified Heller operation with gastric fundoplication. Esophageal achalasia is functional disorder of lower esophagus of which symptoms are dysphasia, regurgitation, and weight loss. Preoperative diagnosis was made by clinical manifestations and radiologic examination, esophagoscopy and confirmed at operative table. Operative procedures are variable, but modified Heller operation is common method to handicap reflux esophagitis and postoperative esophageal stricture. Now, our patients who were surgically treated were well alive without complaining of specific symptoms for followed period.

성문하 기관 협착증의 수술 치험 2예 (Surgical Treatment of Subglottic Stenosis -2 Cases-)

  • Subglottic stenosis is a disorder characterized by narrowing of the airway below the glottis. In children, the stenosis is usually due to scar formation secondary to prolonged airway intubation, rather than to external trauma. The location and extent of the stenosis are highly variable, consequently, corrective measures need to be selected to suit the individual problem. Conservative treatment is adequate for lesser degrees of stenosis but those with more severe scarring require external laryngeal surgery. We managed 2 children with subglottic stenosis due to prolonged intubation after open heart surgery who needed a resectional surgery of the stenotic upper airway. The preoperative evaluation and surgical technique for subglottic stenosis were reviewed.
