전인적 지지간호중재가 두경부암 수술환자의 절망감, 자아존중감 및 자아개념에 미치는 효과 (Effect of Supportive Nursing Intervention on Hopelessness, Self-Esteem, Self-Concept of Operative Patient with Head and Neck Cancer)

  • 목적: 본 연구는 전인적 지지간호중재가 수술한 두경부암 환자의 절망감, 자아존중감, 자아개념에 미치는 효과를 규명하기 위한 비동등성 대조군 전 후 시차 유사실험 설계연구이다. 방법: 연구대상은 P시에 소재한 K 대학교 부속 병원에서 두경부암으로 진단 후 수술을 받은 입원환자로써 대조군 20명, 실험군 20명으로 총 40명이었다. 자료수집 기간은 대조군이 1999년 12월 1일부터 2000년 2월 19일까지였고, 실험군은 2000년 2월 17일부터 4월 11일까지였다. 연구도구는 실험도구로서 전인적 지지간호중재 protocol은 연구자가 문헌고찰을 중심으로 전문가의 도움을 받아 제작한 것으로 신체적, 정서적, 정보적, 영적중재로 구성되어 있다. 측정도구는 절망감을 측정하기 위해 Beck(1974)의 절망감 측정도구를[15]이 번역한 척도를 사용하였다. 자아존중감에 대한 측정도구는 [29]가 개발하고 [41]가 번역한 도구를, 자아개념에 대한 측정도구는 [20] 등이 사용한 것을 [21]이 수정보완한 도구를 사용하였다. 자료처리는 SPSS/PC 9.0 프로그램을 이용하여 실수, 백분율, 평균, 평균평점, 표준편차, $x^2-test$, t-test로 분석하였다. 결과: 본 연구의 결과는 아래와 같았다. 1. 제 1가설 '전인적 지지간호중재를 받은 실험군은 받지 않은 대조군보다 절망감 정도가 낮을 것이다' 는 지지되었다(t=4.550, P=.000). 2. 제 2가설 '전인적 지지간호중재를 받은 실험군은 받지 않은 대조군보다 자아존중감 정도가 높을 것이다' 는 지지되었다(t=-6.442, P=.000). 3. 제 3가설 '전인적 지지간호중재를 받은 실험군은 받지 않은 대조군보다 자아개념 정도가 높을 것이다'는 지지되었다(t=-6.065, P=.000) 결론: 전인적 지지간호중재는 두경부암 수술환자의 절망감 감소와 자아존중감, 자아개념의 증진시키는데 효과가 있으므로, 두경부암 환자의 삶의 질을 위해 임상실무에 전인적 지지간호중재 프로토콜을 적용할 필요가 있으리라 사료된다.

악액질 완화를 위한 안전한 Nuclear Factor-kappa B 전사인자 제어 물질 발굴 (Safe Nuclear Factor-kappa B Inhibitor for Cachexia Management)

  • Purpose: Cachexia is a complex metabolic syndrome associated with wasting of skeletal muscle which contributes to nearly one-third of all cancer deaths. Cachexia lowers the frequency of response to chemotherapy and radiation and ultimately can impact survival as well as quality of life during treatment. NF-kappa B is one of the most important molecular mediators of cachexia. In this study, therefore, possible candidates for inhibitors of NF-kappa B were searched. Methods: Amino acids that regulate cellular redox potential by adjusting the level of NAD/NADH ratio, such as aspartate, pyruvate, and isocitrate were selected. Results: Pyruvate effectively inhibited luciferase activity in TNF-stimulated 293T cells transfect with an NF-kB dependent luciferase reporter vector. Pyruvate also showed protective effect on muscle atrophy of differentiated C2C12 myocyte induced by TNF/IFN. Conclusion: We might be able to develop the nutritional management strategy for cancer cachexia patients with pyruvate supplementation.

지압이 암환아의 오심 구토 정도와 체중변화에 미치는 효과 (Effect of Acupressure on Nausea-vomiting and Weight Change among Pediatric Cancer Patients Receiving Anti-cancer Chemotherapy)

  • Purpose: This study was to confirm the effect of acupressure on the emesis control and the weight change among pediatric cancer patients receiving anti-cancer chemotherapy. Method: Forty pediatric cancer patients, receiving the induction stage of chemotherapy with MTX and vincristine, were divided into control(n=20) and the intervention group(n=20). Both groups received regular anti-emesis medication, but the intervention group was added acupressure maneuver for 5 minutes on P6 point for 3 times a day for 5days: before chemotherapy, lunch and dinner by investigator during the hospitalization and by mother at home. The instruments for this study were Rhode's(1986) Index of nausea, vomiting and retching(INVR), Cas electric scale and pamphlet developed by researcher. Result: Significant differences in the degree of nausea and vomiting were observed between the control and the intervention group as measured by INVR(t=4.73; p=.01). Repeated measures ANOVA also shows that the group effect was significant(F=22.39, P=.01) as was the time effect(F=380.35, P=.01). The group by time interaction was also significant(F=5.27, P=.01). Acupressure maneuver was apparently effective in reducing the degree of chemotherapy-induced nausea and vomiting. There were also statistically significant weight loss noted in the control group than the intervention group(t=5.42, p=.01). Conclusion: Acupressure on P6 point shows an effective adjunct maneuver in reducing the degree of nausea and vomiting and conserving the weight in pediatric cancer patients. Therefore, it is proposed that acupressure should be applied as supportive nursing intervention strategies to relieve chemotherapy induced nausea and vomiting and to prevent weight loss in pediatric cancer patients.

Assessment of Jordanian Patient's Colorectal Cancer Awareness and Preferences towards CRC Screening: Are Jordanians Ready to Embrace CRC Screening?

  • Background: Colorectal cancer (CRC is increasingly becoming a major cause of cancer morbidity and mortality in Jordan. However the population's level of awareness about CRC, CRC screening test preferences and willingness to embrace screening are not known. The aim of this study was to assess the level of CRC awareness and screening preferences among Jordanian patients. Materials and Methods: A survey assessing the CRC knowledge levels was distributed among patients attending outpatient gastroenterology clinics in public hospitals throughout Jordan. A total of 800 surveys were distributed and of these 713 (89.1%) were returned. Results: Only 22% of the participants correctly judged CRC among the choices provided as the commonest cause of cancer related deaths. The majority of participants (68.3%) underestimated their risk for CRC. Only 26.8% correctly judged their life time risk while 5% overestimated their risk. Two thirds of participants (66%) were willing to pay 500 Jordanian Dinars (equivalent to 706 US$) in order to get a prompt colonoscopy if recommended by their physician, while 25.5% reported that they would rather wait for 6 months in order to get a free colonoscopy. Conclusions: Although the participants tended to underestimate their risk for CRC, they were mostly aware of CRC as a major cause of mortality and were willing to embrace the concept of CRC screening and bear the related financial costs. These findings about CRC awareness and propensity for screening provide a good foundation as the Jordanian health system moves forward with initiatives to promote CRC screening and prevention.

Breast Cancer Risk Assessment Using the Gail Model: a Turkish Study

  • Purpose: This study was conducted to determine risk of developing of breast cancer among Turkish women. Materials and Methods: Using a descriptive and cross-sectional approach, data were collected from 231 women. Breast cancer risk was calculated using the National Cancer Institute's on-line verson of called as the Breast Cancer Risk Assessment Tool or the Gail Risk Assesment Tool. Results: The average age of women was $45.0{\pm}8.06$ years. It was revealed that 6.1% of participants reported having first degree relatives who had had breast cancer, with only four women having more than one first-degree relative affected (1.7%). The mean five-year breast cancer risk for all women was $0.88{\pm}0.91%$, and 7.4% of women had a five-year breast cancer risk >1.66% in this study. Mean lifetime breast cancer risk up to age 90 years was $9.3{\pm}5.2%$. Conclusions: The breast cancer risk assessment tool can help in the clinical management of patient seeking advice concerning screening and prevention. Healthcare providers in Turkey can use this approach to estimate an individual's probability of developing breast cancer.

부인암환자의 항암치료에 대한 지식정도 및 교육요구도 (Knowledge and Learning Needs Related to Cancer Treatment in Gynecological Cancer Patients)

  • Purpose: This study was to investigate the knowledge and learning needs of chemotherapy in gynecological cancer patients. Method: The subjects consisted of 103 gynecological cancer patients receiving chemotherapy from April 2005 to August 2005. Data was collected using a questionnaire about knowledge and learning needs of chemotherapy. The data was analyzed by t-test, ANOVA, Scheffe test, and Pearson's correlation coefficient using SAS. Result: Average scores of knowledge and learning needs of general treatment and care were 2.74, and 3.30 respectively. Average score of knowledge and learning needs of chemotherapy were 2.54 and 3.23 respectively. Learning needs of general treatment and care and of chemotherapy were significantly different in relation to marital status, educational level, family support, the operation, and the amount of chemotherapy received. Items with the highest level of learning needs were the symptoms of recurring illness of general treatment, and minimizing side effects of chemotherapy. There were a negative correlation between knowledge and learning needs on general treatment and a positive correlation between knowledge and learning needs on chemothearpy but there were not significant statistically. Conclusion: The level of learning needs related to cancer treatment was high, whereas, that of knowledge was low. Therefore, when designing an educational program for gynecological cancer patients, understanding of learning needs is necessary. Also, consideration of a patient's characteristics, and a systematic and detailed educational program should be provided.

유방암 생존자의 삶의 질 영향요인 (The Influencing Factors on Quality of Life among Breast Cancer Survivors)

  • Purpose: This study was aimed to identify the influencing factors on the quality of life among breast cancer survivors. Methods: The subjects were 159 female patients who visited out-patient department (OPD) after the mass removal surgery for breast cancer and had completed adjuvant treatments such as chemotherapy, radiation therapy at a university hospital and a general hospital. Data collection was conducted using the Ferrell QOL scale, the Mishel uncertainty scale, the Fitts & Osgoods body image scale revised by Jeon & Kim. the Rosenberg self-esteem scale, and the Kang family support scale. Results: The level of QOL in the participants was in the middle. There were a significant correlation between QOL, uncertainty, self-esteem, and family support. There were significant differences in QOL with the perceived health condition and the best support person. In a regression analysis, the most powerful predictor of QOL was body image (21.7%). Altogether uncertainty and perceived health condition explained 28.6% of the variance of QOL of the participants. Conclusion: Body image, uncertainty, and perceived health condition were important predictors of QOL. These results demonstrated the need for developing interventions to improve QOL of breast cancer survivors.

항암화학요법을 받는 위암 환자의 우울과 영향요인 (Influencing Factors on Depression in Stomach Cancer Patients Receiving Chemotherapy)

  • Purpose: The purpose of this study was to identify factors that influence depression in stomach cancer patients receiving chemotherapy at outpatient clinic. Methods: A cross-sectional design was used. Ninety two subjects diagnosed with stomach cancer and receiving chemotherapy agreed to participated in the study. Subjects completed a survey which included questions about depression, symptom experience, social support and self-efficacy. Results: The level of depression was $13.45{\pm}6.92$. Reported depression differed according to patient's perceived health status. Depression was significantly correlated with symptom experience, social support and self-efficacy. The most important factor related to depression was symptom experience, followed by social support and self-efficacy. These three factors could explain about 47% of depression in stomach cancer patients receiving chemotherapy. Conclusion: Although the level of depression in stomach cancer patients receiving chemotherapy was relatively low, 34.8% of patients reported to have depression. It is important to apply nursing intervention including screening of depression level in stomach cancer patients receiving chemotherapy that focused on symptom experience and to consider about social support and self-efficacy at outpatient clinic.

암 환자의 건강정보탐색 및 관련요인 조사연구 (Health Information Seeking Behaviors Among Persons with Cancer Disease)

  • Objectives: The purpose of this study was to examine health information seeking behavior of people with cancer in terms of information seeking or avoiding behavior and the differences of the two groups in demographic and disease variables. Methods: Data were collected from October 21 to December 5 in 2013 from out-patient clinics of a university affiliated hospital in Suwon, Korea. One hundred (100) patients with cancer were participated in this study. Health Information National Trends Survey Questionnaires (National Cancer Institute, 2007) was used. Results: Three quarters (n=77) of the participants were in health information seeking group while 23% were in health information avoiding group. Participants who were female, higher education were more likely to seek health information than their counter parts. Among participants, a considerable number of cancer patients intentionally avoided information due to either the lack of accessibility of information source or the lack of credibility of information from the sources. Conclusion: Significant differences in gender and educational level were found in the two groups. Different approaches for both groups were necessary to increase information seeking behavior. And barriers of the information avoiding group should be considered in designing interventions to fill the gap between seekers and avoiders.

암환자 가족의 죽음 태도 유형에 관한 연구 (A Classification of Death Orientation of Cancer Patient's Family Members : A Q-Methodological Approach)

  • This study was designed to identify, describe and classify orientations of cancer patient's family members to death and to identify factors related to their attitudes on death. Death to the male is understood as a comprehensive system and believed to be highly subjective experience. Therefore attitude on death is affected by personalities. As an attempt to measure the subjective meaning toward death, the unstructured Q-methodology was used. Korean Death Orientation Questonaire prepared by Kim was used. Item-reliability and Sorting-reliability were tested. Forty five cancer patients' family members hospitalized in one university medical center in Seoul were sampled. Sorting the 65 Q-itmes according to the level of personal agreement ; A forced normal distribution into the 11 levels, were carried out by the 45 P-samples. The demographic data and information related to death orientation of the P-sample was collected through face to face in depth interviews. Data was gathered from August 30 till September 22, 1995. The Z-scores of the Q-items were computed and principal component factor analysis was carried out by PC-QUANL Program. Three unique types of the death orientation were identified and labeled. Type I consists of twenty P-samples. Life and death was accepted as people's destiny, They firmly believed the existence of life after life. They kept aloof from death and their concern was facing the and of the life with dignity, They were in favor of organ donation. Type II consists of Nine P-Samples. They considered that death was the end of everything and did not believed the life after life. They were very concerned about the present life. Type III consists of Sixteen P-samples. They regarded the death as a natural phenomena. And they considered that the man is just a traveller and is bound to head for the next life which is believed to be free of agony, pain or darkness. They neither feared death nor its process. Their conserns were on the activities to prepare themselves for the eternal-life after death. Thus, it was concluded that there were three distinctiven type of attitudes on death among cancer patient family members, and their death attitudes were affected by demographic and socio-cultural factors such as sex, education, and religion.
