A Case Report on the CVA Patient with the Sense Disorder of Equilibrium (평형감각이상을 호소하는 태음인 CVA 후유증환자 1례에 대한 사암침법의 치료 증례 보고)

  • CVA is a kind of cerebrovascular disease which has a few local functional lesions of brain tissue, and it is generally caused by cerebral infarction, cerebral hemorrhage, and so on. It is also known as a stroke of paralysis, which leaves a lot of sequelae in a patient such as lesion of movement, perceiving, memory, sense, emotion, etc. A CVA patient which has a sequela, the sense disorder of equilibrium, took the Saam Acupuncture Therapy and Dr. Dong's Acupuncture Therapy, and then he was cured.

Quality of life and the related Factors in Cerebrovascular Surgery Patients (뇌혈관 수술 환자의 삶의 질과 관련요인)

  • Purpose: The purpose of this study was to investigate the quality of life and related factors of CVA surgery patients. Method: The subjects were 64 CVA surgery patients who had discharged and received follow up care at the OPD. Data were collected using William's SS-QOL instrument and IADL. And Data were analyzed with SPSS WIN program in which frequency, percentage, t-test, Pearson's correlation and stepwise regression were used to examine the quality of life and related factors. Result: General characteristics related to QOL were marital status and economical status. Depression was the most important factor with influence on QOL in CVA disease patients after surgery. There were further positive correlation between patient's IADL and QOL and between patient's social support and QOL. Meanwhile the patient's depression level was negatively correlated with QOL. Conclusion: Depression was the most important factor with influence on QOL in CVA patients after surgery. Therefore active nursing intervention to decrease depression and to improve patient's physical functional status in needed. And the patient's family should be included in all nursing intervention and patient education so that the patient's quality of life is prompted by the maintenance of optimal wellbeing.

A Study on the Stress of Family-Caregivers and Level of Daily Living Performance with Patients of Cerebra Vascular Accident(CVA) (뇌졸중환자의 일상생활 수행수준과 가족원의 스트레스)

  • The purpose of this study was to explore the degree of stress in caregivers caring for CVA patients and the level of daily living performance of CVA patients. The subjects for the study were caregivers of 112 CVA patients who enter a hospital or out-patient-department (OPD) at two Oriental medical hospital in Jeonbuk province. The survey instruments used in this study were Kang's ADL check list for daily living performance of patients and Choi's 4 sore scale for stress of caregivers. The survey was conducted from July 4th to August 30th in 1999. The survey results were analyzed with the Statistical Package for Social Science(SPSS) program and can be summarized as follows: 1. The level of daily living performance for the CVA patients was: 1) complete dependence (M=14.9, 13.1%), 2) complete independence (M=23.6, 20.9%), 3) incomplete independence (M=23.9, 21.0%), 4) incomplete dependence (M=26.6, 25%), 5) dependence and independence (M=23.0, 20.0%). The items for with there was a high level daily living performance were: 1) drinking (M=3.62), 2) eating(M=3.25). 3) position returning (M=3.18) : and the items for which there was a low level of daily living performance were: 1) ascending and descending stairs (M=2.08), 2) walking (M=2.47), 3) dressing and undressing trousers (M=2.55). 2. Degree of caregiver stress was: Mean=2.39 at 40 score. The items for which was a high level caregiver stress were: 1) medical fee (M=3.25), 2) being handicapped or recurrence (M=3.02) : and the items for which there was a low level of caregiver stress were: 1) discontinuity of patient's treatment (M = 1.98). 2) change of home atmosphere caused by patient's disease (M = 1.98), 3) desire of patient's knowing about disease (M= 1.99). 3. There was statistically significant difference in the degree of caregiver stress according to the following caregiver's demographic characteristics: education level (F=3.52, P=0.03). change of caregiver (F=5.41. P=0.02). 4. There was a statistically signifiant difference in the level of daily living performance according to the CVA patients demographic characteristics: patient's paralytic status (F=4.48, P=0.01), duration of disease (t=2.76, P=0.03). 5. There was significant difference in degree of caregiver stress according to the CVA patient's demographic characteristics: CVA status (F=4.75, P=0.01). 6. There was statistically significant difference in the degree of caregiver stress according to the level of daily living performance in CVA patients(r=-0.482, P<0.00).

Comparison in Quality of Life, Relationship with Patients and Family Support, between Caregiver of Patients with Cerebro-Vascular Accident(CVA) in Western and Oriental Medicine (양.한방 뇌졸중 환자 주간호자의 삶의 질, 발병 전 환자와의 관계 및 가족지지 비교)

  • Purpose: This study was a descriptive study to compare of quality of life, relationship with CVA patients, and family support between caregiver of patient with CVA in western and oriental medicine. Method: Total 270 caregivers for CVA patients were selected from western medicine department (114) and oriental medicine department (156) at K hospital in Seoul. The instrument tools utilized in this study were quality of life, Family support, Caregiver's relationship with patients, daily activity. Collected data were analyzed by $X^2$-test, t-test using SPSS Windows 12.0 Program. Result: The result are as follows: 1) There was no significant difference in quality of life between caregivers of patients with CVA in western and oriental medicine(t=.26, p=.792). 2) There was significant difference in relationship with patients(t=2.64, p=.009) and family support(t=2.12, p=.035) before onset of disease between caregivers of patients with CVA in western and oriental medicine. Conclusion: As a result, caregivers of oriental medicine with CVA patients showed closer relationship with patients before the onset of disease and higher family support compare to caregivers of western medicine with CVA patients.

  • The purpose of this study suggest that Park Golf for rehabilitation training of the patients with hemiplegia. The increase of the aged caused various problems. Among the problems, chronic illnesses the old people suffer from charge their family with mental, physical, social and economic burden as well as the person. Especially, the more old people are the more risks and attacks of cerebral vascular accident(CVA) exist. Therefore, the increase of the aged means the increase of cerebral vascular accident(CVA) patients. The cerebral vascular accident(CVA) patient get into difficulties to secure their livelihood due to troubles and disoders, so medical care facilities and rehabilitation training are really necessary to help them return to their normal life with mental, physical and social capability. The rehabilitation of patient with hemiplegia is ongoing process. It may possibly accomplish throughout the whole life because it takes a lot of time and effort. In addition, an ultimate aim of the rehabilitation is returning and adaptation to society. In this way, it is hardly to expect a satisfied results of the rehabilitation without motive. Therefore, patient with hemiplegia rehabilitation motive will is the most important. If interesting of park golf the patient with hemiplegia can be absorbed in easily are applied to the rehabilitation training, the patient are will to participate in the training without troubling.

A Study on the Care Needs of Family-Caregivers and the Level of Self Care for Patients of Cerebral Vascular Accident(CVA) (뇌졸중환자의 자가간호 수준과 가족구성원의 간호요구)

  • The purpose of this study was to explore the care needs of family-caregivers caring for patients with a CVA and the level of self care of the patients. The subjects for the study were 112 patients with a CVA and their caregivers. These patients were seen in a hospital or out-patient-department(OPD) at two oriental medical hospital in Jeonbuk province. The survey instruments used in this study were Kang's ADL checklist for self care of patients and Kim's Likert-style checklist for care needs of family-caregivers to patient with CVA. The survey was conducted from July 4 to August 30, 1999. Internal validity by calculation of Cronbach's alpha was 0.95, which was regarded as high. The survey results were analyzed using the SPSS program, with percentages, means, t-test, ANOVA and Pearson's correlation coefficients. The results of this study are as follows : 1. The level of self care for patients with a CVA was : 1) complete dependence(M=14.9, 13.1%), 2) complete independence(M=23.6, 20.9%), 3) incomplete independence(M=23.9, 21.0%), 4) incomplete dependence(M=26.6, 25.0%), 5) dependence and independence(M=23.0, 20.0%). The items for which there was a high level of self care were : 1) drinking(M=3.62), 2) eating (M=3.25), 3) position returning(M=3.18) : and the items for which there was a low level of self care were : 1) ascending and descending stairs(M=2.08), 2) walking(M=2.47). 3) putting on and taking off trousers(M=2.55). 2. The mean score of the sum of the care needs of the family-caregivers was : 1) need for immediate care and help: 2) need of the way to communicate with patient: 3) need for education and assistance related to physical functional level: 4) need to be informed about the disease, treatment and care: 5) need for social support and consultation: 6) need for appreciation: 7) need for management of nursing problems related to immobility. The highest meed factor was the need for immediate care and help(M=3.47): and lowest need factor was the need for management of nursing problems related to immobility(M=2.80). 3. There were significant differences between the level of care need and general characteristic of the caregivers, there were family-caregivers age(P=0.001), marital status (P=0.276), occupation (P=0.006), monthly income(P=0.000), Patient's relationship to caregivers(P=0.004) and health(P=0.000). 4. There were significant differences between the level of self care and general characteristic of the patients, there were patient paralytic condition(P=0.01), blood pressure(P=0.01), and length of suffering(P=0.03). 5 There were significant differences between the level of care need and the general characteristic factors, which were CVA patient's blood pressure (P=0.05), problem of medical fee (P=0.05). 6. There was significantly correlation with the family-caregivers care need and the level of self care in the CVA patient(r=0.300, P=0.000). As a result, need to promote the level of self care in patients and to meet the care need of family-caregivers for more efficient nursing of CVA patients, is emphasized. Therefore more study is needed on an efficient way to provide rehabilitation and quality nursing interventions for family-caregivers and patients with CVA.

Three Cases Report of the Sequela of CVA Threated with Seongnoe-gaegyubeop (성뇌개규법(醒腦開竅法)을 이용한 뇌졸중으로 인한 운동장애 환자 치험 3례)

  • This case study was reported to show the effects of Seongnoe-gaegyubeop acupuncture treatment. The methods were applied to patients who had the sequela of cerebral vescular accident(CVA) and had no progression for 3~6 months after the onset of CVA. The patients had no progression in spite of receiving medical treatment for 3~6 months after the onset of CVA. The patients were treated with Seongnoe-gaegyubeop acupuncture treatment. The patient's standing, sitting state and activities of daily life like dressing, feeding etc were assessed by Berg balance scale, Barthel Index and Manual Muscle Testing. After 3 months of this treatment, the standing, sitting state and activities of daily life were improved. I think that acupuncture treatment of Seongnoe-gaegyubeop is effective treatment method to treat the patient who caused the sequela of CVA.

Analysis of Services and Cost in CVA Patients by Severity in Hospital-based Home Health Care (병원중심 가정간호 뇌혈관질환자의 중증도별 서비스 특성 및 비용 분석)

  • Purpose: The purpose of this study was to analyze the differences in terms of services and cost between CVA without typical diseases (Group I), and CVA with typical diseases (Group II), in their Hospital-based home health care. Method: The subjects of this study were 308 CVA patients who used home care nursing during the second phase demonstration project of their hospital-based home health care. Results: The results of the study was as follows 1. Group II had more home visit (15.3/12.7) (p>0.05), and cases of death when home care (16.8/11.4) (p<0.05). 2. Group II needed more services than Group I such as bladder irrigation, skin care, bed sore care, glycerin enema, finger enema, lung care, urine sugar test, monitoring and surveillance of fluid infusion and R.O.M exercise (p<0.05). 3. The variables that showed statistical significance in the regression analysis were family style, OPD visit, level of consciousness, patient's state on termination of home care, and some extend of home health care services (R2=0.373, 0.205). Conclusion: Home nursing care needs to be planned by severity in Hospital-based home health care for CVA patients.

A Study of Caregiver Stress as related to the Level of Self-care in Patients Who had a Cerebro Vascular Accident(CVA) (뇌졸중환자의 자가간호 수준에 따른 보호자의 스트레스)

  • The purpose of this study was to explore the degree of stress in caregivers caring for patients who had had a cerebro vascular accident as the stress is related to the self- care ability of the patient. The subjects for the study were caregivers of 111 CVA patients, hospitalized at two University affiliated hospitals and two general hospitals in Daejon. The survey instruments used in the study were Kang’s ADL Check List and a modified form of Lee’s Stress Inventory. The survey was conducted from July 16th to August 30th, 1992. The survey results were analyzed using the Statisitical Package for Social Sciences (SPSS) and can be sumerized as follows: 1. The level of self- care for the CVA patients was : 1) complete dependence (M=34.7, 31.2%), 2) complete independence (M=14.8. 13.3%), 3) in-complete independence ( M=17.5, 15.8% ), 4) in-complete dependence (M=14.8, 13.3%) and 5) dependence and independence (M=14.1, 12.7%). The items for which there was a high level of self-care were 1) drinking (M=3.640), 2) returning (M=2.351) and 3) eating (M=2.351) : and the items for which there was a low level of self -care were : 1) ascending and descending stairs (M=2.351), 2) dressing and undressing trousers (M=2.514) and 3) dressing and undressing jacket (M :2.532). 2. There was a statistically difference between the paralytic status and the level of self- care accord-ing to their demographic characteristics ( F=24. 7056, p(.001). 3. There was no significant difference in the degree of caregiver stress according to patient's demo-graphic characteristics. 4. There was a statistically significant difference in the degree of caregiver stress according to the following demographic characteristics : age (F=7.4189, p(.001), education level (F=5.8336, P(.01), family structure (t=2.10, p(.05) and their relationship with the patient (F=6.5099, P〈.01). 5. There was no significant difference in the degree of caregiver stress according to the level of patient self - care.

Clinical Study about the CVA Patients with Urinary Tract Infection (CVA 환자의 UTI에 대한 임상적 관찰)

  • This study was designed to investigate about the efficiency of herb treatment at the patients with urinary tract infection. The Patient group was consisted of 33 CVA patients who were diagnosed as abnormal on urinalysis at the Oriental Medical hospital of Dong-eui University from July in 2006 to April in 2007. And we divided the patient group to 3 of small groups(Bojungikitang, Kumokpaljungsan, and the others herb). Then we treated the patients with each herbs and checked urinary nitrate, leukocyte, bacteria, WBC after 3 days, 7 days. The results are as follows: Each herbs improved the results of urinalysis inspection. The efficiency of each herbs about UTI begins to be indicated after 3days. Each herbs applied to UTI is more effective after 7 days than after 3 days. As the results, the herb treatments improve effectively the result of urinalysis inspection when precsribing to UTI patients more 7 days.
