병원단위의 임상진료지침 개발과정 (Development of Clinical Practice Guidelines in a Hospital)

  • Background : With increased concerns about variation among physician's practice pattern and their impact on the quality of care, clinical practice guidelines have been developed by many different organizations, with differing aims and incentives. From the same point of view, there is growing interest in the development of clinical practice guidelines in Korea, but with only a few examples. As a result, there is not much exploration on the incentive and barrier to develop guidelines as well as description on the development process. The purposes of this study are to describe the process of the four different clinical practice guidelines in a hospital setting, and to identify incentives and barriers in the development of guidelines. Methods : For this research, a clinical practice guideline development committee and four clinical practice guideline development teams were organized in a university hospital which has more than 1,200 bead. Twenty eight doctors, three nurses, and one technician participated as members of development teams for eight months. Four to six meetings were held, and three to seven departments in the hospital were involved. Results : The topics which developed into clinical practice guidelines were cardiopulmonary resuscitation(CPR), blood transfusion, anticoagulation, and angiography. The main goals set by teams were education(CPR, angiography), risk management(blood transfusion), and to enhance quality of care(anticoagulation). Among four teams, only in the team for anticoagulation guideline medical record review and pilot-testing were performed. Also literature review was not carried out systematically. However, all the guidelines were developed by multidisciplinary be used as standard protocols in the practice. Conclusion : Experience and skill in developing process has to be improved to have a more valid and useful practice guideline. In particular, literature review and problem identification by examining medical record should be emphasized. Also further studies on the clinical outcomes of the guidelines application and changes in physicians' behaviors would be required.

일부지역 초등학생들의 심폐소생술 지식 및 태도가 자신감에 미치는 영향 (Influence of Knowledge and Attitude toward Cardiopulmonary Resuscitation in Elementary school Students of Some Regions upon Self-confidence)

  • 본 연구는 일부지역 초등학생들의 심폐소생술 지식 및 태도가 자신감에 미치는 영향에 대하여 알아보고자 시도 하였으며, 심정지 환자에게 효과적인 심폐소생술 수행과 자신감에 도움을 주고자 하는데 그 목적이 있다. 2009년 6월 25일부터 7월 15일까지 J도 소재 J시 K시 4개교에 재학하는 초등학생 381명에 대하여 실시하였다. 통계처리는 SPSS WIN 12.0 Version 프로그램을 사용하였으며, 통계적 유의성은 p<.05로 설정하였다. 심폐소생술 특성의 변수들과 지식, 태도, 자신감의 상관관계는 정보습득유무(r=.174, p<.01), 심폐소생술 경험유무(r=.143, p<.01), 심폐소생술 교육유무(r=.164, p<.01), 교육필요성(r=.184, p<.01), 지식(r=.342, p<.01), 태도(r=.736, p<.01)가 자신감에 상관관계를 보였다. 심폐소생술 지식 및 태도가 자신감에 미치는 영향은 태도($\beta=.703$, t=17.884, p<.01)가 자신감에 유의한 영향을 주는 것으로 나타났다. 초등학생들이 응급상황 시 효과적으로 대처 할 수 있도록 심폐소생술 교육 프로그램 마련과 효과적인 인력 배치와 제도적 장치가 마련되어야 할 것으로 여겨진다.

종합병원 경비업무의 발전방안에 관한 연구 (A Study on development of general hospital security service)

  • 본 연구는 종합병원 경비업무현장에서 병원 시설의 경비업무와 의료진, 환자 및 보호자 등의 내방객의 안전에까지 종합적인 경비업무를 수행 하고 있는 종합병원 경비업무 경비원들이 현재 인지하고 있는 문제점들을 바탕으로 종합병원 경비업무의 발전방안을 모색하는데 목적이 있다. 이러한 연구의 목적을 달성하기 위해 종합병원 경비업무에 6년 이상 종사하고 있는 현장전문가 9명에게 면담조사를 실시하여 자료를 분석하였다. 종합병원 경비업무를 수행하고 있는 종합병원 경비업무경비원들은 종합병원 경비업무의 발전방안을 다음과 같이 제시하였다. 첫째, 종합병원 경비업무 경비원들은 업무현장의 성격에 알맞은 신임교육 프로그램을 이수하여야 한다. 둘째, 종합병원 경비업무 경비원 직무교육도 실질적인 상황에 맞는 교육내용으로 프로그램의 개선이 필요하다. 셋째, 종합병원 경비업무 경비원들에게는 추가적으로 CS교육과 CPR 및 제세동기 사용법등의 응급상황 대처요령과 전염성 질병대처교육 등이 진행되어져야 한다. 넷째, 종합병원에서의 난동 및 폭력 행사자에 대한 대응범위가 구체적으로 명시되어져야 할 것이다. 다섯째, 경비협회에서는 종합병원경비업무의 발전을 위하여 경비원들을 위한 적극적인 프로그램 개발 및 다양한 노력이 필요하다. 여섯째, 종합병원 경비업무에 대한 인식 전환이 필요하며, 하도급에 의한 계약방식보다는 직접고용의 형태가 필요시 되어 진다.

대학생의 개인성향 및 심리적 변인이 심폐소생술 시 가슴압박깊이에 미치는 영향 (Effects of Individual Tendencies and Psychological Variables of College Students on the Depth of Chest Compression During Cardiopulmonary Resuscitation)

  • 본 연구는 대학생의 개인성향과 심리적 변인이 심폐소생술 시 가슴압박깊이에 미치는 영향을 확인하고자 수행하였다. 이를 위해 대학생 127명을 대상으로 개인성향, 피로감, 수행자신감, 수행불안을 조사한 후 심폐소생술 시 가슴압박깊이를 측정하였다. 가슴압박깊이의 영향 요인을 파악하기 위해 개인성향과 심리적 변인을 독립변수로 하고, 가슴압박깊이를 종속변수로 하여 다중선형회귀분석을 실시하였다. 또한 변인 간의 상관성을 확인하기 위해 Pearson의 상관분석을 실시하였다. 분석결과, 수행자신감이 높을수록 가슴압박깊이가 깊고, 수행불안이 높을수록 가슴압박깊이는 낮아졌으며(p<0.05), 가슴압박깊이는 개인성향, 수행자신감과 양의 상관성을 보인 반면, 피로감 및 수행불안과는 음의 상관성을 보였다(p<0.01, p<0.05). 이러한 결과를 토대로 올바른 가슴압박깊이를 수행하기 위해 수행자신감을 높이고 수행불안을 낮출 필요가 있으며 이를 위한 프로그램의 개발, 교육, 연구 등 다각적 노력이 필요하다.

교원 대상 학교안전관리 역량 강화 프로그램 개발을 위한 교육 요구분석 (A Needs Assessment for Teachers' School Safety Management Competency Development Program)

  • Purpose: Recently, various accidents and incidents occurred in our school environment, making it necessary to enhance teachers' competency in school safety management. To this end, this study was conducted to analyze the needs of teachers related to school safety management competency development. Methods: For this purpose, the existing teacher's task analysis related to school safety management was revised and surveys were conducted to measure teachers' present level, expectative level and importance of the items based on the task analysis. To analyze the data, Borich Needs Assessment and The Locus for Focus Model were used. Results: In the t-test between the teachers' present level and expectative level about school safety management, all 39 tasks were related (p<.001). The priorities in developing teachers' competency in school safety management were found to be the 6 tasks listed below: 'To deal with each type of accident,' 'To implement CPR and defibrillator,' 'To use fire extinguishers and fire hydrants,' 'To use descending life lines,' 'To give first aid,' and 'To cope with elevator accidents.' Conclusion: By utilizing the results of the teacher's competency and need assesment as components, teachers' training programs can be made more applicable to the school setting. In addition, more experience-based programs should be considered when designing teachers' training programs in order to apply the priority found from the teacher's need assessment.

특성화고등학교 현장실습 사고와 건강문제 예방을 위한 교육자료 개발 및 적용 (Development of Educational Materials for Prevention of Accidents in Vocational High Schools)

  • Purpose: The purpose of the study was to develop educational materials on field practice safety based on existing field practice data and accident cases regarding vocational high schools and apply them to identify their impact. Methods: The existing data, accumulated since 2010, on field practices of vocational high schools were analyzed, and educational materials regarding field practices were developed by six experts. 195 students in three vocational high schools were surveyed before and after being taught with the materials. The survey asked about their knowledge of industrial safety and health and attitudes toward industrial safety and health. Results: As a result of the study, harmful environments for field practices of vocational high schools were physical, chemical, ergonomic, and emotional labor, and the consequences were accidents, death by overwork, musculoskeletal diseases, etc. The materials covered students' rights and how to respond to workplace accidents in the 1st round, how to organize a workshop in the 2nd round, workplace safety and health signs in the 3rd round, prevention of musculoskeletal diseases in the 4th round, management of physical risk factors in 5th round, management of hazardous chemicals in 6th round, wearing and managing protective equipment in 7th round, first aid depending on the situation in the 8th round, CPR and defibrillator in the 9th round, sexual harassment in the 10th round, and prevention of sexual violence in the 11th round. After completing the education, their knowledge of industrial safety and health increased significantly from 6.52 points to 7.01 points. Conclusion: The results of this study suggest: first, to statistically organize the data on accidents that have occurred during field practices of vocational high schools; second, to develop a systematic curriculum for high school 1st to 3rd graders on accidents that may happen during field practices of vocational high schools.

스마트폰 기반 응급상황 대처 교육에 대한 요구도 조사 - 초등학생 자녀를 둔 부모를 대상으로 (Survey on the Educational Needs for Smartphone Based Emergency Situations - Targeting Parents with Elementary School Children)

  • 본 연구의 목적은 응급 상황에 대한 아동 부모의 스마트폰을 사용한 교육적 요구도를 조사하기 위한 것이다. 본 연구는 서술적 단면 조사 연구방법을 통해 설문지를 사용하여 자료를 수집하였으며 총 200명의 6 학년 이하 초등학생 자녀를 둔 부모를 대상으로 조사되었다. 자료 수집은 2017 년 10 월 17 일부터 2017 년 10 월 27 일까지 수집되었다. 연구 결과 67.5%의 부모가 아동의 응급 사고로 인해 응급실을 방문했다고 응답하였으며, 이중에서 부모의 74%는 어린이의 긴급 상황에 대처하는 방법을 몰랐다고 응답했다. 교육을 받지 못한 대부분의 부모는 응급 처치에 관한 정보를 얻기 위해 스마트 폰을 사용하고 있다고 응답했다. 부모의 아동 응급상황 대처 교육에 대한 요구도는 4점 만점에 평균 3.40점 (±.42)으로 높은 편이었다. 전체 항목 중에서 교육 요구도는 "이물질기도 폐쇄 및 심폐 소생술 관리" 항목의 평균이 가장 높았다 (3.64점). 본 연구 결과는 아동의 응급 처치에 대한 부모의 교육 요구를 이해하기 위한 기본 정보를 제공하며, 스마트 폰이 부모에게 효과적인 교육 방법 일 수 있음을 보여준다.

응급구조학과 대학생들의 안락사에 대한 생명의료윤리 의식 연구 (A Study on the Biomedical Ethics Consciousness for Euthanasia of the Paramedic Students)

  • Objective : This study is designed to present the direction and the necessity of education on the biomedical ethics the establish desirable ethical view of paramedic students by understanding on the consciousness of students of department of emergency medical technology on biomedical ethics on euthanasia. Method : Data were collected from on 335 students of department of emergency medical technology in 4-year-college located in districts of Chungcheong-do, from March 4 to April 22, 2010. The questionnaire consisted of total 29 items, i.e., 11 items of general characteristics, 11 items of characteristics related to biomedical ethics and 7 items of consciousness on euthanasia. Result : In general characteristics, the first grade showed the most dense distribution of 37.3%, the second grade was 20.0%, the third grade was 20.9% and the fourth grade was 21.8%. In the consciousness of biomedical ethics on euthanasia, the third and the fourth grades showed a higher ratio(${\chi}^2$ = 136.327, p= .000), in comparison with the first and the second grades, the higher year they were, the higher degree of consciousness they had. The consciousness on each item of euthanasia showed significant differences in grade (F = 8.203, p= .000), experience of clinical practice or none (t = 3.731, p= .000), experience of biomedical ethics education or none (t = 1.997, p = .047) and degree of consciousness on DNR (F = 8.240, p = .000) regarding 'CPR shall not be required in any cases if a patient wants DNR', in experience of biomedical ethics education or none (t = 2.452, p= .015) and degree of consciousness on DNR (F = 2.725, p = .044) regarding if a patient refuses all treatments, the patient's opinion shall be respected', and in degree of consciousness on DNR (F = 2.858, p = . 037) regarding 'after determination of DNR, if the family wants a positive treatment, it shall follow the family's request'. Conclusion : It seemed that the differences in consciousness according to grade and experience of clinical practice were influenced by experiences to treat patients in personally. Moreover, it showed that the higher grade they are, the more experience of education they have, the experiences of education had influences on the consciousness. Development of educational programs is required in order to supply accurate knowledge on biomedical ethics for euthanasia to lower-grade students the to let them establish ethical views properly, and in order to correct wrong knowledge and to establish ethical views while supplying knowledge on biomedical ethics for euthanasia to higher-grade students.

DNR에 대한 간호사의 인식 및 태도조사 (Nurses' Understanding and Attitude on DNR)

  • The study was intended to identify the nurses' experiences, understanding, and attitudes on DNR. Also, the study was to provide the data base for a standard of DNR decision-making and practice. The sample consisted of 347 nurses in eight general hospitals. The data were collected between August 1 and August 31, 2000. The data were analyzed using descriptive statistics and $x^2-test$. The results of the study were as follows : 1. Regarding DNR-related experience, 74.6 percent of the participants experienced DNR situations. Eleven percent of the participants received DNR education. DNR was most frequently (81.5%) requested by family members and relatives of patients. The decision-making on DNR was most frequently (76.8%) made by agreement between family members and medical staff. The DNR order was recorded at 81.9 percent on charts. Problems after DNR order were negligence in treatment and nursing care (30.6%) and guilty feelings due to doing the best (22.1%). CPR (cardiopulmonary resuscitation) was performed about 49.8 percent of DNR cases. 2. Regarding understanding and attitude on DNR, most of the participants (93.1%) thought DNR was necessary. The major reasons for the necessity of DNR were impossible recovery (44.4%) and death with dignity (41.1%). The decision-making on DNR was most frequently made by patient and family members (47.8%) and followed by agreement between family members and medical staff (25.6%), and patients themselves (16.4%). Most of the participants thought that medical staff must explain DNR to critical and end-of-life patients and their family members. Forty four percent of the participants thought that the most appropriate time for DNR explanation was when patients with critical disease were admitted to hospitals. Most of the participants (90.2%) thought a guide book for DNR is necessary to be made in hospitals. 3. There were significant differences in the participants' understanding and attitudes on DNR according to religion career education and experience of DNR. Of the participants those who have religions and education experience on DNR thought that there would be more DNR requests after DNR is explained to patients and family members (p<.05). In addition, there was higher understanding on the necessity of DNR in those who have more career and DNR experience(p<.01). The findings of the study suggest that a guide book for DNR need to be made with inclusion of legal, ethical, and cultural aspects. Also, there needs to be more education on DNR in medical ethics to health care professional and to provide more information on DNR to the general public.

제세동 유형에 따른 제세동 효율성 비교 연구 (A Comparative Study on Defibrillation Efficiency According to Defibrillation Type)

  • 본 연구는 1급 응급구조사들을 대상으로 제세동 유형에 따른 제세동 효율성을 비교하기 위해 시뮬레이션 교육을 적용하여 가슴압박 중단시간, 제세동 효율성을 비교 분석하여 제세동 효율성을 알아보고자 함이다. 이를 위해 2016년 3월 2일부터 8월 31일까지 K도에 소재하고 있는 소방서 구급대원 중 1급 응급구조사 15명을 대상으로 단일군의 교육 전 후 설계 실험연구이다. 수집된 자료는 Windows용 SPSS Win 21.0을 사용하였다. 연구 결과는 다음과 같다. 첫째, 시뮬레이션 교육 후 심폐소생술 과정 중 가슴압박 중단시간은 반자동제세동기의 수동패들과 수동패드를 사용했을 때 유의미하게 시간이 감소하였다. 둘째, 제세동 수행시간은 반자동제세동기의 수동패들과 수동패드를 사용할 때 통계적으로 유의미하게 시간이 감소하였다. 이러한 결과로 보아 시뮬레이션 교육을 통해 1급 응급구조사의 반자동 제세동기의 수동모드를 이용한 신속한 심전도 판독 및 제세동을 통해 환자 소생률을 높이며 응급구조사 업무확대를 할 수 있는 기초자료로 제시하고자 한다.
