Occupational therapist is required for patient-centered approaches to actively seek the perspectives of patients and their families in clinical settings. The purpose of this study was to investigate the current trends and to suggest future-oriented view of examination and assessment used by neurological occupational therapist in clinical settings. Sixty-six occupational therapists who work in persons with neurological disorders participated in this study. The survey was measured from Seoul and GyeongGi by means of E-mail about commonly used assessment tools and selecting considerations. The participants were 66 neurological occupational therapists. The number of patients by one day was from 10 to 14 persons, and the length of time for initial evaluation was 20-40 minutes per one patient, and reexamination periods was every 1 month or as functional changes were detected. The using tool was not limited only neurological tools, and choice consideration was the reliability and validity of clinical measures. The most frequently used tools for adults were: JHFT for motor function in upper extremity, MMSE-K for cognitive perceptual assessment, MBI for daily activity assessment, and COPM for occupational performance. The most frequently used tools for child were: MVPT for cognitive perceptual assessment and Wee-FIM for daily activity assessment. The results of this study suggest that it is necessary to integrate and associate patient-report, care-giver report, and results of performance-based assessment for estimating plan of care more quality.
Objective : The purpose of this study was to investigate the effects of occupational therapeutic intervention including functional task training on activities daily living of falls experienced elderly. Methods : This study used a single subject experimental design with alternating treatment and reversal design. One participant who have had falls experience among the community-dwelling elderly completed. This study a total of 18 sessions were conducted. Fall Efficiency Scale(FES), Berg Balance Scale(BBS) and task modification scale were used to assess the activities of daily living during each session, and Canadian Occupational Performance Measure(COPM) and Assessment of Home-based Activities(AHA) were used to evaluate the activities of daily living at pre-and post-test. Results : The participant showed significant improvements in the FES, BBS scores. However, the improvement was greater when progressive resistance exercise was practised along with functional task training. COPM and AHA scores improved at post-test. Conclusions : These findings provide evidence for occupational therapeutic intervention for falls-experienced elderly.
Objective: The purpose of this study was conducted to systematic review about assessment tools for Activities of Daily Living (ADL) for stroke patients. Methods: Studies tools was administered by using four electronic databases (Pubmed, Embase, Cochrane and NDSL). For the main key words,"stroke AND activities of daily living AND occupational therapy AND assessment OR evaluation OR measurement"was used. We analyzed the types and frequency of evaluation tools. In addition, the evaluation tools for activities and participation were classified based on the classification criteria of International Classification of Functioning, Disability and Health (ICF). Results: In this study, 111 studies were analyzed and 30 assessment tools were identified. As the number of studies on stroke patients has been increased recently, the types and frequency of evaluation tools have been also increased. The most commonly used evaluation tools were Functional Independence Measure (FIM), Barthel Index (BI) and Canadian Occupational Performance Measure (COPM). In addition, according to classification based on ICF, we found that the types of assessment tools which assess participation were few. Conclusion: Although there were many kinds of assessment tools, the types of evaluation tools which were used in the research or field were very limited. Using various assessment tools, more research should be conducted to support evidence-based occupational therapy. Evaluation tools for participation also should be developed.

Purpose: This study was conducted to provide occupational performance evaluated by stroke patients themselves as an intervention according to the periods of its outbreak and examine the effects of it on their activities of daily living and life quality by using the Canadian Occupational Performance Measure (COPM). Method: The subjects of this research were divided into nine persons for the experimental group receiving the client-centered occupational therapy intervention and another nine for the control group getting the conventional occupational therapy intervention. And the intervention was performed for 30 minutes, three times a week, for total eight weeks. The level of activities of daily living was evaluated by the Functional Independence Measure (FIM), and life quality was tested by using SS-QOL. Results: First, about the difference of change in the FIM points of the two groups, there was statistically significant difference in Mauchly's sphericity verification (p<.05). Second, about the difference of change in the SS-QOL points of the two groups, there was statistically significant difference in Mauchly's sphericity verification (p<.05). Third, regarding the coefficient of difference in the change of FIM and SS-QOL points according to the two groups, there existed statistically significant correlation in all the evaluations of before, in the middle of, and after the FIM points and also before, in the middle of, and after the SS-QOL points (p<.05). Conclusion: With the findings of this research, we can see that the client-centered occupational therapy training group shows more improved activities of daily living and life quality than the conventional occupational therapy training group. Therefore, it is expected that the client-centered occupational therapy training will be applied usefully to clinical situations as an intervention to improve stroke patients' activities of daily living as well as life quality.
Background: The International Classification of Functioning, Disability, and Health-core set (ICF-core set) for children and youth (CY) with cerebral palsy (CP) provides a useful conceptual framework and a guide for health care planning and measuring the changes brought by interventions across a multitude of dimensions from body functions to personal activities, social participation, and environmental factors for them. Objects: This single case study was reported to illustrate the use of a goal directed approach in applying the ICF-core set for CY with CP from a physical therapist perspective. Methods: An eleven year old boy with spastic CP, Gross Motor Function Classification System (GMFCS) level V, and his mother participated in an evaluation of his functioning state. The intervention goal was set through an interview using the ICF-core set, Canadian Occupational Performance Measure (COPM) and Goal Attainment Scale (GAS). Physical therapy was carried out on an outpatient basis using a goal directed approach for 30 min, 1 time/week during 12 weeks and the boy's gross motor function was assessed using the Gross Motor Function Measure (GMFM)-66 version (item set 2) before and after the intervention. Results: As measured by the boy's mother, the COPM score showed a meaningful clinical change (performance=mean 3.5, satisfaction=mean 2.5) and the T-score of GAS changed 34.4 after the goal directed approach. The GMFM-66 (item set 2) score changed from 31.8 to 38.7 and evaluation using the ICF-core set displayed improvement in 6 items of activity level between before and after the intervention. Conclusion: The ICF-core set for CY with CP is useful for understanding the overall functioning of CY with this condition and provides an opportunity to share and integrate information and opinions from different disciplines. We consider it as a useful tool in the universal language for the therapy and education of CY with CP.
